Shoulder pain was probably the most frequent complication of hemiplegia. Despite the extensive interest, there continued to be uncertainty about its etiology.In this study, 71 hemiplegic patients were regularly followed up after their cerebrovascular accidents (CVA) for 3 months (33 men, 38 women, with the mean age of 57 years), with the exclusion from another 21 patients of the previous disorders of hemiplegic shoulders, eg. fractures, dislocations, arthritis, or rotator cuff ruptures, etc. During the 3 months, a full rehabil-itation program was given, which consisted of physical therapy, occupational therapy, training in activities of daily living and speech therapy. The cooperation of Taichung Hsien Assiciation for Retarded Children and Provincial Feng-Yuan Hospital to set up the Day Center of Children Development began since Apr. 4, 1992. This center accept children before school age with psychomotor retardation or cerebral palsy. The main duty was to educate and rehabilitate these children during the day time.The evaluation one year later showed marked improvement in several aspects, including gross motor, fine motor, psychosocial development and self daily care. Spinal cord injury (SCI) patients suffered from motor and sensory impairement, as well as neurogenic bladder dysfunction and gastrointestinal (GI) problems. The pathogenesis and clinical presentation of GI problems in SCI patients are different from that of the general populations. This study is to evaluate the GI problems in SCI patients.Questionares were mailed to 52 SCI patients lived in Tainan or Chai-I from Sept 1991 to Dec 1991. Fourty-three questionares were sent back. Myotonia dystrophica is one of the common adult-onset muscular dystrophies. It is characterized by its multisystemic involvement as compared with other myotonic disorders. We report here a 33-year-old man with myotonia dystrophica who was diagnosed after transfer to the rehabilitation department for the sequelae of traumatic brain injury, The clinical, electrophysiological and histopathological findings supported the existence of myotonia dystrophica. The presentation of myotonia did not change in association with his central paralysis.

Sacral perineurial cyst is an uncommon entity. It can be the cause of a sciatica or cauda equina syndrome. Reportd here is a case of multiple lumbosacral perineurial cysts with cauda equina syndrome, who experienced remarkable improvement of symptoms after each course of pelvic traction which was initially prescribed under the impression of lumbar spondylosis. Pelvic traction may probably serve as a palliative treatment temporarily for some patients with only mild symptoms or patients whose general condition is not suitable for surgery.

A 32 year-old male patient fell down from 4th floor and T12 paraplegia developped. Three months after injury, right thigh swelling and pain were noted. Range of motion of right hip joint was also limited. X-ray of right hip joint didn't reveal any abnormal finding. But Tc 99m-methylene diphosphonate (MDP) three phase bone scan showed estra-osseous accumulation anterior-medial to greater trochancter of right femur bone. The serum alkaline phosphatase was also increased. Indomethacin 25 mg Tid was given and passive range of motion exercises were performed. Traumatic common carotid artery (CCA) injury with subsequent cerebral infarction is a rare cause among stroke patients. we report a male aged 33 years who suffered from weakness of left limbs about 12 hours after blunt neck trauma. About 48 hours later, computerized tomography (CT) of the head revealed a low-density lesion over the territory of the right middle cerebral artery (MCA). Carotid Doppler ultrasound examination showed two thrombi within the right proximal CCA. The purpose of this study was determine the influence of the biofeed-back index to the muscle tension of the shoulder in Cello student. Data analysis of 14 students included before performance, during performance and after performance. The result revealed biofeed-back index higher in left shoulder then right shoulder after performance. This is a case of 30-year-old female patient who suffered from Rheumatoid Arthritis for 8 years. She also has the problems of Systemic Lupus Erythematosus and the sequsela of Poliomyelitis. After bilateral total knee replacement, she was almost bed ridden with multiple joints pain. The best muscle power of both lower limbs was only fair then. She received oral methotrexate therapy and got moderate improvement, such as decrease in unmber of joint pain. Resting splints and anterior direct molding ankle-foot orthosis were made for her. Qi Gong is a special type of Chinese traditional conditioning exercise, consisting of three major components: dynamic sitting posture, meditation, and diaphragmatic exercise.In order to evaluate the differences of the cardiorespiratory function between Qi Gong and Tai Chi Chuan practitioners, there are three groups, each group including 10 male middle-aged subjects, with matched age and body size: (1) Qi Gong practitioners for at least one year; (2) Tai Chi chuan players for average 5.8 rears; (3) sedentary subjects. Triathlon, an ultraendurance exercise, is a race involving consecutive swimming, bicycling and running which is currently enjoying a tremendous increase in popularity abroad. This study is aimed at assessing the overuse injuries in the training period and the sports injuries during the first triathlon race in Taiwan in 1992.The triathlon race composed of 1.5 Km swimming, 40 Km bike and 10 Km run which must be finished within 3 hours and 40 minutes. With 177 effective questionnaires (86.3% of the 205 race entrants), 94.3% (167 competitor) were men and 5.6% (10 competitor) were women. The average age was 32±9.1 years. …

The study was designed to determine the extent of significant pulmonary　dysfunction in selected residents of chronic care facilities, to find out any relationhip between pulmonary dysfunction and general disability, and explore pulmonary treatment options.Thirty-six nursing facility residents over 80 year old, or with a diagnosis of chronic obstructive pulmonary disease or generalized neurological or neuromuscular disease, were surveyed for extent of disability and pulmonary dysfunction. According to the previous literature, leg length discrepancy within 2.54 cm will be compensated by individual; no management is indicated. Insole modification and external shoe modification will be given for those with leg length discrepancy between 1 to 2 inches. Surgery intervention will be given for those with leg length discrepacy beyong 5 cm or 10 to 15% shorter than that of the sound leg. Thirty four adults with traumatic brain injury [TBI] who had magnetic resonance imaging [MRI] within three months were evaluated motor outcome 6 to 18 months after injury. MRI were classified into four groups according to the depth of abnormality detected and the motor function evaluated with revised Fugl-Meyer method. The results showed no positive relationship between motor outcome and the depth of lesion detected by MRI. It was different from the result of Wilson's studies which had positive finding between the depth of brain lesions detected by MRI andneuropsychological outcome. Spinal cord tumors, sometimes without spinal symptoms, occasionally cause signs and symptoms of intracranial disease in the form of papilledema and/or hydrocephalus.The association of hydrocephalus with benign extramedullary spinal cord tumors is particularly rare. The incidence was only 1% to 3%. So, in the diagnosis of intraspinal tumor, the associated presentations of intracranial hypertension were as important as hydrocephalus. Spinal epidural abscess (SEA) is a rare infectious disorder often subject to a delayed diagnosis. In two United States hospitals, the incidence was between 0.2 and 1.2 per 10000 admissions. There are still no reports about the incidence of SEA in Taiwan. In this report, we present a 57-year-old diabetic woman who suffered from severe lower back pain for one week prior to admission. A patient with headache and nuchalgia for three years was initially diagnosed as psychogenic origin. Spinal cord compression syndrome was unrecognized until motor evoked potential study showing abnormal findings. Excision of a nearly 3x2x2 cm dumbbell extra-and intradural tumor, which arose from the right C2 root, was performed after MRI documentation. Histologic diagnosis was neurilemmoma. Motor evoked potential study is useful in spinal cord diagnostics. The particular value of complete history-taking and exact neurological examination should not be underestimated.

Correction of equinus contracture of feet offers the chance for ambulation. The casereported here describes a 34-year-old paraplegic woman with scald burn resulted in equinus contractures of both feet associated with extensive scar formation who received the Ilizarov external skeletal fixation for the equinus correction and completed the ambulation training program. This technique offers a new horizon in the correction of joints contractures and deformities in traumatized patient as well as in patient suffering from spinal cord injury.

The purpose of this study is to quantitatively analyze the effect of reference electrode size on the amplitude, surface area, latency and duration of CMAP during routine NCV studies in median and ulnar nerves. We dilivered supramaximal stimulations over the wrist in the median and ulnar nerve examinations and obtained the data of CMAP parameters from APB and ADQ muscles. Developmental language delay is one of the most common developmental problem found in preschool children. There is increasing evidence showing that early language delay is associated with later learning and behavior problems, and the language problem may also persist into later childhood. We followed 33 cases who had been diagnosed as language delay, at least 5 years ago, by questionnair. Neck pain is the most clinical complain in PM&R out patient department. In spite of , the upper cervical is rare. But the lesion is variety and dangerous.The upper cervical lesion, non-pathologic lesion is always obviously traumatic history. But pathologic lesion has not obviously traumatic history that the diagnosis is often delayed. This study was carried out on 68 cases (age 3-76, M/F 51/17) who took CT or MRI at TAICHUNG VETERAN GENERAL HOSPITAL from SEP. 1983 to NOV. 1991. These cases consisted 51 male, 17 female.
