Judges and judicial systems have become more specialized in response to increasing legal complexity. While the theoretical literature has discussed judicial specialization extensively, deliberating its pros and cons and its implications—arguably an appealing background for empirical investigation—few empirical studies have attempted to quantify the relationship between judicial specialization and case outcomes. This Article fills that lacuna. It analyzes a hand-coded dataset of tax cases in which rulings were made by both specialized tax judges and generalist judges in Israeli District Courts. The COVID-19 pandemic has imposed a significant economic burden globally, particularly in regions with stringentresponse measures. This study aimed to assess the economic impact of COVID-19 in Taiwan, focusing on both directand indirect costs. A cost-of-illness analysis was conducted, utilizing data from the Taiwan Centers for Disease Control(CDC), national databases, epidemiological studies, and economic surveys. The analysis included both direct costs (e.g.,hospital admissions, outpatient care) and indirect costs (e.g., productivity losses due to long COVID, absenteeism, caregiving duties). Plant-derived nanovesicles (PDNVs) offer a promising, biocompatible therapeutic platform. In this study, we isolatednanovesicles from protocorm-like bodies of Dendrobium officinale (PLBs-NVs) and evaluated their efficacy in dry eyedisease (DED) models. PLBs-NVs protected human corneal epithelial cells against hyperosmotic stress, supportedwound repair, and inhibited inflammation in vitro. In a desiccating stress-induced DED mouse model, topical PLBs-NVstreatment significantly restored tear secretion, reduced corneal epithelial damage, and lowered levels of proinflammatorycytokines. These results suggest that PLBs-NVs are a safe, natural, and effective therapeutic strategy for DED, sup-porting their potential for future clinical translation in ocular surface disorders.

A series of chromatographic separation on the liquid-state fermented products of Didymocyrtis brachylaenae Km1530 derived from the littoral medicinal herb Atriplex maximowicziana Makino, resulted in the isolation of compounds 1—13. Their structures were determined by spectroscopic analysis as fivepreviously unreported C13 polyketides, namely brachylactones A—D (1—4) and brachic acid (5), along with eight known compounds 6—13. Among these, brachy-lactones A (1) and B (2) exhibited nitric oxide production inhibitory activity in lipopolysaccharide-induced murine BV- 2 microglial cells at a concentration of 20 μM without any cytotoxicity.

This study aims to explore the relationship between thetherapeutic mechanism of YQXF and the microflora in COPD. Parkinson's disease (PD) is a progressive neurodegenerative disorder marked by dopaminergic neuronal loss,oxidative stress, neuroinflammation, and a-synuclein aggregation. Despite advances, current treatments only offersymptomatic relief without altering disease progression. This review aims to evaluate the therapeutic potential of plant-derived phytochemicals in PD through evidence from both in vivo and in vitro experimental models. Key findingsindicate that rodent models (e.g., MPTP, rotenone) and neuronal cell lines (e.g., SH-SY5Y) help replicate PD pathologyand mechanistic insights. Postoperative shivering is a common complication that can affect patient comfort and stability. Pethidine and tramadol are widely used to relieve this symptom, but their relative effectiveness and safety remain under investigation. In this randomized trial, 50 patients (aged 25–35 years) with postoperative shivering were divided into two groups: one group receiving intravenous tramadol (n = 25 patients) and one group receiving pethidine (n = 25 patients). Onset of drug action, vital signs, and side effects were systematically assessed. Pethidine had a significantly faster onset of action than tramadol (12.20 ± 0.35 min vs 7.16 ± 0.28 min, p < 0.001). No significant differences were observed between the two groups regarding body temperature, heart rate, oxygen saturation, or blood pressure (P < 0.05). Side effects, such as dizziness (16% vs. 8%), nausea and vomiting (20% vs. 8%), and nausea alone (48% vs. 20%), were more common and showed a statistically significant increase in the tramadol group. Both drugs were effective in suppressing postoperative tremor, but pethidine demonstrated superior efficacy, a faster onset of action, and a better safety profile. These results confirm pethidine as the recommended choice at an intravenous dose of 0.5 mg/kg.

The mechanistic target of rapamycin complex 1 (mTORC1) integrates growth factor (GF) and nutrient signals to stimulate anabolic processes connected to cell growth and inhibit catabolic processes such as autophagy1,2. GF signalling through the tuberous sclerosis complex regulates the lysosomally localized small GTPase RAS homologue enriched in brain (RHEB)3. Direct binding of RHEB-GTP to the mTOR kinase subunit of mTORC1 allosterically activates the kinase by inducing a large-scale conformational change4. BACKGROUND: Factors influencing in-hospital mortality in patients with well-differentiated gastroenteropancreatic neuroendocrine tumors (GEP-NETs) remain understudied, highlighting gaps in optimizing acute clinical outcomes. This study aimed to identify sociodemographic and clinical predictors of all-cause in-hospital mortality in this population.

METHODS: Using 2016 - 2020 data from the National Inpatient Sample (NIS), patients with malignant well-differentiated GEP-NETs were identified via the International Classification of Diseases, 10th Revision (ICD-10) codes. The primary outcome was in-hospital mortality. Sociodemographic and clinical variables (heart failure (HF), malnutrition, Charlson Comorbidity Index (CCI), and tumor site) were analyzed using multivariable logistic regression.

We report direct measurements of changes in diving and movement behavior for 53 goose-beaked whales (Ziphius cavirostris) in relation to experimentally controlled mid-frequency (3–4 kHz) active sonar (MFAS) signals. These signals simulate powerful Navy sources that have been associated with multiple mortal stranding events for this species. We deployed a multi-scale combination of tags to monitor individual whales, including 50 long-duration (weeks), coarse-resolution satellite-transmitting tags and 3 short-duration (hours), high-resolution archival depth, orientation, and acoustic tags. BACKGROUND: Sonic hedgehog (SHH) medulloblastoma is the most common molecular group of infant and early childhood medulloblastoma (iMB) and has no standard of care at relapse. This work aimed to evaluate the post-relapse survival (PRS) and explore prognostic factors of patients with nodular desmoplastic (ND) and/or SHH iMB.

METHODS: This international retrospective study included 147 subjects diagnosed with relapsed ND/SHH iMB between 1995 and 2017, < 6 years old at original diagnosis, and treated without initial craniospinal irradiation (CSI). Univariable and multivariable Cox models with propensity score analyses were used to assess PRS for those in the curative intent cohort.

RESULTS: The 3-year PRS was 61.6% (95% confidence interval [CI], 52.2-69.6). The median age at relapse was 3.4 years (interquartile range [IQR], 2.6-4.1). Those with local relapse (40.8%) more often received salvage treatment with surgery …

This Doctor of Nursing Practice (DNP) project was a staff education project that addressed the stigmatization of patients with mental illness (PMI) within psychiatric contexts. The project site healthcare organization’s mental health professionals had a high rate of stigmatization of PMI. Stigma is linked to poor medication adherence rates, delays in accessing healthcare, lower healthcare utilization among PMI, psychiatric morbidity and mortality, and poor quality of life.
