The Influence Of Sports-Related Concussion On Lower Extremity Injury Risk In Adolescent And Collegiate Athletes, Jason M. Avedesian

Sports-related concussion (SRC) represents a major epidemiological concern in the adolescent and collegiate athlete populations, with conservative estimates of approximately four million SRCs occurring each year in the United States. While the majority of previous research has emphasized the management and recovery of the cognitive aspects of SRC, recent data indicates subtle motor behavior is altered following a concussive event. Injury surveillance literature indicates that adolescent and collegiate athletes are at an approximately 2–4 greater risk for lower extremity (LE) injury following an SRC when compared to matched controls and pre-SRC LE injury rates. Context: Lateral ankle sprains are one of the most commonly occurring sport-related injuries. The somatosensory system is primarily responsible for monitoring the position of the joints in space during movement. This system functions by way of neurological structures that provide proprioceptive and neuromuscular input to the brain. Motor control of the lower extremity relies on proprioceptive input to the central nervous system (CNS) and local neuromuscular control of each joint to produce coordinated, global muscular responses to external events. The purpose of this dissertation was to examine the systemic neuromechanical implications in individuals who have had an ACL reconstruction (ACLR) compared to healthy controls. The specific aims addressed were to: 1) examine differences in inter-limb coordination during walking at different speeds, 2) examine differences in trunk, neck and head acceleration during gait, and 3) investigate whether the reaction time responses assessed during stepping are negatively affected by ACLR.

The findings of study 1 revealed that maximal coordination stability was achieved when walking at the person’s preferred gait speed. However, individuals with a previous ACLR exhibited reduced coordination stability between …

Purpose This study aimed to compare the cognitive measures of reaction time and dual-task performance among recreationally active, master athletes, and sedentary older adults aged 50 years or older. Methods 59 late middle-aged and older adults between the ages of 50 to 88 years old participated in reaction time and dual-task tests of which consisted of multiple trials on the same day. Subjects were placed either into the recreationally active, masters athlete, or sedentary activity level group based on the Rapid Assessment of Physical Activity (RAPA) form. Context: Various intrinsic factors such as high exposure, poor endurance of core muscles, previous injury, strength deficits, suboptimal neurocognitive function, and orthopedic abnormalities have been found as predictors for sprains and strains among collegiate football players. Objective: Assess the applicability of pre-participation assessments as predictors of core or lower extremity injury. Design: Cohort Study. Setting: National Collegiate Athletic Association Division I football program. Patients or Other Participants: Athletes who underwent mandatory pre-participation examinations before preseason football training over two consecutive seasons (n=225). The purpose of this study was to measure the choice reaction time and myoelectric activity of the right and left rectus abdominus, and right and left external obliques required to initiate movement in response to a visual stimulus that signaled performance of four different closecombat movements (left or right cross and left or right dodge). Aging is associated with the increased onset of diseases such as cognitive impairment, and ultimately dementia. Participants, 55+ years, with a self-reported cognitive complaint completed a 6-month exercise intervention. They were randomized to either a multiple modality exercise program (M2), consisting of aerobic, stretching and balancing exercises or a multiple modality plus mind motor program (M4), whereby a square stepping exercise was incorporated. Participants were assessed for reaction time (RT), movement time (MT) and % errors at baseline and at 6-months using an eye tracking 1000 system. During activities of daily living, there are demands placed on the body both cognitively and physically. These demands are multiplied when performing an athletic endeavor. Many studies have demonstrated the negative impact dehydration has on cognitive and muscle functions, independently. The purpose of this study is to investigate the effect of dehydration on central nervous system and peripheral nervous system fatigue as expressed through concomitant factors of cognitive function and balance. Objectives: Quantify the effects of cold water immersion of the ankle on fractioned response time of the dominant lower limb. Design and Setting: A 2x2x5x5 crossover design with repeated measures on time and treatment directed data collection. The independent variables were gender, treatment, time (pretreatment, and post 15 seconds, 3 minutes 6 minutes and 9 minutes) and trial (5 trials for each time group). Objective: To examine the influence of a 6-week training program on the electromechanical delay (EMD) and reaction time of the peroneus longus muscle.

Design and Setting: The study was guided by a 2 x 2 factorial design with repeated measures on the time factor.
