Improving Early Utilization Of Palliative Care Services To Impact Quality Of Life, April Mcdonald

Despite studies indicating the benefits of palliative care, many barriers remain that prevent individuals from accessing this specialty service. Palliative care needs improved utilization. The purpose of this quality improvement project was to improve the utilization of palliative services and increase general knowledge and confidence among nursing staff within one nursing home facility. In the intervention bundle, training was provided on the use of a Palliative Referral Flowchart and the Palliative Performance Scale (PPS) along with evidence-based materials made available to residents and families. Data was collected pre and post intervention. Knowledge was improved by 26 %. Confidence was improved …

Medical advancesmean a growing array of interventions, therapies, and technologies are available to support care for children with chronic and serious conditions. Some of these approaches are supported by robust data drawn from populations that perfectly reflect an individual patient's physiologic, psychologic, and social situation. But much more often, clinicians and families face decisions in the context of some—or much—uncertainty about whether the intervention will do a child more harm than good. This is particularly true for seriously ill children with a limited lifespan— whether the child is a neonate born with a life-threatening brain anomaly or an adolescent with …

Early Case Management Involvement For Long-Term Care Planning In The Primary Care Setting To Reduce Avoidable Social Admissions In High-Risk Patients, Elizabeth Blondeau

There are no protocols for case management referrals of patients at risk for social hospital admissions by their outpatient providers. Social admissions lead to adverse outcomes such as delirium, infections, polypharmacy, falls, functional dependence, pose a resource and financial strain on organizations, and are antithetical to patient-centered care. Timely case management intervention may prevent these hospitalizations by establishing a plan for future care needs. Project activities included the analysis of retrospective admission data from a N.H. community hospital, collaboration with interdisciplinary colleagues among and between both inpatient and outpatient settings, and an evaluation of best practices from the literature, resulting …
