Ganglion cysts are among the most common benign soft tissue masses encountered in podiatric clinical practice, and their diagnosis is often presumed based on location, clinical observation, and transillumination. However, this case highlights a diagnostic idiosyncrasy: a lesion clinically presumed to be a ganglion cyst within the plantar fascia was ultimately identified as an angioleiomyoma following surgical excision and histopathological evaluation. This finding underscores the importance of maintaining a broad differential diagnosis when assessing cystic lesions, particularly those that transilluminate, as benign vascular tumors may closely mimic more common entities in both clinical presentation and imaging. The United States faces a critical dermatology workforce shortage, with more than 60% of counties lacking specialist access. Primary care providers (PCPs) are often tasked with managing suspicious skin lesions but demonstrate lower diagnostic accuracy than dermatologists. Artificial intelligence (AI) tools offer potential solutions, and meta-analytic evidence demonstrates that AI assistance improves PCP sensitivity by 13 percentage points and specificity by 11 percentage points. Yet only 1 Food and Drug Administration–authorized device (January 2024) is cleared for nondermatologist use in the United States. This device achieves 96% sensitivity but only 21% specificity, meaning most positive results are false alarms. A …

Introduction: Hypopituitarism results from insufficient pituitary hormone production, often due to adenomas—accounting for ~15% of intracranial tumors. Non-functional adenomas, typically affecting older adults, may present with visual field deficits, headaches, or cranial nerve palsies when extending into the cavernous sinus. Isolated oculomotor (third nerve) palsy is a rare initial manifestation.

Case: A 46-year-old man with uncontrolled type 2 diabetes presented with acute right-sided headache, diplopia, ptosis, and right leg weakness. Introduction: Deep vein thrombosis (DVT) in young adults is uncommon and typically associated with identifiable risk factors. In cases where DVT occurs without apparent provocation, congenital anatomical anomalies should be considered. May-Thurner Syndrome (MTS), a vascular compression disorder involving the left iliac vein, is a frequently overlooked cause of iliofemoral DVT in young individuals, particularly women.

Prostate cancer is one of the major health concerns, with increasing incidence and persistent therapeutic resistance worldwide. Despite various available treatments (radiation therapy, chemotherapy, hormone therapy, cryotherapy, biological therapy, and high-intensity focused ultrasound), patients still show treatment-related side effects over many years. Introduction:  Diabetic foot ulceration and peripheral vascular disease remain major causes of morbidity and limb loss, yet early identification of tissue compromise can be challenging, particularly across diverse skin tones. Hyperspectral imaging (HSI) is a non-invasive technology that quantifies tissue oxygenation by measuring the absorption and reflection of near-infrared light.

Introduction: Periprosthetic joint infection (PJI) is a rare but devastating complication following total ankle arthroplasty (TAA). Acute infections are commonly managed with debridement, antibiotics, and implant retention (DAIR), in which irrigation plays a critical role in reducing bacterial burden. Although irrigation protocols have been investigated in total hip and total knee arthroplasty, evidence specific to infected TAA remains unclear. This systematic review evaluated the available literature regarding irrigation practices in the surgical management of infected TAA.

Introduction: Parkinson’s disease (PD) is a progressive neurodegenerative disorder characterized by the degeneration of the dopaminergic neurons of the substantia nigra that leads to motor and non-motor dysfunctions. Emerging evidence highlights mitochondrial dysfunction as a key factor in PD pathogenesis. The COVID-19 pandemic highlighted the need for accurate epidemic forecasting to support public health decision-making. Most existing approaches depend heavily on human mobility data, while largely neglecting population behavior shaped by socio-cultural norms. In this study, we analyze daily COVID-19 mortality and Google mobility data from 72 countries during the first 130 d of the pandemic, a period characterized by high uncertainty and behavioral heterogeneity. In particular, we examine whether Hofstede’s country-level cultural dimensions can serve as latent behavioral forecasters of mortality in lieu of dynamic mobility indicators. Skin cancer disparities persist among racial and ethnic minority populations, yet sociocultural factors shaping engagement with preventive health information remain understudied. This study examines whether internalized colorism functions as a barrier to skin cancer information avoidance among Hispanic college students and whether these associations differ by sex. Using survey data from 300 undergraduates at a Hispanic-serving institution, hierarchical regression analyses examined how three dimensions of colorism, including self-concept, attractiveness, and upward mobility, were associated with skin cancer information avoidance. Influenza A remains a major cause of respiratory mortality worldwide, motivating accurate forecasting to support timely preparedness and resource allocation. This study presents a comparative evaluation of two traditional seasonal time series baselines, ARIMA and Holt–Winters exponential smoothing (ETS), and six deep learning (DL) architectures (Simple RNN, LSTM, GRU, BiLSTM, BiGRU, and a Transformer) for forecasting monthly Influenza A case counts in the United States. Background: Metabolic dysfunction–associated steatotic liver disease (MASLD), including metabolic dysfunction–associated steatohepatitis (MASH), is a leading cause of chronic liver disease worldwide. Current guidelines recommend fibrosis-based risk stratification using noninvasive tools such as the Fibrosis-4 (FIB-4) index in patients with metabolic risk factors, followed by secondary assessment (e.g., transient elastography or enhanced liver fibrosis(ELF) testing) for those with elevated scores. Behaviors arise from coordinated neural activity across diverse spatial and temporal scales. Prior work has linked better task performance and cognitive functioning to patterns of global network connectivity requiring minimal reconfiguration when switching between task demands. This metric indexing similarity in functional connectivity across task and rest has been termed “neural efficiency.” Here we assess stability of neural efficiency over approximately 3 years in adolescence, specificity across two task-rest combinations and associations with anxiety. Several combination therapies for the early outpatient treatment of COVID-19 were proposed by independent research groups at the onset of the pandemic during 2020 and 2021. In this observational study, we report on the outcomes of an off-label triple combination therapy, consisting of ivermectin, doxycycline, and zinc, with adjunct vitamin C and D3 supplementation, which was used on high-risk COVID-19 patients. These patients refused an initial recommendation to seek inpatient care, despite a high-risk presentation compounded with one or more comorbidities and/or severe hypoxia. Importance: Subtle, age-related declines in occupational performance among at-risk older adults may go undetected when assessed solely with self-report measures, potentially leading to missed intervention opportunities. Understanding discrepancies between self-reported and performance-based occupational performance outcomes can inform assessment selection and interpretation.

Objective: To examine the discrepancies between the two types of measure among community-dwelling older adults at risk for losing independence.

Design: Cross-sectional study using secondary data analysis.

Charcot-Marie Tooth (CMT) disease is a clinically and genetically heterogeneous inherited peripheral neuropathy for which there is no treatment. CMT patients often present with weakness, fatigue, and muscle atrophy in the distal limbs. Improving function at the neuromuscular junction (NMJ) may improve function in some CMT patients. Using mouse models, we investigated eight CMT subtypes for NMJ phenotypes by morphology and functional deficits assessed by electromyography (EMG). We did not find NMJ abnormalities in mice with mutations in Gjb1Y/Δ2 (CMT1X), or Yars1E196K/E196K (diCMTC). Altered immune function is increasingly recognized as a contributor to Alzheimer's disease (AD); however, it remains unclear whether peripheral immune alterations reflect constitutive inflammation or stimulus-dependent changes in immune responsiveness. Addressing this distinction is critical for understanding immune dysregulation in neurodegenerative diseases. In this study, we applied a challenge-based ex vivo immune profiling approach to characterize functional immune responsiveness in patients with AD and in cognitively healthy older adults. Peripheral blood mononuclear cells were exposed to defined innate, antigenic, and mitogenic stimuli, and cytokine and β-amyloid (Aβ) responses were quantified in culture supernatants. Despite advances in therapies, cardiogenic shock (CS) remains associated with high mortality rates, often necessitating mechanical circulatory support (MCS) to preserve end-organ perfusion. Despite increasing rates of MCS utilization, less is known about sex-related differences in clinical outcomes to guide practice. Additionally, standardized management protocols for CS patients requiring MCS that account for sex differences are lacking. Breast cancer (BC) remains a leading cause of cancer-related mortality in women worldwide. Scutellaria barbata, a traditional Chinese medicinal herb, possesses recognized anticancer properties, but its mechanistic role in BC is not fully elucidated. This study employed an integrated in silico approach to identify the key flavonoids, targets, and pathways through which S. barbata exerts its anti-BC effects. A multi-step computational methodology incorporating network pharmacology, molecular docking, and dynamics simulations was utilized to profile bioactive compounds from S. barbata. Introduction Introducing surgical skills early in medical training may enhance students' confidence and readiness for clinical training. Simulation-based skill workshops provide students with a comfortable learning environment, resulting in effective learning. Introduction: Angiotensin-converting enzyme inhibitor–induced angioedema (ACEI-AE) is a rare but potentially serious adverse effect of a widely used class of antihypertensives. It typically presents as non-pitting, non-pruritic swelling of the face, lips, tongue, or upper airway, often without associated urticaria. Although most cases occur within the first weeks of treatment, delayed presentations, sometimes after years of stable therapy, have been increasingly recognized. The condition is bradykinin-mediated and often does not respond to standard allergy medications such as antihistamines or corticosteroids.

Introduction: Menopause is associated with significant physiological changes, including alterations in lipid profiles. Estrogen therapy plays a crucial role in managing symptoms and preventing long-term complications. This study aims to compare oral and transdermal routes of estrogen therapy among postmenopausal women and assess changes in lipid parameters from baseline.

Methods: Randomized clinical trials comparing oral and transdermal estrogen therapy in postmenopausal women were identified through searches in PubMed, Scopus, Web of Science, and ClinicalTrials.gov. Pooled mean differences (MDs) with 95% confidence intervals (CIs) were calculated using a random-effects model. Statistical analysis was conducted using R version 4.3.2.

Introduction: The comparative effects of oral versus transdermal estrogen therapy on cardiovascular parameters in postmenopausal women remain incompletely understood. This analysis aimed to evaluate blood pressure and heart rate responses between administration routes.

Methods: A systematic review of randomized trials comparing oral and transdermal estrogen therapy in postmenopausal women was conducted. Primary outcomes included changes in systolic blood pressure (SBP), diastolic blood pressure (DBP), and heart rate. Random-effects meta-analysis was performed to calculate pooled mean differences (MD) with 95% confidence intervals (CI).

Background: Ulcerative colitis (UC) is a chronic inflammatory bowel disease often complicated by extraintestinal manifestations, with skin involvement seen in up to 15% of patients. Typical lesions include erythema nodosum and pyoderma gangrenosum, but abscess-like cutaneous lesions are almost always assumed to be infectious. Rare case reports describe sterile, culture-negative abscesses arising during UC flares, yet their true prevalence is unknown. We present the case of a young man with UC who developed multiple aseptic skin abscesses in parallel with active proctitis.

Background: Telemetry monitoring is a critical tool in detecting arrhythmias but is frequently overutilized outside of guideline-based indications, contributing to alarm fatigue, unnecessary costs, and resource strain. National guidelines (AHA 2017; BHRS 2020) recommend targeted use, yet adherence remains inconsistent. This quality improvement (QI) project was designed to assess baseline adherence and implement a resident-led intervention to optimize telemetry utilization in a general medicine unit.
