The Health Wagon has been providing care for the rural population of southwest Virginia for the past 40 years. The mission of the Health Wagon is to provide quality health care to the medically underserved people in the mountains of Appalachia. It has expanded to two stationary clinics, three mobile units, and a mobile dental unit, logging over 19,000 patients encounters in the past year.

Listening To Voices In Appalachia: Gathering Wisdom From The Field About Substance-Abuse Recovery Ecosystems, Bruce Behringer

New qualitative data collected through six regional community-based listening sessions and community meetings are presented that describe elements of the Appalachian Regional Commission’s Recovery Ecosystem Model. These data informed the Model, which was used in formulating the new ARC Recovery-to-Work initiative. Input was intentionally solicited from multiple sectors, including persons recovering from substance abuse disorder, treatment and recovery service providers, workforce development agencies, employers, and community advocacy groups.

Responding To Appalachian Voices: Steps In Developing Substance-Use Recovery Ecosystems, Bruce Behringer

A description is presented of the four-step process used by the Appalachian Regional Commission to develop a new Recovery to Work initiative. The Commission identified, defined, and described issues facing individuals who complete substance abuse disorder treatment and who seek reentry into the workforce. Key elements were identified for resources and supports to develop and maintain community-based substance abuse recovery ecosystems. The steps included conceptualization, data collection, analysis, and review to formulate recommendations for program and policy development. The full process of development was accomplished in twelve months.

Connected cancer care is of increasing importance in light of the COVID-19 pandemic. The Linking & Amplifying User-Centered Networks through Connected Health (L.A.U.N.C.H.) Collaborative in Appalachian Kentucky has pioneered a new roadmap for equipping communities with the transformative power of broadband to innovate around the future of cancer care and to better scale their ideas. The roadmap involves reaching across disciplines, including public health, anthropology, telecommunications, and user-centered design. The goal is to leverage connectivity and cancer communication research and practice to make a real difference for patients and families.

Introduction: The Appalachian region is often characterized by poor health outcomes and economic depression. Health centers (HCs) are community-based and patient-directed organizations that deliver comprehensive, culturally competent, high-quality primary healthcare services in high need areas, including Appalachia, where economic, geographic, or cultural factors can hinder access to healthcare services.

Purpose: The study compares the clinical quality performance in preventive care and chronic disease management between Appalachian HCs and their non-Appalachian counterparts.

Introduction: Physical activity has positive health benefits across the lifespan including reduced rates of chronic disease. Despite having ample availability of outdoor space for physical activity in the Appalachian Mountain region, there are low rates of physical activity along with high rates of sedentary time and increased prevalence of overweight individuals across all age groups. Therefore, there is a need to understand the factors that influence family’s physical activity and sedentary time.

The Appalachian Regional Commission (ARC) has long addressed issues of substance abuse through funded community-based interventions, research, and sponsored conferences. Recently, the opioid crisis created a new urgency for cross-sector collaboration among various partners and funders dealing with this issue. This commentary provides an overview of recent efforts by the ARC to convene stakeholders to focus on assisting individuals with substance abuse disorder to access recovery services while pursuing education and training necessary to reenter the workforce and gain employment. The hallmark of public health is population-level intervention. However, current public health funding in Kentucky is largely programmatic or disease-based. As a result, public health leaders are not able to appropriately utilize present resources to pursue population health endeavors. However, a recent transformation of the public health system has emphasized multisector partnerships and efficient funding mechanisms that may increase resources to pursue population-level health interventions based on community health assessments.

The Barren Springs Songbook is a poetry collection exploring Appalachian themes through the lens of three representative characters and my own experience. The poems presented are in blank verse and lean heavily on musicality, as each poem features an epigraph from my own Great Uncle Henry’s song lyrics. The poetry explores themes of poverty, folklore, feminism, and Christianity within the context of Barren Springs, an insular Appalachian community. The purpose of this study was to examine the pre-college arts experiences of Appalachian college students who participated in place-based and non-place-based performance arts ensembles and, using a qualitative research approach informed by Kuh et al.’s (2005) study on positive student engagement, understand the influence that participation in these ensembles might have on Appalachian students who are the first in their generation to pursue higher education. In this essay, I investigate how the historic and current economic structures operating in Appalachia from the 1920s to the 2010s are represented in two contemporary Southern Appalachian novels – Serena (2008) by Ron Rash and Flight Behavior (2012) by Barbara Kingsolver. Through the lens of postcolonial theory, I show how Serena represents Appalachia as functioning under the colonial model outlined by Robert Blauner and Helen Mathews Lewis in 1978. Then, still under the theory of postcolonialism, I explore how Kingsolver’s work depicts regional identity in response to a post-colonial environment and the ever-expanding global economy.

When women are lured away from home, they become vulnerable and cannot survive the violence inflicted upon them by their ‘lovers.’ This thesis explores the ties between two distinct cultural regions, Arabic and Appalachian, to examine the violence against women and what allows these women to escape such situations by using Hanan al-Shaykh’s One Thousand and One Nights: A Retelling and three traditional Appalachian murdered girl ballads.

This thesis analyzes Southeastern Kentucky’s political and military support for the Union during the Civil War era. In the decades prior to the 1860 election, Kentucky developed deep social and economic ties with all sections of the country. After the secession winter that followed Abraham Lincoln’s presidential election, the statewide population divided and pockets of significant Confederate sympathies emerged. Kentucky’s southeastern counties aligned with the Union at the outbreak of the Civil War because of a strong national identity and the absence of a large slave population. In this thesis, I demonstrate how analysis through literary criticism can provide a commentary on Appalachian song. While literary analysis of both sacred and secular song lyrics is an approach that is largely overlooked in this region’s traditional music, it nonetheless provides insightful perspective on the art form itself. As I argue, one particular duo of Appalachian musicians, the Louvin Brothers, are uniquely suited to this inquiry. I propose that themes that are found in many of the Louvin Brothers’ songs, such as love, acceptance, and rejection, create a bridge between the historically documented theoretical gap between bluegrass and country …

This thesis asks what transcriptions of old-time fiddle tunes might tell us about their underlying purposes and the nature of transcription. How could differing approaches to transcription reflect the intentions of the author, and what are those intentions? What does this suggest about how aural information is prioritized? Contemporary Appalachian poetry offers a lens through which we can see the immense impact that the Tennessee Valley Authority has had in Appalachia. In this thesis, I explore the powerful presence of dams in Appalachian poetry by analyzing three poems. Jesse Graves’s “The Road into the Lake” centers on personal and familial loss, Jackson Wheeler’s “The TVA Built a Dam” mourns the loss of communities, and Rose McLarney’s “Imminent Domain” focuses on the ecological destruction that has occurred in Appalachia and around the globe as the result of the construction of TVA dams. Introduction: Appalachian communities experience elevated rates of cancer incidence and mortality relative to other regions in the U.S. Specifically, melanoma mortality rates are higher in Appalachia compared to the national average, despite comparable incidence rates.

Purpose: To examine differences in self-reported history of skin cancer and prevalence of two UV exposure behaviors between Appalachian and non-Appalachian adults in a nationally representative sample.

Introduction: The Affordable Care Act (ACA) enacted on March 23, 2010 significantly impacted access to healthcare for people living with HIV (PLWH). Expansion of care was accomplished in three areas: eliminating exclusions for pre-existing conditions, elimination of lifetime caps on healthcare expenditures, and expansion of Medicaid eligibility.

Introduction: Food insecurity has emerged as a public health problem among college students in Appalachia, jeopardizing their physical, mental, and emotional health and academic success. Campus food pantries have been established in this region, but no data are available concerning student use or perception of services.

Purpose: This study measured use and perceptions of a campus food pantry by students at a mid-sized university in rural North Carolina.

Methods: An online questionnaire collected behavioral and perceptual data, and follow-up interviews explored these variables. Descriptive statistics with significance at p<0.05 and thematic analytical procedures were used.
