On December 15, 2023, Northview Village, the largest skilled nursing facility in the Saint Louis region, closed abruptly, leading to the nighttime discharge of 170 residents. This emergency, while unique to that facility, is indicative of a broader crisis in both local and, to a significant extent, national nursing homes. This crisis has partly been catalyzed by the COVID-19 pandemic, but it rests on underlying factors that are unfortunately integral to the financing and operation of long-term care. Quality and compliant care delivery in nursing homes remains elusive. Four main interests are currently misaligned, thereby putting nursing home residents at risk of harm. Without a clearly defined commitment to quality and compliant care and alignment of these interests, nursing home residents will remain vulnerable to severe harm.

The first key interest concerns nursing home ownership and management. Currently, over seventy percent of nursing homes are owned by for-profit entities. Previous ownership models, including non-profit, religious-based, and county-owned facilities, have largely closed or transitioned to for-profit entities. In early 2020, as the nation was just becoming familiar with COVID-19, over a million nursing facility residents were already experiencing crisis-level disasters. Extreme lockdowns, increased hospitalizations, and high mortality rates plagued nursing facilities earlier and far worse than the rest of the population. Data gradually showed that facilities with higher numbers of residents of color were facing particularly dire outcomes.

Although the severity of COVID-19 was surprising, the pandemic highlighted existing and long-standing problems with the country’s long-term care facilities. Many of the problems that led to the horrible disparities during COVID-19 are rooted in a wholly insufficient long-term …

The impacts of human activity on ecosystems are increasingly evident through ecological degradation and climate change. Despite this, in many jurisdictions across the world, action to address and curtail destructive human activities is slow and resisted at all levels. In this article, we suggest that this resistance is connected to deep ecological grief. We introduce a dual approach to understanding ecological grief, using concepts of unprecedented and unacknowledged grief. Unprecedented grief is felt in response to the loss of our ecosystems and unacknowledged grief connected to the anticipated loss of lifestyle necessary to curtail destructive human behaviour. [Case] Mr H is a 74-year-old man with history of end-stage heart failure (ESHF) secondary to hereditary transthyretin cardiac amyloidosis. Our patient underwent placement of a HeartMate II (Abbott) left ventricular assist device (LVAD) as destination therapy (DT) at an outside hospital and later came to us (Sentara Heart Hospital) for continued management because he resided in the area. Over a 2-year period, the patient experienced multiple hospital admissions for acute heart failure (HF) exacerbations, delirium, and recurrent low-flow alarms. The seamless transfer and assimilation of healthcare data are foundational to delivering holistic, timely, and effective patient care across the healthcare spectrum. However, disparities in Electronic Health Record (EHR) system adoption, especially in long-term and post-acute care (LTPAC) settings, consistently obstruct interoperability. This study explores the factors that impede and facilitate health information exchange in LTPAC environments, focusing on technical and organizational facets. Grounded theory guided our qualitative case study research, involving 35 stakeholder interviews. Key technical findings highlight the need for integrated, reliable data structures, robust infrastructure, and standardized practices. This thesis is a critical autoethnographic exploration of embodied knowledge amid shifting reality and radical possibility. Beginning and ending with my own body, I explore how grief, as an embodied companion, offers wisdom and power in moments of profound change. Rooted in a methodology of slowness, rest, walking, and active listening, this work resists the extractive pace of modernity and instead invites building a relationship with grief and discomfort. This research is an experiment in co-creating knowledge with bodies and their intersectionality within systems that would keep us separate from knowing our embodied knowledge. Kentucky has an overwhelming need for both palliative and hospice care services because of the high percentage of individuals who struggle with chronic or life-threatening conditions. Studies have shown that access to palliative and hospice care may be inhibited by the knowledge patients have of these services. The purpose of this study was to examine the awareness and understanding Kentuckians have of palliative and hospice care and to determine the factors influencing them. In the summer of 2024, the Supreme Court of the United States overturned the Chevron doctrine, which required federal courts to defer to agency interpretations of ambiguous statutes. While the debate about agency deference in federal litigation generated significant attention in the legal community and from media outlets, the use of agency deference in state courts has received relatively little fanfare. This Article examines how the concept of agency deference entered North Carolina's administrative law jurisprudence and the North Carolina Supreme Court's recent efforts to reject the use of deference in the state's courts. This project applies machine learning to predict whether diabetic patients will be readmitted to a hospital within 30 days of discharge. Early readmissions are a costly and critical issue in healthcare, often signalling gaps in post-discharge care and risk management. Diabetic patients face unfair high readmission rates compared to the general population. According to the CDC Diabetes Report Card 37.3 million people in the U.S. or 11.3% of the population had diabetes as of 2019 (CDC, 2021). Our goal here is to develop a binary classification model capable of flagging high risk patient (< 30-day readmission) based on their clinical, demographic, and administrative data. Grounded in a case study in Barbuda in the Caribbean, this research examines sustainability from the perspective of what arts and heritage can contribute to community engagement and local and broader understandings about the environment. This article documents a growing body of literature recognizing the role of arts and culture, including local knowledge and traditional ecological knowledge (TEK), in climate change endeavors. Art and TEK present expansive world views. Contextual information situates research done on the island of Barbuda pre- and post-Hurricane Irma. Daratumumab Monotherapy: Reconsidering Relapsed And Refractory Multiple Myeloma Maintenance Therapy, Gabrielle O. Carreon Pharmd, Ms

Results from this study support the continued use of daratumumab monotherapy in carefully selected patients who may not be candidates for more intensive combination therapies due to age, comorbidities, or prior toxicities. However, further research is needed to confirm these early observations.

Palliative care includes effective communication, relief of suffering and symptom management with an underlying goal of improving the quality of life for patients with serious illness and their families. Best practice palliative care is delivered in parallel with life-sustaining or life-prolonging care. Palliative care affirms life and regards death as a normal process, intends neither to hasten death nor to postpone death and includes but is not limited to end-of-life care. Oral health is a critical component of overall wellbeing, yet disparities in access to dental care disproportionately affect vulnerable populations, particularly elderly residents in nursing homes. Poor oral hygiene is linked to systemic health conditions, such as cardiovascular disease, diabetes and cognitive decline. Despite federal regulations mandating oral health assessments in nursing homes, many facilities lack adequate resources, and care often falls to undertrained staff. Introduction: To determine if the use of glucagon-like peptide-1 receptor agonists (GLP-1RA) in type 2 diabetic mellitus (T2DM) patients affects outcomes after hemiarthroplasty for femoral neck fractures (FNFs). Methods: A retrospective cross-sectional analysis of a local hospital system database was performed using Current Procedural Terminology (CPT) and International Classification of Diseases (ICD-10) codes to identify T2DM patients who were at least 18 years of age who underwent hemiarthroplasty (HA) for FNFs and were on a GLP-1RA at the time of injury. Background: Disseminated fusarium, primarily affecting immunocompromised individuals, is associated with persistent fevers, rash, and lung involvement. Despite interventions, mortality rate remains high, exceeding 75% for disseminated disease. There are only 26 documented cases worldwide between 1974 and 2022 with CNS involvement. This case highlights a rare presentation of disseminated fusarium infection involving the lungs, liver, and brain in a 53-year-old male with acute promyelocytic leukemia (APL), highlighting critical diagnostic and therapeutic challenges. Introduction: This case study explores an uncommon triad of encephalitis, ventriculitis, and meningitis complicated by ischemic strokes and seizures in a COVID-19-positive patient. Prior studies suggest that bacterial meningitis and encephalitis can lead to cerebral infarctions due to inflammation and vascular injury.1,2,5 Ventriculitis, in particular, is rarely observed and typically is seen as a severe complication of meningitis, and can be managed with neurosurgical interventions such as CSF shunt placement, ventricular drains, or other intracranial devices.3,4 The simultaneous presentation of all three conditions in a single patient is rare and poses significant challenges from a diagnostic and management standpoint.
