The following study examined the presence of risk factors among hand therapists for developing carpal tunnel syndrome. A review of the literature indicated a significant relationship between the incidence of carpal tunnel syndrome and manual workers whose occupational tasks involve repetitive wrist and hand movements. Hand therapists were characterized as manual workers while possessing specialized knowledge and skill in the evaluation and treatment of hand and wrist disorders. Biomechanical analysis of various "hands on" techniques revealed repetitive wrist and hand movements including the use of force and increased risk for injury associated with carpal tunnel syndrome. SMagnetic stimulation applied to the scalp over the normal motor cortex caused short latency response of contralateral limb muscles. 12 patients with unilateral subcortical stroke for the first time were selected. They had hemiparesis and good motor recovery up to individual finger movements, although mild weakness and clumsiness of affected limbs were still complainted of. Of the twelve patients (eleven men and one woman) the mean age were 54.8±9.3 years. Lesion sites included basal ganglia, thalamus and corona radiata, with six hemorrhage and six infarction. H-Reflex is the most common method in measuring the excitability of a-motor neuron. The aim of this study is to evaluate the suppression periods on H-Reflex in young adults. In clinical application, the amplitude of H-Reflex will be decreased after repetitive stimulation by low frequency current. This inhibitory phenomenon will influence badly on the accurancy of H-Reflex measurement. Triathlon, an ultraendurance exercise, is a race involving consecutive swimming, bicycling and running which is currently enjoying a tremendous increase in popularity abroad. This study is aimed at assessing the overuse injuries in the training period and the sports injuries during the first triathlon race in Taiwan in 1992.The triathlon race composed of 1.5 Km swimming, 40 Km bike and 10 Km run which must be finished within 3 hours and 40 minutes. With 177 effective questionnaires (86.3% of the 205 race entrants), 94.3% (167 competitor) were men and 5.6% (10 competitor) were women. The average age was 32±9.1 years. …

The study was designed to determine the extent of significant pulmonary dysfunction in selected residents of chronic care facilities, to find out any relationhip between pulmonary dysfunction and general disability, and explore pulmonary treatment options.Thirty-six nursing facility residents over 80 year old, or with a diagnosis of chronic obstructive pulmonary disease or generalized neurological or neuromuscular disease, were surveyed for extent of disability and pulmonary dysfunction. By comparison with age, height, and sex norms, there were significant restrictive pulmonary syndrome, poor inspiratory muscle strength and endurance and inadequate airway secretion clearance mechanisms in all of the residents studied. According to the previous literature, leg length discrepancy within 2.54 cm will be compensated by individual; no management is indicated. Insole modification and external shoe modification will be given for those with leg length discrepancy between 1 to 2 inches. Surgery intervention will be given for those with leg length discrepacy beyong 5 cm or 10 to 15% shorter than that of the sound leg. Post-Stroke Proximal Femoral Fracture (PSPFF) is not uncommon. In the study, We had followed up 156 patients with Proximal Femoral Fracture (PFF) happened during Jul. 1987 and Dec. 1990 via OPD for 1 year to 4 and a half years. Some patients being unable to visit our clinic were followed up by phone or by the chart record. The focus was on the 28 cases with Stroke prior to the PFF.We found PSPFF is apt to happen in cases with motor status around Brunnstrom Stage III&IV. Shoulder pain was probably the most frequent complication of hemiplegia. Despite the extensive interest, there continued to be uncertainty about its etiology.In this study, 71 hemiplegic patients were regularly followed up after their cerebrovascular accidents (CVA) for 3 months (33 men, 38 women, with the mean age of 57 years), with the exclusion from another 21 patients of the previous disorders of hemiplegic shoulders, eg. fractures, dislocations, arthritis, or rotator cuff ruptures, etc. During the 3 months, a full rehabil-itation program was given, which consisted of physical therapy, occupational therapy, training in activities of daily living and speech therapy. The cooperation of Taichung Hsien Assiciation for Retarded Children and Provincial Feng-Yuan Hospital to set up the Day Center of Children Development began since Apr. 4, 1992. This center accept children before school age with psychomotor retardation or cerebral palsy. The main duty was to educate and rehabilitate these children during the day time.The evaluation one year later showed marked improvement in several aspects, including gross motor, fine motor, psychosocial development and self daily care. The pathogenesis and clinical presentation of GI problems in SCI patients are different from that of the general populations. Spinal cord tumors, sometimes without spinal symptoms, occasionally cause signs and symptoms of intracranial disease in the form of papilledema and/or hydrocephalus.The association of hydrocephalus with benign extramedullary spinal cord tumors is particularly rare. The incidence was only 1% to 3%. So, in the diagnosis of intraspinal tumor, the associated presentations of intracranial hypertension were as important as hydrocephalus. Spinal epidural abscess (SEA) is a rare infectious disorder often subject to a delayed diagnosis. In two United States hospitals, the incidence was between 0.2 and 1.2 per 10000 admissions. There are still no reports about the incidence of SEA in Taiwan. In this report, we present a 57-year-old diabetic woman who suffered from severe lower back pain for one week prior to admission. Neither physical infectious signs nor obvious neutrophilia was apparent.The responses to electrotherapy and nonsteroidal anti-inflammatory drugs were poor. Sacral perineurial cyst is an uncommon entity. It can be the cause of a sciatica or cauda equina syndrome. Reportd here is a case of multiple lumbosacral perineurial cysts with cauda equina syndrome, who experienced remarkable improvement of symptoms after each course of pelvic traction which was initially prescribed under the impression of lumbar spondylosis. Pelvic traction may probably serve as a palliative treatment temporarily for some patients with only mild symptoms or patients whose general condition is not suitable for surgery.

A patient with headache and nuchalgia for three years was initially diagnosed as psychogenic origin. Spinal cord compression syndrome was unrecognized until motor evoked potential study showing abnormal findings. Excision of a nearly 3x2x2 cm dumbbell extra-and intradural tumor, which arose from the right C2 root, was performed after MRI documentation. Histologic diagnosis was neurilemmoma. Motor evoked potential study is useful in spinal cord diagnostics. The particular value of complete history-taking and exact neurological examination should not be underestimated.

A 32 year-old male patient fell down from 4th floor and T12 paraplegia developped. Three months after injury, right thigh swelling and pain were noted. Range of motion of right hip joint was also limited. X-ray of right hip joint didn't reveal any abnormal finding. But Tc 99m-methylene diphosphonate (MDP) three phase bone scan showed estra-osseous accumulation anterior-medial to greater trochancter of right femur bone. The serum alkaline phosphatase was also increased. Indomethacin 25 mg Tid was given and passive range of motion exercises were performed. Neuronal dropout accompanied with axonal degeneration may occur below the level of cord lesion after spinal cord injury (SCI). By using a sensitive electrodiagnostic method of stimulated single fiber electromyography (SFEMG), it is possible to assess the functional integrity of neuromuscular transmission in paralytic muscles of SCI. Neuromuscular jitter and fiber density were measured in 21 anterior tibial muscles of 18 patients with SCI and 11 muscles in 11 normal controls. The results showed an increased mean jitter value but without increased fiber density in SCI patients. There is a positive correlation between the increased jitter and the disease duration …

The purpose of this study was determine the influence of the biofeed-back index to the muscle tension of the shoulder in Cello student. Data analysis of 14 students included before performance, during performance and after performance. The result revealed biofeed-back index higher in left shoulder then right shoulder after performance. From 1989 to 1991, five cases of humeral shaft fractures and one case of ulnar shaft pathological fracture were treated with the low temperature thermoplastic functional fracture brace, while three cases of lumbar spine fractures were treated with a custom-made spinal brace molded by low temperature thermoplastics.All patients with humeral shaft fractures achieved bone healing within 8 to 12 weeks. There was varus angulation averaging 7°and anterior angulation averaging 8°present. The majority of patients were found to have minimal loss of range of motion in the shoulder. Traumatic common carotid artery (CCA) injury with subsequent cerebral infarction is a rare cause among stroke patients. we report a male aged 33 years who suffered from weakness of left limbs about 12 hours after blunt neck trauma. About 48 hours later, computerized tomography (CT) of the head revealed a low-density lesion over the territory of the right middle cerebral artery (MCA). Carotid Doppler ultrasound examination showed two thrombi within the right proximal CCA. Acquisition of diagnostic reasoning skills is difficult for students, however, because of paradoxical developmental limitations faced during transition from classroom to clinical learning environments. Educators in pre-professional occupational therapy curriculums are searching for educational methods to prepare students for diagnostic reasoning tasks inherent in practice.

This experimental study compared a computer-simulated diagnostic reasoning activity with a more traditional case study reviewing activity in terms of effect on accuracy of problem identification among occupational therapy students. Qi Gong is a special type of Chinese traditional conditioning exercise, consisting of three major components: dynamic sitting posture, meditation, and diaphragmatic exercise.In order to evaluate the differences of the cardiorespiratory function between Qi Gong and Tai Chi Chuan practitioners, there are three groups, each group including 10 male middle-aged subjects, with matched age and body size: (1) Qi Gong practitioners for at least one year; (2) Tai Chi chuan players for average 5.8 rears; (3) sedentary subjects.
