Promoting Self-Management Through Adherence Among Heart Failure Patients Discharged From Rural Hospitals: A Study Protocol., Lufei Young, Sue Barnason, Van Do
Journal Articles: College of Nursing
UNLABELLED: Background Heart failure is one of the most prevalent chronic conditions in adults, leading to prolonged morbidity, repeated hospitalizations, and placing tremendous economic burden on the healthcare system. Heart failure patients discharged from rural hospitals, or primarily critical access hospitals, have higher 30-day readmission and mortality rates compared to patients discharged from urban hospitals. Self-management improves heart failure patients' health outcomes and reduces re-hospitalizations, but adherence to self-management guidelines is low. We propose a home based post-acute care service managed by advanced practice nurses to enhance patient activation and lead to the improvement of self-management adherence in heart failure …

There is a pressing need to retain dually eligible elderly Medicare and Medicaid beneficiaries in the community. The objective of this study was to examine median personal care services (PCS) hours, and how increasing PCS to the median (for those below), might delay nursing home placement (NHP) and save cost. Methods: Retrospective study of PCS hours, costs, and NHP in a statewide home and community based waiver program in the Midwest serving 6525 dually eligible Medicare-Medicaid beneficiaries aged 65 and older. Two Minimum Data Set-Home Care assessments and Medicaid claim files were examined to identify PCS hours and cost. A …

Utilizing individualized exercise training programs that are inclusive of appropriate resistance training protocols can attenuate the deleterious effects of sarcopenia and enhance quality of life, functional capacity and also diminish its negative effect on independence. Nurses should be educated in how the utilization of proper resistance training (RT) in individuals can be used to ameliorate the effect of sarcopenia, especially considering its relationship to Quality of Life (QoL), functional capacity and independence. In part, this educational process should include how to refer patients to an appropriate healthcare provider trained in assessing sarcopenia and the use of resistance training. However, it …

Considerations of how gender-related factors influence smoking first appeared over 20 years ago in the work of critical and feminist scholars. This scholarship highlighted the need to consider the social and cultural context of women's tobacco use and the relationships between smoking and gender inequity. Parallel research on men's smoking and masculinities has only recently emerged with some attention being given to gender influences on men's tobacco use. Since that time, a multidisciplinary literature addressing women and men's tobacco use has spanned the social, psychological and medical sciences. To incorporate these gender-related factors into tobacco reduction and cessation interventions, our …

Good Intentions?: A Consideration Of Short-Term, Medical Mission Trips, Kayla M. Steele
Mahurin Honors College Capstone Experience/Thesis Projects
I review the current trend of international volunteer travel with a focus on advantages and disadvantages of short-term (1-2 week) medical volunteer trips and how cross-cultural communication can influence the effectiveness of volunteers. Advantages include making health care available to populations who normally do not have access to care. Disadvantages to trips include a lack of follow-up care and community dependence on outside aid. To increase the effectiveness of these trips, it is recommended that volunteers coordinate with full-time health care workers in target communities and utilize pre-departure training.

Hemodialysis is a treatment process utilized by kidney patients to filter their blood. These patients are required to undergo an intensive regimen including hemodialysis treatments multiple times per week lasting 3-5 hours each. As a result of hemodialysis, patients experience decreased autonomy, freedom, and independence. An estimated 50% of these patients do not adhere to their treatment regimen. Patients on hemodialysis may be more compliant to their treatment if they are satisfied with the level of care and empathy they perceive from their healthcare providers. Currently there is no literature measuring empathy levels of healthcare professionals or students towards patients …

Antibiotic resistance is a problem that is growing worldwide. Misuse and overprescribing of antibiotics leads to a rise in pathogens that are resistant to the conventional drugs prescribed, causing increases in morbidity and mortality rates. Antibiotic resistance occurs when microbes still survive despite the presence of an antibiotic. The remaining microbes grow in strength and multiply until there is a population that are now resistant to treatment. One such treatment is azithromycin, an antibiotic often prescribed for upper respiratory tract infections (URI). A URI is any infection of the respiratory tract from the nasal cavities to the larynx that can …

Abstract
Background
Medication nonadherence, defined as “the number of doses not taken or taken incorrectly that jeopardizes the patient’s therapeutic outcome,” is a major health problem with about 43% of the general population nonadherent to their medications. Medication nonadherence accounts for an estimated 125,000 deaths per year in the US, 33-69% of medication-related hospital readmissions, and an estimated $100 to $300 billion in direct and indirect medical costs. Medication therapy management (MTM), defined as “a distinct service or group of services that optimize therapeutic outcomes for individual patients,” has been found to reduce medication nonadherence. However, many individuals eligible for …

ABSTRACT:
Background: There is a high potential for medication discrepancies to occur during patient care transitions. However, health professionals must find ways to reduce these and improve patient care, such as with medication reconciliation. This intervention is used to identify a patient’s most accurate medication list by comparing the medical record to another list obtained from the patient, hospital, or other provider. Pharmacists have a major role in this process because of their medicinal expertise, but paying them is expensive, so using students may be more cost-effective. Research has examined fourth-year professional pharmacy students (P4s) performing medication reconciliation …

Vitamin K, a fat-soluble vitamin synthesized by plants, is found in large quantities within green leafy vegetables, such as collard, mustard, turnip and spinach. These greens are commonly cooked with fat to enhance flavor. The vitamin K in these greens can leach out during cooking into the liquid portion of the pot, which is referred to as potlicker. Vitamin K activates clotting factors (II, VII, IX, and X,), which can lead to the formation of a thromboembolism (blood clot). People at a higher risk of forming blood clots are often prescribed warfarin, an anti-coagulant that acts as a competitive inhibitor …

Medication adherence is a bigger problem than many of us are willing to admit. If patients do not take their medications as prescribed, they will not get the full therapeutic benefits and will put themselves at risk if serious harm. An estimated 125,000 deaths occur in the United States annually due to non-adherence. The objective of this study is to establish a pharmacy intervention model that best provides patient satisfaction and improved medication adherence through the use of home visits and follow-up calls by pharmacists and pharmacy interns at Clark’s Pharmacy. In order to do this, patients receiving home visits …

Background: Immigrant women may be regarded as a vulnerable population with respect to access and navigation of maternity care services. They may encounter difficulties when accessing culturally safe and appropriate maternity care, which may be further exacerbated by language difficulties and discriminatory practices or attitudes. The project aimed to understand ethnocultural food and health practices and how these intersect in a particular social context of cultural adaptation and adjustment in order to improve the care-giving capacities of health practitioners working in multicultural perinatal clinics.
Methods: This four-phase study employed a case study design allowing for multiple means of data collection …

Background Weight gain during the childbearing years and failure to lose pregnancy weight after birth contribute to the development of obesity in postpartum Latinas. Methods Madres para la Salud [Mothers for Health] was a 12-month, randomized controlled trial exploring a social support intervention with moderate-intensity physical activity (PA) seeking to effect changes in body fat, fat tissue inflammation, and depression symptoms in sedentary postpartum Latinas. This report describes the efficacy of the Madres intervention. Results The results show that while social support increased during the active intervention delivery, it declined to pre-intervention levels by the end of the intervention. There …
Government Efforts And Personal Opinion Explain The Medicalization Of Pregnancy And Childbirth Through Time In Lower Mustang, Nepal, Ruth Baker
Independent Study Project (ISP) Collection
The way that women approach pregnancy and childbirth in rural Nepal has seen an amazing change in the past twenty to thirty years. The medicalization of this entire process, from pre-‐ to post-‐natal care, comes with government efforts for the increased education of women about family planning, nutrition, hygiene, and the proposed benefits of institutional versus in-‐home delivery. In 9 villages of Lower Mustang, interviews conducted with Government Health Post workers, Female Health Volunteers, and women of different ages sought to discern personal experience and opinion about pregnancy and childbirth from the perspective of both local women and those with …
Sexual And Reproductive Health And Rights Of Aymara Women In The Intercultural Health System In Chile / Salud Y Derechos Sexuales Y Reproductivos De Las Mujeres Aymara En El Sistema De Salud Intercultural De Chile, Ariela Schnyer
Independent Study Project (ISP) Collection
This investigation seeks to understand how Aymara women navigate their sexual and reproductive health and rights (SRHR) in Chile’s intercultural health care model. Indigenous communities have their own practices that complicate the provision of sexual and reproductive health by requiring health care providers to be aware of two different worldviews and how they may conflict, as well as what is necessary to provide respectful care. However, an ethnically and culturally pertinent framework is vital to actually assuring successful SRHR provision, whose tenants include autonomous choice and care free of discrimination, coercion or violence. These interactions were investigated through semi-structured interviews …

This article shares a unique model of collaboration between a school of nursing and a community business to offer an on-site occupational health clinical experience for RN to BSN students. This activity could be easily replicated by others to provide a learning opportunity for nursing students. Hospitals remain the primary site of employment for nurses. However, with health care reform nurses are exploring many new opportunities including many settings in the community (Black, 2014). Incorporating community health into nursing curriculum is a necessity; however, providing meaningful clinical experiences can be a challenge for faculty. To meet students' learning needs, faculty …
