Retrospective analysis of 45 patients (33 females, 12 males) with cytologically-proven malignant ascites is presented. Abdominal pain was the most frequent symptom (69%). Fiftythree percent cases had low serum albumin. Ascitic fluid was haemorrhagic or serosanguinous in 48% cases, in the rest it was clear or straw-coloured. Peritoneal effusion was exudative in 84% cases. Mean glucose content of ascitic fluid was 95 mg/dl and the mean white cell count of 919 cells/cmm. Vast majority (82%) of the cases had metastatic adenocarcinomas. Primary malignancy was mostly ovarian (47%) followed by non-Hodgkin's lymphoma (11%) and gall bladder carcinoma (9%). After circumcision the first operations done on man were for bladder stone. Done by charlatans the operation often resulted in loss of life and caused the early lithotomists to maintain a peripatetic existence, vanishing from the town the night after surgery was performed. About two centuries ago stones became a common disease amongst kings: surgeons took over from the charlatans and commenced blind skilful mechanical lithotripsy where a curved instrument was introduced per urethram, its jaws opened to engage the stone which was then crushed by a mechanical (screw driven) closure of the jaws. Reports from the Aga Khan University indicate that 58% of 400 school children studied had undesirably high serum cholesterol levels. The present study was undertaken to determine whether the high cholesterol levels are present at birth and to determine the relationship between cord blood, maternal blood cholesterol and maternal diet. Cord blood from 58 neonates and fasting venous blood form 45 mothers were analyzed for total serum cholesterol. Mothers were interviewed regarding their usual diet during pregnancy. Mean cord blood cholesterol was 56.90 mg/dl (range 26 to 123 mg/dI). Mean maternal blood cholesterol was 232.4mg/dl (range 141-382 mg/dl). Mean maternal …

Serum alkaline phosphatase (AP) was estimated in a total of 786(418 males and 386 females) apparently healthy people aged between 1-75 years selected randomly from a sample of the Karachi population. Reference ranges for AP level were obtained for the males and females stratified into ten successive age groups. The study population was also divided into two main age groups, a paediatric aged between 1-14 years and an adult group aged between 15 to over 50 years to see if the mean AP levels differ significantly between the two groups. Sera of 2000 pregnant women attending the antenatal clinic of the Aga Khan University Hospital were tested for rubella antibodies. Of these 1684(84.2%) were Immune and 316(15.8%) were susceptible to rubella. Majority (99.8%) of the women were Asian in origin. Susceptibility decreased with increasing age and parity. We conclude that apart from vaccination of all young children greater attention should be paid to immunization of women of child-bearing age

Congenital hypothyroidism is a preventable cause of mental retardation. Since clinical signs of congenital hypothyroidism do not generally become obvious before three months of age, screening programmes have been introduced in North America and Europe, which consist of T4 or TSH screening on newborn infants on the third day of life. The screening for congenital hypothyroidism was initiated in Pakistan by the Aga Khan University Hospital (AKUH) in March 1987. By April 1988, 5000 neonates were screened and five cases of congenital hypothyroidism were diagnosed. The study revealed the incidence of hypothyroidism to be one case per 1000 newborns which …

The analgesic effect and the usefulness of EMLA cream 5% in connection with venous blood-sampling was investigated in 42 Tanzanian schoolchildren. Approximately 2.5 g EMLA was applied to the right cubital fossa for a minimum of 120 min. The analgesic effect was pronounced - 93% of the venepunctures were pain-free and no child experienced severe pain. No adverse reactions were observed and the children could continue normal school work during the application time.

With increasing automation in Clinical Laboratories, the requirements for quality control material have greatly increased in order to monitor performance. The constant use of commercial control material is not economically feasible for many countries because of non-availability or the high cost of these materials. Here we describe a simple technique to prepare Home Made Quality Control serum using blood from polycythaemic patients. This preparation is stable for about six months without any alterations in the concentration of any of the chemical constituents. Over two years, 9892 mid-stream urine samples from patients attending the Aga Khan University Hospital, Karachi were cultured. Significant bacterial growth was seen in 23.5% samples. Further identification of these organisms Eevealed 40% of E.coli, 16% Pseudomonas aeruginosa, 11% Klebsiella aerogenes, 5.0% Enterobacter sp., 13% Protdus sp., 4.0% Serratia liquifaciens, 1.0% Acinetobacter sp., 3.0% Citrobacter sp., 4.0% Enterococci, 0.5% Staphylococcus aureus. Results of sensitivity tests performed with antibiotics Ampicillin, Cotrimoxa.zole, Nitrofurantoin, Nalidixic acid, Gentamicin, Amikacin, Pipemedic acid, Cefotaxime, Azactain and Carbenicillin did not reveal any distinct pattern

An outbreak of icteric non-A non-B (NANB) hepatitis occurred in a residential community of urban Karachi, Pakistan, from August 1986 through October 1986. Of the 114 cases reported from this community during the 1986 calendar year, a clustering of 85 cases was seen during the above period. Twenty-seven percent of 226 households and 9% of 1,250 individuals were affected. Five persons were hospitalized and 1 death occurred in a young pregnant woman. Cases occurred predominantly in the less than or equal to 29-year-old age group (72%), with a male:female ratio of 1.8:1. Thirty-four cases occurred singly within households, while in …

According to previous WHO standards, anaemia in pregnancy denoted a haemoglobin level of 11.0 gms/dl or less, but this is now considered to be variable1 as it differs from country to country depending on local reference ranges and socio-economic conditions. Quality control in chemical pathology is based on internal and external quality assessment. The internal quality control in clinical chemistry section of AKUH laboratory is based on the analysis of commercially available Beckman norma! and abnormal range control sera. These have known concentrations of blood chemistry constituents. After repeated estimations, mean values of all constituents were established for both normal and abnormal sera. The effect of galvanic stimulation of the calf muscles during surgical operations on the development of postoperative deep-vein thrombosis was assessed using 125I fibrinogen in 64 patients over 40 years of age undergoing major surgery. Alternate legs were stimulated in alternate patients using the opposite leg as a control. No significant difference in the incidence of deep-vein thrombosis was observed either in the immediate postoperative period or later.

Orbital metastasis is a rare occurrence found only in about 3 - 10% of all prostate cancers. A 72 years male presented with proptosis of the left eye associated with pain, blurred vision and frequent headaches for the past 8 months. Past medical history had symptoms of bladder outflow obstruction for 3 years. MRI brain and orbit with contrast was consistent with a large soft tissue mass in the left frontal region. The mass was surgically excised in order to achieve palliation. Histopathology revealed poorly differentiated malignant neoplasm with immunohistochemistry favoring metastatic prostate carcinoma.
