Unusual Lymphomas Developing In Chronic Lymphocytic Leukemia, Sundara B. K. Raman, Sheikh M. Saeed, Joseph P. Abraham

We report three patients with chronic lymphocytic leukemia (CLL) who developed malignant lymphomas of unusual character and modes of presentation. Two of the patients had received low doses of chlorambucil for several years before they developed malignant lymphoma, diffuse, large cell type (LCL). In one of these patients LCL manifested as a grossly evident osteolytic lesion. In the second patient LCL developed initially as a localized lesion in the iliac bone. Both patients died within a few weeks after LCL was diagnosed. The third patient, who was found to have CLL during a routine examination, did not receive any therapy …

A 35-year-old woman, who had a renal transplant flve years ago, developed malignant lymphoma of the mediastinum. The lymphoma was of lymphoblastic type and had T-cell immunophenotype. Most transplant-related lymphomas are of B-cell type. T-cell lymphoma in this setting is extremely rare, and the mechanism of development may be different from that of B-cell lymphomas.

Pseudopolyposis of the colon is a well-recognized local complication of ulcerative colitis. Giant pseudopolyposis is uncommon, and its presentation as a complete obstruction of the colon is a rare occurrence. We report such a case, where retrograde flow of barium on double-contrast barium enema was completely obstructed at midtransverse colon. After resection by total abdominal colectomy, pathology examination ofthe specimen confirmed massive pseudopolyposis without malignancy. A brief review of the pathology and current literature is presented.

Ocular Irrigating Solutions: A Comparison Between Balanced Salt Solution And L-410 (Po-Eis), Philip C. Hessburg, Paul M. Johnston

The ability of two ocular irrigating solutions to reduce postoperative corneal edema after intracapsular cataract extraction was compared using ultrasonic pachymetry. All patients had anterior chamber intraocular lenses implanted. The solutions were balanced salt solution and solution L-410 (PO-EIS), an eye irrigation solution containing dextran 40 and bicarbonate. Corneas irrigated with either solution showed no significant difference in postoperative edema. The use of L-410 (as an alternative to balanced salt solution) may not be necessary in intracapsular cataract extraction.

The highly selective permeability of cerebral blood vessels, termed the blood-brain barrier, is due to endothelial tight junctions (zonulae occludentes). Hyperosmotic solutions reversibly open the blood-brain barrier. The purpose of this study is to determine the immediate effect of hyperosmotic arabinose perfusion upon the cerebrovascular endothelium, the morphology of the cerebral cortex, and the cortical extracellular space.
Twelve adult male Sprague-Dawley rats were separated into three groups of four animals each: control, intermediate, and extreme. The time delayed complementation of plant proteins was tested in weanling Sprague-Dawley male rats by measuring growth rates as well as plasma and tissue free amino acids. Diets of pinto beans with rice, corn or wheat were pair-fed in combination or alternating 4 times a day at 6 and 11 am, and 4 and 9 pm for 4 weeks. Plasma, liver and muscle samples were obtained 2 hours postprandial, a time determined most suitable for sacrifice, and analyzed for free amino acids via ion-exchange chromatography. There were no significant differences in growth of rats fed combination or alternating diets.during week …

A cDNA clone encoding the B1 chain of human laminin has been isolated from a human endothelial cell cDNA library. With use of this probe and a panel of rodent/human somatic-cell hybrids and in situ hybridization, the gene for the human laminin-B1 chain has been localized to chromosome 7, band q31.

n-Alkylphosphates and adenosine derivatives have been observed to be competitive inhibitors of coenzyme molecules in many NAD+ requiring enzymes. In earlier studies, adenosine derivatives such as AMP, ADP and ADP-ribose were found to be coenzyme competitive inhibitors of the Lactate Dehydrogenase catalyzed reaction. However, n-alkylphosphates, to the contrary, + were found to be noncompetitive inhibitors of NAD+.
Furthermore, it was observed previously in Dr. Yuan's laboratory that the effect of n-octylphosphate on the LDH catalyzed reaction was changed from noncompetitive to competitive in the presence of low concentration of AMP. Rossmann et al. reported that the binding …

Duplex formation between oligo(C:G) n where n=3 to 4 was shown not to occur under conditions favorable for duplex formation between poly G and poly C. Instead, a stable guano sine self-structure was found to form which a Tm of 50°C for (Gp)3 and 80°C for (Gp)4 at strand concentrations of 10-5M in 1M NaCl. Neither a duplex nor a self-structure formed in the absence of salt.

Five men with non-insulin-dependent diabetes mellitus and fourteen nondiabetic men, were given test meals with soy protein, casein or additional carbohydrate in place of protein. Blood levels of insulin, glucagon, glucose and lipids were measured at fasting, one-half hour and two hours postprandial. To all meals, the nondiabetic subjects responded with a quick rise of the insulin level which decreased at two hours. In contrast, the diabetics showed a significantly lower insulin response at one-half hour (p < 0.05), but a tendency to higher levels at two hours. The same pattern was observed in the insulin to glucagon ratio. No significant difference between the meals was found in regard to insulin or glucagon responses, suggests that a sustained, although delayed postprandial insulin response, may be a precursor to the higher cholesterol and increased risk for cardiovascular disease among diabetics.

Problem drinkers (N=7) were given 500mg disulfiram daily for one week. Urine was collected before and after disulfiram intake. 5-Hydroxytryptophol (5-HTOL) was determined by a modification of the method of Davis et al. and adapted to high-performance liquid chromatography (HPLC) with an electrochemical detector setting of 0.75V. 5-Hydroxy-indoleacetic acid (5- HIAA) was determined using the HPLC method of Shihabi et al. (1980). Urinary excretion of 5-HTOL in ug/gm creatinine before and after disulfiram was 25.4± 3.3 and 110.5± 27.1 respectively, p < 0.02. 5-HIAA in mg/gm creatinine before and after disulfiram was 4.9±0.9 and 3.6±0.8 respectively, p > 0.16. The data shows that disulfiram administration alters serotonin metabolism resulting in increased 5-HTOL which is known to increase …

The study was designed to investigate the hypothesis that various environmental stimuli would elicit specific individual responses, as reflected in autonomic patterns. It then became the aim of the study to see whether the stimuli which controlled the physiological pattern would also control the verbal response. Three female and two male students volunteered for the single-subject experiment, where they were exposed to a 15-slide sequence of environment stimuli. Results indicated that when allowed to verbally respond without rating restrictions, responses varied greatly, limiting the possibilities of comparison. During a survey of the vertebrates in the Bighorn Canyon National Recreation Area, 46 mammal, 210 bird, 9 reptile, 5 amphibian, and 28 fish species were found. Habitat structure and moisture were two environmental variables associated with species numbers across this 1,820-ha area. Eight distinct habitats were evaluated. In coniferous forests, 27% of the birds and 54% of the mammals were observed. Sagebrush/grasslands and upland shrublands were very arid and had relatively few vertebrates. Five introduced game birds occurred in sagebrush habitat. Riparian and wetland habitats had the largest number of unique vertebrates, 38 and 77 respectively. The establishment of …

In a large family with multiple endocrine neoplasia rype 2 A (MEN-2 A), 20 patients were identified by the diagnosis of medullary thyroid carcinoma (MTC) and/or pheochromocytomas. Another five subjects had neck surgery on the basis of slightly increased results of a C-cell provocative test. Retrospectively, however, although the immunohistochemical diagnosis of C-cell hyperplasia was confirmed, the diagnosis of MEN-2 was doubtful in these five subjects, and the C-cell hyperplasia observed was probably within the limits of normal variation. Using chlorine as a drinking water disinfectant may have potential health effects due to its reactivity with organic amino nitrogen compounds found in the stomach. Organic N-chloramines have been shown to form in the stomachs of laboratory rats. The possible reactions of N-chloramines in the stomach fluid were examined in this study using a model radiolabeled N-chloramine. 36Cl-N-Chloropiperidine, was synthesized and purified to remove 36Cl-chloride. Stomach fluid was obtained from Sprague-Dawley rats which had been first fasted for 24 or 48 hours and then administered 3 mL of deionized water. Eighteen cases of medullary thyroid carcinoma (MTC) (11 hereditary) had parallel measurements of basal plasma levels of somatostatin (SRIF), levels of SRIF extracted from fresh tumor tissue (radioimmunoassay), and immunohistochemistry (peroxidase-antiperoxidase method) using an anti-SRIF antibody on paraffin sections. Immunohistochemistry disclosed some cell positivity in nine cases (50%). Plasma levels of SRIF were above normal (> 50 pg/mL expressed in terms of the 14 amino acid SRIF peptide) in only two of 12 patients. Prospective annual screening for hereditary medullary thyroid carcinoma (MTC) in the J-kindred, currently a 117-member family with multiple endocrine neoplasia type 2A, began in 1969. During the initial screening, 12 patients were found to have MTC. Subsequent screening has detected C-cell abnormalities (C-cell hyperplasia or microscopic MTC) in 22 of 23 addilional family members thyroidectomized for abnormal calcium- or pentagastrin-provocative calcitonin (CT) test results. Seven of the initial 12 patients thyroidectomized in 1970 to 1971 and 19 of 23 individuals thyroidectomized since 1971 remain disease-free by all criteria; three patients thyroidectomized since 1971 have had clearly abnormal serum CT measurements …

We report two patients in whom pheochromocytoma was treated by subtotal adrenalectomy leaving a rim of vascularized cortical tissue in situ. Both patients are doing well without cortisol supplementation although they have subnormal cortisol responses to ACTH stimulation.

We describe the potential benefits and the limitations of the use of highly polymorphic minisatellite DNA probes for genetic linkage analysis of multiple endocrine neoplasia type 2A (MEN-2 A). The advantage of these probes is that up to 34 loci can be examined in a single experiment, and since the loci are highly polymorphic, almost every individual in every family is informative. The disadvantage is that the DNA fragment lengths of the alleles at any given locus differ from one family to another, so that families cannot be combined, and large single sibships are needed to obtain significant linkage data. …

This report presents pairwise linkage results from our search for the locus of the gene (MEN2A) for the multiple endocrine neoplasia type 2A (MEN-2A) syndrome in one large kindred (the N kindred), clearly segregating for an autosomal dominant form. About 25% of the autosomal genome is excluded when these new results are combined with those we have published previously. The genetic markers employed are distributed across at least 19 of the 22 autosomes. Seven genetic markers whose chromosomal locations are not yet established have also been studied.

In this retrospective study of 68 cases of medullary thyroid carcinoma (MTC), morphological features were studied, which could be helpful in prognosis and in the distinction between hereditary and sporadic forms of the disease. Necrosis within the tumors seemed to carry a poor prognosis. Bilateral MTC. unquestionable C-cell hyperplasia, glandular or follicular pattern, and thyroglobulin-positive cells seemed to be more prevalent in hereditary forms of the disease. A smaller proportion of calcitonin-immunoreactive cells were encountered in hereditary tumors than in sporadic tumors.

We have employed the TT cell line, a model for the human medullary thyroid carcinoma cell, lo study the regulation of peptide hormone production by glucocorticoids. Complementary DNA probes were used to measure the calcitonin (CT), CT gene-related peptide (CGRP), and somatostatin (SRIF) mRNA levels. Dose-response experiments in serum-free medium showed that dexamethasone (six-day treatment) lowered somatostatin (to 1% of basal) and CGRP mRNA (to 50% of basal) and stimulated CT mRNA (threefold to thirteenfold) with a half-maximal effective concentration of 10−8 M. The four-year experience of the French collaborative study on medullary thyroid carcinoma (MTC) has permitted improvement in family screening by a common discussion of data and a network of coordinators throughout France involved in the detection of new cases. Investigations have been directed at improving the discrimination between sporadic and hereditary forms of MTC and between individuals who have or have not inherited the disease within families.
