In this introduction to the Division for Early Childhood (DEC) Recommended Practices, we will briefly explain what we mean by the environment and what it means for young children with disabilities and their families. This will provide a framework for conceptualizing the inter-relatedness of environmental settings and features. We will also define types of environments in which children develop and grow and outline key ideas to consider when implementing the Environment recommended practices. This chapter will use vignettes to illustrate important themes and show examples of how practitioners can best use the Environment recommended practices.

Attention Intervention (JAI) conducted by graduate researcher, parent, and caregiver, would change the use of joint attention (JA) by a three-year-old suspected of having Autism Spectrum Disorder (ASD). The participant was a three-year-old child suspected of having ASD. Intervention was conducted twice per week for 30-45 minute duration by the student graduate researcher at a university clinic under the supervisor of Dr. Bonnie Berg, CCCSLP. The parent and child's caregiver were trained in JAI and implemented the intervention daily in the home environment. Alabama has piloted the Gaining Access to Employment project, a collaborative effort between the state’s Department of Mental Health/ Developmental Disabilities (MH/DD) and its Department of Vocational Rehabilitation (VR). Through this project, known as Project GATE, the two agencies work together to help local service providers use funds to support integrated employment opportunities. MH/DD and VR have a long history of partnering, including joint efforts on a supported employment workgroup, due to the strong relationships between colleagues at each department.

In 2010, when the New Hampshire Bureau of Developmental Services (BDS) received grant funds to strengthen multisystem service delivery, its administrators partnered with area agencies; community rehabilitation providers, or CRPs (employment providers); and other stakeholders to improve and streamline the process of collecting employment data. Originally a multi-system process, BDS continued the data-collection effort when other systems withdrew. BDS’s new data-collection system allows unprecedented access to clear data displays, as well as the ability to run a variety of detailed reports to guide the statewide promotion of integrated employment.

In the mid-1980s, the state of Washington was awarded a five-year federal systems change grant to kick-start their supported employment efforts via the Washington State Employment Initiative. Funding from this grant was used to develop training on best practices and to generate high-quality integrated employment supports among agencies. At the end of this five-year period, with state funding and support from state leadership, the Washington State Employment Initiative re-formed as WISE, an independent training and technical assistance (TA) organization. WISE now contracts with the Washington Developmental Disabilities Administration to provide ongoing, high-quality training and TA across the state and to …

Adherence is considered a prerequisite for the success of exercise programs for musculoskeletal disorders. The negative effects of non-adherence to exercise recommendations impact the cost of care, and also treatment effectiveness, treatment duration, the therapeutic relationship, waiting times, the efficiency of personnel and use of equipment. Adherence to therapeutic exercise intervention is a multifaceted problem.
The World Health Organization (WHO) established the multidimensional adherence model (MAM). The MAM describes five interactive dimensions (socioeconomic, healthcare team and system, condition-related, therapy-related, and patient-related factors) that have an effect on patient adherence.

Breast cancer is the most common cancer diagnosis of women, with an estimated 232,670 new cases in 2014. With 89.2% of breast cancer patients surviving five years or longer, studies are needed to investigate the long-term impact of breast cancer on women and families (National Cancer Institute, 2014).
The purpose of this study was to examine, using a mixed methods approach, the impact of chemotherapy on mothering occupations for patients diagnosed with breast cancer. Decreased adherence to therapy protocols after upper extremity injury has been correlated with increased risk of re-injury and decreased general therapy outcomes. Occupation-based therapy has been shown to increase adherence to therapy protocol. To date, there have been several occupation-based protocols in upper-extremity musculoskeletal rehabilitation, but never one that addresses occupation across the lifespan and attempts to develop a tool to assist therapist in developing occupation based protocols specific to the developmental group.

Objective
This study was undertaken to better understand the high variability in response seen when treating human subjects with restorative therapies poststroke. Preclinical studies suggest that neural function, neural injury, and clinical status each influence treatment gains; therefore, the current study hypothesized that a multivariate approach incorporating these 3 measures would have the greatest predictive value.
Methods
Patients 3 to 6 months poststroke underwent a battery of assessments before receiving 3 weeks of standardized upper extremity robotic therapy. Background: Individuals post hip fracture decrease force on the involved limb during sit to stand tasks, creating an asymmetry in vertical ground reaction force. Joint specific differences that underlie asymmetry of the vertical ground reaction force are unknown. The purpose of this study was to compare differences in vertical ground reaction force variables and joint kinetics at the hip and knee in participants post-hip fracture, who were recently discharged from home care physical therapy to controls.

The Functional Movement Screen (FMS) and Y-Balance Test (YBT) are functional measurement tools utilized for injury prevention, performance predictability, and as return to sport criteria. The FMS is comprised of 7 standardized tests of movement patterns that are rated from 0-3 and includes: Deep Squat, Hurdle Step, In-Line Lunge, Shoulder Mobility, Active Straight Leg Raise, Trunk Stability Push-Up, and Rotary Stability. —Literature shows that some health outcomes (e.g., eating, breathing, and speaking) are directly related to posture. Evidence of outcomes mediated by wheelchair seated posture is limited to interface pressure, physical function, and wheelchair skills and safety. This study’s purpose was to develop and validate a rapid, low-burden, paper-pencil assessment of wheelchair seated posture for research use and to test feasibility of its use with a sample of older adults. Human locomotion is phenomenon that is extraordinarily complex. It is evident that a complete description of locomotion involves consideration of kinematics, kinetics, and muscle activity of the extremities in all of their various movements. Race walking (RW) is a form of upright locomotion that differs from normal walking and running by its form dictated by the International Amateur Athletics Federation (IAAF). Spinal manipulative therapy (SMT) is a therapeutic procedure employed by various healthcare practitioners for alleviating acute and chronic musculoskeletal complaints. This form of treatment is also delivered to enhance the performance and augment the rehabilitation of athletes. However, despite research findings alleging the strength-modulating effects of SMT alongside numerous professional athletes’ positive anecdotal claims concerning its results, the physiological processes to explain its effects remain largely unexplained. The purpose of the current study was to evaluate the efficacy of a cognitive training program among those with moderate cognitive impairment. A total of 23 individuals participated in the study and were randomly assigned to a wait-list control group or a cognitive training program that consisted of 24 cognitive classes for a total of 12 weeks. The cognitive training classes aimed to activate the six primary cognitive domains impacted with dementia, reaction time, attention, memory, language, visual-spatial skills, and executive functioning. All participants were evaluated with a battery of neurocognitive assessments pre-and post-treatment. The findings tentatively support the use …

A key determinant of geriatric frailty is sarcopenia, the age-associated loss of skeletal muscle mass and strength. Although the etiology of sarcopenia is unknown, the correlation during aging between the loss of activity of satellite cells, which are endogenous muscle stem cells, and impaired muscle regenerative capacity has led to the hypothesis that the loss of satellite cell activity is also a cause of sarcopenia. We tested this hypothesis in male sedentary mice by experimentally depleting satellite cells in young adult animals to a degree sufficient to impair regeneration throughout the rest of their lives. Previous research has proposed that the somatosensory feedback generated during exercise is a key component in regards to the mechanism underlying the therapeutic effects of exercise on the motor symptoms of Parkinson’s disease (PD). This thesis aimed to further examine the contributions of different forms of somatosensory feedback during exercise in PD in order to understand the mechanism for symptom improvements that certain exercise studies report.

Introduction: In Canadian society, there is a growing prevalence of older adults and one of the main problems facing this generation today is the risk of falling. Tai Chi (TC) is a martial art that has demonstrated improvements in balance control. A literature review of 40 evidence-based journal articles addressed adult learning principles; guidelines for facilitating learning; types of, advantages, and disadvantages of using gaming in higher education; and preparation methods for high-stakes examinations. In 2009, Cherry, Kitchens, Nicholson, Soden, Tomkiewicz, Kedia & Shah found that 32.8% of their respondents did not know about the demand for culturally competent occupational therapy (OT) services and cultural factors that could influence a client's occupational performance. Background: In occupational therapy education, it is unclear what educational methods best facilitate the development of critical thinking. Therefore, this pilot study explored whether Level I fieldwork coupled with reflective learning opportunities impacts the development of critical thinking skills.
Methods: The researchers employed a pre and post-test design which measured both self-perception of critical thinking and actual performance of critical thinking. Specifically, the Health Science Reasoning Test (HSRT) and a reflective writing assignment graded with a rubric were utilized to measure actual performance. The purpose of the scholarly project is to incorporate a therapy/service dog within the occupational therapy department at the University of North Dakota (UND). The therapy/service dog will be utilized as a support system for student’s and faculty’s health and well-being and as an educational tool for enhancing student learning. The authors completed the process of implementation by progressing through the hierarchical process of UND gatekeepers. Through the development of a legality and safety protocol, loan agreement, and purpose proposal, the authors obtained official approval to implement a therapy/service dog in the UND Occupational Therapy program.

Intergenerational programs are known to bring older adults together with children in order to promote social inclusion, and increased self-confidence and motivation for both age groups (Hatton-Yeo, 2010). Programs of this nature have proven to influence greater academic outcomes and school performance, decreased criminal behavior and greater personal adaptability in younger populations (Hatton-Yeo, 2010). The purpose of this scholarly project was to provide occupational therapists with a practical guide for implementing occupation-based leisure interventions for individuals with spinal cord injury (SCI). In recent years, the occupational therapy (OT) profession has seen a shift from a biomechanically driven paradigm back to the occupation-centered practice from which OT is rooted. The purpose of this scholarly project was to develop an interdisciplinary after-school program for fifth graders living in low socioeconomic neighborhoods. This program entitled Let’s Move, Cook, and Learn With Friends! is intended to reduce obesity through promoting healthy habits in children by increasing physical activity, consumption of healthy food, and positive interactions with peers. Childhood obesity is a worldwide epidemic that can result in lifelong health problems if not treated proactively (Vizcaino et al., 2008). Therefore, the goal of this scholarly project is to raise their knowledge about obesity and decrease the risk factors for childhood obesity.
