Developing A Pbpk Model For Intranasal Naloxone And Opioid Displacement From Brain Receptors, Jasmin Akther Hossain
Developing A Pbpk Model For Intranasal Naloxone And Opioid Displacement From Brain Receptors, Jasmin Akther Hossain
Selected Full-Text Dissertations 2020-
The therapeutic and nontherapeutic use of potent opioid agonists has increased dramatically over the past 20 years, with overdose trends following suit. Currently, naloxone is the primary drug used for treating emergency rescue from an opioid overdose. Naloxone works by displacing opioid agonists that are bound to receptors such as the μ-opioid receptor (MOR) in the brain, which are thought to be the sites of action responsible for symptoms due to overdosing, such as respiratory depression.
Intranasal (IN) administration of naloxone is an excellent alternative to the invasiveness of injections and poorly bioavailable oral formulation. However, the exact mechanism of …

Previous meta-analyses assessed andexanet alfa (AA) or prothrombin complex concentrate (PCC) products for the treatment of Factor Xa inhibitor (FXaI)-associated major bleeding. However, they did not include recent studies or assess the impact of the risk of bias. We conducted a systematic review with meta-analysis on the effectiveness of AA versus PCC products for FXaI-associated major bleeding, inclusive of the studies' risk of bias. PubMed and Embase were searched for comparative studies assessing major bleeding in patients using FXaI who received AA or PCC. We used the Methodological Index for NOn-Randomized Studies (MINORS) checklist and one question from the Joanna …
Naloxone And Dexmedetomidine Reversal Of The Effects Of Methamphetamine And Fentanyl Co-Administration, Wesley R. Tackett
Naloxone And Dexmedetomidine Reversal Of The Effects Of Methamphetamine And Fentanyl Co-Administration, Wesley R. Tackett
Theses, Dissertations and Capstones
Agitation is common amongst individuals who have taken methamphetamine (METH) when they arrive at the emergency room (ER). Fentanyl (FENT) and other opioids can mask this agitation which could then unexpectedly present after the reversal of these drugs by naloxone (NLX). Commonly used medications for agitation caused by METH include benzodiazepine sedatives (like alprazolam and similar drugs), but these drugs can produce dangerous sedation and inhibition of breathing when combined with opioids such as FENT. Therefore, the purpose of this MS thesis project was to use a rat model of METH-FENT overdose to test the ability of opioid antagonist NLX …

An Uncommon Culprit In Sma Syndrome: Navigating Through Duodenal Adenocarcinoma-Induced Obstruction, Deepak Benny Dr, Vikram Patil, Shrisagar R A
Radiology Teaching Files
Superior Mesenteric Artery (SMA) Syndrome is an infrequent vascular compression disorder, characterized predominantly by the extrinsic obstruction of the third part of the duodenum by the SMA, leading to a spectrum of gastrointestinal symptoms. While most commonly associated with rapid weight loss and anatomical variations, its occurrence in the context of adjacent malignancies remains exceedingly rare and clinically significant.
Here I detail a compelling case of a 49-year-old male, initially presenting with nonspecific symptoms of intermittent abdominal pain and vomiting. The diagnostic journey, propelled by computed tomography (CT) and subsequent investigations, unveiled an underlying duodenal adenocarcinoma exerting extrinsic pressure, precipitating …

Background: In children, digital media, lifestyle, and the COVID pandemic have impacted sunlight exposure, exercise, and diet patterns - cues that entrain the circadian clock. We hypothesized that low morning cortisol reflects a weak circadian clock, impacting the pro-inflammatory state. The primary objective was to test relationships between diurnal cortisol fluctuations and the inflammatory state in children as a means of providing indirect support for this hypothesis. Methods: The Cardiovascular Health Intervention Program (CHIP) was a population-based cross-sectional and longitudinal study of circadian health in public elementary school children in Southern Maine, USA (recruitment period 2012–2017). Participants were 689 students …
Modulation Of Diet-Induced Gut Inflammation In Slowing Initiation Of Hematologic Malignancies, Esther Nyarkoa Mensah
Modulation Of Diet-Induced Gut Inflammation In Slowing Initiation Of Hematologic Malignancies, Esther Nyarkoa Mensah
Theses, Dissertations and Capstones
The intestinal tract serves as a host to an extremely complex microbial system. Microbes in the gut regulate their metabolism in response to chemicals generated by other microorganisms as well as nutrition supply from dietary intake. Alterations in gut bacteria have been implicated in the development of metabolic disorders such as obesity and the progression of diseases such as Myelodysplastic syndromes (MDS). Although the cause of obesity is multifactorial, the consumption of high-fat or sugar-rich diets (Western diets) has been proposed as a significant contributor to this global epidemic. Obesity is a major public health concern that has been extensively …
In-Vitro Time-Kill Assays And Semi-Mechanistic Pharmacokinetic-Pharmacodynamic Modeling Of A Beta-Lactam Antibiotic Combination Against Enterococcus Faecalis: Optimizing Dosing Regimens For The Geriatric Population, Zeel Saumin Shah
Selected Full-Text Dissertations 2020-
Pharmacokinetic-pharmacodynamic (PKPD) modeling and simulation have emerged as pivotal tools in drug development and usage. Such models characterize typical trends in data and quantify the variability in relationships among dose, concentration, and desired effects. For antibacterial applications, models characterizing bacterial growth and antibiotic-induced bacterial killing offer insight into interactions between antibiotics, bacteria, and the host. Simulations from these models predict outcomes for untested scenarios, refine study designs, and optimize dosing regimens.
Enterococcus faecalis, a significant opportunistic bacterial pathogen with increasing clinical relevance, is commonly found in the gastrointestinal tract but can lead to severe infection, such as endocarditis. Treatments for …

An Ex-Vivo Investigation Of The Effect Of Skin Conditioning With Formulation Vehicles On The Dermal Disposition Of Diphenhydramine Perfused Directly Into The Dermis, Nasim Rezvani Haghighi Shirazi
Selected Full-Text Dissertations 2020-
It is well known that the composition/excipients of a topical dermatological drug product (TDDP) affect the permeation of an active pharmaceutical ingredient (API) through the skin. While it is generally accepted that the vehicle mostly modulates the release of the API from the product, little is known whether the vehicle might also influence the kinetics of the API within the skin itself. In one attempt to answer this question, we developed an ex-vivo model using pig ear and a retrodialysis/microdialysis technique to deliver the API directly into dermis and measure the API dermal pharmacokinetics (dPK). We compared the dPK of …

Pediatric Vaccination Adherence: Enhancing Compliance Among First-Generation Asian American Parents, Katelyn Leong
Nursing | Senior Theses
Background: Vaccine adherence in the pediatric population refers to receiving vaccines as per the recommended schedule, which significantly reduces vaccine-preventable diseases and enhances community immunity. Vaccine hesitancy, driven by concerns such as safety, misinformation, or cultural beliefs can affect adherence. National coverage of state-mandated vaccines among kindergarteners declined from 95% to around 93% between 2019 and 2022, varying across different vaccines. Objective: This research proposal’s focus is to determine if an educational intervention targeting first-generation Asian Americans, who have adopted their immigrant parents' misinformation about vaccines, will effectively increase vaccination rates among their children. The study aims to assess the …

Chemotherapy poses a significant challenge for cancer patients due to drug-associated toxicity, which often results from their effects on both healthy (normal) and cancerous cells. While various options aim to reduce toxicity and optimize beneficial effects, a comprehensive solution remains elusive. Cyclotherapy is one such approach developed to protect normal cells from the toxic effects of chemotherapy drugs. The basic principle underlying cyclotherapy is p53- dependent cell cycle arrest of normal cells while killing cancer cells via a p53-independent mechanism using a second drug. In our research, we investigated the inhibitory effects of a combination of two low-dose anticancer drugs, …

Patients expect safe, effective, high-quality medicines that are available when needed. Meeting these expectations is increasingly challenging in a global pharmaceutical supply chain, as evidenced by persistent product quality and supply issues. A key contributory factor to these issues is disruptions in manufacturing operations, which in turn are often caused by a hesitancy to change and to continuously improve approved processes. Regulatory agencies repeatedly indicate that they will consider increased autonomy for post-approval changes, if pharmaceutical manufacturing operations can demonstrate the effectiveness of Quality Risk Management (QRM) within the Pharmaceutical Quality System (PQS). Thus, the question was posed by Dr …
Synthesis Of Leishmania Spp Antigens For The Development Of Vaccine Candidates, Dipesh Budhathoki
Synthesis Of Leishmania Spp Antigens For The Development Of Vaccine Candidates, Dipesh Budhathoki
Theses and Dissertations
Leishmaniasis is a neglected tropical disease caused by the protozoan parasite of the genus Leishmania. Presently no vaccine against Leishmaniasis is available, so there is a need for new anti-leishmanial treatments, including an effective preventive vaccine. We focus on developing vaccine candidates based on leishmania’s cell surface carbohydrate antigens. Saccharides heavily envelop the cell surface of Leishmania, lipophosphoglycan (LPG) becoming a significant glycan in most leishmania species. Its intrinsic antigenicity makes this polysaccharide a useful target for developing conjugated vaccine candidates against all leishmaniasis variants. In this report, we present our progress towards the synthesis of the fragments of LPG …

Neuroprognostication In Patients With A Subarachnoid Hemorrhage: A Quasi-Review Of Anti-Inflammatory Agent Administration Versus Blood Pressure Management, Julia Gabalski, Marcie Mcmahon
Soaring: A Journal of Undergraduate Research
A subarachnoid hemorrhage (SAH) is a type of stoke where a blood vessel bursts and causes the subarachnoid space to become filled with blood. Traditional nursing management of SAH includes close monitoring of blood pressure goals in order to prevent complications such as vasospasm. Vasospasm causes delayed cerebral ischemia and neurological deficits secondary to the initial injury. Recent research has expanded understanding of the underlying mechanisms behind the neuroinflammatory cascade that occurs after a SAH. In this study, a methodical search was conducted through the CINAHL and PubMed databases to form a quasi-review of current research. The comparison of traditional …
Evaluating Functional Inhibitors Of Acid Sphingomyelinase As Potential Negative Modulators Of Microvesicle Particle Release In Response To Skin Stressors, Taskin Sabit
Browse all Theses and Dissertations
Many diseases including lupus erythematosus, atopic eczema, and rosacea can be associated with photosensitivity, which is an abnormal response to sunlight. Recent studies by our group have implicated microvesicle particles (MVP) as playing an important role in causing inflammation and systemic immunosuppression. Previous studies show that functional inhibitors of acid sphingomyelinase (FIASMA) can block MVP release in response to ultraviolet B (UVB) radiation or Platelet-activating Factor (PAF) agonist, which are skin stressors. However, more research needs to be conducted to evaluate the role of PAF, UVB and FIASMA in MVP generation and to formulate a therapeutic medication to be potentially …

Influence Of Human And Viral Il-6 On Kshv Infection In Human Tonsil Lymphocytes, Wajd Zakir
Influence Of Human And Viral Il-6 On Kshv Infection In Human Tonsil Lymphocytes, Wajd Zakir
Pharmaceutical Sciences (MS) Theses
Infection of B cells by the lymphotropic gamma-herpesvirus Kaposi sarcoma-associated herpesvirus (KSHV) causes Multicentric Castleman Disease, Primary Effusion Lymphoma, and the newly-identified inflammatory syndrome designated KICS. Despite proven connections to lymphoproliferative disorders, little is known about how KSHV invades the human immune system or the early events governing infection in B lymphocytes. IL-6 signaling plays a critical role in KSHV-mediated disease, where elevated hIL-6 levels are positively associated with KSHV viral load and disease progression. This dynamic is even more complex due to the co-existence of human IL-6 (hIL6) and KSHV-encoded viral IL-6 (vIL6) in these diseases. Although the impact …

Nanoscale Characterization Of Interaction Of Nucleosomes With H1 Linker Histone, Ahmed Yesvi Rafa, Shaun Filliaux, Yuri L. Lyubchenko
Journal Articles: Pharmaceutical Sciences
In eukaryotic nuclei, DNA is wrapped around an octamer of core histones to form nucleosomes. H1 binds to the linker DNA of nucleosome to form the chromatosome, the next structural unit of chromatin. Structural features on individual chromatosomes contribute to chromatin structure, but not fully characterized. In addition to canonical nucleosomes composed of two copies each of histones H2A, H2B, H3, and H4 (H3 nucleosomes), centromeres chromatin contain nucleosomes in which H3 is replaced with its analog CENP-A, changing structural properties of CENP-A nucleosomes. Nothing is known about the interaction of H1 with CENP-A nucleosomes. Here we filled this gap …

Neuroimmune Response And Neurodegeneration In Adolescent Rats Following Binge Alcohol Exposure, Catherine Van Doorn
Theses and Dissertations--Pharmacy
Studies of adolescent drinking show a distinct prevalence of binge drinking, including at extreme levels, and this type of drinking has been identified as increasing risk of developing an Alcohol Use Disorder (AUD). Additionally, females suffer greater levels of harm from binge drinking and AUD and in an abbreviated timeline compared to males. Neuroimmune reactivity, specifically with microglial reactivity following binge drinking, may influence the sex differences seen in negative alcohol-related health disparities.
The first set of experiments sought to examine a marker of microglial reactivity, TSPO, in adolescent females and males and found the sexes displayed different temporal and …

A Closer Look At The Humanistic Aspects Of The Opioid Epidemic And Its Implications For Pharmaceutical Ethics And Law, Eriel Burns
Gaines Fellow Senior Theses
This paper elucidates the humanistic elements of this crisis, that is, the ethical challenges it poses and the role of culture in it. The opioid crisis disturbs several bioethics principles as indicated by the fact that it burgeoned out of the health care system as opposed to the illicit environments of the underground market. The book Dreamland by investigative journalist and storyteller, Sam Quinones encapsulates the complexity of the opioid epidemic through research and interviews with important figures related to the crisis such as physicians, people with opioid use disorder, and pharmaceutical sales representatives, among others. Quinones details that the …

Apurinic/apyrimidinic endonuclease 1 (APE1) is involved in DNA repair and transcriptional regulation mechanisms. This multifunctional activity of APE1 should be supported by specific structural properties of APE1 that have not yet been elucidated. Herein, we applied atomic force microscopy (AFM) to characterize the interactions of APE1 with DNA containing two well-separated G-rich segments. Complexes of APE1 with DNA containing G-rich segments were visualized, and analysis of the complexes revealed the affinity of APE1 to G-rich DNA sequences, and their yield was as high as 53%. Furthermore, APE1 is capable of binding two DNA segments leading to the formation of loops …
