The body of knowledge that exists regarding cracked teeth is limited. The purpose of this study was to determine the prevalence of cracks among extracted teeth. This retrospective longitudinal cohort study included patients of the Virginia Commonwealth University School of Dentistry that underwent extraction procedures over a 6 year period. The sample consisted of 20,408 patients and 40,870 teeth. Statistical analysis software was used to identify diagnoses of a crack, fracture, or split tooth prior to extraction of the tooth by analyzing the Electronic Health Record (EHR) (axiUm™, Version 6.03.03.1035, Exan Corporation, Vancouver, BC, Canada). The aim of this study was to evaluate whether volumetric changes in persistent periapical lesions can be detected in follow-ups six months to five years after apicoectomy using cone-beam computed tomography. Patients with a previous treatment history of apicoectomy and for whom a pre-surgical CBCT scan was taken between November 2010 and December 2015 were invited to participate in the study. A post-surgical CBCT image of the treated tooth was obtained at the recall visit. Objective: The purpose was to compare orthodontists’ and parents’ perception of orthodontic treatment outcomes in the anterior-posterior (AP) dimension. Assessment of treatment time and compliance were also investigated.
Material and Methods: Parallel surveys for orthodontists (n=1000) and parents (n=750) displayed occlusions from 3 mm Class III (Cl III:3) to 3 mm Class II. Participants rated occlusal relationships on a 100 mm VAS from least to most acceptable (0-100).
Results: 233 orthodontists (23%) and 243 parents (32%) responded. Orthodontists (mean=93.9, 25.9) and parents (mean=80.7, 40.9) rated Class I (Cl I) occlusion most and Cl III:3 least acceptable. No significant difference was …

A Comparison Of Moderate Oral Sedation Drug Regimens For Pediatric Dental Treatment: A Pilot Study, Ojas A. Parikh

Purpose: Compare moderate oral sedation of pediatric patients using Hydroxyzine and Meperidine with either Diazepam or Midazolam in management of pediatric dental patients.
Methods: Randomized, double-blind, crossover pilot study of patients 3 to 7 years of age requiring two sedation visits. Frankl and Houpt behavior scores recorded at injection time, initiation of treatment and 100% oxygen at end of treatment. Postoperative phone call surveys conducted within eight hours and within 24 hours of discharge. Wilcoxon Signed-Rank tests, Fisher’s Exact Chi-squared test and 0.10 significance level.

Objective To investigate the effect of adding ferrous sulphate (FeSO4) as a chemical activator to hydrogen peroxide tooth bleaching gels on color change and treatment time. Methods One hundred and twenty extracted human upper premolars, hue A or B, were selected. One enamel-dentin specimen was prepared from each tooth. The specimens were divided into twelve groups (n=10), based on bleaching product, FeSO4 and treatment time. Three in-office bleaching products were used in this study: Opalescence® Boost TM, PolaOffice® and ZOOM!® Whitespeed TM. The obstructive sleep apnea–hypopnea syndrome (OSAHS) is a common sleep-related breathing disorder characterized by repetitive obstructions of the upper airway during sleep. Nasal continuous positive airway pressure (CPAP) is the primary choice of treatment for OSAHS, but many patients are unable or unwilling to comply with the treatment. The modification of pharyngeal patency by oral appliances (OA) is an alternative treatment for OSAHS. This article provides a brief review and information of the oral appliance application in treating OSAHS.

This report describes a non-surgical orthodontic treatment for a 36-year-old male patient who had Class II division 2 malocclusion (Class II/2) with deep bite more than 100%, gingival recession, and periodontal abscess in the mandibular anterior region. Patient had history of trauma in region. The surgical intervention was once suggested but was refused by the patient. The compromised but acceptable final results were achieved by nonsurgical approach. The treatment plan included periodontal control, maxillary incisor intrusion and restoration of the collapsed occlusion. The total treatment duration was 43 months. The maxillary canine impaction is a specific topic in orthodontic treatment. Because of its important role in esthetics and occlusal function, preservation and alignment of the impacted canine would always be the priority in teenage orthodontics. In addition to the routine analysis of the space distribution for the impacted canines, orthodontists should also consider the growth potential to achieve proper treatment outcome. Facial asymmetry is common in humans. Significant facial asymmetry causes both functional as well as esthetic problems. The etiologies of facial asymmetry include congenital disorders, acquired diseases, and traumatic and developmental deformities. The causes of many cases of developmental facial asymmetry are indistinct. The principle of facial asymmetry management would be correcting the underlying disorder. Introduction: Medical disputes have received increasing attention in recent years. Under the assistance of the academic and medical affairs committees of the Taiwan Association of Orthodontists, this study explored medical disputes among orthodontists performing orthodontic treatments in Taiwan in 2016. Methods: This study adopted a questionnaire survey, which was divided into three sections, namely the surveyed orthodontists’ demographic information, medical services, and subjective views on orthodontic-related medical disputes. Early treatment of Class II, Div. 1 malocclusion is a much debated approach and some evidence based studies using a single appliance concluded that this approach was often not worthwhile. This article will discuss the pros and cons of early treatment with functional appliances and present guidelines for early treatment. The role of facial growth and dentoalveolar development, and growth intensity will be discussed as they relate to treatment outcome. A retention protocol for early treatment with functional appliances will be suggested.

Pulp vitality may occasionally change while tooth experiencing orthodontic movement, and orthodontists are the particular group who concern this phenomenon if it happens. The respiration rate of dental pulp will be depressed inevitably in short term when tooth is subject to orthodontic force. If force level is extreme, circulatory interruption can occur and further induce pulp necrosis. When orthodontic movement on pulpalcompromised teeth (e.g. teeth with trauma history or large restorations) is required, the pulp response to orthodontic force should be monitored. This case report illustrates a combined orthodontic-orthognathic surgical treatment option for correction of Class II division 2 malocclusion. A 31-year-old female patient, presenting a chief complaint of gummy smile and backward position of anterior teeth, came to seek for treatment. She had skeletal Class II with hypodivergent facial type and mild asymmetric face. Intraoral examination revealed a severe deep bite with 13 mm overbite. A 20-year-old young male complained of his protrusive and deviated chin. He also presented occlusal plane cant, anterior open bite and unrestorable maxillary right first molar and mandibular left first molar. The severe maxilla-mandibular discrepancy was solved with surgery-first double jaw orthognathic surgery. The space of two extracted first molars was closed by protraction of the second and third molars after surgery. Two protraction techniques were used in this case, including closing maxillary right first molar space with reciprocal retraction, and closing mandibular left first molar with one miniscrew as an absolute anchorage. A rare case-a 11-year-old boy with ectopic eruption of all the permanent second molars associated with resorption of all the permanent first molars was reported. Severe root resorption was found in the mandibular first molars, while moderate root resorption was found in the maxillary first molars. This study aims to consider the microbial distribution in oral disease, as well as gene analysis and expression, in elucidating: 1, the fundamental underpinnings of oral disease, and 2, the potential relationship between oral diseases and systemic health. A key focus is identifying the microbiota associated with oral disease manifestations characterized by both conventional microbiological and molecular methods. Variations in the observed microbial populations characterized by conventional and molecular approaches have been identified for caries, periodontitis, peri-implantitis, and stomatitis. Porphyromonas gingivalis, a Gram negative anaerobe implicated in the progression of periodontal disease, is capable of surviving and causing infection despite high levels of reactive nitrogen species found in the oral cavity due to its efficient nitrosative stress response. HcpR is an important sensor-regulator that plays a vital step in the initiation of the nitrosative stress response in many Gram negative anaerobic bacteria. We employ a combination of X-ray crystallography, SAXS, resonance Raman spectroscopy, UV-Vis spectroscopy, and molecular biology techniques to better understand this key regulator. This report describes the diagnosis and prosthodontic management of 2 patients with a history of chronic gastroesophageal reflux disease and worn dentition. Different treatment approaches were used for oral rehabilitation. Use of conventional and contemporary restorative materials resulted in functional and esthetic prosthodontic rehabilitation with a favorable prognosis.
Gastroesophageal reflux disease (GERD) is a “condition which develops when the reflux of stomach contents causes troublesome symptoms and/or complications." Purpose: Interleukin-1 beta (IL-1β) is a cytokine that participates in the regulation of immune responses and inflammatory reactions. It is hypothesized that IL-1 levels may be elevated in patients suffering from temporomandibular joint dysfunction. The purpose of this study was to determine the association of IL-1β expression with TMD using an immunohistochemical approach to evaluate the joint disc.

The oral cavity is the first component of the digestive tract, which is delimited by the lips anteriorly and the oropharynx posteriorly. The oral cavity functions as a protective barrier and is an essential component for speech and swallowing, mastication, digestion, and taste sensation.
The oral examination comprises a uniform and consistent inspection of the head and neck and an intraoral evaluation of the hard and soft tissues (see the images below) in conjunction with a thorough medical and dental history. Objectives
To examine the relationships between fluoride intake levels and fluorosis of late‐erupting permanent teeth.
Methods
The current study used information collected from 437 children in the longitudinal Iowa Fluoride Study. Participants' fluoride intake information was collected using questionnaires from birth to age 10 years. Estimated mean daily fluoride intake was categorized into low, moderate, and high intake tertiles for each age interval (2‐5, 5‐8, and 2‐8 years). Bivariate analyses were performed to study the relationships between self‐reported fluoride intake levels during three age intervals and dental fluorosis.

Objective: Temporomandibular disorders (TMD) are a group of conditions affecting the temporomandibular joint (TMJ), leading to jaw dysfunction, joint and muscle pain, and a decrease in quality of life. A communication network of pro- and anti-inflammatory mediators called cytokines maintains the homeostasis of the TMJ. This review will focus on the Interleukin (IL) family of cytokines, which have been quantified in TMJ synovial fluids in a variety of studies. IL-1α and IL-1β have pro-inflammatory effects, while the endogenous receptor antagonist (IL-1RA) inhibits the pro-inflammatory effects of IL-1.
