An intranasal formulation of midazolam, Nayzilam, has been FDA-approved to treat intermittent, stereotypic episodes of frequent seizure activity. Nayzilam is easy to administer and can quickly treat seizures that occur outside of the hospital. The intra-nasal route of administration allows non-medical personal to administer the drug which makes it more accessible and user-friendly in the event of a seizure. Many studies have indicated quick cessation of seizures with Nayzilam compared to rectal diazepam, which has been the standard of care treatment. Phenothiazines, a diverse class of drugs, can be used to treat multiple mental health and physical conditions. Phenothiazines have been used for decades to treat mental illnesses, including schizophrenia, mania in bipolar disorder, and psychosis. Additionally, these drugs offer relief for physical illnesses, including migraines, hiccups, nausea, and vomiting in both adults and children. Further research is needed to prove the efficacy of phenothiazines in treating physical symptoms. Phenothiazines are dopaminergic antagonists that inhibit D2 receptors with varying potency. Alzheimer's disease (AD) is the most common form of dementia affecting millions of individuals, including family members who often take on the role of caregivers. This debilitating disease reportedly consumes 8% of the total United States healthcare expenditure, with medical and nursing outlays accounting for an estimated $290 billion. Cholinesterase inhibitors and N-methyl-D-aspartate receptor antagonists have historically been the most widely used pharmacologic therapies for patients with AD; however, these drugs are not curative. Migraine is a common form of primary headache, affecting up to 1 in every 6 Americans. The pathophysiology is an intricate interplay of genetic factors and environmental influence and is still being elucidated in ongoing studies. The trigeminovascular system is now known to have a significant role in the initiation of migraines, including the release of pain mediators such as CGRP and substance P. Traditional treatment of migraine is usually divided into acute and preventive treatment. Plasmodium falciparum causes the most lethal form of malaria. Peroxide antimalarials based on artemisinin underpin the frontline treatments for malaria, but artemisinin resistance is rapidly spreading. Synthetic peroxide antimalarials, known as ozonides, are in clinical development and offer a potential alternative. Here, we used chemoproteomics to investigate the protein alkylation targets of artemisinin and ozonide probes, including an analogue of the ozonide clinical candidate, artefenomel. We greatly expanded the list of proteins alkylated by peroxide antimalarials and identified significant enrichment of redox-related proteins for both artemisinins and ozonides. The Thai pharmaceutical industry aims to achieve self-reliance in vaccines, drugs, and biologics as per the National Strategic Master Plan. Challenges include low research capacity and higher drug imports compared to domestic production. To promote self-reliance, more research on incrementally modified drugs (IMDs) is essential. This study analyzes the financial feasibility of developing IMD dosage forms for policy and investment decisions. A mixed-method approach, including type selection, investment models, cost structures, and financial analysis, was employed. Results favor sustained release, oro-dispersible tablet, and nasal spray formulations. IMD research and development took 7 to 13 years due to formulation complexity and …

Immune checkpoint antibodies disrupt the binding of receptor-ligand pairs which regulate immune response in cancer treatment. PD-1/PD-L1 pathway has become one target of the cancer immunotherapy approaches. Atezolizumab, the first FDA-approved antibody to PD-L1 in the treatment of metastatic urothelial, non-small cell lung, small cell lung, and hepatocellular cancers, is expressed in Chinese Hamster Ovary (CHO) cell lines with several limitations, i.e., high costs of production, limited-capacity yields, and pathogen risks. Objective: This study evaluated the budget impact of introducing upadacitinib for patients with uncontrolled moderate-to-severe atopic dermatitis (AD) from a United States (U.S.) private payer perspective.
Methods: The model estimated costs before and after the adoption of upadacitinib for a hypothetical one million covered lives over 3 years. The model included immunosuppressant agents and dupilumab. Market uptake was assumed to be 2% per year. Treatments incur a cost for drug acquisition, and the costs associated with drug administration, laboratory testing, and clinic visits. Background: Hypertension is highly prevalent in the United States, affecting nearly half of all adults (43%). Studies have shown that pharmacy-physician collaborative care models (PPCCM) for hypertension management significantly improve blood pressure (BP) control rates and provide consistent control of BP. Time in target range (TTR) for systolic BP is a novel measure of BP control consistency that is independently associated with decreased cardiovascular (CV) risk. There is no evidence observed improvement in TTR for systolic BP with PPCCM is cost effective.

The assessment of patients presenting with chest pain or symptoms indicative of cardiac ischemia remains a diagnostic challenge. Many types of research have focused on the search for ideal biological markers for the rapid detection of cardiac cell injuries. Markers of inflammation and oxidative stress are the way forward. At present, the biomarker most widely used for diagnosing acute coronary syndrome is cardiac troponin though it has some limitations. To fill the gap in existing literature related to immune-related adverse events (irAEs) induced by immune checkpoint inhibitors (ICIs) in non-small cell lung cancer (NSCLC), this dissertation focused on: 1) assessing the incidence, spectrum and risk factors for the development of irAEs; 2) assessing the clinical impact of irAE occurrence on survival and treatment discontinuation; 3) analyzing and estimating the incremental economic burden of irAEs among an older population in the United States (US). The Techniques of Studying and Other Variables Impact on Initial Drug Card Exam Pass Rates is a study performed on the Top 200 Drug Card Exams presented to professional pharmacy students at South Dakota State University. The study utilized Fisher's exact test with a 0.05 significance value to find statistical significance in a variety of categories relating to different variables that were suspected to impact the pass rate of the exams. Poor customer complaint-handling strategies can negatively affect customer loyalty. Leaders in the pharmaceutical industry who fail to improve complaint-handling strategies can experience a decrease in profitability. Grounded in the double-loop learning theory, the purpose of this qualitative single-case study was to explore effective strategies business leaders use to manage customer complaints in a pharmaceutical company. Participants included six business leaders in Northwest New Jersey, United States, who successfully used strategies to improve customer complaint handling in the pharmaceutical industry. Data were collected from semistructured interviews and company documents relevant to customer complaint handling and were analyzed using thematic data analysis. …

Clozapine และ Olanzapine เป็นยาต้านโรคจิตกลุ่มใหม่สองชนิดที่มีผลต่อการเพิ่มขึ้นของน้ำหนักตัวมากที่สุดเมื่อเปรียบเทียบกับยาต้านโรคจิตชนิดอื่น ๆ การศึกษานี้เป็นการศึกษาเชิงวิเคราะห์แบบเก็บข้อมูลย้อนหลัง เพื่อเปรียบเทียบการเปลี่ยนแปลงน้ำหนักตัวในผู้ป่วยจิตเวชชาวไทยระหว่างกลุ่มที่ได้รับ Clozapine กับ Olanzapine เป็นระยะเวลา 12 เดือน ณ โรงพยาบาลศรีธัญญา ตั้งแต่เดือนมกราคม พ.ศ. 2555 ถึง เดือนธันวาคม พ.ศ. 2564 ติดตามน้ำหนักตัวและค่าดัชนีมวลกายทั้งหมด 5 ครั้ง คือ ช่วงเดือนที่ 0, 3, 6, 9 และ 12 วิเคราะห์ข้อมูลด้วยสถิติ Two Way ANCOVA และ Repeated measure ANCOVA ผลการวิจัย พบกลุ่มตัวอย่างทั้งหมดในการศึกษา จำนวน 381 ราย (Clozapine จำนวน 205 ราย และ Olanzapine จำนวน 176 ราย) ณ เดือนที่ 12 เดือนกลุ่ม Olanzapine มีค่าเฉลี่ยของน้ำหนักตัว (71.87 ± 0.55 กิโลกรัม) สูงกว่ากลุ่ม Clozapine (69.22 ± 0.52 กิโลกรัม) อย่างมีนัยสำคัญ (P=0.001) และในทุกช่วงของการติดตาม กลุ่ม Olanzapine มีน้ำหนักตัวเฉลี่ยสูงกว่ากลุ่ม Clozapine รวมถึงพบแนวโน้มของน้ำหนักตัวเพิ่มสูงขึ้นหลังการใช้ยาทั้งสองตัวตามระยะเวลาที่ผ่านไป นอกจากนี้พบ Plateau ของน้ำหนักตัวจากการใช้ Clozapine และ Olanzapine ในเดือนที่ 6 และ 9 ของการรักษาตามลำดับ โดยสรุป ผู้ป่วยจิตเวชชาวไทยที่ได้รับ Clozapine หรือ Olanzapine ต่อเนื่องเป็นระยะเวลา 12 เดือนจะมีการเปลี่ยนแปลงของน้ำหนักตัวเพิ่มสูงขึ้นตามระยะเวลาที่ผ่านไป โดย Olanzapine …

This thesis deals with mental health, with a focus on Black women. Historically, Black women are often so compromised, being constant caregivers and helping everyone else, that they forget to help themselves, not having the time and financial means to do so. If we go back in the time of slavery, many Black women were taking care of slave owners' children and suckling the white women’s babies instead of their own. วัตถุประสงค์: ประเมินผลคุณภาพชีวิตด้านการใช้ยา คุณภาพชีวิตด้านสุขภาพ จำนวนปัญหาจากการใช้ยาและผลลัพธ์ทางคลินิกของการแก้ไขปัญหาจากการใช้ยา รวมถึงสัดส่วนผู้ป่วยที่ได้รับการแก้ไขปัญหาจากการใช้ยาในผู้ป่วยโรคไตเรื้อรังที่ไม่ได้บำบัดทดแทนไตที่ได้รับการบริบาลทางเภสัชกรรมโดยใช้แบบสอบถาม PROMPT เปรียบเทียบกับกลุ่มที่ได้รับการบริการตามมาตรฐานปกติของโรงพยาบาล วิธีการวิจัย: การวิจัยเชิงทดลองหลายศูนย์แบบสุ่มชนิดมีกลุ่มควบคุมแบบปกปิดทางเดียว ศึกษาในผู้ป่วยโรคไตเรื้อรังที่ไม่ได้บำบัดทดแทนไตที่เข้ารับบริการในโรงพยาบาลสองพี่น้องและโรงพยาบาลท่าใหม่ จังหวัดจันทบุรี ระหว่างเดือนพฤศจิกายน 2564 ถึงเดือนสิงหาคม 2565 จำนวนทั้งสิ้น 292 คน โดยจัดสรรผู้ป่วยเป็น 2 กลุ่ม คือ กลุ่มศึกษา (กลุ่มที่ได้รับการบริบาลทางเภสัชกรรมโดยใช้แบบสอบถาม PROMPT และกลุ่มควบคุม (กลุ่มที่ได้รับการบริการตามมาตรฐานปกติของโรงพยาบาล) ประเมินผลคุณภาพชีวิตด้านการใช้ยาและคุณภาพชีวิตด้านสุขภาพของผู้ป่วยด้วยแบบสอบถาม PROMPT และแบบสอบถาม KDQOL ตามลำดับ ผลการวิจัย: กลุ่มศึกษามีผลคะแนนคุณภาพชีวิตด้านการใช้ยาดีขึ้นอย่างมีนัยสำคัญทางสถิติจำนวน 3 มิติจาก 8 มิติเมื่อเทียบกับกลุ่มควบคุม ได้แก่ มิติการได้รับข้อมูลยาและโรคจากบุคลากรทางการแพทย์ (p < 0.001) มิติความพึงพอใจต่อผลการใช้ยา (p = 0.003) และมิติคุณภาพชีวิตโดยรวมของการใช้ยา (p = 0.018) รวมถึงผลคะแนนรวมของแบบสอบถามคุณภาพชีวิตด้านการใช้ยา (p < 0.001) สำหรับผลคุณภาพชีวิตด้านสุขภาพระหว่างผู้ป่วยทั้งสองกลุ่มพบความแตกต่างอย่างมีนัยสำคัญทางสถิติจำนวน 2 มิติจาก 10 มิติ ได้แก่ มิติความยากลำบากจากโรคไต (p = 0.045) และการให้คะแนนสุขภาพโดยรวม (p = 0.011) นอกจากนี้กลุ่มศึกษาค้นพบปัญหาจากการใช้ยามากกว่าและมีผลลัพธ์ทางคลินิกโดยรวมของการแก้ไขปัญหาจากการใช้ยาที่มีผลดีขึ้นมากกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p < 0.001 และ p = 0.013 ตามลำดับ) แต่สัดส่วนผู้ป่วยที่ได้รับการแก้ไขปัญหาจากการใช้ยาและมีผลดีขึ้นไม่พบความแตกต่างอย่างมีนัยสำคัญทางสถิติระหว่างผู้ป่วยทั้งสองกลุ่ม (p = 0.329) สรุปผลการวิจัย: การบริบาลทางเภสัชกรรมโดยใช้แบบสอบถาม PROMPT ส่งผลให้ผู้ป่วยมีคุณภาพชีวิตด้านการใช้ยาดีขึ้น ค้นพบปัญหาจากการใช้ยาได้เพิ่มมากขึ้น และผลลัพธ์ทางคลินิกโดยรวมของปัญหาจากการใช้ยาที่ได้รับการแก้ไขและมีผลดีขึ้นในผู้ป่วยโรคไตเรื้อรังที่ไม่ได้บำบัดทดแทนไตเพิ่มมากขึ้น แม้ว่าผลคุณภาพชีวิตด้านสุขภาพและสัดส่วนผู้ป่วยที่ได้รับการแก้ไขปัญหาจากการใช้ยาและมีผลดีขึ้นจะมีผลลัพธ์ที่ยังไม่ชัดเจน

The present study introduces the synergistic analgesic potential of curcumin (Cur) and metformin (Met), encapsulated within the chitosan-alginate nanoparticle (CTS/ALG-NP) delivery system. The aim is to improve the therapeutic efficacy of this combination without potentiating its side effects. The primary objectives were to gauge the therapeutic potency of Cur and Met, improve their delivery using CTS/ALG-NPs, and ascertain the possible toxicity of these nanoparticles. วัตถุประสงค์ของงานวิจัยนี้ เพื่อศึกษาผลของแรงตอกอัดต่อการเปลี่ยนแปลงรูปแบบของตัวยาสำคัญโรซูวาสแตตินแคลเซียมอสัณฐาน และพัฒนาวิธีวิเคราะห์โรซูวาสแตตินแคลเซียม อสัณฐานแบบโซลิดสเตตเชิงปริมาณ โดยใช้เทคนิครามานสเปกโทรสโกปี โดยเริ่มจากการเตรียมตัวอย่างการสอบเทียบที่อัตราส่วน 0, 25, 50, 75 และ 100 เปอร์เซนต์โดยน้ำหนัก โรซูวาสแตตินแคลเซียมอสัณฐาน นำไปตอกด้วยแรงขนาดต่ำให้กลายเป็นเม็ดยา และทำการวิเคราะห์โดยใช้เทคนิครามานสเปกโทรสโกปี จากนั้นนำสเปกตรัมรามานมาทำการประมวลผลล่วงหน้า (Preprocessing) และทำการวิเคราะห์ข้อมูลโดยใช้การวิเคราะห์องค์ประกอบหลัก (Principal component analysis, PCA) และการวิเคราะห์การถดถอยกำลังสองน้อยที่สุดบางส่วน (Partial least squares regression, PLSR) ด้วยโปรแกรมอันสแครมเบลอร์ (Unscrambler®) ซึ่งพบว่า แบบจำลองการสอบเทียบที่ได้รับการประมวลผลล่วงหน้าโดยการปรับแก้การกระเจิงแบบผลคูณ (Multiplicative scatter correction, MSC) ของสเปกตรัมรามานช่วง 1,700 ซม-1 ถึง 1,490 ซม-1 มีประสิทธิภาพในการทำนายและให้ผลการวิเคราะห์ใกล้เคียงกับวิธีวิเคราะห์อ้างอิงมากที่สุด โดยมีค่าสัมประสิทธิ์การพิจารณาการสอบเทียบ (Coefficient of determination of calibration, R-square) เท่ากับ 0.9993466, ค่าเฉลี่ยกำลังสองของความผิดพลาดมาตรฐานของการสอบเทียบ (Root mean square of the standard error of calibration, RMSEC) เท่ากับ 0.8746274, ค่าเฉลี่ยกำลังสองของความผิดพลาดมาตรฐานของการทวนสอบความถูกต้องข้าม (Root mean square of the standard error of cross-validation, RMSECV) เท่ากับ 3.8250928 และค่าเฉลี่ยกำลังสองของความผิดพลาดมาตรฐานของการทำนาย (Root mean the square error of prediction, RMSEP, RMSEP) เท่ากับ 4.8952222

The aim of this study was to select insect-pathogenic fungi and screen them for their antibacterial and antibiofilm activities against plant pathogenic bacteria that cause an 80% decrease in rice yield. The selected insect pathogenic fungi belong to a group that exhibits complex characteristics that can potentially lead to misidentification. Therefore, a combination of morphological study and phylogenetic analysis was used to accurately identify these fungi. A total of 166 specimens were selected from three genera, including Ascopolyporus, Beauveria, and Metarhizium. This research presents the screening and identification of 41 lactic acid-producing bacteria collected from various tree barks in different provinces of Thailand. They belonged to six groups within five genera, including Sporolactobacillus (9 strains), Weizmannia (18 strains), Terrilactibacillus (1 strain), Lactococcus (2 strains), and Enterococcus (11 strains). Based on 16S rRNA gene sequence similarity (98.65%), the novel species in the genera Weizmannia (1 strain), Sporolactobacillus (4 strains), and Enterococcus (1 strain) are found. The lactic acid screening of spore-forming bacteria, S. laevolacticus CUH1-2, exhibited high temperature tolerance and produced D-(-)-lactic acid at high yield and optical purity. Therefore, it was …
