IgE-mediated sensitivity (true allergy) to latex occurs in well-defined groups. The common feature is a high degree of repeated exposure to latex. Approximately one percent of the total U.S. population and five to 17 percent of health care workers are affected. The objective of this article is to provide a rational approach to managing latex allergy in health care workers. Information is provided on the basis of a review of recent literature (primarily since 1986) and on the basis of work by the University of Iowa Hospitals and Clinics Multidisciplinary Latex Allergy Committee. In most nursing programs, limited options exist beyond the required nursing curricula for students to continue career growth in a nursing specialty area at the undergraduate level. The authors describe the Young Nurse Clinician Programs, an innovative approach that provides individualized educational opportunities to select nursing students to promote leadership and foster interest in geriatric, pediatric, and community health nursing.

Nurses who work with patients with Alzheimer's disease (AD) are especially vulnerable to the effects of stress and burnout. Many are poorly trained to cope with the emotional and behavioral problems of these residents and are repeatedly frustrated by their inability to manage these recurrent problems. Specialized care units (SCUs) reduced stress in the areas of staff knowledge, abilities, and resources. The SCU was designed specifically to provide the special environmental structures, support, and service systems required for the care of persons with AD that would enhance functioning and decrease associated behavioral problems. The purpose of this study was to determine the effectiveness of a community-based strength training program for older adults in decreasing falls. Subjects who completed a strength training program and an age and gender matched comparison group of older adults who did not participate in the strength training program were questioned about falls during the preceding year. Functional fitness measures for coordination, balance, and strength significantly improved in strength training subjects after completion of a six week program. Furthermore, after controlling for gender effects, the findings revealed that the strength training group reported significantly fewer falls in the preceding year …

Most nurses function as generalists; however, some function in "expert" roles based on informal training as Resource Nurses. Training usually focuses on assessment and management of a specific problem, with the goal of creating a readily available "expert" for every unit. The primary activity of the Resource Nurse is to provide expert care, education, and consultation for patients, families, and staff. Despite the high prevalence of acute confusion among elders and the importance of its early detection, there are few reports of systematic efforts to increase staff competence. This article describes the development and evaluation of an 8-hour educational program designed to prepare staff nurses to perform in a new role, the unit-based acute confusion Resource Nurse (ACRN). Pre- and posttest scores were obtained for the 80 registered nurses who participated in the program. Paired t tests showed that knowledge and confidence significantly increased for participants as a result of their participation in the educational program. This program can serve as …

Psychological factors, including depression, are often involved in the development and continuation of chronic pain problems. Increased understanding of the role of depression in the etiology and maintenance of chronic pain can improve assessment and intervention for the elderly with chronic pain complaints. It is often impossible to determine whether the stress of living with chronic pain has caused the elder to become depressed or whether a depressive disorder is the cause of the pain experience. The nursing and medical literature reveals considerable debate over whether graduate or continuing education is the appropriate level for nurse practitioner preparation. These arguments have been rhetorical since the research literature lacks data on the scope of practice of master's- and nonmaster's-prepared nurse practitioners. The purpose of this study was to examine possible differences between the two types of practitioners in one area--well-child care provided by pediatric nurse practitioners (PNPs). A total of 236 master's- and nonmaster's-prepared PNPs completed a background questionnaire and checklists of nursing activities involving infant and preschool well-child care. The hospital admission rate for dehydration is one of the Agency for Healthcare Research and Quality Prevention's Quality Indicators, which are considered screening tools for potential quality issues. Thus, admission for dehydration may reflect the quality of care provided in community settings. Using a case-control design, this study estimated the incidence, risk factors, and outcomes of dehydration in adults admitted to the hospital. The overall prevalence rate for three International Classification of Diseases codes for dehydration at admission was 0.55%. In the past year there has been continued discussion within professional organizations regarding the emergence of the practice doctorate. This in turn has caused concerns among some nurse practitioners. A great deal of work has been done during the past several years since the American Association of Colleges of Nursing voted in October 2004 to move advanced practice nursing to the doctoral level by 2015. Following the approval, task forces were formed to address curriculum issues and the strategies for transitioning advanced practice nursing education from the master's level to the doctorate of nursing practice (DNP). The purpose of this article was to provide the background and rationale for the practice doctorate in nursing. The American Association of Colleges of Nursing's Position Statement on the Practice Doctorate in Nursing, approved in October 2004, will be discussed. Outlined are some of the changes that will be needed in education, regulation, and advanced practice. Common questions and concerns that advanced practice nurses have, including titling, salary, and transitioning to the doctor of nursing practice degree, will be addressed.

Reenvisioning gerontological mental health nursing begins with reinterpreting the meaning of aging and radically transforming the curriculum of the subspecialty. The commentary suggests 11 specific recommendations that expand on Morris and Mentes's article as a way of reenvisioning and rerooting gerontological mental health nursing. Among the recommendations are renaming the subspecialty "gerontological mental health nursing" as a means to more appropriately reflect the multidisciplinary focus and mental health perspective, placing an equal emphasis on positive aspects of healthy aging, developing a standardized model curriculum, increasing the number of programs offering specialization in gerontological mental health nursing, and reinvigorating continuing education …
