Muscle Pathology In Total Knee Replacement For Severe Osteoarthritis: A Histochemical And Morphometric Study, Mark A. Glasberg, Janet R. Glasberg, Richard E. Jones

We evaluated a series of 12 biopsies from 11 patients with total knee replacements for severe osteoarthritis. All 12 biopsies showed denervation atrophy, while five cases had significant myopathic changes. Morphometric studies indicated a positive atrophy factor (greater than 150) in the type I myofibers in seven cases, the IIA myofibers in nine cases, and the IIB myofibers in all 12 cases. Type I predominance occurred in six cases, IIA paucity in two cases, and IIB paucity in two cases. The results indicate that patients with severe osteoarthritis of the knee have both significant neuropathic and myopathic changes in quadriceps …

A young woman presented with recurrent hemoptysis temporally associated with menstruation. Catamenial hemoptysis, an extremely uncommon disorder, is usually caused by the presence of ectopic endometrial tissue within the lung. The use of progesterone suppressed menstruation and hemoptysis during four months of treatment. Chest x-ray was normal.

Innovative developments over the past 15 years have resulted in increased sophistication and effectiveness of therapeutic neuroembolization procedures. As a result, percutaneous transcatheter embolization has assumed an increasingly important role in the management of patients with vascular lesions of the head, neck, and spine. By reducing bleeding during surgery, this technique can markedly facilitate the surgical approach to difficult vascular lesions. The technique may also be applied in cases where surgery is contraindicated. Therapeutic neuroembolization has been performed in 23 patients with vascular lesions of the head, neck, or spine at Henry Ford Hospital. Devascularization of the extraaxial blood supply …

The effect of STA-MCA bypass on regional cerebral bloodflow (rCBF) is unclear Since asymmetries of rCBF have previously been shown to identify the focus of maximal cerebral ischemia, a study was performed to assess the effects of surgical revascularization on rCBF in this focus. Sixteen candidates for STA-MCA bypass were studied preoperatively and postoperatively. The rCBF vms measured by the '"Xenon inhalation technique with eight contralateral probe pairs. The probe pair with the maximal percentage difference in initial slope index (ISI) was identified and corresponded to the clinically involved hemisphere in 14 patients. The importance of mind, emotion, and behavior as they relate to the heart, particularly coronary artery disease, is growing in recognition. A review of the research shows increasingly sophisticated demonstrations of links unifying heart function and mind. "Psychocardiology" would be an appropriate term for the diverse areas of interest which focus on the psychosocial correlates of heart disease. This paper illustrates this concept through a review of psychosocial and behavioral aspects of coronary artery disease.

Intussusception of the appendix, though uncommon, is usually diagnosed during surgery. Endometriosis in an intussuscepted appendix is rarely described. A case of appendiceal intussusception of a rare morphologic type associated with endometriosis is presented, and the literature is briefly reviewed. Etiology, classification, and diagnosis are discussed.

A case concerning a 21-year-old male college student with thrombocytopenia and neutropenia complicating infectious mononucleosis is presented. Although the patient had no prior history of alloimmunization, a broad spectrum anti-HLA antibody was strongly positive in his serum. Concomitant with resolution of the hematologic abnormalities, the titer of the antibody diminished. This case is unique both in the severity of the thrombocytopenia and neutropenia and for the circulating HLA antibody most likely of viral origin.

A 58-year-old man with acute inferior wall infarction developed a loud systolic murmur hypotension, and pulmonary edema four days after onset of infarction. M-mode echocardiography demonstrated an unusual diastolic echo that appeared between the mitral valve leaflets. Complete rupture of the posterior papillary muscle was confirmed at surgery for mitral valve replacement.

Congenital absence of platelet glycoproteins IIb and IIIa (GPIIb and GPIIIa) results in a severe bleeding disorder characterized by defective platelet aggregation and failure of fibrinogen to bind to platelets. GPIIb is a two-chain protein containing disulfide-linked alpha and beta subunits. GPIIb and GPIIIa are present as a heterodimeric, noncovalent complex in the platelet plasma membrane and function as the fibrinogen receptor. To characterize synthesis of these two proteins, RNA isolated from a human leukemia cell line that contains GPIIb and GPIIIa was translated in a wheat germ cell-free system. At age 17 two motor signs, mirror movements and dysdiadochokinesis, were found in more than half the subjects known to have had the respective signs at age 7. These rates were significantly higher than rates found within the group of subjects who were sign free at age 7.

Few Western Australian ewe flocks are diagnosed for pregnancy despite the precence of high numbers of barren ewes. Surveys of farm flocks in the early 1970s recorded that between 14 and 29 per cent of ewes failed to lamb each year.

Techniques for diagnosing pregnant ewes have been available for many years, but ultrasonic devices and computer technology have made it possible for skilled operators to determine whether ewes are pregnant and more importantly, whether they have one, two or more foetuses.

Once the number of pregnant ewes is known farmers can use labour more efficiently. They can minimise the …

The effects of quinidine on steady state serum and tissue digoxin concentrations in the neonatal dog were studied. To determine the effects of quinidine on serum digoxin concentrations, two groups of neonates were evaluated: Group I (n = 11) was digitalized with 40 μg/kg body weight, intramuscularly, and placed on a 10 μg/kg per day maintenance dose; Group II (n = 7) was digitalized with 50 μg/kg per day, intraperitoneally, and placed on a 20 μg/kg per day maintenance dose. After 10 days of digoxin alone, quinidine was coadministered (30 mg/kg per day, intraperitoneally) for 7 days. Serum digoxin concentrations …

To assess 'soft-sign' persistence and its correlates outside a referred sample, 159 members of a local birth cohort of the United National Collaborative Perinatal Project were traced and their performance on six neurological test scales was measured at age 17 by examiners blind to their status at age seven. A comparison group was also formed, who had been 'sign-free' at age seven. On four of the six tests (dysdiadochokinesis, mirror movements, dysgraphesthesia and motor slowness) index boys did significantly worse than the comparison boys; by contrast, index girls scored significantly worse than comparisons only on motor slowness.

Improvements in intraocular lens implants during the past ten years have revolutionized cataract surgery. Both anterior and posterior chamber lenses are currently in wide use, but the optical superiority of the posterior chamber location has yet to be demonstrated. To do so, a more detailed analysis is required of visual function than is clinically routine. Also required is appreciation that the retina is an integral part of the eye's optical system because of its directional characteristics, which are known as the Stiles-Crawford effect.
