Outcomes in patients with relapsed/refractory (RR) AML are poor. We sought to investigate if CPX-531 in combination with venetoclax (CPX + VEN) was tolerable and effective in RR AML. This was a single institution phase 1b/2 trial of CPX + VEN. Patients aged ≥ 18 years with RR AML who were fit for intensive chemotherapy were eligible. Prior venetoclax exposure was allowed. The phase 1b portion followed a 3 + 3 design to identify the recommended phase 2 dose (RP2D) for the expansion cohort. BACKGROUND: Finerenone, a selective nonsteroidal mineralocorticoid receptor antagonist, and sodium-glucose cotransporter 2 inhibitors (SGLT2is) both reduce chronic kidney disease (CKD) progression and improve kidney/cardiovascular (CV) outcomes. The CONFIDENCE (COmbinatioN effect of FInerenone anD EmpaglifloziN in participants with chronic kidney disease and type 2 diabetes using a UACR Endpoint) study (NCT05254002; EudraCT 2021-003037-11) hypothesis is that early combination of finerenone and empagliflozin, an SGLT2i, is superior to either drug alone in reducing urine albumin-to-creatinine ratio (UACR) over 6 months.

INTRODUCTION: The treatment of post-traumatic elbow arthritis poses a significant challenge for orthopedic surgeons, especially in young active patients. Total elbow arthroplasty is often the recommended treatment option in older patients; however, interpositional arthroplasty remains a viable option in high-demand patients.

Coordinated growth of multiple tissues is fundamental to shaping our body, but the underlying mechanisms remain underexplored. In zebrafish embryos, midline tissues composed of the notochord, floorplate, and hypochord elongate synchronously with their lengths aligned. We show that floorplate and hypochord cells collectively migrate posteriorly along the nascent notochord extracellular matrix as it extends posteriorly, maintaining the tripartite configuration. Fibroblast growth factor-mediated migration in a spatially graded manner causes cell stretching, which triggers Yap-dependent proliferation and controls floorplate and hypochord growth. PURPOSE: We tested whether Müller cells utilize autophagy to support immune privilege in the eye.
METHODS: The essential autophagy gene Atg5 was deleted in retinal Müller cells. Inflammation was induced by intravitreal injection of lipopolysaccharide (LPS) that was monitored by hematoxylin and eosin (H&E) staining, immunofluorescent confocal microscopy, and flow cytometry. Single-cell RNA sequencing was performed on retinal Müller cells isolated from control and Atg5-deficient mice. Markers of Müller cell gliosis were assessed, and cytokine production in the eye was measured. Small interfering RNA knockdown techniques were used to examine LPS-induced inflammatory pathways in culture.

In the emergency department-initiated buprenorphine for opioid use disorder (EMBED) trial, a clinical decision support (CDS) tool had no effect on rates of buprenorphine initiation in emergency department (ED) patients with opioid use disorder. The Agency for Healthcare Research and Quality (AHRQ) recently released a CDS Performance Measure Inventory to guide data-driven CDS development and evaluation. In the phase II NeoCOAST-2 platform study, 202 patients with untreated, resectable stage IIA–IIIB non-small-cell lung cancer (NSCLC) were randomized to receive neoadjuvant durvalumab plus platinum-doublet chemotherapy with oleclumab, a CD73 inhibitor (Arm 1), or with monalizumab, a NKG2A inhibitor (Arm 2), or neoadjuvant durvalumab plus single-agent platinum chemotherapy with the TROP-2 antibody–drug conjugate (ADC) datopotamab deruxtecan (Arm 4), followed by surgical resection and adjuvant durvalumab with oleclumab or monalizumab (Arms 1 and 2) or durvalumab alone (Arm 4). Introduction The shoulder joint is a common site for joint dislocation, with many individuals suffering from recurrent dislocations following treatment. The purpose of this study was to evaluate the epidemiology of shoulder dislocations in the United States from 1990 to 2019. Methods The Global Burden of Disease database was utilized to collect epidemiological data on shoulder dislocations in the United States (U.S.) from 1990 to 2019. These data included age-standardized rates of years lived with disability (YLDs), prevalence rates, and incidence rates per 100,000 people. Derived from a Social Cognitive Theory framework (Bandura, 1986), the aim of this study was to assess the impact of different drinking environments on college students’ expected use of alcohol protective behavioral strategies, perceived effectiveness of those strategies, and perceived self-efficacy to use them. To accomplish this, we sought out to create and employ experimental vignettes consisting of a bar/club or party drinking environment in a two-part study. RNA biology is a rapidly expanding field, with many avenues of discovery among marine organisms. In this study, we analyze both mollusks and sawfish. First, we perform an evaluation of sRNA biogenesis pathways throughout Mollusca, with emphasis on the easter oyster, Crassostrea virginica. Understanding these molecules prescribes RNAi-based gene silencing approaches, benefiting genetic investigation and biotechnology. Similar to other animal groups, mollusks have conserved microRNAs (miRNAs), with some shared with ecdysozoans and deuterostomes; however, there is no evidence of an endogenous small interfering RNA (siRNA) pathway. Importance Accurate prediction of surgical case duration is critical for operating room (OR) management, as inefficient scheduling can lead to reduced patient and surgeon satisfaction while incurring considerable financial costs.

High-dose olanzapine has shown efficacy in treating adult patients with treatment-resistant schizophrenia, though evidence in adolescents remains limited. This report describes the case of a 17-year-old male patient with treatment-resistant schizophrenia who showed significant clinical improvement with a supratherapeutic dose of orally disintegrating olanzapine. The patient, with a 16-month history of schizophrenia and multiple hospitalizations, presented with acute psychosis and aggression after medication non-adherence. Following failed trials of paliperidone and standard-dose olanzapine, and unable to initiate clozapine due to blood draw refusal, the patient was gradually titrated to 50 mg daily of orally disintegrating olanzapine (20 mg early morning, 30 …

The nucleus reuniens (RE) is a midline thalamic structure interconnecting the medial prefrontal cortex (mPFC) and the hippocampus (HPC). Recent work in both rodents and humans implicates the RE in the adaptive regulation of emotional memories, including the suppression of learned fear. However, the neural correlates of aversive learning in the RE of rodents and humans remain unclear. To address this, we recorded RE activity in humans (BOLD fMRI) and rats (fiber photometry) during Pavlovian fear conditioning and extinction. In both rats and humans, we found that conditioned stimulus (CS)-evoked activity in RE reflects the associative value of the CS. …

The authors show that a large class of 2-adic Schrödinger equations is the scaling limit of certain continuous-time quantum Markov chains (CTQMCs). Practically, a discretization of such an equation gives a CTQMC. As a practical result, new types of continuous-time quantum walks (CTQWs) on graphs using two symmetric matrices are constructed. The transport between nodes in one direction is described by one matrix, while the transport between nodes in the opposite direction. This Phase 1 clinical trial evaluated ¡Iniciando! la Adultez, a culturally adapted intervention for Latino autistic young adults (n = 56) and their parents (n = 63). Young adults showed significant post-treatment improvements on measures of social responsiveness (SRS-2), adaptive behavior (ABAS-3), executive functioning (BRIEF-A), quality of life (WHOQOL-BREF), transition readiness (TRS-A), coping self-efficacy (CSES), and emotion regulation (DERS-SF). Parents reported significant improvements in their perceptions of their child’s social responsiveness, adaptive behavior, and executive functioning, as well as increased transition readiness ratings. Integrating Distributed Energy Resources into microgrids modifies the network’s system parameters and causes it to exhibit behaviour, particularly during fault conditions. This is due to the greater variations in current direction and magnitude. When a microgrid runs in autonomous mode, Distributed Generators result in a reduced short circuit current compared to a system working in grid-connected mode. Microgrid protection issues, namely, blinding of protection, sympathetic tripping, and bidirectional power flow, occur in the power network under such circumstances. The Whipple operation is oftentimes considered one of the most challenging and complex abdominal operations, usually reserved for duodenal and hepatopancreatobiliary malignancies. In cases of duodenal and/or pancreatic trauma with disruption of the biliary systems, a Whipple reconstruction can be performed. Such operative traumas are rare and carry with them a high level of morbidity and mortality. This case report presents a challenging 46-day clinical course of a 22-year-old African American female, who suffered multiple gunshot wounds, resulting in severe abdominal injuries. These injuries included a grade 3 duodenal injury, grade 4 pancreatic injury, and a grade 4 renal injury. …

Integrated modular motor drive (IMMD) is an effective approach for realizing high-efficiency, high-power-density, and fault-tolerant electric machines. However, designing an efficient thermal management system (TMS) for the motor drive becomes a challenge, particularly due to space constraints. This article presents the design of a TMS based on 3-mm heat pipes for a 250-kW IMMD intended for aviation applications. The power electronics module is simulated using PLECS software where an electrothermal analysis is conducted. Objectives: Cortical reorganization has been proposed as a mechanism for motor recovery of the upper limb. The objective of this study was to determine if intervention-related clinical improvement in post-stroke hemiparesis is associated with enlarged motor cortex representation. Methods: Data were analyzed from a single site subset of the NICHE trial. Background: Metabolic dysfunction-associated steatotic liver disease (MASLD) is a chronic liver condition associated with lifestyle and metabolic risk factors. While exercise is a beneficial treatment strategy, many patients struggle to implement and perform exercise regularly. It is important for practitioners to know about patient awareness, attitudes, and preferences regarding exercise in the con-text of MASLD. Aim: To evaluate the early impact of the Centre. Background: The educational quality of nursing and midwifery student clinical placement learning varies. The National Placement Evaluation Centre has a pivotal role in quality improvement by systematically collecting student and supervisor feedback on placement experiences. Design: Multimodal impact evaluation. Methods: Data were collected from website analytics and a survey shared via stakeholder registration emails and social media. Background: Increased nut consumption has positive effects on physical and cognitive function, but whether these translate into lower functional limitations in older adults is unknown. The aim of this study was to investigate the association between nut intake, functional limitations, the severity of these limitations and health-related quality of life (HRQOL) in older adults. Methods: This cross-sectional study included data from 5807 adults (53.4% female) aged 60 years and older who participated in the US National Health and Nutrition Examination Survey (NHANES) between 2003 and 2012. This study explores the influence of perceived similarity on pro-environmental behavior, focusing on plastic reduction. Participants' daily plastic use and reduction were tracked over 30 days via online chat software, with controlled nudges from an agent. Each group included two examinees and one agent. Behavioral data were analyzed to evaluate predictability from various perspectives and its relationship with behavioral change. Results showed significant differences in predictability based on perceived similarity, particularly during the first 10 days. Furthermore, nudges, consumption levels, and behavioral changes significantly affected predictability within the first 20 days. This study delves into the lived experiences of grandmothers grappling with grief following the “out-of-order” death of a child, child-in-law, or grandchild, using an Interpretative Phenomenological Analysis grounded in the Dual Process Model-Revised. With 70% of U.S. adults over 65 being grandparents, the research highlights their often-invisible role as both grievers and caregivers amid family loss. Findings reveal three key themes: navigating personal grief, intergenerational support dynamics, and reconstructing family identity. Grandmothers oscillate between loss-oriented and restoration-oriented coping, with grief intensity varying by relational proximity—most profound when losing their own child. Background: Social prescribing link workers formed part of the Additional Roles Reimbursement Scheme introduced into primary care in England from 2019. Link workers assist patients experiencing issues affecting their health and well-being that are 'non-medical' (e.g. lack of social connections, financial difficulties and housing problems). They give patients space to consider these non-medical issues and, when relevant, connect them to support, often within the voluntary-community-social-enterprise sector. The development of Industry 4.0 has accelerated the adoption of sophisticated technologies, including Digital Twins (DTs), Artificial Intelligence (AI), and cybersecurity, within Additive Manufacturing (AM). Enabling real-time monitoring, process optimization, predictive maintenance, and secure data management can redefine conventional manufacturing paradigms.
