The survey of dancing injury in the special dancing class of a primary school was done on 1990. From 108 effective questionare, the prevalence of dancing injury was 77.78% in which 11.54% became chronic.In the 59 girls of grade 5, 6 students, the most frequent injury area was ankle 34.48%, next was the knee 20.2% and inner thigh muscle 11.72%. The causes of the injury were 1) unenough warm up (47.61%) 2) poor dancing floor (29.76%) 3) over training (13.09%). For the better understanding of problems of patients with spinal dysraphism, 51 cases were studied retrospectively by questionnaire, telephone and clinical followup. They had been admitted to Taipei Area Chang Gung Memorial Hospital from Jan 1983 to Dec. 1989Among them 24 patients were male and 27 were female. Their age distribution were from 2 to 39 years old. Thirty patients with meningomyelocele, 5 of them had hydrocephalus; 14 with tethered cord; 7 patients had meningomyelocele with tethered cord; only one case had Arnold Chiari Malformation. Early detection will lower the risk of neurological sequelae. Sarmiento introduced the functional fracture brace for the management of tibial shaft fracture in 1963. However, tibial angulation with varus deformity may still occur, especially in the presence of an intact fibula. We report here a case of tibial shaft fracture with varus angulation treated with a modified below knee fracture brace. Our modification included a wooden rod embedded in the under-side of the anterior shield with nod positioned laterally between tibia and fibula. Intoxication of lithium carbonate may present as a multiple systematic dysfunction, but central and peripheral nervous involvement is rare and more troublesome. In the past, there was only one case of lithium intoxication with polyneuroparhy reported by Chang et al in Taiwan. We report here an additional 49 year-old man with manic-depressive psychosis. He received lithium carbonate medication treatment for 5 years and developed a severe cerebellar dysfunction and a mild sensori-motor polyneuropathy. Blood lithium level was 1.80meq/l two days after quitting lithium. This is a case of a 64-year-old male with difficulty in ambulation for 40 years. Physical examination revealed a defect in vibration and positional sense over the lower extremities. Muscle power of the upper extremities was normal. The lower extremities could still resist force, but there was a little atrophic change in the quadriceps muscle and weakness over the iliopsoas muscle.Laboratory data were all negative, including CSF, routine blood, urine and stool specimens. Diurnally fluctuating hereditary progressive dystoria is a very rare disorder. In 1975, Segawa regarded this disorder as an entity, and there are 46 cases reported till 1986 since then. The clinical manifestation of this disease are: normal development, early onset, dyotonia, diurnal change, excellent response to L-dopa. In this report, a 21-year-old female, who had had symptoms onset since 8 years old; became wheel chair bound at 15 years of age; was not diagnosed correctly until 20 years old. A 53 year-old male patient, who sustained infarction of the right medial frontal lobe with extension into the corpus callosum, demonstrated inattention, slow initiation and left hemiparesis initially. As the strength recovered, forced grasping of the left hand, and intermanual conflict appeared. Besides, left ideomotor apraxia, left constructional apraxia, left apraxic agraphia, and abnormalities of cross-replication of hand posture were detected after specific clinical testing. Most of these deficits improved eight weeks after stroke. The effects of ice and compression wrap applications on intramuscular temperature at the skin surface, 1 cm below the fat layer, and 2 cm below the fat layer were recorded in 11 subjects. Each subject was tested under all treatment conditions including control, compression only, ice only, and ice+ compression. Subjects underwent a 5-minute pre-application, a 3 ominute application, and a 20-minute post-application. Using thermocouples, surface and intramuscular temperatures were recorded at 30-second intervals for the entire study. This study described biomechanical deviations from proper biceps curl technique specifically in the shoulder, trunk, body and knee angles, that occurred in response to varying intensities of an individual's 1 RM (repetition maximum). Ten college age males participated in the study. Subjects were required to perform one repetition at 60%, 70%, 80%, 90% and 100% of his 1 RM. The only differentiating factor was the order in which the five intensities were performed. Condition order was randomized to minimize bias. Movement was filmed with a high-speed motion picture camera, digitized and then analyzed with computer software.

The Effect Of Vestibular Stimulation On Expressive Language Of Pre-Schoolers With Down Syndrome, Tiffany Bergo

Effects of vestibular stimulation, when presented prior to language therapy, on expressive language abilities in pre-school children with Down syndrome were examined. Two subjects participated; one subject completed the study. Vestibular stimulation was therapist- and child-initiated and provided by various types of equipment. The subject's expressive language use (verbal and non-verbal) was videotaped during fifteen minutes of language therapy across a six-week, eleven-session period.

Results of this study indicate that expressive language use increased or improved across many parameters during the vestibular stimulation phase of the study. The subject displayed greatest gains in the use of multiple word utterances and …

Peripheral neuropathy is an important complication of diabetes mellitus. However, neuropathological changes in diabetic neuropathy with axonal degeneration or segmental demyelination are still unclear. By using a newly developing electrodiagnostic method of single fiber electromyography (SFEMG) with a computer-aided program, mean jitter value and muscle fiber density were measured in extensor digitorum communis (EDC) and anterior tibial (AT) muscles from 36 diabetic patients with neuropathy (group 1) and 12 patients without neuropathy (group Ⅱ) proved by nerve conduction study. One hundred and fifty nine patients who experienced the first episode of stroke between 15 to 45 year-old is analyzed for possible risk factors. These patients whose diagnosis of stroke were all approved by the computerized tomography, were collected from the National Taiwan University Hospital (NTUH) from 1983 to 1990. They included 79 cases of cerebral hemorrhage (48 males and 31 females) and 80 cases of cerebral infarction (47 males and 33 females). The frequency of male stroke was about 30% higher than female. To investigate the nature of exposure factors, and effectiveness of countermeasures in head injuries in Taiwanese children with head injuries consecutively admitted to Chang Gung Memorial Hospital from Jan. 1985 to Dec. 1989. Post-event massage therapy has been proposed to hasten muscle recovery and improve ensuing performance in multistage ultraendurance cycling events. Six elite male cyclists competed in two 4·d stage races of 161 kmd^·1 on a computerized race simulator over challenging terrain in a controlled laboratory environment with an average completion time of 275 ± 28 min. In a crossover experimental design, subjects received either 30 min post-event massage therapy or a 30 min inactivated diathermy treatment serving as a blind placebo after each 161 km race. This paper describes an occupational therapy intervention designed for family caregivers of persons with dementia. The intervention, based on the framework of a competence-environmental press model and the principle of collaboration, was implemented during 5 home visits. Each visit was designed to build caregiving skills through collaboration in identifying problem areas, developing and implementing environmental strategies, and modifying management approaches. A case vignette illustrates the therapeutic process and outcomes. The theoretical rationale and structure of the intervention and innovative documentation for evaluation of the theoretic process are also presented.

Cultural sensitivity is a crucial component of health care provision, particularly in psychiatric settings. As society becomes more multicultural, it is essential for occupational therapists to continue to develop cultural competence, which is defined in this paper as an awareness of, sensitivity to, and knowledge of the meaning of culture. At San Francisco General Hospital, an innovative multicultural model consisting of special focus programs is used. The key to the success of such programs is a culturally competent professional staff.

The F wave or F response results form the recurrent discharge of antidromically activated anterior horn cells. F response latency represents an impulse which travels to and from the spinal cord through the central segment of a nerve. Study of the F wave is commonly done to assess proximal conduction in motor nerves, to measure motor nerve conduction velocity over a longer pathway than in orthodromic motor studies, and to assess motor neuron excitability. The purpose of this study was to report the relationship among perceived intoxication, performance impairment, and actual blood alcohol concentration (BAC) levels. Fifteen subjects, aged 21 to 40, completed both single- and double-dose sessions of alcohol consumption_ BACs, reaction and anticipation time, and perceived intoxication data were collected during both sessions. Analysis of data showed that perceived intoxication was significantly related to performance impairment, but the actual BAC was not.
