This thesis examines how long-term partners sustain intimacy when one partner lives with dementia. Through an Appreciative Inquiry (AI) workshop with four couples from the Empowering Partnership Network—a joint initiative of the Yale Social Gerontology Lab and the Livewell Institute—this research explores how relationships adapt and endure amidst cognitive change. Drawing on Irene Smith’s Touch Awareness methodology and theories of relational autonomy, the study challenges dominant deficit-based narratives by highlighting the enduring capacity for meaningful connection. In this analysis we define the system boundary as Portland's downtown core, encompassing the downtown and immediately adjacent districts immediately affected by the effects of the "Urban Doom Loop." We selected this boundary for the concentration of reinforcing feedback loops. In Portland's downtown core remote work, commercial vacancy, homelessness, public safety concerns, and narrative media depiction of decline converge and amplify each other most intensely. Media focus on downtown disproportionately shapes public perceptions of the entire city. Interventions by city agencies are being proposed to take place there. The first two years after a child’s death, defined by parents as early bereavement, are a particularly vulnerable period for parents. More than 40% of bereaved parents report severe, prolonged grief symptoms during early bereavement. High-quality parent-clinician communication, which is delivered honestly, clearly, and with sensitivity, helps parents make informed decisions about medical care, address their complex needs during their child’s end-of-life (EOL), and is related to lower severity of prolonged grief symptoms many years after a child’s death. This qualitative grounded theory study examined how new graduate nurses who entered practice just prior to and during the COVID-19 pandemic developed and sustained resilience while navigating extraordinary professional and personal challenges. The study focused on associate degree nurses who graduated between December 2018 and May 2022 and were employed in acute and sub-acute care settings with direct patient care responsibilities, including the care of patients with COVID-19. Data were collected through semi-structured interviews and reflective journal entries and analyzed using open, axial, and selective coding.

Introduction: Medication regimen complexity may be an important risk factor for adverse outcomes in older adults with heart failure. However, increasing complexity is often necessary when prescribing guideline-directed medical therapy at the time of a heart failure hospitalization. We sought to determine whether increased medication regimen complexity following a heart failure hospitalization was associated with worse post-hospitalization outcomes.

Methods: This retrospective cohort study included Reasons for Geographic and Racial Differences in Stroke (REGARDS) participants aged at least 65 years hospitalized for heart failure between 2003 and 2014. We examine the economics of health care fraud and enforcement using theory and data and connect to a growing literature on the topic. We first offer a new economic definition of health care fraud that captures and connects the wide range of activities prosecuted as fraud. We define fraud as any divergence between the care an insurer says a patient qualifies for, the care a provider provides, and the care a provider bills for. การศึกษาวิจัยเรื่อง “เส้นทางสู่วาระท้ายของผู้ต้องขังที่ป่วยในเรือนจำ” เป็นการวิจัยเชิงคุณภาพที่มุ่งสะท้อนประสบการณ์ของผู้ต้องขังป่วยระยะท้ายผ่านการสร้างเรื่องเล่า (Narrative) เพื่อทำความเข้าใจสภาวะและความหมายของการดำรงอยู่ในช่วงวาระสุดท้ายภายใต้ข้อจำกัดของเรือนจำ ผู้วิจัยดำเนินการศึกษาข้อมูลจากเอกสาร (Documentary Study) ทั้งหนังสือ บทความทางวิชาการ รายงานการวิจัย และวิทยานิพนธ์ที่เกี่ยวข้อง ทั้งในและต่างประเทศ โดยเน้นประเด็นด้านแนวคิดและทฤษฎีการดูแลผู้ป่วยระยะสุดท้าย การดูแลแบบประคับประคอง (Palliative Care) สิทธิขั้นพื้นฐานของผู้ต้องขังตามหลักสิทธิมนุษยชน มาตรฐานสากลด้านสิทธิเสรีภาพของผู้ต้องขัง ตลอดจนกรอบสิทธิมนุษยชนระหว่างประเทศเป็นฐานในการวิเคราะห์ นอกจากนี้ งานวิจัยยังพิจารณาปัจจัยเชิงระบบที่มีผลต่อการดูแลผู้ต้องขังป่วยระยะท้าย เช่น ความแออัดของเรือนจำ โครงสร้างพื้นฐานด้านสาธารณสุข ความพร้อมของบุคลากรทางการแพทย์ และข้อจำกัดด้านทรัพยากร ตลอดจนศึกษาแนวปฏิบัติในต่างประเทศซึ่งมีการพัฒนาระบบการดูแลผู้ต้องขังป่วยระยะท้ายอย่างเป็นรูปธรรมมากกว่า ได้แก่ ประเทศสหราชอาณาจักร ฝรั่งเศส และสหรัฐอเมริกา ซึ่งมีรูปแบบบริการด้านการดูแลประคับประคองในเรือนจำตั้งแต่การจัดตั้งหน่วยดูแลเฉพาะทาง การพัฒนาอาสาสมัครผู้ต้องขัง ไปจนถึงกลไกการปล่อยตัวด้วยเหตุผลด้านมนุษยธรรม การเก็บข้อมูลภาคสนามใช้การสัมภาษณ์เชิงลึก (In-depth Interview) จากผู้ให้ข้อมูลสำคัญ (Key Informants) ได้แก่ แพทย์ พยาบาลวิชาชีพ นักทัณฑวิทยาที่มีประสบการณ์ดูแลผู้ต้องขังป่วยระยะท้าย และผู้ต้องขังจำนวน 2 นายที่กำลังเผชิญภาวะป่วยระยะสุดท้าย ข้อมูลที่ได้ถูกนำมาวิเคราะห์เพื่อสร้างภาพรวมของประสบการณ์ในวาระสุดท้ายของผู้ต้องขังในด้านร่างกาย จิตใจ จิตสังคม และจิตวิญญาณ รวมถึงอุปสรรคที่พบในการเข้าถึงบริการสุขภาพภายในเรือนจำ ผลการศึกษาคาดว่าจะสะท้อนให้เห็นถึงความท้าทายสำคัญของการดูแลผู้ต้องขังป่วยระยะท้ายภายใต้ข้อจำกัดด้านทรัพยากร โครงสร้างพื้นที่ และระบบบริการสาธารณสุขในเรือนจำ อาทิ การดูแลรักษาที่ไม่ต่อเนื่อง การขาดบริการเฉพาะทางด้านประคับประคอง และความจำกัดในการตอบสนองความต้องการทางจิตใจและจิตวิญญาณของผู้ต้องขัง นอกจากนี้ การศึกษายังมุ่งเสนอแนวทางพัฒนาระบบการดูแลผู้ต้องขังป่วยระยะท้ายในเรือนจำไทยให้มีลักษณะครอบคลุมและสอดคล้องกับมาตรฐานสากล โดยเน้นความสำคัญของการดูแลแบบองค์รวม การเคารพศักดิ์ศรีความเป็นมนุษย์ และการสนับสนุนให้ผู้ต้องขังมีคุณภาพชีวิตที่ดีที่สุดในช่วงวาระสุดท้ายของชีวิต

BACKGROUND: Transitions in care from hospitalization to home are a high-risk period for patients. Without care coordination during these transitions in care, patients are left trying to manage their care without any support. The VA has a mandated two-day post-discharge contact process that requires a member of the patient’s primary care team to contact the patient within two business days of a VA discharge.

Objectives: Pain is the most common patient-reported outcome in rheumatoid arthritis (RA), frequently managed with long-term opioid therapy (LTOT), despite limited evidence supporting its efficacy and safety. Given the inflammatory nature of RA pain, biologic or targeted synthetic disease-modifying antirheumatic drugs (b/tsDMARDs) may relief pain and facilitate opioid withdrawal. This study compared rates of any opioid tapering, rapid opioid tapering, and opioid discontinuation among Medicare beneficiaries with RA who initiated b/tsDMARDs, including tumor necrosis factor inhibitors (TNFi bDMARDs), non-TNFi bDMARDs, or Janus kinase inhibitors (JAKi).

The prevalence of Obstructive Sleep Apnea (OSA) in the general adult population has been estimated to be between 9-17% in women and 25-30% in men. However, the prevalence of OSA among medical intensive care unit (MICU) patients is unknown. A 2015 systematic review by Knauert et al demonstrated the associations of OSA with many chronic diseases commonly seen in MICU patients (for instance, hypertension, type II diabetes mellitus, and congestive heart failure). Statement of the Problem: Age-related macular degeneration (AMD) and diabetic retinopathy (DR) are leading causes of blindness and low vision in the United States. In 2019, approximately 19.8 million Americans were living with AMD, which is classified into dry (non-neovascular) and wet (neovascular) forms, with the latter being more severe. Catholic Faith-Based Organizations (CFBOs) in Mexico and Russia are unique entities of study and comparison to understand the power dynamics between majority/minority institutional religions and their media representation. Although both countries recognize their religious diversity, Catholicism is still the major religion in Mexico, not only because of its large number of adherents but also its significant cultural and political capital.
