การกระจายของโรคฟันผุและสภาพความรุนแรงของโรคปริทันต์ ในประชากรมีอายุ 30-70 ปี ของจังหวัดสกลนคร, สุนทร ระพิสุวรรณ, สุรสิทธิ์ เกียรติพงษ์สาร

จากการสุ่มสํารวจประชากรที่อยู่ในหมู่บ้านอําเภอรอบนอกของจังหวัดสกลนคร อายุ 30-70 ปี เพื่อหาความชุกของโรคฟันผุจําแนกตามซี่ฟันและสถานะของโรคปริทันต์ โดยตําแหน่งของฟัน จํานวน 165 คน, เป็นเพศหญิง 96 คน (58.18%) พบว่า 93.90% ของ periodontal sextant มีหินปูนเกาะรอบคอฟัน, ตําแหน่ง ฟัน #11 มีความชุกของการเกิดหินปูนเกาะสูงกว่าตําแหน่งอื่น (89.31%) ในแง่ของความต้องการบริการรักษา 36% ของประชากรศึกษาต้องการการบําบัดที่ค่อนข้างซับซ้อน ค่าเฉลี่ยของฟันผุ ถอน อุด เท่ากับ 5.48 ต่อคน โดยฟันกรามล่างซี่สุดท้าย มีเปอร์เซ็นต์การผุมากที่สุด (16.97%) และฟันกรามซี่สุดท้ายมีเปอร์เซ็นต์การถอนมากที่สุด (22.12%) ฟันกรามบนซี่แรกจะมีเปอร์เซ็นต์การผุมากกว่าฟันกรามล่างซี่แรก ค่าเฉลี่ยของความต้องการอุด และถอน เท่ากับ 2.93 ที่ต่อคน ฟันกรามโดยทั่วไปของคนกลุ่มนี้จะพบฟันสึกจนถึงชั้นเนื้อฟันทุกคน

เพื่อเปรียบเทียบผลทางคลินิกของน้ำยาคลอร์เฮกซิดีนความเข้มข้นร้อยละ 0.03 และ 0.06 และน้ำยาควบคุม ร่วมกับการฉีดล้างช่องปากแบบมีแรงดัน ในการควบคุมการเกิดคราบจุลินทรีย์และเหงือกอักเสบ ในผู้ป่วย 10 คน ซึ่งฉีดน้ำยาวันละ 1 ครั้ง เสริมการแปรงฟัน เป็นเวลา 3 สัปดาห์ต่อน้ำยาแต่ละอย่าง มีระยะพัก 2 สัปดาห์ สําหรับการเปลี่ยนน้ำยา ผู้ป่วยทุกคนแปรงฟันโดยวิธีที่แปรงอยู่เดิม โดยใช้ยาสีฟันชนิดเดียวกัน ไม่มีการดูแลสุขอนามัยในช่องปากด้วยวิธีอื่น ผู้ป่วยทุกคนจะได้รับการทําความสะอาดช่องปากให้มีดัชนีคราบจุลินทรีย์เท่ากับ 0 ทุกครั้งที่เปลี่ยนน้ำยา ผู้ป่วยจะได้รับการตรวจดัชนีคราบจุลินทรีย์ ดัชนีการมีเลือดออก ดัชนีดัดแปลงพีเอ็มเอและผลข้างเคียงก่อนและทุกสัปดาห์ของการศึกษา ผลการศึกษาพบว่า การฉีดล้างช่องปากด้วยน้ำยาทั้งสามอย่าง สามารถลดคราบจุลินทรีย์และเหงือกอักเสบได้อย่างมีนัยสําคัญทางสถิติ (P<0.05) ในทุกสัปดาห์เมื่อเทียบกับก่อนการศึกษาและเมื่อสิ้นสุดการศึกษาไม่พบความแตกต่างอย่างมีนัยสําคัญทางสถิติ (P > 0.05) ระหว่างน้ำยาคลอร์เฮกซิดีนทั้งสอง ความเข้มข้น อาการไม่พึงประสงค์ของน้ำยาคลอร์เฮกซิดีนความเข้มข้นร้อยละ 0.03 น้อยกว่าความเข้มข้นร้อยละ 0.06 อย่างไรก็ตามพบความแตกต่างอย่างมีนัยสําคัญทางสถิติ (P < 0.05) สําหรับดัชนีคราบจุลินทรีย์และดัชนีดัดแปลงพีเอ็มเอ (ส่วนเหงือกสามเหลี่ยมระหว่างฟัน) ของน้ำยาคลอร์เฮกซิดีนทั้ง 2 ความเข้มข้นกับน้ำยาควบคุมจากข้อมูลแสดงให้เห็นว่า การฉีดล้างแบบมีแรงดันด้วยน้ำยาคลอร์เฮกซิดีนความเข้มข้นร้อยละ 0.03 จํานวน 400 มิลลิลิตรวันละ 1 ครั้ง เสริมการแปรงฟันตามปกติ เป็นวิธีที่เหมาะสมมากกว่าใช้คลอร์เฮกซิดีนที่ความเข้มข้นร้อยละ 0.06 ในการลดคราบจุลินทรีย์และเหงือกอักเสบ ดังนั้นอาจกล่าวได้ว่า เครื่องฉีดล้างแบบมีแรงดันมีประสิทธิภาพ ในการควบคุมคราบจุลินทรีย์และเหงือกอักเสบโดยน้ำยาคลอร์เฮกซิดีนจะช่วยเสริมประสิทธิภาพ

การเกิดฟันกรามแท้ซี่ที่สองที่เป็นฟันชน อาจมีสาเหตุได้หลายประการ การรักษาฟันกรามแท้ซี่ที่สองที่เป็นฟันชน และมีสภาวะความรุนแรงของฟันชนปานกลาง สามารถทําได้โดยอาศัยเครื่องมือทางทันตกรรมจัดฟันทั้งชนิดติดแน่นและชนิดถอดได้ รายงานผู้ป่วยนี้ ได้แสดงถึงวิธีการประยุกต์ใช้เครื่องมือจัดฟันชนิดถอดได้ ร่วมกับการใช้ยางเพื่อตั้งฟันกรามล่างแท้ซี่ที่สองที่มีลักษณะเป็นฟันชน ส่วนประกอบที่สําคัญที่ใช้ในการตั้งฟัน ได้แก่ การใช้ตะขอดัด 2 ตัว ร่วมกับการดึงยางขนาดเส้นผ่าศูนย์กลาง 1/8 นิ้ว แรง 3.5 ออนซ์ ภายหลังผู้ป่วยใช้เครื่องมือนี้ 8 สัปดาห์ ฟันกรามแท้ซี่ที่สองที่เป็นฟันชนนั้นสามารถขึ้นสู่ช่องปากได้ตามปกติ เครื่องมือจัดฟันชนิดนี้สามารถทําได้ง่ายและใช้ได้ผลดี ซึ่งทันตแพทย์ทั่วไปสามารถนําเทคนิคนี้มาใช้ในการรักษาได้ ถึงแม้ว่าจะมีอุปกรณ์ทางทันตกรรมเพียงบางชิ้นเท่านั้น

ความยาวใบหน้าเป็นเกณฑ์สําคัญประการหนึ่งในการวินิจฉัยความสวยงามของใบหน้า ในการวางแผนรักษาผู้ป่วยที่กําลังเจริญเติบโตทันตแพทย์จัดฟันต้องมีมาตรการที่เหมาะสมเพื่อควบคุมหรือเปลี่ยนแปลงความยาวใบหน้า บทความนําเสนอปัจจัยที่เกี่ยวข้องกับการเปลี่ยนแปลงความยาวใบหน้า ประกอบด้วย การเจริญเติบโตของใบหน้า การบําบัดทางทันตกรรมจัดฟันด้วยเครื่องมือประเภทต่าง ๆ กล้ามเนื้อบดเคี้ยว ทางเดินหายใจ และปัจจัยอื่น ๆ โดยทั่วไปปัจจัยดังกล่าวข้างต้นโดยเฉพาะการบําบัดทางทันตกรรมจัดฟันมักทําให้ความยาวใบหน้าเพิ่มขึ้น ซึ่งทําให้ผลการรักษาด้อยลงในผู้ป่วยที่มีโครงสร้างใบหน้ายาว แต่จะเป็นผลดีต่อผู้ป่วยโครงสร้างใบหน้าสั้น

In Dentistry, bonding between resin cement and metal alloy plays an important role in many clinical applications. From several studies, the failures at resin-metal interface are the most frequently reported. Numerous methods are available for bonding resin to alloy. The sillica coating methods have been developed to improve chemo - mechanical retention. Three different silica coating methods were described. The original Silicoater system is a pyrolytic deposition of an intermediate SiOx layer to the alloy surface. Mycobacterium avium-M. intracellulare complex (MAC) is the most frequent cause of opportunistic bacterial infection in patients with AIDS. Previous studies have indicated that liposome-encapsulated aminoglycosides are highly effective in treating MAC infections in mice. We investigated whether the fluoroquinolone sparfloxacin is effective in treating MAC infection in the murine macrophage-like cell line J774. Sparfloxacin was encapsulated in the membrane phase of multilamellar liposomes composed of phosphatidylglycerol-phosphatidylcholine-cholesterol (1:1:1 molar ratio). MAC-infected macrophages were treated for either 24 h or 4 days with free or liposome-encapsulated sparfloxacin. It is estimated that up to 10% of the American public has some form of Temporomandibular Joint Disorder (TMD), of whom perhaps 5% may seek or need treatment. The purpose of this study is to demonstrate that chewing gum for 5 minutes on the involved side at levels less than maximal contraction will decrease resting muscle tone in the masseter muscle as demonstrated through surface electromyography.; This study examined 40 normal subjects as well as 6 people diagnosed with TMD. The purpose of this study was to determine the transformation temperatures for the austenitic, martensitic, and rhombohedral (R) structure phases in representative as-received commercial nitinol (NiTi) orthodontic wire alloys, to reconcile discrepancies among recent publications. Specimens were examined by differential scanning calorimetry (DSC) over a temperature range from approximately −170° C to 100° C, with a scanning rate of 10° C per minute. Two different pathways, with the intermediate R structure either absent or present, were observed for the transformation from martensitic to austenitic NiTi, whereas the reverse transformation from austenitic to martensitic NiTi always included the R structure. This article discusses the current information on the mechanism of the biological signaling that takes place to bring about orthodontic tooth movement.
Currently, there are two general theories as to how orthodontic mechanical force is translated to biologically meaningful signals for the cells of the periodontium. These include a cellular-humoral response and a bioelectric response.

The purpose of this study is to evaluate skeletal and dental asymmetry in unilateral class II malocclusion. A study group of 25 unilateral subdivision patients was compared to 25 class I malocclusion patients serving as the control group. Submento-vertical and frontal radiographs were used for the evaluation. Symmetry was evaluated by comparing measurements between right and left sides for both groups and matching the middle points with the midsagittal axis. The purposes of this study were to determine if a relationship exists between levels of dentist supervision and the ability of dental hygienists in nontraditional settings to provide care, and to ascertain differences in the ability of dental hygienists practicing in legal jurisdictions with direct, indirect, general, and no dentist supervision. The Dental Hygiene supervision in Nontraditional settings Questionnaire (DHSNSQ) was sent to a purposive sample consisting of dental hygienists employed in nontraditional settings within one of the four levels of dentist supervision. รายงานผู้ป่วยหนึ่งราย เป็นหญิงจีน อายุ 36 ปี มีก้อนเนื้องอกสีแดงจัด ลักษณะอ่อนนุ่ม เลือดออกง่าย งอกขึ้นมาจากแผลถอนฟันของเบ้ากระดูกรองรับฟันที่ไม่หายและเหงือกบริเวณ 22 วินิจฉัยทางคลินิกว่าเป็นมะเร็ง ลักษณะจุลพยาธิวิทยาของเนื้องอกนี้ ประกอบด้วยไซโตทรอโฟบลาสและซินไซติโอทรอโฟบลาสและพบบริเวณที่มีเลือดออกและเนื้อตายจากขาดเลือดอยู่ใต้เยื่อบุผิวช่องปาก ผลการวินิจฉัยทางจุลพยาธิวิทยาเป็น "เมตาสแตติคคอริโอคาร์ซิโนมาของเหงือกและกระดูกแม็กซิลลา บริเวณ 22"

การสํารวจภาคตัดขวางเพื่อศึกษาสภาวะโรคฟันผุของนักเรียนอายุ 12 ปี ในโรงเรียนสังกัดกรุงเทพมหานคร 18 โรง จํานวน 338 คน พบว่า มีอัตราชุกของโรคฟันผุร้อยละ 82.8 ค่าเฉลี่ยฟันผุถอนอุดเป็นซี่ ± ส่วนเบี่ยงเบนมาตรฐานเท่ากับ 2.94±2.20 ต่ำกว่าผลการสํารวจในปี พ.ศ. 2534 โดยกองทันตสาธารณสุข สํานักอนามัยกรุงเทพมหานคร แสดงว่าผลการดําเนินงานทันตสาธารณสุขของกองทันตสาธารณสุข สํานักอนามัย กรุงเทพมหานคร ประสบความสําเร็จในระดับหนึ่ง แต่อัตราฟันผุถอนอุดของฟันกรามแท้ซี่ที่หนึ่งยังคงสูงมาก คือ ปริมาณฟันผุถอนอุดของฟันกรามแท้ซี่ที่หนึ่งบนขวา ฟันกรามแท้ซี่ที่หนึ่งบนซ้าย ฟันกรามแท้ซี่ที่หนึ่งล่างซ้าย และฟันกรามแท้ซี่ที่หนึ่ง ล่างขวา เท่ากับร้อยละ 46.5, 47.9, 63.0 และ 65.4 ตามลําดับ ดังนั้นจึงแนะนําให้ใช้สารเคลือบหลุมร่องฟัน ป้องกันการผุของฟันกรามแท้ซี่ที่หนึ่งเป็นมาตรการหนึ่งของการดําเนินงานทันตสาธารณสุขในกลุ่มเป้าหมายคือเด็กนักเรียนประถมศึกษาของกองทันตสาธารณสุข สํานักอนามัย เพื่อให้ประสบความสําเร็จดียิ่งขึ้น
