Apoptosis, the programmed death of cells, is primarily regulated by a delicate balance between pro-apoptotic and anti-apoptotic signals. The Bcl-2 (B-cell lymphoma 2) family of proteins acts as anti-apoptotic agents, promoting cell survival. Dysregulation of these proteins is a common occurrence in conditions such as cancer and fibrosis, where overexpression of anti-apoptotic members can foster tumor cell survival and fibroblast activation. In this study, our aim was to explore the therapeutic potential of Bcl-2 inhibitors, both as a small molecule (specifically Navitoclax (Navi)), inhibitor and as Bcl-2 siRNA, for targeted treatment. Despite notable progress in lipid nanoparticle (LNP)-mediated gene delivery, achieving selective transfection of specific cell types, such as cancer cells, remains a significant hurdle, hindering the advancement of innovative gene therapies. In this study, we engineered an LNP formulation encapsulating plasmid DNA (pDNA) encoding the monomeric Green Lantern (mGL) fluorescent reporter protein. The DT7 peptide ligand targeting human transferrin receptor 1 (hTfR1) was also conjugated to the LNP surface for targeted delivery to hTfR1-expressing cells. Optimization of LNP composition yielded favorable particle diameter, ζ-potential, yield, and pDNA encapsulation efficiency. To enhance patient safety, surgical education is increasingly incorporating simulation for formative skills assessment and training. However, many standardized assessment tools rely on human raters for performance assessment, which is resource-intensive and subjective. Simulators that provide automated and objective metrics from sensor data can address this limitation. This thesis presents an instrumented bench suturing simulator, patterned after the Clock Face (CF) radial suturing model from the Fundamentals of Vascular Surgery (FVS), for automated and objective assessment of open suturing skills by particularly focusing on biomechanical analysis of hand movements. Introduction: Activity-based locomotor training improves trunk control in children with spinal cord injury (SCI), and there is need for additional activities to allow these children to activate trunk muscles. The purpose of this research was to design, fabricate, and evaluate a rocking chair for children with SCI, and to investigate the activation of muscles during use. Sensorization of the chair to confirm muscle activation is also explored. This dissertation explores applications of representation learning and generative models to challenges in healthcare, astronautics, and aviation.
The first part investigates the use of Generative Adversarial Networks (GANs) to synthesize realistic electronic health record (EHR) data. An initial attempt at training a GAN on the MIMIC-IV dataset encountered stability and convergence issues, motivating a deeper study of 1-Lipschitz regularization techniques for Auxiliary Classifier GANs (AC-GANs). An extensive ablation study on the CIFAR-10 dataset found that Spectral Normalization is key for AC-GAN stability and performance, while Weight Clipping fails to converge without Spectral Normalization. Traditional treatment methods for glioblastoma multiforme (GBM) are largely unsuccessful, with a 5-year survival rate of 5.6%. Temozolomide (TMZ), the only chemotherapy FDA-approved for GBM treatment, is a hydrophobic prodrug that is stable in an acidic pH and begins converting to its active form at a physiological pH of 7.4. However, complete conversion of TMZ is observed only at a higher pH, and only about 30% of administered TMZ reaches the central nervous system. In sports science, ensuring maximum athlete safety and optimizing data utilization are pivotal yet leave room for further work. My project, Unbeaten SafeWare, addresses these critical issues by focusing on two primary concerns: preventing heat-related and cardiac illnesses, which are significant causes of athlete fatalities, and enhancing the transparency and utility of sports data. This initiative involves developing a shirt integrated with sensors to monitor vital signs and an athlete management system to handle data input, storage, analysis, and accessibility for athletes.

The enormous potential of multifunctional bilayer wound dressings in various medical interventions for wound healing has led to decades of exploration into this field of medicine. However, it is usually difficult to synthesize a single hydrogel with all the required capabilities simultaneously. This paper proposes a bilayer model with an outer layer intended for hydrogel wound treatment. By adding gelatin methacrylate (GelMA) and tannic acid (TA) to the hydrogel composition and using polyvinyl alcohol-carboxymethyl chitosan (PVA-CMCs) foam layer as supports, a photocrosslinkable hydrogel with an optimal formulation was created. Background: Nucleus replacement devices (NRDs) are not routinely used in clinic, predominantly due to the risk of device expulsion. Rigorous in vitro testing may enable failure mechanisms to be identified prior to clinical trials; however, current testing standards do not specify a particular expulsion test. Multiple methods have therefore been developed, complicating comparisons between NRD designs. Thus, this study assessed the effectiveness of four previously reported expulsion testing protocols; hula-hoop (Protocol 1), adapted hula-hoop (Protocol 2), eccentric cycling (Protocol 3), and ramp to failure (Protocol 4), applied to two NRDs, one preformed and one in situ curing. The high prevalence of dental caries among children and adolescents, especially those from lower socio-economic backgrounds, is a significant nationwide health concern. Early prevention, such as dental sealants and fluoride varnish (FV), is essential, but access to this care remains limited and disparate. In this research, a national dataset is utilized to assess sealants' reach and effectiveness in preventing tooth decay, particularly focusing on 2nd molars that emerge during early adolescence, a current gap in the knowledge base. Understanding the factors that contribute to the risk of hospitalization among individuals with schizophrenia is crucial for optimizing treatment strategies and improving patient outcomes. In this study, we used recent Medicaid adminis trative claims data in Missouri (from 2016 to 2019) and predicted the risk of hospitalization of patients diagnosed with Schizophrenia (N=143446). We ana lyzed the association between previous hospitalizations and the likelihood of future hospitalizations. We developed machine learning techniques, including logistic re gression and decision tree classifiers, to predict the risk of hospitalization based on demographic factors, clinical variables, and historical hospitalization records. A report issued by the Pilot Mental Health Aviation Rulemaking Committee in April 2024 underscored seven significant barriers impeding pilots from seeking necessary support for mental health concerns. In the field of brain tumor segmentation, accurately capturing the complexities of tumor sub-regions poses significant challenges. Traditional segmentation methods usually fail to accurately segment tumor subregions. This research introduces a novel solution employing Graph Neural Networks (GNNs), enriched with spectral and spatial insight. In the supervoxel creation phase, we explored methods like VCCS, SLIC, Watershed, Meanshift, and Felzenszwalb–Huttenlocher, evaluating their performance based on homogeneity, moment of inertia, and uniformity in shape and size. Current drug-eluting coatings have demonstrated controlled long-term, sustained release but have only tried to mitigate short-term burst release as a negative side effect. For applications like wound healing, there is a need for a drug-eluting coating which is adjustable in both short- and long-term release, independent of each other. We present a tunable dual-phase dual-drug delivery coating composed of drug-loaded polymer microparticles and drug-loaded hydrogel which can control short term and long term release individually in this study. This coating was created using lidocaine and dexamethasone encapsulated in poly(D,L-lactide-co-glycolide) particles combined with lidocaine- and dexamethasone-loaded poly(vinyl alcohol) hydrogel. In the first chapter, the focus is on the evaluation of digestion methods for the analysis of trace metals in mammalian tissues and the Standard Reference Material NIST 1577c. Since its introduction, vaccination has improved health outcomes tremendously. However, modern vaccination efforts are still difficult to implement, particularly in developing countries with warm climates. This includes complex and expensive cold-chain management of vaccines to maintain stability, a lack of medical personnel to administer parental vaccines, and needle phobia. As an alternative, dissolvable microneedles have recently been studied for their simple and painless application. In this thesis, I present a novel manufacturing process for polymer-based dissolvable microneedles, loaded with virus-like particles vaccines conjugated to TRIO or Sialokinin peptides to target malaria and arboviruses, respectively. Recent advancements in bioimpedance technology have demonstrated significant promise in the application of cardiac health monitoring. This research explores the design and development of a forearm-based wearable bioimpedance device for non-invasive measurement of heart rate and respiratory rate at an accuracy level comparable to medical-grade monitors. It utilizes a tetrapolar electrode configuration to analyze bioimpedance changes in the radial artery due to blood flow.

Every major U.S. airline has experienced a violent inflight assault on one of their flight attendants. The problem is becoming more widespread, but industry leaders have few coping strategies. The first step toward addressing the issue is understanding the various aspects of the event, including the actions of other passengers (bystanders) who may witness the assault. There is ample literature on bystander reactions to similar events such as bullying, medical emergencies, injuries, and violent attacks, but none dedicated to a flight attendant assault or passenger reactions to it.
