The prevention and cure of malaria is difficult as it thrives under poor socio- and pharmacoeconomic conditions in third world, tropical and subtropical regions. It is a parasitic disease spread through the bite of an infected Anopheles mosquito. In humans, malaria can be caused by several Plasmodium species that include: Plasmodium falciparum, Plasmodium vivax, Plasmodium ovale, Plasmodium malariae and Plasmodium knowlesi. The natural product artemisinin is a sesquiterpene lactone with an endoperoxide bridge isolated from the Chinese annual herb qinghao (Artemisia annua L or sweet wormwood). Lipid based systems and topical ocular inserts of various drugs were developed for the intervention/treatment of posterior segment ocular complications. Indomethacin (IN) was developed into solid lipid nanoparticles (SLNs) and Nanostructured lipid carriers (NLCs). Effect of surface functionalization using chitosan (CS) on lipid nanocarriers was tested to evaluate corneal penetration. Surface modification of SLNs with CS increased ocular penetration of IN. NLCs maintained significantly higher IN concentrations in all ocular tissues tested, compared to the other formulations evaluated in vivo. Objectives: To examine adherence to ADHD stimulant medications and determined the condition-related, therapy-related, patient-related, health-system related, and social and economic related predictors of medication nonadherence in children/adolescents (4 – 18 years) with ADHD enrolled in Mississippi Medicaid Program. Methods: This was a retrospective cohort study of the Mississippi Medicaid administrative claims data. Data was analyzed from March 01, 2014 to February 29, 2016. Adherence was calculated as the proportion of days covered (PDC). Glycosaminoglycans (GAGs) are complex linear chain carbohydrate molecules found on virtually all animal cell surfaces. Owing to their negatively charged nature, GAGs interact with a number of different proteins. Thus, although they have great potential as therapeutic agents, their apparent promiscuous interactions increase their side effect risk. GAG mimetics, including GAG oligosaccharides and non-saccharide GAG mimetics (NSGMs) are viable approaches to address this. This work discusses sulfated benzofuran thrombin inhibitors with submaximal protease inhibition, sulfated diflavonoid inhibitors of plasmin and GAG oligosaccharides with selectivity for human neutrophil elastase (HNE).

The purpose of this study was to evaluate the use of semi-structured interviews as an outcome measure in a clinical trial for children with fragile X syndrome (FXS) ages 2-6. This qualitative approach was used to analyze twenty-five interviews of parents with children in a double-blind medication trial - sertraline or placebo. The aim was to assess occupational improvements in their child that may not have been detected with the use quantitative outcome measures alone. Results showed greater improvements in the sertraline group in areas of behavior, social participation, sensory-related behaviors, receptive language, education, family impact and therapeutic strategies. In December 2016, a panel of experts in microbiology, nutrition and clinical research was convened by the International Scientific Association for Probiotics and Prebiotics to review the definition and scope of prebiotics. Consistent with the original embodiment of prebiotics, but aware of the latest scientific and clinical developments, the panel updated the definition of a prebiotic: a substrate that is selectively utilized by host microorganisms conferring a health benefit. This definition expands the concept of prebiotics to possibly include non-carbohydrate substances, applications to body sites other than the gastrointestinal tract, and diverse categories other than food. วัตถุประสงค์: เปรียบเทียบอุบัติการณ์การเกิดแผลติดเชื้อในผู้ที่ได้รับยา amoxicillin และ amoxicillin-clavulanic acid เพื่อป้องการการติดเชื้อจากสุนัขและแมวกัด/ข่วน และศึกษาปัจจัยที่เกี่ยวข้องกับการเกิดแผลติดเชื้อ
วิธีการดำเนินการวิจัย: การวิจัยเชิงทดลองแท้จริงและใช้วิธีการปกปิดแบบทางเดียว ศึกษาในผู้ป่วยที่เกิดแผลจากสุนัขหรือแมวกัด/ข่วนที่มีระดับการสัมผัสโรคพิษสุนัขบ้าระดับ 3 และเข้ารับการรักษาภายใน 24 ชั่วโมง ณ แผนกฉุกเฉิน โรงพยาบาลบ้านบึง จังหวัดชลบุรี ระหว่างเดือนกุมภาพันธ์ พ.ศ. 2561 ถึงเดือนพฤษภาคม พ.ศ. 2561 โดยการสุ่มแบบบล็อกให้ผู้ป่วยได้รับยา amoxicillin หรือยา amoxicillin-clavulanic acid เป็นระยะเวลา 5 วัน และประเมินผลการเกิดแผลติดเชื้อในวันที่ 3 และ 7 หลังจากได้รับยาปฏิชีวนะ
ผลการวิจัย: ผู้ป่วยทั้งหมด 236 คน เข้ารับบริการที่โรงพยาบาลด้วยสาเหตุสุนัขกัด แมวกัดและแมวข่วนร้อยละ 75, 18.2 และ 6.8 ตามลำดับ อุบัติการณ์การเกิดแผลติดเชื้อระหว่างกลุ่มที่ได้รับยา amoxicillin (ร้อยละ 1.7) กับยา amoxicillin-clavulanic acid (ร้อยละ 4.2) ไม่มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p=0.447) ผู้ป่วยที่ได้รับยา amoxicillin-clavulanic acid เกิดอาการไม่พึงประสงค์มากกว่ากลุ่มที่ได้รับยา amoxicillin ร้อยละ 17.5 และ 10.3 ตามลำดับ (p=0.113) แผลบริเวณเท้าจากสุนัขกัดและแผลเจาะจากแมวกัดมีความสัมพันธ์กับการติดเชื้ออย่างมีนัยสำคัญทางสถิติ (p=0.048 และ p=0.029 ตามลำดับ)
สรุปผลการวิจัย : ยา amoxicillin สามารถใช้เป็นยาปฏิชีวนะเพื่อป้องกันการเกิดแผลติดเชื้อจากสุนัขและแมวกัดได้ โดยไม่มีความแตกต่างของอุบัติการณ์การเกิดแผลติดเชื้อระหว่างกลุ่มที่ได้รับยา amoxicillin กับยา amoxicillin-clavulanic acid โดยการเกิดแผลบริเวณเท้าจากสุนัขกัดและการมีแผลเจาะจากแมวกัดอาจเพิ่มความเสี่ยงต่อการเกิดแผลติดเชื้อ

Silver nanoparticles (AgNPs) and nanoparticles in the form of silver-metal alloy, such as silver-copper (AgCuNPs) and silver-tin (AgSnNPs) have been applied in various industries and medical applications. To assess effects of these nanoparticles on human immune system, this study aimed to assess toxicity of AgNPs, AgCuNPs and AgSnNPs on human monocytes, THP-1 cells, and THP-1 differentiated macrophage cells. The nanoparticles were characterized and assessed for cytotoxicity and generation of intracellular ROS. Effects of the nanoparticles on the release of cytokines (TNF-α, IL-1β and IL-8) were also assessed. Introduction: High dose insulin (HDI) therapy, defined as > 0.5 units/kg/hour, has been postulated to improve hemodynamics in both calcium channel blocker (CCB) and beta-blocker overdose. Proposed benefits of HDI include coronary and systemic vasodilation due to enhancement of nitric oxide synthase activity leading to improved contractility and decreased systemic vascular resistance, respectively. HDI also directly improves cardiac contractility by increasing glucose uptake and improving calcium handling within cardiac cells. We present a case of a patient with amlodipine overdose and her response to HDI therapy.

Background and purpose: It is still inconclusive which method of iontophoresis delivers the most medication deepest through the skin, and therefore most effective in treating lateral epicondylitis. The purpose of this systematic review is to analyze the efficacy of treatments for lateral epicondylitis using iontophoresis.
Method: The review included articles from peer-reviewed journals with sufficient data related to the purpose and focus of the study. Inclusion criteria included randomized control trials, cohort studies, case studies, systematic reviews, meta-analyses, and pilot studies published since 2000.

The misassembly of soluble proteins into toxic aggregates, including amyloid fibrils, underlies a large number of human degenerative diseases. Cardiac amyloidosis, which is most commonly, caused by aggregation of Immunoglobulin (Ig) light chains or transthyretin (TTR) in the cardiac muscle, represent an important and often underdiagnosed cause of heart failure. TTR-mediated amyloid cardiomyopathies are chronic and progressive conditions that lead to arrhythmias, biventricular heart failure, and death. The synthesis and evaluation of structure-activity relationships of saturated nitrogen heterocycles is the focal point of various pharmaceutical companies thanks to the high biological activity of previously isolated azacycles. Here, we describe an operationally simple and highly efficient approach to macrocyclic lactams bearing vicinal stereocenters and a challenging cycloalkyne motif. The outcomes are achieved through a novel [4 + 2] cycloaddition reaction between an N-iodoarylated-1,3-azadiene and cyclic anhydrides, followed by interception of the cycloadducts in cross-coupling manifolds (e.g., Sonogashira coupling) and concomitant lithiation-cyclization of the tethered alkyne. The Electrospray (ES) technique is a promising particle generation method for drug delivery due to its capabilities of producing monodisperse PLGA composite particles with unique configurations and high drug encapsulation efficiency. Venous thromboembolism (VTE) which comprises pulmonary embolism (PE) and deep vein thrombosis (DVT) has been a public health concern for several years. VTE accounts for an estimated 60,000 to 100,000 deaths per year in the United States. Specifically, patients who present with an acute PE, it has been estimated 25% of these patients have sudden death as the first symptom. Accurate energetic modeling of large molecular systems is always desired by chemists. For example, ligand-protein binding simulations and enzymatic catalysis studies all involve with a small energy difference. The energetic accuracy depends largely on a proper handling of electronic correlations. Molecular mechanics (MM) methods deliver a parameterized Newtonian treatment to these problems. They show great capability in handling large calculations but give only qualitatively good results. Quantum mechanics (QM) methods solve Schrödinger equations and exhibit much better energy accuracy, though the computational cost can be prohibitive if directly applied to very large systems.

Cancer has been a pervasive and deadly problem for many years. No treatments have been developed that effectively destroy cancer cells while also keeping healthy cells safe. In this work, the knob-socket construct is used to analyze two systems involved in cancer pathways, the PDZ domain and the Bcl-BH3 complex. Application of the knob-socket model in mapping the packing surface topology (PST) allows a direct analysis of the residue groups important for peptide specificity and affinity in both of these systems. PDZ domains are regulatory proteins that bind the C-terminus of peptides involved in the signaling pathway of cancer progression. …

Transthyretin (TTR) amyloidosis is a progressive, fatal disease in which deposition of amyloid derived from either mutant or wild-type TTR causes severe organ damage and dysfunction. TTR cardiomyopathy is an infiltrative, restrictive cardiomyopathy characterized by progressive left and right heart failure. Familial amyloid cardiomyopathy (FAC) is driven by pathogenic point mutations in the TTR gene that destabilize the TTR tetramer, prompting its dissociation into dimers and monomers, with subsequent misfolding, aggregation and deposition of toxic TTR amyloid aggregates in the myocardium. In previous experiments, the steroidal saponin, dioscin, which was extracted from the roots of the wild yam (Dioscorea villosa), was observed to have potent anti-proliferative effects on the human breast cancer cell lines MCF-7 and MDA-MB-231. Dioscin was also able to activate the transcription of tumor suppressor genes GATA3 and CDH1 in these cell lines. We hypothesize that the anti-proliferative effect of dioscin on human breast cancer cells could be mediated through the epigenome.
