There are currently three options when it comes to managing obesity: (1) making nutritional and activity changes; (2) weight loss medications; and (3) weight loss surgery. The existing limited options for managing obesity are even more restricted for Medicare patients. Making lifestyle changes might not be effective for a variety of reasons, including persistent food cravings and experiences of weight stigma from healthcare providers. To qualify for weight loss surgery, Medicare patients must satisfy several criteria in addition to the already specific weight loss surgery requirements. The right to reproductive privacy is under attack in the United States, and it is losing ground. Dobbs v. Jackson Women’s Health Organization, the Supreme Court’s 2022 decision that overruled Roe v. Wade’s constitutional protection of abortion and jeopardized privacy rights by proxy, reflects this losing posture. Scholarship in reproductive privacy varyingly critiques federal privacy initiatives, evaluates regulatory interventions, and proposes civil rights frameworks in response to Dobbs. This column addresses whether a dental practice can hire a Certified Registered Nurse Anesthetist (CRNA) to administer anesthesia. A CRNA can be hired if the requirements of Michigan's MCL 333.17210 and the Michigan Administrative Code are met. The CRNA must have sufficient experience or advanced degrees and work collaboratively in a patient-centered care team. The dental office must have policies for immediate clinical response, and all involved personnel must be certified in basic cardiac life support. Additional rules apply depending on the sedation level provided.

Innumerable inventions implicate public health—including drugs, vaccines, dietary supplements, and sewage treatment plants. Over the past century, the Patent Office and the courts have modulated the ability to obtain or enforce patents for these inventions—whether in response to a public health crisis or to protect the credulous public from unscrupulous inventors. Over the coming decades, physicians in the US can expect to see more patients with disabilities as medical advances extend the lives of persons with complex congenital or early-onset conditions and as the population ages with a higher incidence of multimorbidities and functional impairments. As reported by the Centers for Disease Control and Prevention, the overall age-adjusted disability prevalence in 2021 among adults aged 18 years or older was 27.2%, with estimates of 12.1% for mobility disability, 6.1% for hearing disability, and 4.8%for vision disability. In 2021, however, only 63.2%of persons with any disability aged 18 to 44 years reported …

Medical-legal partnerships (MLPs) try to mitigate health inequity by uniting legal and health professionals to respond to legal determinants of patients’ health. While there is a long tradition of “patients-to-policy” work in MLPs, the current empirical evidence base has evaluated MLP effectiveness by assessing benefits to individual patients, clinicians, and hospital and legal systems. This article calls for future research to measure how community power, which includes shifting power to impacted communities to develop and lead equity-focused agendas, is built as both a process and an outcome of MLPs.

MLP teams meet the needs of individual patients while also detecting structural problems at the root of health inequities and advancing solutions at the institutional, community, and system levels. Yet MLPs today operate in limited settings and survive on scant budgets. Expanding their impact requires secure funding. Economists have identified important adaptations that immigrant workers have made to weather economic crises. During times of economic contraction, immigrant workers have moved across industries or geographical locations, downshifted to part-time work, and accepted lower wages to stay employed. Evidence from the Great Recession (2007–2009) shows the benefits of that economic resilience: immigrant workers were more likely than native-born workers to remain continuously employed, to have shorter periods of unemployment when they lost their jobs, and to regain jobs more quickly in the recovery period. Of course, these adaptations had significant personal costs for immigrant workers and their families, but …

The MDA Foundation's Mission of Mercy (MOM) dental clinic in Flint provided free dental care to 659 patients over June 14-15, delivering 6,678 procedures valued at $1,156,285. The 120-chair clinic, set up by volunteers, saw patients from 35 Michigan counties and neighboring states. Patients, many of whom queued overnight, received an average of ten procedures each. Volunteers, including retired dentists, performed a wide range of services, from cleanings to complex surgeries. Translators were available to assist non-English speakers, and patients were informed about ongoing free or reduced-cost care options. Policy declarations from the G7 and other high-level meetings call for increased incentives for antimicrobial research and development (R&D). Governments fund push incentives like CARB-X and GARDP, and G7 countries are now designing pull incentives—financial rewards given to manufacturers post-market authorization that are intended to encourage the creation and introduction of novel antimicrobials. Germany has declared previously at the G7 that it has developed a pull incentive that will increase revenues from sales of important new antimicrobials, principally by exempting them from some aspects of health technology benefit assessments and reference pricing, which should result in higher prices.
