Ectopic pregnancy in a woman with T9 AIS A (motor complete, sensory complete) paraplegia secondary to a high-grade spinal cord pilocytic astrocytoma

Background

In the United States, 20,000 women of childbearing age have a spinal cord injury (SCI). While SCI induces transient amenorrhea immediately after injury, women generally have normal reproductive function after SCI, and their fertility is unimpaired. Medications that commonly manage complications of SCI should be avoided or discontinued during pregnancy. Complications inherent to SCI, including urologic issues, are often exacerbated during pregnancy.

Methods

This is a case report on a female Veteran who initially presented to the …

Getting On The Same Page: A Quality Improvement Project To Reduce Overnight Nursing-To-Resident Physician Pages In An Inpatient Rehabilitation Unit, Renae Fisher Md, Rajbir Chaggar Md, Anthony Zenger Md, Susan Hamilton Rn, Bsn, William Carter Md

Graduate Medical Education (GME) Resident and Fellow Research Day Posters

Introduction
Many Physical Medicine and Rehabilitation (PM&R) residency programs utilize “home call” systems for nighttime coverage of inpatient services. In this system, trainees are expected to work at full capacity the following day after taking nocturnal call. While “on call,” trainees receive both urgent and non-urgent pages throughout the night, which disrupt sleep. To mitigate fatigue, improve hand-offs and improve timing of patient care, this study targeted reducing non-urgent overnight pages from nurses to home call PM&R residents between 11pm to 6am.

Methods

We implemented a prospective quality improvement study with interventions including holding “Getting on the Same Page” meetings …

Journal Articles

© 2020 Simone et al. Introduction: Contract negotiation is a high-stakes interaction, yet most physicians are never taught negotiation skills. Studies suggest that women, as compared with men, display a lower propensity to initiate negotiations and negotiate less competitively, highlighting a need for training to help level the playing field for female physicians. Methods: We devised a learner-centered workshop for female physicians that included a mini-didactic on negotiation principles, a question-and-answer time with a lawyer, an interactive role-play on contract negotiation style, and guided reflection. The workshop was intended for women in medicine from the level of medical student to …

How Partnerships For Community-Based Health Professions Training Were Affected By National Changes In Funding, Patricia A. Carney, Eric M. Wiser, Cynthia Taylor, Debbie Cole, Shelley Dougherty, Chris Gustaferro, Meredith Lair, Curt Stilp

Faculty Publications - College of Medical Science

Background: Area Health Education Centers (AHEC) have contributed to U.S. healthcare workforce training since 1971. National funders recently refocused efforts from K-12 students to matriculated health profession students, which reduced annual funding by $75,000 (25%) per year per Center.

Objectives: To describe how community partnership changed due to funding reductions.

Methods: Key informant interviews were conducted with all four regional center directors with community partnerships.

Lessons learned: Hosted regional centers navigated partnerships in ways that did not significantly change programs because the host institutions supported continuing the partnerships. Independent centers experienced significant changes in partnerships by ending well-established programs and …

An Evaluation Of The Advance Directives-Live Action Simulation Training (Ad-Last) Program, Alexandra Rosa Spinelli

Theses and Dissertations

Advance Care Planning (ACP) is a process that captures a patient’s wishes in the case of future circumstances in which they are unable to express them. Studies show that less than one third of the general population has completed some type of formal Advance Directive (AD). There are barriers to completing ADs, and these barriers operate on multiple levels, including, patient, provider and institutional. To improve providers’ capacity to help patients complete ACP, and overcome these barriers, a provider-focused intervention was conducted. The current study is an analysis of archival data collected from the Advance Directives-Live Action Simulation Training (AD-LAST) …

Strategies For Safe And Effective Treatment Of Patients With Alzheimer Disease, Jafar Alabdullah, Ahmed Almuntashiri

Community & Environmental Health Faculty Publications

Alzheimer disease is a progressive neurodegenerative disorder that causes cognition impairment and memory loss, affecting activities in daily living. In the United States, an estimated 4.5 million people have AD and this figure is expected to reach 14 million by 2050. Because patients with AD experience a deterioration of their mental functions, patients need more help with their activities of daily living, including oral hygiene, as their AD progresses. Studies have found that individuals with AD often experience poor oral health. As advanced age is a risk factor for AD and the number of older adults in the US continues …

A Module Based Method Of Teaching A Novel Approach To Electrocardiogram Interpretation, Emily Fishbein, Alexandra Koutsoubis, Jennifer White, Md, Dimitrios Papanagnou, Md

Phase 1

Purpose: The traditional approach to 12-lead electrocardiogram interpretation has limitations in the ED setting because it is tedious and does not emphasize pattern recognition for potentially fatal conditions that ED physicians must recognize. Additionally, while lecture-based learning is necessary, learning modules can help learners gain a better understanding of the material. The purpose of this study was to create and implement a self-paced learning module that teaches a novel way of ECG interpretation through the following steps: is it sinus? is it wide? is there ischemia? and does this herald sudden death?

Methods: The objective endpoint of this study was …

Screening For E-Cigarette And Tobacco Use In Standardized Patient Encounters, Michael A. Schmidt, Emily J. Noonan, Laura A. Weingartner

Undergraduate Arts and Research Showcase

E-cigarette usage has grown significantly in recent years, with over 5 million US middle and high school students reporting recent usage. The long-term health consequences of these devices are still being investigated, but it is known that e-cigarette aerosol could contain harmful substances including nicotine, heavy metals, and carcinogens. Therefore, it is important for physicians to ask patients about e-cigarette usage specifically, as this may contribute to future health problems. The goal of this study was to investigate if and how medical students screen for e-cigarette usage. Screening language was reviewed in standardized patient encounters, which are a type of …

A Multinational Study Of The Etiology And Clinical Teleology Of Moral Evaluations Of Patient Behaviors, Anna Yu Lee

CGU Theses & Dissertations

This dissertation is a collection of four studies which collectively explore a hypothesized construct of ‘moral evaluation of patient behaviors’ (MEPB) as a driver of health professionals’ readiness to interact humanistically with their patients. In these studies, ‘humanistic interactions’ refer to the non-technical, intangible skills and factors of clinical competence; the factors specifically explored in these studies were compassion toward patients, self-efficacy for treating patients, and optimism toward patient treatment. For the purpose of specificity, all factors were examined as they pertained to patients with substance use disorders. Survey data from a convenience sample of 524 health professionals (i.e. physicians, …

Race And Implicit Bias In The Jeffmd Case-Based Learning Curriculum, Abigail Wetzel, Denine Crittendon, Mph, Rosemary Frasso, Phd, Andres Fernandez, Md, Susan Truong, Md, Traci Trice, Md

Phase 1

Purpose: Race is related to health disparities, not to genetic predispositions, but race is often presented to medical students as a biological variable. At Sidney Kimmel Medical College, the curriculum has never been reviewed to analyze how race is presented to students. This Scholarly Inquiry project aims to analyze how the case-based learning (CBL) narratives presented to medical students perpetuate or challenge implicit biases in medicine, especially in regard to race. This paper will describe the methods of the project, which can serve as a model for other institutions to review their own curricula.

Methods: Thirty-nine CBL cases from the …
