INTRODUCTION: Monitoring an athlete’s psychological, physiological, and performance level is important when preparing for a major competition. No study to date has tracked a high-level weightlifter peaking for a major competition all the way up to the day of competition. Membrane-coated extracellular vesicles (EVs) released by cells can serve as vehicles for delivery of biological materials and signals. Recently, we demonstrated that alcohol-treated hepatocytes cross-talk with immune cells via exosomes containing microRNA (miRNAs). Here, we hypothesized that alcohol-exposed monocytes can communicate with naive monocytes via EVs. We observed increased numbers of EVs, mostly exosomes, secreted by primary human monocytes and THP-1 monocytic cells in the presence of alcohol in a concentration- and time-dependent manner. Background As late provision of specialist care, prior to starting dialysis therapy, is believed to be associated with increased morbidity and mortality, a systematic review was undertaken to evaluate clinical outcomes relating to early versus late referral of patients to nephrology services.

Methods: Cochrane Central Register of Controlled Trials (CENTRAL), MEDLINE and EMBASE were searched up until September 2008 for studies of early versus late nephrology referral in adult (>18 years) patients with chronic kidney disease. Early referral was defined by the time period, at which patients were referred to a nephrologist.

Findings: No randomized, controlled trials were found. …
