Metabolic disorders affect around thirty-four percent of the population in the United States. Among these disorders is lactose intolerance, which results from diminished production of the human lactase enzyme. This disorder and others like it are genetically determined and cannot be cured. However, the use of transformed bacteria implanted in the colon may provide a means by which the faulty pathway can be bypassed. It is estimated that over 370 million people worldwide suffer from diabetes, and this number is increasing rapidly. Normally, β-cells in the pancreas secrete insulin, which is necessary for proper glucose absorption and storage. Both Type 1 and Type 2 diabetes are characterized by decreased functional β-cell mass and insulin production, and increased circulating glucose and fatty acid levels. Diabetics’ pancreata maintain small amounts of functional β-cells, but these surviving cells are damaged and destroyed over time due to the harmful effects of hyperglycemia and hyperlipidemia. Diabetes, a disease characterized by the inability of the body to maintain a normal blood glucose level, continues to affect the lives of many. In both Type I and Type II diabetes, eventual β-cell destruction results in decreased β-cell mass. Regeneration of functional β-cells and protection of such, could help reverse the effects of this disease and could possibly lead to a cure. Many studies have been done to increase functioning β-cell mass, but protecting regenerated β-cells from further apoptotic insults could greatly increase the effectiveness of β-cell transplants and other future treatments of the disease.

Type 1 and Type 2 diabetes are classified as a decrease in functional ß-cell mass, which results in impaired blood glucose regulation. Functional ß-cell mass is defined as the glucose stimulated insulin secretion rate multiplied by the total cellular mass which is determined by the proliferation and cell death rates. Though ß-cell mass proliferation rates generally decrease by adolescence, obesity and pregnancy have been shown to be times of significant ß-cell proliferation (1). Background and Purpose: In the United States, 79 million people are pre-diabetic: most have no symptoms. If left untreated, 37% will develop diabetes within 4 years. Fourteen percent of United States health care expenditures are related to diabetic complications. This Evidenced Based Project (EBP) involved a behavioral lifestyle change with the purpose of 7% weight loss and a minimum of 150 minutes of physical activity a week. It was also designed to reduce risk factors and glycated hemoglobin A1c (HgA1c) levels.

Type 1 and Type 2 diabetes are major global health concerns. Both types of diabetes result in loss of functional β-cell mass, which is defined as the β-cell number multiplied by insulin secretion rate. The number of β-cells is derived from the cellular proliferation and death rates. Diabetes is increasing globally and nationally. Diabetes complications and costs can be reduced through modification of lifestyle risks and diabetes self-management education (DSME). The Cooperative Extension System (CES) is uniquely positioned to implement DSME. This study assessed the role and impact of the Cooperative Extension System (CES) in DSME. A survey was sent to CES professionals throughout the U.S. a total of 43 participants provided information on 73 DSME programs. Image-based dietary records have limited evidence evaluating their performance and use among adults with a chronic disease. This study evaluated the performance of a 3-day mobile phone image-based dietary record, the Nutricam Dietary Assessment Method (NuDAM), in adults with type 2 diabetes mellitus (T2DM). Criterion validity was determined by comparing energy intake (EI) with total energy expenditure (TEE) measured by the doubly-labelled water technique. Relative validity was established by comparison to a weighed food record (WFR). Inter-rater reliability was assessed by comparing estimates of intake from three dietitians. The purpose of this research was to examine food security for people with prediabetes participating in the Supplemental Nutrition Assistance Program (SNAP), focusing on their food choice decisions and coping strategies over a 30-day benefit cycle that potentially increases the risk of Type 2 diabetes. A cross-sectional, quantitative design based on food choice process model constructs was used. SNAP participants (n = 36) with prediabetes, aged 21-70 years, were recruited as outpatients from Bronx Lebanon Hospital and completed self-reported questionnaires on demographics and health, food security, and food frequency. Grape pomace contains considerable amounts of polyphenols and it has been reported to exhibit an inhibitory activity against alpha- glucosidases. Due to dramatic increases in type II diabetes, studies on insulin resistance are especially applicable to this current health issue. Previous epidemiological studies examining selenium’s effect on prostate cancer observed a possible correlation between increased risk for type II diabetes mellitus and increased selenium levels in humans (Lippman, S.M.2009). However, in other studies contradictory results have been recorded. Due to these inconsistencies we are investigating the effects of selenium on insulin resistance. If increased dietary selenium causes insulin resistance, we will examine the mechanism by which this occurs.

Numerous epidemiology studies suggest a correlation between exposures to polychlorinated biphenyls (PCBs) and the development and severity of type 2 diabetes (T2D); however, mechanisms remain largely unknown. Previous studies demonstrated that PCBs that are ligands of the aryl hydrocarbon receptor (AhR) promote the expression of proinflammatory cytokines, including tumor necrosis factor-α (TNF-α), that are linked to insulin resistance in adipocytes. To explore potential mechanisms linking PCB exposures to diabetes, we developed a mouse model of glucose and insulin intolerance induced by acute and chronic exposures to PCB-77. Long term consumption of a high fat diet (HFD) increases the risk of developing Metabolic Syndrome and type 2 diabetes. This led us to hypothesize that long term HFD consumption impairs immune tolerance to the intestinal bacteria. Our studies had two goals. First, we characterized the effect of long term HFD consumption on the systemic immune response by comparing C57BL6 mice fed a HFD and low fat diet (LFD). Lipodystrophy caused by fat deficiency contributes to metabolic disease for which several treatment modalities have been implemented, with leptin therapy being the most effective to date. In addition to playing a role in energy homeostasis and metabolism, leptin was also shown to play a pro-thrombotic role in mice. This role was not examined in fatless mice, neither was thrombosis measured. Overweight and obesity are paradoxically associated with better survival in patients with heart failure (HF). This association is poorly understood, in part because the impact of diabetes (DM) on survival of overweight and obese HF patients has not been considered. Inflammation may contribute to worse survival in overweight and obese HF patients with DM, and levels of inflammation may be associated with fruit and vegetable consumption. Studies have reported mixed results for people with or without diabetes with cognitive decline or Alzheimer's disease (AD). Cognitive decline and AD among people with diabetes will be the focus of much discussion since results have been controversial.

The study examined whether diabetes is associated with cognitive decline and whether it is an independent risk factor for the development of AD among elderly residents of Cache County, Utah.

The Diabetes Stepping up to the Plate program was developed to determine the effectiveness of food portion based diabetes education. One hundred fifty-one individuals enrolled in the diabetes series. Food portion knowledge and skills tests, height, weight, and waist-to-hip ratio (WHR) were measured pre- and post-program. Data was analyzed through paired t-tests and correlations. Evaluation showed a decrease in HbA1C, waist/hip circumference, WHR, and BMI, and an improvement in food portion knowledge and skills. Seventy-five participants completed pre- and post-data collection parameters, although more participants completed all classes in the series. Extension can play a role in successful diabetes education.

The purpose of this study was to increase knowledge of diabetes and promote healthy lifestyle behaviors to reduce diabetes onset in a community with a relatively high incidence of type 2 diabetes. Three hundred eighty seven students grades 7 – 12 participated in the program and taste test. Changes in knowledge before and after an educational program focusing on diabetes prevention were measured by comparison of identical pre, post, and retention tests. The effect of time on taste threshold was examined in 30 diabetics and 30 control subjects (ages 22-30) who had participated in a sensory study 14 years previously. Detection and recognition taste thresholds for sweet (sucrose), salty (sodium chloride), sour (citric acid), and bitter (quinine sulfate) were assessed using triangle testing. Food preferences related to concentration of the stimuli in model food systems were tested using a nine-point hedonic scale. Mashed potatoes were used as the carrier for different levels of salt.
