Background & aims Celiac disease (CD) is a multisystem disorder triggered by dietary gluten in genetically predisposed individuals that may affect any organ system, including the liver. We evaluated a change in patient model for end-stage liver disease (MELD)-Na and albumin level from the time of celiac disease diagnosis to six months later, after implementing a gluten-free diet. Methods A retrospective study was conducted from January 1, 2006, to June 30, 2018. CD was diagnosed based on celiac antibodies and/or histopathological data. Currently, the prescription opioid and heroin overdose epidemic is the worst public health crisis in Pennsylvania and addiction problems within the Lehigh Valley far outpace the number of health professionals trained to provide care in the field of Addiction Medicine. Additionally, negative attitudes towards individuals with substance use disorder are prevalent among health care providers. Unfortunately, this trend may continue as preprofessional health care curricula across many disciplines still lack the social, emotional, and behavioral competencies to treat patients with substance use disorder. Persistent left superior vena cava (LSVC) is an asymptomatic congenital heart disease. It is usually found incidentally on imaging, during central line placements or while undergoing electrophysiological procedures. We present a case of a 91-year-old female who initially presented with seizures and was diagnosed with tachy-brady syndrome. She was planned to undergo dual-chamber permanent pacemaker placement. However, during the procedure, she was incidentally found to have an LSVC without a right superior vena cava. Due to challenging anatomy, her pacemaker was changed to a single-chamber atrial lead pacemaker. This case highlights the clinical implications of this unusual structural anomaly, technical …

Anti-signal recognition particle (SRP) antibodies are typically associated with immune-mediated necrotizing myopathy. Some patients with anti-SRP antibodies may have extramuscular manifestations including mild respiratory symptoms secondary to interstitial lung disease. We present a case of a 40-year-old female presenting with acute hypoxic respiratory failure secondary to anti-SRP antibody-associated interstitial lung disease with mildly elevated creatinine kinase but without evidence of necrotizing myopathy on muscle biopsy. After a complicated six-month hospitalization, the patient successfully received double lung transplantation and was eventually discharged on room air. Quality measurements (QMs) have emerged as quantitative tools for measuring "quality", an elusive term that has been historically difficult to define and quantify. However, current literature has demonstrated that these measurements are flawed. The purpose of this study was to identify the strengths and weaknesses of quality measurements and provide a novel scorecard for evaluating quality measurements. In this retrospective analysis, 246 quality measurements that are integrated into the most significant payer-provider contracts within our institution were analyzed. Each measurement was dissected based on type of measurement, evidence, precision, data exchange, alignment, and how patient-oriented. Our research showed a significant …

OBJECTIVES: The aim of this study was to determine the feasibility of establishing a mechanical thrombectomy (MT) program for acute ischemic stroke in a community hospital using interventional cardiologists working closely with neurologists.
BACKGROUND: American Heart Association/American Stroke Association 2018 guidelines give a Class I (Level of Evidence: A) recommendation for MT in eligible patients with large vessel occlusion stroke. Improvement in neurological outcomes with MT is highly time sensitive. Most hospitals do not have trained neurointerventionalists to perform MT, leading to treatment delays that reduce the benefit of reperfusion therapy.
