We have retrospectively reviewed our bronchoscopic experience in patients with suspected pulmonary complications of the acquired immunodeficiency syndrome (AIDS) or AIDS-related complex at Henry Ford Hospital between 1984 and 1986. The charts of 52 patients, representing a total of 87 bronchoscopic examinations, were available for review. The bronchoscopic findings as well as correlative chest roentgenograms and arterial blood gases are reviewed, and implications for therapy are discussed.

To evaluate the effectiveness of ultrasound in the management of postpartum problems, ultrasonographic examination was performed on 98 patients with suspected postpartum infection. The study includes only those patients who did not respond to antibiotic treatment for at least 48 hours and those patients who were readmitted to the hospital with suspected postpartum infection. Echogenic tissue was found in the uterine cavity in 12 patients, and significant fluid collection was found in 24 patients. Gas collection appeared in the uterine cavity in five patients and around the uterine incision in four patients. Fluid collection was found around the uterine incision …

Bilateral Anterior Cerebral Artery Occlusion In An Alcohol Abuser With Sickle-Cell Trait, Thomas H. Swanson, John L. Zinkel, Patti L. Peterson

The contribution of ethanol ingestion to brain infarction is unclear, although many studies suggest that the two may be causally related. We report an unusual case of bilateral anterior cerebral artery occlusion in a young female ethanol abuser with sickle-cell trait whose platelets showed hyperaggregability during epinephrine and adenosine diphosphate-induced aggregation experiments. It is concluded that ethanol withdrawal and dehydration along with direct effects of ethanol on platelet aggregation may result in cerebral artery thrombosis. Sickling of red blood cells in the distal circulation also may be a compounding factor, but this is not proven.

This chapter reports on finding from a study of ethnic older men, aged 65 an older (Jewish, Irish, and Italian) who were widowed from 2 to 8 years after a long-term study. It focuses on life reorganization after the initial bereavement period. It identifies key issues in the process concerning continuity and change in identity reformulation, changes in health and activity patterns, ethnic identity and lingering attachment to the deceased spouse. Ethnicity as a dynamic life course process, shaped by contextual and historical dimensions, and personal meaning processes are highlighted. Supported by NIH# R01-AG005204

The Effects Of Alpha Adrenergic Blockade On Arachidonic Acid Metabolism And Shock Sequelae During Septic Shock In The Rat, Jeffrey Armstrong

Increased activity of the alpha adrenergic nervous system may be a significant factor promoting tissue ischemia in endotoxic shock. The suggestion that increased sympathetic activity affects prostaglandin synthesis prompted investigation of the potential interaction between the adrenergic nervous system and arachidonic acid metabolites during endotoxemia and septicemia. In addition, the possibility that the underlying patterns of arachidonic acid metabolites seen in two different shock models may provide a rationale for therapeutic intervention designed to block either or both the synthesis of arachidonic acid metabolites or the activity of the adrenergic nervous system was examined. Cultured human keratinocytes were grown in vitro for transplantation purposes in case of severe burns. Single keratinocytes were harvested from human epidermis and grown in culture flasks such that multilayered sheets of keratinocyte cells were harvestable in approximately three weeks. In order to evaluate cultured epidermis for equivalency with normal human epidermis, biochemical and histological studies were performed. Thin sections of cultured epidermis were shown to be composed of cells that had undergone developmental differentiation, but not terminal differentiation. Biochemical analyses of keratin and prekeratin proteins isolated from cultured epidermis indicated that cultured keratinocytes have electrophoretic patterns similar to keratins/prekeratins …

The purpose of this study was to evaluate the effectiveness of myofascial release (MFR) compared to ultrasound (US) in treating musculoskeletal pain. Data in the form of number of trigger points, active range-of-motion of the affected joint, pain scale measurement, and a Treatment Response Questionnaire score, were collected.

The Mann-Whitney U value supported that the MFR group outranked the US group 47 percent when ranking the percent change in trigger point number from pre to post treatment.

Use Of A "Permission Giving" Patient Checklist In Identification Of Social And Sexual Problems, William B. Anderson

A checklist derived from the ACOG bulletin entitled "Communication of Sexual Problems in Office Gynecology" was given to 614 women. The findings demonstrate that social and sexual concerns are encountered frequently in the practice of clinical gynecology and that a checklist is effective in identifying these concerns.

Echocardiographic Features Of Metastatic Pericardial And Myocardial Malignancy, Carey Green, Mohsin Alam, Howard S. Rosman, Jeffrey B. Lakier

Clinical and echocardiographic features of a patient with extensive metastasis to the pericardium and myocardium are presented. Echocardiography revealed pericardial effusion, markedly thickened pericardium, and myocardium with increased echogenicity. These findings were subsequently confirmed during surgery and autopsy, which revealed marked tumor encasement of the heart and pericardial myocardial infiltration.
