Breast cancer is the second most common type of cancer in the world. Hormone receptor (HR) positive breast cancer (BC) is a prevalent disease accounting for approximately 2 million new cases globally. Almost 70-80% of breast cancer patients are women with a positive score for the estrogen receptor (ER). Radiology departments have contributed significantly to greenhouse gases including release of toxic imaging contrast media to environment. We feel Radiology also has several spectroscopy and imaging tools that may apply to monitor and support cleaner environmental goals. The current manuscript is one of the firsts to prompt Radiology to move in that direction by non invasive imaging of bio metals that are less abundant in biological tissues but play key roles as co-factors in tissue structure and function. Pharmacogenomics (PGx) is considered an emergent field in developing countries. Research on PGx in the Latin American and the Caribbean (LAC) region remains scarce, with limited information in some populations. Thus, extrapolations are complicated, especially in mixed populations. In this paper, we reviewed and analyzed pharmacogenomic knowledge among the LAC scientific and clinical community and examined barriers to clinical application. We performed a search for publications and clinical trials in the field worldwide and evaluated the contribution of LAC. Curcumin is a natural active ingredient from traditional Chinese medicine (TCM) that has multi-target characteristics to exert extensive pharmacological activities and thus has been applied in the treatment of various diseases such as cancer, cardiovascular diseases, nervous system, and autoimmune disorders. As an important class of membranous organelles in the intracellular membrane system, lysosomes are involved in biological processes such as programmed cell death, cell metabolism, and immune regulation, thus affecting tumor initiation and progression. It has been shown that curcumin can modulate lysosomal function through the aforementioned pathways, thereby affecting tumor proliferation, invasion, metastasis, drug resistance, and immune function. …

ที่มาและความสำคัญ: การตรวจยีนแพ้ยา HLA-B*58:01 ก่อนเริ่มใช้ยา allopurinol สามารถช่วยป้องกันการแพ้ยาทางผิวหนังจากการใช้ยา allopurinol ได้ วัตถุประสงค์: เพื่อเปรียบเทียบอุบัติการณ์การแพ้ยาทางผิวหนังจากยา allopurinol โดยรวมทั้งชนิดที่ไม่รุนแรงและรุนแรง และศึกษาความแตกต่างของค่าใช้จ่ายทางตรงที่เกี่ยวข้องกับการรักษาเมื่อพบการแพ้ยาในกลุ่มผู้ป่วยที่ได้รับการตรวจยีนแพ้ยา (กลุ่มศึกษา) และผู้ป่วยที่ไม่ได้รับการตรวจยีนแพ้ยา (กลุ่มเปรียบเทียบ) วิธีการศึกษา: การวิจัยจากเหตุไปหาผลแบบย้อนหลัง ผู้ป่วยที่เข้ารับการรักษาในโรงพยาบาลเจริญกรุงประชารักษ์ กรุงเทพมหานคร ระหว่างวันที่ 1 มกราคม พ.ศ. 2562 ถึงวันที่ 31 มีนาคม พ.ศ. 2567 ผู้ป่วยที่ผ่านคุณสมบัติตามที่ผู้วิจัยกำหนดทั้งหมด 1,026 ราย แบ่งเป็นกลุ่มศึกษา 281 รายและกลุ่มเปรียบเทียบ 745 ราย ผลการศึกษา: ผู้ป่วยกลุ่มศึกษา 281 ราย ตรวจพบยีนแพ้ยาเป็นบวก 53 ราย (ร้อยละ 18.8) ได้รับการรักษาด้วยยาลดกรดยูริกในเลือดอื่น ๆ 32 ราย ตรวจพบยีนแพ้ยาเป็นลบ 228 ราย (ร้อยละ 81.14) ได้รับการรักษาด้วยยา allopurinol 163 ราย และกลุ่มเปรียบเทียบ 745 ราย ผู้ป่วยทุกรายได้รับการรักษาด้วยยา allopurinol ที่ระยะเวลาการติดตามการแพ้ยา 90 วัน ในกลุ่มศึกษาพบอุบัติการณ์การแพ้ยาทางผิวหนังโดยรวม และชนิดที่ไม่รุนแรงเท่ากันร้อยละ 1.4 และกลุ่มเปรียบเทียบร้อยละ 5.9 และ 5.0 ตามลำดับ อย่างมีนัยสำคัญทางสถิติ (p = 0.007, 0.02 ตามลำดับ) และในกลุ่มศึกษาไม่พบอุบัติการณ์การแพ้ยาทางผิวหนังชนิดที่รุนแรง แต่กลุ่มเปรียบเทียบพบร้อยละ 0.9 (p = 0.216) ความแตกต่างของค่าใช้จ่ายทางตรงที่เกี่ยวข้องกับการรักษาเมื่อพบการแพ้ยาระหว่างกลุ่มศึกษาน้อยกว่ากลุ่มเปรียบเทียบ 78,198.52 บาทต่อราย สรุปผลการศึกษา: การตรวจคัดกรองยีน HLA-B*58:01 ก่อนเริ่มใช้ยา allopurinol มีประสิทธิผลในการลดอุบัติการณ์แพ้ยาทางผิวหนัง และลดค่าใช้จ่ายทางตรงที่เกี่ยวข้องกับการแพ้ยา

Lung, breast, and colorectal cancers significantly contribute to the high incidence and mortality rates of cancer worldwide. Naphthoquinone is a well-known anticancer agent, including lawsone and napabucasin. The series of lawsone (bicyclic system) and napabucasin (tricyclic system) were designed to find the suitable target proteins though computational modelling, predicting the interactions between 172 naphthoquinone analogues and 12 potential cancer-related target proteins. The virtual study proposed the naphthoquinone analogues based on their superior docking scores, interactions, and absorption, distribution, metabolism, and elimination (ADME) predictions compared to lawsone and napabucasin. Interestingly, the PI3K/AKT/mTOR proteins emerged as the promising target for the naphthoquinones. …

Background: Epidemiological modeling studies in snakebite envenoming research are evolving. Their techniques can be essential in filling the knowledge gap needed to attain the World Health Organization’s goal of halving the burden of snakebite envenoming by complementing the current data scarcity. Hence, there is a need for a scoping review to summarize epidemiological models used in estimating the burden of snakebite envenoming. Methods: We conducted a scoping review by searching PubMed, EMBASE, and Scopus to identify articles reporting epidemiological models in snakebite envenoming from database inception to 31st December 2023. An increase in lysosomal membrane permeabilization (LMP) impaired lysosome integrity causing release of cathepsin B, and subsequently may contribute to cell death. Indomethacin, which is a non-steroidal anti-inflammatory drugs (NSAIDs), caused apoptosis through generation of cell stress, and disruption of mitochondria and lysosome functions. Recently, it was reported that naringenin, a natural bioactive flavonoid, elicited cytoprotection against indomethacin toxicity in Caco-2 cells. A simple, accurate, and low-cost effective method has been introduced in this study for separation of valsartan enantiomers using ligand-exchange chromatography as chiral mobile phase additives. The optimized method was found using a mobile phase system of 57%(v/v) methanol, adding 1.5 mM of L-isoleucine as the chiral selector along with 0.5 mM of cupric sulfate and 10 mM of sodium acetate, adjusted to pH 6.1. The chemical analysis of crude extracts from Dendrobium ochreatum ethanol revealed 12 compounds. Among them, dendroochreatene, a novel discovered compound, and 11 previously identified compounds were found: atraric acid, docosyl trans-ferulate, moscatilin, cinnamic acid, β-hydroxypropiovanillone, loddigesiinol C, dihydroconiferyl dihydro-p-coumarate, indole-3-aldehyde, vanillic acid, naringenin, and 2,4,7-trihydroxy-9,10-dihydrophenanthrene. Structural elucidation of these compounds was achieved through MS, IR, UV, and NMR spectroscopic analyses, corroborated by comparison with existing literature data. The cytotoxicity evaluation against NCI-H460 non-small cell lung cancer cells employed two methods: direct cell testing and literature review. This research aimed to study the physicochemical properties, including physical appearance, percentage of labeled amount (%LA), and release profile, of Asiatic acid (AA) encapsulated in solid lipid microparticles (SLM) composed of 10% and 15% cetyl alcohol with poloxamer 188 (F6 and F12). The results indicated that F12 demonstrated a significantly higher %LA and exhibited a release kinetic that fit the Higuchi model with a slower sustained-release profile compared to F6. วัตถุประสงค์: เพื่อศึกษาผลลัพธ์ของการเริ่มต้นและปรับขนาดยาโดยใช้วาร์ฟาริน โนโมแกรมในผู้ป่วยที่เริ่มใช้ยาวาร์ฟาริน ณ โรงพยาบาลเจริญกรุงประชารักษ์ วิธีการดำเนินการวิจัย: การศึกษานี้เป็นการทดลองแบบสุ่มและมีกลุ่มควบคุม ในอาสาสมัครที่เริ่มใช้ยาวาร์ฟารินขณะเข้าพักรักษาตัวในโรงพยาบาลเจริญกรุงประชารักษ์ ระหว่างเดือน สิงหาคม ถึง พฤศจิกายน พ.ศ. 2566 โดยกลุ่มทดลองจะได้รับการเริ่มต้นและปรับขนาดยาวาร์ฟารินโดยใช้วาร์ฟารินโนโมแกรม และกลุ่มควบคุมจะได้รับการเริ่มต้นและปรับขนาดยาวาร์ฟารินโดยดุลยพินิจของแพทย์ ประเมินสัดส่วนของอาสาสมัครที่มีค่า INR อยู่ในช่วง 2.0 – 3.0 ภายใน 14 วัน ผลการวิจัย: จากกลุ่มตัวอย่างทั้งหมด 98 ราย แบ่งเป็นกลุ่มควบคุม 49 ราย และกลุ่มทดลอง 49 ราย ผลลัพธ์ของการวิจัยพบว่าสัดส่วนของอาสาสมัครที่มีค่า INR อยู่ในช่วง 2.0 – 3.0 ภายใน 14 วันหลังได้รับยาวาร์ฟารินของกลุ่มที่เริ่มต้นและปรับขนาดยาโดยใช้วาร์ฟารินโนโมแกรมมากกว่ากลุ่มที่เริ่มต้นและปรับขนาดยาโดยแพทย์อย่างมีนัยสำคัญทางสถิติ (44.9% และ 24.5%, p=0.034) ขนาดยาวาร์ฟารินเริ่มต้นเฉลี่ยของทั้งสองกลุ่มไม่แตกต่างกัน (2.1 มิลลิกรัมต่อวัน และ 2.2 มิลลิกรัมต่อวัน, p= 0.666) สรุป: การใช้วาร์ฟารินโนโมแกรมในการเริ่มต้นและปรับขนาดยาวาร์ฟารินส่งผลให้ค่า INR เข้าสู่ช่วงเป้าหมายของการรักษาเพิ่มขึ้น นอกจากนี้การใช้วาร์ฟารินโนโมแกรมมีความปลอดภัยเทียบเท่ากับการเริ่มต้นและปรับขนาดยาวาร์ฟารินโดยแพทย์

วัตถุประสงค์: เพื่อเปรียบเทียบการเปลี่ยนแปลงอัตราการกรองของไต (estimated glomerular filtration rate, eGFR) ในผู้ป่วยโรคเบาหวานชนิดที่ 2 ที่ได้รับการรักษาเสริมด้วยยากลุ่ม sodium-glucose cotransporter 2 inhibitors (SGLT2 inhibitors) ร่วมกับยาต้านระบบ renin-angiotensin (RAS blockers) เทียบกับกลุ่มที่ได้รับ RAS blockers วิธีการ: งานวิจัยนี้เก็บข้อมูลย้อนหลังจากเวชระเบียนอิเล็กทรอนิกส์ในโรงพยาบาลสวรรค์ประชารักษ์ การศึกษาคัดเลือกผู้ป่วยโรคเบาหวานชนิดที่ 2 อายุ 18 ปีขึ้นไป ระยะเวลาตั้งแต่มกราคม พ.ศ. 2560 ถึงธันวาคม พ.ศ. 2564 เข้างานวิจัยจำนวน 350 ราย กลุ่มศึกษา คือ กลุ่มที่ได้รับยากลุ่ม SGLT2 inhibitors ร่วมกับ RAS blockers 175 ราย กลุ่มควบคุม คือ กลุ่มที่ได้รับยากลุ่ม RAS blockers 175 ราย ผลลัพธ์ของการศึกษา คือ ค่าอัตราการกรองของไต น้ำตาลสะสมฮีโมโกลบิน ความดันโลหิต และน้ำหนักตัวซึ่งวัด ณ เดือนที่ 0, 3, 6 และ 12 ผลการวิจัย: อัตราการกรองของไตระหว่างกลุ่มศึกษาและกลุ่มควบคุมทั้งในเพศหญิงและเพศชายไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ โดยมีเพศหญิงเป็นปัจจัยที่มีผลต่อการเปลี่ยนแปลงอัตราการกรองของไตที่ P=0.024 น้ำตาลสะสมฮีโมโกลบินของทั้ง 2 กลุ่มไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ โดยมีระยะเวลาการเกิดโรคเบาหวานและค่าน้ำตาลสะสมฮีโมโกลบินพื้นฐานเป็นปัจจัยที่มีผลต่อการเปลี่ยนแปลงค่าน้ำตาลสะสมฮีโมโกลบินที่ P=0.003 และ P

การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่มีผลต่อการเกิดแกรนูลที่มีขนาดมากกว่า 2 มิลลิเมตร ซึ่งไม่สามารถผ่านแร่งในกระบวนการทำแกรนูลเปียกของยาเมตฟอร์มิน (Rejected granules) ที่ถูกออกแบบให้ผลิตด้วยเครื่องจักรที่มีระบบปิด มีขนาดการผลิต 600 กิโลกรัม จากการเก็บรวมรวมข้อมูลปริมาณแกรนูลของยาเมตฟอร์มินที่ไม่สามารถผ่านแร่งได้จำนวน 100 รุ่นการผลิต พบว่ามีปริมาณสูงสุดถึง 21.54 กิโลกรัม จึงออกแบบการทดลองแบบแฟคทอเรียลเต็มรูป เพื่อศึกษาปัจจัยของกระบวนการทำแกรนูลเปียกของยาเมตฟอร์มิน จำนวน 5 ตัวแปร ตัวแปรละ 2 ระดับ ได้แก่ เวลาที่ใช้ในการลดขนาดก้อนวัตถุดิบเมตฟอร์มิน (15 และ 20 นาที) อัตราการไหลของลมทางเข้าของเครื่อง Fluid Bed Dryer (1,700 และ 1,900 ลูกบาศก์เมตรต่อชั่วโมง) อุณหภูมิที่ใช้ในการถ่ายแกรนูลจากเครื่อง Fluid Bed Dryer ไปยังเครื่อง Dry mill (25 และ 30 องศาเซลเซียส) ความกว้างในการเปิด Flap ขณะถ่ายแกรนูล (80 และ 90%) และระยะห่างระหว่างแร่งและใบพัดของเครื่อง Dry mill (2.5 และ 3.5 มิลลิเมตร) และมีการทำซ้ำ 2 ครั้ง ผลการวิจัยพบว่าพารามิเตอร์ของกระบวนการที่มีผลต่อปริมาณการเกิด Rejected granule อย่างมีนัยสำคัญ ได้แก่ ระยะห่างระหว่างแร่งและใบพัดของเครื่อง Dry mill และเวลาที่ใช้ในการลดขนาดก้อนวัตถุดิบเมตฟอร์มินตามลำดับ แต่ปัจจัยที่ศึกษาไม่มีผลต่อการกระจายตัวของขนาดอนุภาคและปริมาณน้ำที่สูญเสียไปของแกรนูล นอกจากนี้ได้ระดับที่ดีที่สุดของพารามิเตอร์กระบวนการทำแกรนูลเปียกที่ปรับใช้ในระดับอุตสาหกรรม คือ เวลาที่ใช้ในการลดขนาดก้อนวัตถุดิบเมตฟอร์มิน 20 นาที อัตราการไหลของลมทางเข้าของเครื่อง Fluid Bed Dryer 1,900 ลูกบาศก์เมตร/ชั่วโมง อุณหภูมิที่ใช้ในการถ่ายแกรนูลจากเครื่อง Fluid Bed Dryer ไปยังเครื่อง Dry mill 25 องศาเซลเซียส ความกว้างในการเปิด Flap ขณะถ่ายแกรนูล 80% และระยะห่างระหว่างแร่งและใบพัดของเครื่อง …

Cyclodextrins (CDs) are cyclic oligosaccharides composed of glucose units linked by α-1,4 glycosidic bond. CDs are commonly used as pharmaceutical excipients and they offer versatile benefits in various industries. Recently, CDs have been promising for the application of medical imaging, especially nuclear medicine imaging. Herein, we aim to radiosynthesize 68Ga-alpha-cyclodextrin (68Ga-αCD) by directly radiolabeling Ga-68 to αCD. Briefly, Ga-68 was eluted, mixed with αCD solution and heated at 100 °C for 20 min. In this study, six triterpenoids were isolated from the stem of Diospyros gracilis H.R.Fletcher (Ebenaceae), including two friedelane-type triterpenoids namely friedelin and epifriedelanol, and four lupane-type triterpenoids namely lupeol, betulin, betulinic acid and betulinaldehyde. Their chemical structures were identified by spectroscopic analysis (IR, NMR and MS) and comparison with those reported in the literature. In the investigation for their in vitro cytotoxic activity against cancer cells, lupeol, betulin and betulinic acid exhibited cytotoxic effects against glioblastoma (U87) and breast cancer (MDA-MB231) cells (IC50 = 13.16, 15.39, 8.31 µM and 34.41, 74.49, 43.88 µM, respectively). วัตถุประสงค์: เพื่อศึกษาปัจจัยที่สัมพันธ์กับการได้รับยาในกลุ่ม Angiotensin converting enzyme inhibitors (ACEIs) หรือ Angiotensin receptor-neprilysin inhibitor (ARNI) หรือ Angiotensin receptor blockers (ARBs) ขนาดต่ำกว่าเป้าหมาย ในผู้ป่วยคนไทยภาวะหัวใจล้มเหลวเรื้อรังที่เกิดร่วมกับการบีบตัวของหัวใจห้องซ้ายล่างลดลง วิธีการดำเนินการวิจัย: การวิจัยเชิงวิเคราะห์จากเหตุไปหาผลแบบย้อนหลัง ในผู้ป่วยคนไทยภาวะหัวใจล้มเหลวเรื้อรังที่เกิดร่วมกับการบีบตัวของหัวใจห้องซ้ายล่างลดลง แผนกผู้ป่วยนอก โรงพยาบาลตราด และโรงพยาบาลพระปกเกล้า ตั้งแต่เดือนมกราคม พ.ศ. 2559 ถึงเดือนธันวาคม พ.ศ. 2563 ทุกราย วิเคราะห์ปัจจัยที่สัมพันธ์กับการได้รับยาในกลุ่ม ACEIs/ARBs/ARNI ขนาดต่ำกว่าเป้าหมาย ผลการวิจัย: ผู้ป่วยทั้งหมดในการศึกษานี้มีจำนวน 396 ราย แบ่งเป็นกลุ่มที่ได้รับยา ACEIs/ARBs/ARNI ขนาดต่ำกว่าเป้าหมาย จำนวน 289 ราย กลุ่มที่ได้รับยา ACEIs/ARBs/ARNI ขนาดมากกว่าหรือเท่ากับเป้าหมาย จำนวน 107 ราย เมื่อควบคุมปัจจัยอื่นๆที่ศึกษา พบว่าปัจจัยที่มีความสัมพันธ์กับการได้รับยาขนาดต่ำกว่าเป้าหมาย คือ NYHA class III/IV (Odd ratio=1.84), 95%CI (1.11, 3.06) และการได้รับยากลุ่ม beta blockers ขนาดมากกว่าหรือเท่ากับร้อยละ 50 ของขนาดเป้าหมาย (Odd ratio=0.37), 95%CI (0.22, 0.6) สรุปผลการวิจัย: ปัจจัย NYHA class III/IV เพิ่มความเสี่ยงที่ผู้ป่วยจะได้รับยาขนาดต่ำกว่าเป้าหมาย การได้รับยากลุ่ม beta blockers ขนาดมากกว่าหรือเท่ากับร้อยละ 50 ของขนาดเป้าหมาย ช่วยลดความเสี่ยงที่จะทำให้ผู้ป่วยได้รับยา ACEIs/ARBs/ARNI ขนาดต่ำกว่าเป้าหมาย

Background and objective: Urinary tract infections (UTIs) are a common problem in pediatric urgent care medicine. There are multiple quality improvement (QI) projects related to the management of UTIs documented in the pediatric literature. We developed a project to decrease the prescribing of ultimately unneeded antibiotics for possible UTIs in a pediatric urgent care setting. A similar project has not been described in the pediatric literature.
Methods: We first reviewed the charts of patients presenting to a system of pediatric urgent care centers with a possible UTI over a 2-year period. We then launched a QI project with three plan, …

BACKGROUND: A growing body of evidence has shown the involvement of the kynurenine pathway (KP), the primary route of tryptophan (TRP) catabolism, in the pathophysiology of neuropsychiatric disorders.
OBJECTIVE: The study aims to provide a comprehensive and critical overview of the clinical evidence on the KP involvement in the pathophysiology of Alzheimer's disease (AD) and Parkinson's disease (PD), discussing therapeutic opportunities.
METHODS: We searched for studies investigating KP metabolites in human subjects with AD and/or PD.

The current GAG anticoagulants such as heparin, heparin derivatives, and vitamin K antagonists, such as warfarin continue to be the backbone of anticoagulant therapy. These drugs act through an indirect mechanism to convey inhibition of several coagulation enzymes. However, xv their use leads to several serious adverse effects, such as excessive bleeding risk and unpredictability of patient response. Regardless of their clinical achievement, every individual agent is accompanied by several side effects, particularly major/minor bleeding, thrombocytopenia, drug-food or drug-drug interactions, or absence of antidote. The pathogen Ophidiomyces ophidiicola, widely known as the primary cause of snake fungal disease (SFD), has been detected in Texas’s naïve snakes. Our team set out to characterize O. ophidiicola’s spread in eastern Texas. From December 2018 until November 2021, we sampled and screened with ultraviolet (UV) light 176 snakes across eastern Texas and detected 27. O. ophidiicola’s positive snakes using qPCR and one snake in which SFD was confirmed via additional histological examination. Upon finding the ribbon snake with a clear clinical display, we isolated and cultured what we believe to be the first culture from Texas. This cultured …

This commentary highlights the scientific history of the NIH-Moderna COVID-19 vaccine and corroborates Sarpatwari’s theme of private capture of value created by the public. The commentary also identifies missteps by the Trump and Biden Administrations and offers policy recommendations: better contracts with and incentives for pharmaceutical manufacturers and a not-for-profit “public option” for pharmaceutical development.

Water samples were collected in May 2022 from WTH lake and silent lake located in Wind Cave National Park, South Dakota. From the water samples collected, 8 pure fungal colonies were isolated. All samples underwent chemical extraction, growth inhibition testing, proton-nuclear magnetic resonance spectroscopy ( 1H NMR), and high-pressure liquid chromatography (HPLC). Three pure colonies also underwent DNA extraction, and sequencing of the ITS region for genus level identification. These samples were able to be identified to the genus level, and for these three samples were: Talaromyces, sp., Linnemannia, sp., and Gliomatsix, sp. Purpose: Systemic treatment of colorectal cancer involves chemotherapeutic agents which elicit serious and negative side effects from the toxicity of the drug. To address this issue, we are testing dietary supplements for their efficacy against human colon cancer cell lines and also their potential synergistic effects when combined with conventional chemotherapy. Dietary supplements (specifically sesamol and black rice extract) exhibit anticancer, anti-inflammatory, and chemo-preventive properties. Meanwhile, one of the cancer resistance mechanisms is the upregulation of drug elimination mechanisms, leading to multi-drug resistance. We hypothesize that dietary compounds will act as chemo-enhancers, thus enhancing potency of the chemotherapy drug(s) on …
