Introduction: Failure in palatal fusion during palate development results in cleft palate, one of the most common birth defects in the US. During the final phases of palatogenesis, the palatal periderm (a protective cell layer that prevents premature fusion and adhesion with oral epithelium) must be removed, also known as desquamation. This process is a prerequisite for establishing the midline seam in the fused palate. Previous studies have shown that transforming growth factor-β3 (TGFβ3) plays an irreplaceable role in palate development in mice. Mandibular molar distalization plays a critical role in the management of crowding, and correction of the molar relationship.

This thesis explores the development and preclinical assessment of polymeric chloroquine (PCQ)-based nanotherapeutics, leveraging stimuli-responsive designs for disease-specific delivery and action.

Self-assembled polymeric polyplexes have the potential to serve as an RNA interference (RNAi) delivery platform for the combined inhibition of CXCR4 and miR-155 in the treatment of alcohol-associated liver disease (AALD). A central challenge in RNA delivery is balancing the extracellular stability of polyplexes, cellular uptake, and intracellular release of RNA cargo. We developed polymeric polyplexes using the CXCR4-antagonist polymer PAMD, designed to target CXCR4 on activated hepatic stellate cells (HSCs) and deliver therapeutic miRNA to activated Kupffer cells (KCs) in fibrotic liver. Farming is a hazardous occupation. Exposures are multifactorial, variable, and shaped by overlapping tasks, environments, and social contexts. This dissertation aimed to illustrate the complexity of agricultural exposures and the barriers to protective practices through three complementary studies.

The first study examined co-exposures to solvents and noise as risk factors for hearing loss (HL) among 7,495 farmers and ranchers in a 7-state region. Results indicated nearly 60% of co-exposed participants reported hearing loss, with an adjusted odds ratio nearly six-fold (OR=6.03, [4.67-7.78]) for moderate/severe HL. Substantial investments have been made in hospital emergency preparedness. The Hospital Medical Surge Preparedness Index (HMSPI) has been proposed as a metric to assess health system readiness. This index summarizes hospital system characteristics, space and facilities, staff availability, and supply planning.

Methods

The primary objective was to evaluate the relationship between the HMSPI and characteristics of the respective hospitals, including size, type, location, performance on the Hospital Resilience Index (HRI), and Social Determinants of Health (SDoH).

This replication study examines the association between frequent social media (SM) use and bullying victimization, persistent feelings of sadness or hopelessness, and suicide risk among U.S. young adults using data from the 2023 Youth Risk Behavior Survey. The study replicates the findings of Young et al. (2024), the first CDC report to incorporate SM into national adolescent behavioral surveillance. Using a weighted, cross-sectional sample of 15,203 students, the replication analysis estimated unadjusted and adjusted odds ratios for outcomes, controlling for age, sex, race/ethnicity, and sexual identity. 77% of students reported frequent SM use. Smoke produced from these fires creates a multitude of air pollution constituents that can cause breathing and other health issues for humans, particularly those with asthma and other respiratory conditions. In January 2022, the Department of Defense launched a five-year pilot program called the TRICARE Childbirth and Breastfeeding Support Demonstration. This demonstration aimed to cover lactation services for Tricare beneficiaries. This project included an electronic survey and was created and completed by 577 participants. Eligible participants needed to have given birth in the last 24 months and have TRICARE as their primary insurance. Participants completed this survey to help understand if they were aware of lactation benefits, if they utilized the service, and if it improved their breastfeeding experience.

Certified nursing assistants (CNAs) are at a higher risk of back injuries in healthcare, particularly in long-term care settings. This literature review assessed the primary causes of these injuries and the efficacy of current interventions, particularly Safe Patient Handling and Mobility (SPHM) programs. A total of 26 peer-reviewed articles were published between 2014 and 2025, including experimental simulations, cross-sectional and longitudinal studies, quasi-experimental training interventions, and evaluations of SPHM programs. Across the literature, the primary contributors to back injuries included high physical workload, inadequate ergonomic design, limited access to assistive devices, and staff shortage.

Purpose: Care partners of people with Alzheimer’s disease (AD) and related neurocognitive disorders (NCDs) often experience disrupted routines, reduced self-care, and limited participation in meaningful occupations. Occupational therapy (OT) supports care partners’ engagement in health-promoting activities and sustainable caregiving (Rhodus et al., 2023). This systematized review examined interventions that enhance care partner performance and participation.

Purpose: Individuals with intellectual and developmental disabilities (IDD) transitioning to adulthood often receive minimal support, resulting in challenges with education, employment, and independent living. This systematized review aimed to identify interventions within the scope of occupational therapy to aid a smoother transition to adulthood for individuals with IDD. By analyzing existing research, this study identified evidence-based OT strategies to promote independence, self-determination, and community participation.

Kikuchi-Fujimoto disease (KFD) commonly presents with lymphadenopathy, with a third of cases presenting with fevers. It is seen in young female patients of Asian descent, with Taiwan accounting for most of the reported cases. It is imperative to consider KFD in an individual presenting with fever and lymphadenopathy. While it might not be common in the United States, there is an increasing number of cases being reported. Histologic features in KFD are unique, and a lymph node biopsy determines the diagnosis. We outline a case of KFD in the United States and describe histologic details that resulted in the diagnosis.
