Substance Use Research & Evaluation
This report summarizes the Year 1 activities of the Healthy Acadia Downeast Maine MAT Expansion Project. Through a collaborative effort of Healthy Acadia, its providers, the Downeast Substance Treatment Network and Downeast Substance Use Response Coalition, the project utilized multiple evidence-based strategies to combat opioid use disorder (OUD) in Downeast Maine.
Key findings focused on:
- capacity building,
- stakeholder engagement,
- organizational and peer support,
- payment and reimbursement issues,
- the creation of low-barrier access to MAT,
- barriers, including stigma, to MAT
- patient-centered approaches

A number of current events are exacerbating farm stress. Extension and farm organizations have mobilizedresponses to an emerging mental health crisis among farmers. To evaluate these responses, we conducted anonline scan of resources to present a baseline typology of current mental health programs and response effortsin the 12-state Extension North Central Region. We classified responses by type of program, target audience,and delivery format. We identified the need to train mental health counselors and state suicide hotlineresponders on farm issues and farm culture.

Purpose: The purpose of the two studies was to develop a text message intervention and examine its effects on lowering perceived stress and increasing coping self-efficacy among nursing students. We also explored stress perceptions and investigated student satisfaction with the text message stress management program. Methods: For the first study, twenty three students participated in the focus groups for intervention development. Sample messages were presented to participants and feedback was requested. The human gut is populated with hundreds of types of bacteria and fungi. In North America, both intestinal and mood disorders are prevalent, presenting health problems that challenge health care professionals and patients alike. A scholarly literature review was conducted to explore this important relationship.
The investigation of the literature revealed that there is an association between gut health disorders and mood disorders, mainly anxiety and depression. This is due to bi-directionality between the brain-gut pathways, meaning that gastrointestinal health may have an effect on mental health and vice versa.

The purpose of this QI project was to improve provider adherence to the Institute for Clinical Systems Improvement (ICSI) Guidelines for Adult Depression in Primary Care to enhance the screening, diagnosis, treatment, and management of adult major depressive disorder (MDD). MDD is a widespread, disabling, and treatable psychiatric disorder that is primarily treated and managed by primary care providers (PCPs). Untreated depression is prone to become chronic and can lead to pain, suffering, disability, increased morbidity and mortality, and premature death, with serious repercussions for the individual and society. Sexual minority women (SMW) are at increased risk of elevated body mass index (BMI) compared to heterosexual women, which increases their vulnerability to a variety of chronic diseases. This increased risk of elevated BMI is likely due to unique minority stressors faced by sexual minority individuals, such as internalized heterosexism and discrimination. Minority stressors are associated with poorer mental and physical health among SMW, and SMW may engage in unhealthy coping strategies, like binge eating, to cope with these minority stressors. Background: Souroubea sympetala Gilg. is a neotropical vine native to Central America, investigated as part of a targeted study of the plant family Marcgraviaceae. Our previous research showed that extract of S. sympetala leaf and small branch extract had anxiolytic effects in animals and acts as an agonist for the GABAA receptor at the benzodiazepine binding site. To date, the potential effects of S. sympetala and its constituents on reconsolidation have not been assessed. Reconsolidation, the process by which formed memories are rendered labile and susceptible to change, may offer a window of opportunity for pharmacological manipulation of learned fear. …

The purpose of this study was to explore the effects of Created for Connection (CFC), a faith based marriage enrichment program on relationship satisfaction, adult attachment style and adult attachment behavior. This study further adds to the existing knowledge base related to faith-based programs delivered in an intensive weekend format (Johnson & Sanderfer, 2016). Participants were volunteers from the local community who identified as Christian. Aims: We aimed to determine the psychosocial risk factors for deliberate self-harm in patients aged ≥ 16 years presenting to tertiary care hospitals in Kabul, Afghanistan.

In this article we consider a theology of “person” from the perspective of “mental health.” We first outline how a Lutheran theology of the person takes leave from the teaching of justification, which underscores that humans in healthy relationships are shaped by hope. We then outline the problem of mental well-being in Canada, with a higher than average percentage of people with mental health problems against global averages. To examine trends and contributing factors of opioid, heroin, and cannabis-associated emergency department (ED) visits in Nevada. The 2009 to 2017 Nevada State ED database (n = 7,950,554 ED visits) were used. Use of opioid, heroin, and cannabis, respectively, was identified by the International Classification of Diseases, 9th & 10th Revisions. Three multivariable models, one for each of the 3 dependent variables, were conducted. Independent variables included year, insurance status, race/ethnicity, use of other substance, and mental health conditions. The number of individuals with opioid, heroin, cannabis-associated ED visits increased 3%, 10%, and 23% annually from 2009 to 2015, particularly …

Looking into the ways in which we treat anxiety and depression in both clinical and casual settings it seemed apparent that there are similar tools being utilized to alleviate the effects, but there didn’t seem to be a cohesion of these tools. With Dialectical Behavioral Therapy (DBT) a mindfulness based approach to therapy and the use of art as a tool to create dialogue in Art Therapy (AT) it seemed appropriate to combine the modalities to create a workbook that could be used on one's own or in conjunction with a therapist.

Problem: Over 40% of those with mental illness are untreated since the supply of psychiatrists does not meet the demand. The psychiatric mental health nurse practitioner (PMHNP) may be an adjunct in providing those services. The purpose of this quality assurance study was to evaluate outcomes of care between a PMHNP and a psychiatrist (medical doctor [MD]).
Methods: An observational, descriptive design with a retrospective medical record review of adult patients over a six-month period in a suburban, Midwestern, privately owned psychiatric practice.

Background: Depression often co-exists with non-cardiovascular morbid conditions. Whether this comorbidity increases the risk of cardiovascular disease has so far not been studied. Thus, the aim of this study was to determine if non-cardiovascular morbidity modifies the effect of depression on future risk of CVD.
Methods: Data was derived from the PART study (acronym in Swedish for: Psykisk hälsa, Arbete och RelaTioner: Mental Health, Work and Relationships), a longitudinal cohort study on mental health, work and relations, including 10,443 adults (aged 20-64 years). Attrition in psychotherapy, also known as dropout, is a problem that affects clients who terminate, their families, therapists, mental health systems, and the overall community. Research on attrition is vast. However, the majority of this research has been done post hoc, relied on quantitative methods, and looked primarily at client demographic variables as the predictors of attrition. This has resulted in inconsistent findings, offers little to no useful information about attrition, and appears to blame clients for failed therapy. Founded in theory and empirical research, we developed the Sexual Values and Behaviors Discrepancy Model (SVBDM) as reflective model for counselors to follow in order to assist their clients in safely reducing discrepancies between their ideal and practiced sexual beliefs, values, and behaviors. The SVBDM is a wellness-informed and sex-positive approach to working with clients and is comprised of three steps: Identification and Operationalization of Potential Sexual Issues, Counselor’s Self-Assessment, and Reducing Discrepancies and Maintaining Safety. We note practical implications and potential limitations of this model as well as recommendations for future research.

Human immunodeficiency virus (HIV)-associated neurocognitive disorder is an emergent public health problem known to HIV researchers and scientists, but unfortunately, is a concern that still needs to be better recognized by people living with HIV and HIV service providers. Background
People belonging to sexual minority groups have higher levels of suicidality than heterosexuals. However, findings regarding suicide death are sparse. Using unique national data from two countries, we investigated whether individuals entering a same-sex marriage (SSM), a proxy group of sexual minority individuals, had higher suicide rates than those entering opposite-sex marriage (OSM).
Methods
A cohort study of all males and females who entered an SSM (n=28 649) or OSM (n=3 918 617) in Denmark and Sweden during 1989–2016 was conducted. Incidence rate ratios (IRRs) for suicide were calculated using adjusted Poisson regression models.

In positive psychology, a greater emphasis is placed on the presence of indicators of both physical and mental health. This study examined the relationship between 12 health-promoting behaviors and subjective well-being (SWB; e.g., happiness) in a sample of 450 high school aged youth from five high schools in two states. Participants reported on their dietary habits, physical activity, abstinence from tobacco products, abstinence from alcohol, and sleep hygiene (i.e., 8 unique components) as well as a multidimensional assessment of SWB (i.e., life satisfaction, positive affect, and negative affect). Adolescent dating violence (ADV) is a pressing public health problem in North America. Strategies to prevent perpetration are needed, and a substantial body of research demonstrates the importance of applying a gender lens to target root causes of adolescent dating violence as part of effective prevention. To date, however, there has been limited research on how to specifically engage boys in adolescent dating violence prevention.
