Venlafaxine, a serotonin-norepinephrine reuptake inhibitor (SNRI), is widely used to manage depression and anxiety, including in pregnant women. However, its known hypertensive side effects raise concerns about a potential link to pre-eclampsia, a serious pregnancy complication characterized by new-onset hypertension and proteinuria. Pregnant women with psychiatric disorders already have an increased risk of hypertensive disorders, and while untreated depression can lead to adverse outcomes such as preterm birth and fetal cortisol elevation, the safety of venlafaxine remains uncertain. Current research lacks wellcontrolled studies assessing its association with hypertensive complications in pregnancy. This systematic review aims to evaluate whether venlafaxine use …

Comparing Spatiotemporal Gait Measures And Hip Disability And Osteoarthritis Outcome Scores-12 Through Gait Analysis Of Individuals With Hip Osteoarthritis, Collin N. Thompson
Biological Sciences Undergraduate Honors Theses
Hip osteoarthritis (OA) is a joint disease that leads to degenerative damage surrounding the femoroacetabular joint, which can manifest as degraded cartilage and increased inflammation at the joint site. There are several methods physician’s use to measure a patient's condition, and our study looks at one called the Hip Disability and Osteoarthritis Outcome Score-12, or HOOS-12. This is a patient-reported outcome measure where patients can self-report their pain and level of dysfunction in relation to their hip osteoarthritis. This study investigates the association between the participants’ HOOS-12 scores and objective gait characteristics measured via a portable pressure sensing walkway. At …

- Spinal cord stimulation (SCS) is a neuromodulatory treatment for patients experiencing chronic pain resistant to conservative therapies. It is often used for postlaminectomy syndrome, complex regional pain syndrome, and neuropathic pain.
- SCS leads can be categorized into percutaneous leads and paddle leads, the latter requiring a laminectomy for implantation.
- The advantages of paddle leads include lower migration rates and more efficient energy delivery.
- Revision surgery may be required in instances of lead failure, migration or misplacement. Paddle lead replacement can be challenging due to extensive scar tissue formation, often requiring additional dissection.
- Reoperation for paddle leads has raised concerns about …

From Trauma To Treatment: Chronic Neurobehavioral Consequences And Emerging Treatments, Moriah E. Mcguier
Dartmouth College Ph.D Dissertations
Severe trauma exposure related to traumatic brain injury (TBI) or psychiatric disorders like post-traumatic stress disorder (PTSD) leads to neurobehavioral and physiological impairments that reduce the quality of life of individuals and remain difficult to treat with current therapeutic options. Further, these conditions are highly comorbid, notably in PTSD with blast-traumatic brain injury (bTBI) commonly experienced by military members via explosive forces, leading to complex and heterogenous outcomes. To address this, preclinical models have been developed to recapitulate aspects of TBI and PTSD. This thesis utilizes two such models in rats, bTBI and the single prolonged stress (SPS) model for …

Narrative medicine is a tool that the medical field is increasingly utilizing to cultivate patient-centered and culturally competent doctors and to reflect on lived experiences through texts that focus on psychosocial best practices. This study aims to examine the language used in physician-written works that address how physicians in different medical specialties treat the aging and the terminally ill, and how they view the deceased. Qualitative data guided by thematic questions will primarily be used in the analysis and comparison of these texts’ tone, rhetoric, diction, and similar literary devices to uncover if certain patterns arise across such specialties. Findings …

Access to healthcare significantly varies by medical insurance type, particularly for individuals for low socioeconomic status.
• Medicaid often covers these individuals (a government-funded program for low-income adults and children).
• Only 45% of primary care providers accept new Medicaid patients, versus 94% for private insurance.1
• Large non-profit medical centers accept Medicaid but still present considerable access challenges for patients.
• Lower reimbursement rates, complex medical needs, and extensive administrative requirements are key barriers for providers accepting Medicaid.
• Studies, such as those by Deans et al., highlight greater travel burdens for Medicaid patients needing revision hip or knee …

INTRODUCTION & OBJECTIVE
Prior studies have established that the watch and wait protocol for rectal cancer is a reasonable approach in patients with excellent response to total neoadjuvant therapy (TNT). Current guidelines stipulate that patients eligible for this protocol have no residual tumor on re-staging imaging and endoscopy. Patients who have evidence of residual disease are referred for surgery. This study assessed all patients in our institution who underwent TNT for rectal cancer and the subsequent recommendations for treatment. Primary aim: assess patient compliance with surgeon recommendations for residual disease after TNT.
Secondary aim: assessment of oncologic outcomes and the …

Dissertations and Theses (Open Access)
Acute monocytic leukemia (monocytic AML) is a subtype of AML marked by a proliferation of abnormal monoblasts. This subtype represents approximately 10% of AML cases. The prognosis of monocytic AML is poor, with a 5-year survival of ~30%. Most patients are diagnosed at an age when they are unlikely to survive first-line non-targeted cytotoxic chemotherapy as a bridge to hematopoietic stem cell transplant (HSCT). Even patients who achieve remission commonly relapse. This population of patients would greatly benefit from precision-targeted therapies but currently there are none approved for monocytic AML.
