The purpose of this doctoral project was to improve charge nurse leadership competencies through education on the tiered skills acquisition model (TSAM), addressing challenges of high turnover, limited orientation, and compromised care quality in an inpatient psychiatric facility. The background reveals a two-year financial loss of over $1.8 million tied to staff attrition, highlighting the urgent need for a structured training framework. This doctoral project was conducted to develop, implement, and evaluate whether an educational intervention for registered nurses would enhance their knowledge on using the Epworth Sleepiness Scale (ESS) for sleep assessment in primary care. Black adults in the United States are nearly twice as likely to receive a diagnosis of type 2 diabetes mellitus (T2DM) and are more susceptible to severe diabetes-related complications. Health literacy and patient-provider communication (PPC) are extremely important for self-management in patients of all ethnicities however, 58% of non-Hispanic Blacks have basic or below-basic health literacy skills compared to 28% of non-Hispanic Whites. Alcohol withdrawal syndrome (AWS) is a critical condition commonly encountered in hospital settings. If not properly treated, AWS can progress to seizures, delirium tremens, and death. Traditionally, benzodiazepines guided by the Clinical Institute Withdrawal Assessment for Alcohol Scale, Revised (CIWA-Ar), have served as a mainstay of treatment. Despite established fall prevention strategies, the unit continued to experience ongoing incidents, prompting the implementation of structured leadership rounding. The practice-focused question assessed changes in fall rates over a 3-month postintervention period compared to the preceding 3 months. Medication errors represent a persistent threat to patient safety, particularly during transitions of care in outpatient settings. Despite recognition of the problem, standardized medication reconciliation practices among primary care physicians and nurse practitioners (NPs) remain inconsistent, hindered by varied workflows, unclear responsibilities, and knowledge gaps. To address these issues, a staff education program focusing on medication reconciliation was introduced to seven NPs and three physicians in a primary care clinic. Pre- and post-intervention surveys measured participants’ knowledge and confidence in conducting medication reconciliation and documenting medications in electronic medical records. This Doctor of Nursing Practice (DNP) staff education project aimed to address inconsistent and ineffective handoff communication among healthcare staff in a 44-bed observation unit. The project focused on implementing an education program centered on the SBAR (Situation-Background-Assessment-Recommendation) communication technique to improve staff knowledge and communication practices. Using a pretest/posttest design, the program demonstrated significant knowledge gains among participants, with a cohort-normalized gain of 0.85 and statistically significant improvements in test scores (p < .05). The intervention highlighted the importance of structured communication in enhancing patient safety, reducing errors, and fostering interdisciplinary collaboration. Despite limitations such as a small sample size and time constraints, the project highlighted the value of SBAR as a reliable tool for enhancing communication and improving patient outcomes. Recommendations include ongoing SBAR training, integration into onboarding programs, and expansion to other departments to sustain long-term improvements in communication and care quality.

The purpose of this study was to assess the prevalence of MASLD-related advanced fibrosis in the United States and assess whether type 2 diabetes mellitus (T2DM) and cardiovascular disease (CVD) were independent risk factors for MASLD-related advanced fibrosis. A primary care medical clinic experienced an increase in hospital readmission for hypertensive patients, as evidenced by a consistent lack of standardized patient education on medication adherence and a notable increase in readmission rates for uncontrolled hypertension.
