The study purpose was to examine the effects of a 15-min live-music therapy intervention on heart rate, oxygen saturation, level of motor activity, behavioral distress, and behavioral state levels in premature infants in the neonatal intensive care unit. The convenience sample included 20 infants born at 26 to 29 weeks’ gestational age who were hospitalized in a large teaching hospital in the southern United States. In a 1-group repeated-measures crossover design, infants were randomly assigned order of music versus no-music conditions. Occupational back pain is a significant health and safety problem in many industrialized countries for persons who are required to perform work-related forceful movement and lifting. Firefighters, an occupational group at high risk for back injury, perform many job tasks under hazardous work conditions. One-third of firefighters retire due to job-related health and safety problems and 49.4% of this group retire as a result of back pain, the leading cause of early retirements in firefighters. A recent study undertaken by the authors (2007) highlighted that undergraduate nursing students were subjected to varying experiences in clinical practice, which were mediated by a number of factors. Mediating factors included continuity of preceptors, student attitudes, the clinical setting environment, student and preceptor expectations of the clinical practice experience and interactions between the student and preceptor.
Of note, interactions with preceptors were seen to 'make or break' the practical experience. Therefore, the relationship that is forged between preceptor and student is vital in shaping the student's experience of the clinical area and of the real world of nursing work. …

The diagnosis of cancer in a child is one of the most significant stressors a parent can experience, and research in the area of stress-related effects of illness on parents has emerged in many areas. The majority of studies have focused on the potential or actual negative impact on parents during and after treatment of their child's illness. This study concentrates on the voice of registered nurses as they describe their experiences of leadership within the nontraditional, non-institutional, non-hospital environment of assisted living. It further expounds upon regulatory and corporate information as context for the nurses’ leadership experiences. The desire to hear nurses describe their personal experiences of leadership influenced the decision to use grounded theory as a methodological process. The belief that voice requires context to be most effectively understood influenced, in turn, the addition of a situational analysis approach to the grounded theory methodology. Couples wishing to achieve or avoid pregnancy can now use an electronic hormonal fertility monitor to aid home fertility monitoring. The Clearplan Easy Fertility Monitor (CPFM) is a handheld monitor designed to help couples achieve pregnancy. It provides information on three levels of fertility (low, high, and peak) based on urinary threshold levels of estrogen and luteinizing hormone (LH). The monitor can be used as an aid to avoid pregnancy when used with another marker of fertility. Depression is a serious complication of the postpartum period that affects not only women but their children and families as well. Rural women are of particular concern because of the significant barriers to health care that they experience as a result of isolation, poverty, traditional beliefs, and a lack of accessible and adequate health related services. BACKGROUND AND SIGNIFICANCE: Obesity prevalence has doubled since 1980. Current research is investigating how to reduce prevalence and examining obesity's impact on the respiratory system. A unifying meta-theory of nursing is suggested, building from the foundation of the AQAL framework. A definition of nursing as situated caring is presented, along with a historical discussion of nursing epistemology and theory for context. Implications for practice, education, and research are also discussed. A unifying meta-theory will enable nurses at both the practical and academic levels to appreciate the complexity and simplicity of nursing, allowing them to articulate confidently what we do and why we do it.

Nursing schools in the United States have not been teaching evidence-based practices for safe patient handling, putting their graduates at risk for musculoskeletal disorders (MSDs). The specific aim of this study was to translate research related to safe patient handling into the curricula of nursing schools and evaluate the impact on nurse educators and students' intentions to use safe patient handling techniques. Nurse educators at 26 nursing schools received curricular materials and training; nursing students received the evidence-based curriculum module. There were three control sites. African American (AA) grandmothers who parent their grandchildren may experience burden related to these responsibilities. Little is known about perceived caregiver burden and its influence on AA grandmother health and service needs. The purpose of this descriptive, correlational research study was to investigate relationships among personal characteristics, information need, perceived burden, perceived health, service need, and service use in AA grandmothers who are the primary caregivers for one or more of their biological grandchildren. Recruited through churches and community centers, 93 AA grandmothers, ages 36 to 78, participated in this study. These grandmothers were primary caregivers for 211 grandchildren. The …

For the individual with sickle cell disease (SCD), the lifespan is increasing but adults report decreased quality of life (QOL), low self-efficacy, and ineffective coping skills. The care of adult patients with SCD requires a complex multidisciplinary team approach with focus not only on physiological, psychological, and social needs, but also on spiritual needs. The purpose of this study was twofold: (1) to replicate Victor and Cullen's (1987; 1988) studies to confirm whether ethical climates (ECs) arise in skilled nursing facilities (SNFs), and whether organizations and groups within organizations have identifiable ECs; and (2) to test a model that examines the effects of ECs and perceived organizational support (POS) on licensed nurses' turnover intention (TOI) through job satisfaction (JS) and organizational commitment (OC).

Purpose: This study evaluated the psychometric properties of the Dyadic Relationship Scale (DRS), which measures negative and positive dyadic interactions from the perspective of both the patient and the family caregiver. An important aspect of evaluating the DRS was that it be statistically sound and meaningful for both members of the dyad. Design and Methods: The study used a cross-sectional design. Survey packages were mailed to home health care patients and their family caregivers. The unit of analysis was the dyad, and exploratory and confirmatory factor analyses were conducted. The purpose of this study was to determine whether the implementation of a stroke clinical pathway could improve compliance with documentation of specific quality measures for patients admitted with the diagnosis of acute ischemic stroke, hemorrhagic stroke, or transient ischemic attack. The study used the ten standardized performance measures for stroke identified by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and integrated documentation of those measures into a stroke clinical pathway consisting of physician order sets, an interdisciplinary plan of care, and a patient/family education flow sheet. Although the majority of coronary artery bypass graft (CABG) surgery patients are extubated within 6 to 8 hours following surgery, 20% to 40% of patients remain intubated 12 hours after surgery due to dysfunctional ventilator weaning response (DVWR). DVWR associated with increased morbidity and mortality (30% to 43%) following CABG surgery. Finding significant antecedence to predict DVWR could help to identify and prevent the complications from DVWR after CABG surgery. Literature review revealed that there is an association between cardiopulmonary indicators (CPI) and DVWR after CABG surgery. Cardiopulmonary indicators are the selected hemodynamic parameters that have an association with DVWR. …

Objective: Determine the feasibility and outcome of a pilot program to increase farm workers' use of hearing protection when performing activities with high noise exposure. Design: The study was a quasi-experimental study. Sample: Eight intervention farmers and 17 comparison farmers participated in the study. Measures: Before and after the intervention, farmers complted a survey to identify their frequency of use of hearing protection and their beliefs about hearing loss and use of hearing protection. Intervention: The intervention consisted of noise assessments, educational sessions, mailed reminders with brochures, and placement of hearing protection on the farm. Purpose: This integrated review was conducted to evaluate the factors that inhibit or promote decisions by African American and Hispanic women to obtain cervical cancer screening.
Data sources: Research articles were identified using MEDLINE, PubMed, and Cumulative Index to Nursing and Allied Health literature, published between 1999 and 2005.
Conclusions: Cervical cancer screening practices of African American and Hispanic women were influenced by extrinsic motivators including lack of insurance, no usual source of health care, acculturation, and socioeconomic factors. Intrinsic motivators were related to beliefs and perceptions of vulnerability, such as ignoring cervical cancer screening when no symptoms were present; …

The purpose of this project was to explore issues that influence Master Degree completion as perceived by graduate nursing students, using focus groups. All eight graduate nursing student participants recently completed structured coursework, but had not submitted a final paper or project. Margaret Newman's Health as an Expanding Consciousness Nursing Theory was applied as the conceptual framework used to process the information obtained. Dialogue between moderator and participants occurred during accompaniment enabling valuable information to be elicited, and participants' personal clarification of concerns; therefore establishing praxis. Issues emerged related to adult learners seeking defined expectations and effective relationships with advisors. …
