The 7.6-magnitude earthquake on October 8, 2005, caused a large number of deaths, and left more than 100,000 people injured and an estimated 3 million people displaced or homeless. To assist the Government of Pakistan in the preparation of a rehabilitation plan, the first step was to identify the vulnerable people living in the tent camps. The Population Council completed an assessment survey of the camps in the earthquake area. This assessment is distinctive from the surveys carried out previously, since it collects socio-demographic and vulnerability indicators about each individual. Nairobi, Kenya is home to one of the largest slum populations in sub-Saharan Africa. Adolescents living in these slums, especially girls, are cut off from the potentially protective environments of school and family and lack safe economic opportunities. Unemployment is a major concern for adolescent girls, and desperate circumstances force them to survive however they can: 21 percent of sexually active girls aged 15–19 report exchanging sex for money or gifts. This heightened vulnerability is reflected in rates of HIV infection. It is well established that truckers (drivers and helpers) have higher rates of nonmarital sex than any other occupational group. Because of this multipartner sexual activity, truckers form a key group for HIV/STI prevention efforts. Thus far, HIV/STI prevention interventions for truckers have been geared to providing risk-reduction information and services within a heterosexual context. Recent evidence suggests that a large number of truckers engage in male-to-male (MSM) sexual activity that has serious implications for HIV/STI transmission. This issue of Quality/Calidad/Qualité describes the vital role played by the Ghana branch of the Society for Women and AIDS in Africa (SWAA) in introducing the female condom, the only female-controlled method for protection from HIV. By addressing issues of gender inequity and communication within relationships, SWAA used the female condom to empower women to regain control of their bodies. Also in this issue are profiles of three other approaches to programming for the female condom from Brazil, South Africa, and Zimbabwe.

In the absence of intervention, poor, rural, unschooled girls in conservative and low-income communities throughout West Asia and North Africa, and indeed elsewhere, are destined to a life of poverty, illiteracy, early marriage, high fertility, and poor health. This report describes Ishraq, a well-designed, multidimensional skill-building program, that has altered this scenario in Upper Egypt by intervening early (around the time of puberty). The two previous publications contain data and information essential for understanding the project and its results, although they do not analyze the microfinance component in depth. In contrast, this brief was written by microfinance practitioners for a microfinance audience. In 1998, the Population Council and K-Rep (a Kenyan microfinance institution) developed a microfinance approach to address livelihood-strategy constraints for adolescent girls at risk for HIV/AIDS in an urban slum in Kenya. The objectives of this study were to assess the Kenyan Ministry of Health’s capacity to adapt focused antenatal care (ANC) at the service delivery level so that it can be sustained, and to examine the extent to which adaptation of the package has increased coverage and quality of key ANC services and the overall quality of care received by pregnant women. The study showed that focused ANC is acceptable to both clients and providers and the approach has also received tremendous support from both the government and health development partners. This project contributed significantly to the capacity-building of regional professionals in planning, implementing, and monitoring of reproductive health programs in South Asia. During 2001–05, professionals from 17 countries received training in various aspects of reproductive health in nine workshops, including operations research, economic evaluation, qualitative research methods, proposal writing, and process documentation and enhancing the utilization of research findings in reproductive health programs. Forty-three percent of workshop participants were program managers from government health programs and nongovernmental organizations. Systematic screening is a strategy to integrate reproductive health services at the provider level. Integration is the proactive provision of multiple reproductive health services in the same facility at the same time. Systematic screening is a simple intervention to increase the number of services received at a single client visit. In this strategy, providers use a checklist or questionnaire to identify each client’s needs and desires for reproductive health services. The Somali community living in Kenya has practiced the severest form of female genital mutilation/cutting (FGM/C), infibulation, for centuries. To understand the context within which the practice takes place, and how its complications are managed, FRONTIERS undertook a diagnostic study in North Eastern Province and in the Eastleigh area of Nairobi. The study found that the health system is ill equipped to serve women who have been cut, and particularly infibulated women who are pregnant and delivering, stemming from an overall weakness in the availability and quality of maternal and neonatal heath services. A great deal of attention has been focused recently on promoting the “ABCs” of HIV prevention— abstinence or delaying sex, being faithful to one sexual partner or reducing the number of sexual partners, and consistently using condoms during sex. Yet even as programs that focus on the ABCs to prevent sexual HIV transmission are rolled out, questions remain about how well different groups in varied cultural contexts understand the terms, as well as how best to address challenges to adopting the ABC behaviors. This article explores efforts to encourage abandonment of female genital cutting (FGC) in Senegal, Burkina Faso, and Mali. The strategy of encouraging traditional practitioners to stop performing FGC is not effective as community-level support for the practice remains high. Training health staff significantly improved healthcare providers' levels of knowledge about FGC and its complications. Consequently, it was recommended that such training be incorporated into preservice medical training. The use of community-based strategies like the Village Education Program (VEP) showed the most promise, as their multifaceted approach addresses knowledge, attitudes, actions, and communal support in an integrated manner. People living with HIV/AIDS in India, as elsewhere, face stigma and discrimination in a variety of contexts. Research in India has shown that stigma and discrimination against HIV-positive people and those perceived to be infected are common in hospitals and act as barriers to seeking and receiving critical treatment and care services. This Population Council Horizons report presents findings from an exploratory study by the Regional Psychosocial Support Initiative and Catholic Relief Services’ Support to Replicable, Innovative Village/Community-level Efforts Program of vulnerable youth living in and around Bulawayo, Zimbabwe. It describes their demographic characteristics, exposure to stress and trauma, and psychosocial well-being. The report also highlights the relationships between psychosocial well-being outcomes and exposure to stress and trauma, and the differences in psychosocial well-being between males and females, orphaned and nonorphaned youth, and younger and older adolescents. The report concludes with program and research implications.
