Background

Recent publications describe severe disease in pediatric patients with TMP-SMX associated respiratory failure progressing to extracorporeal membrane oxygenation (ECMO), lung transplantation and/or death. ECMO characteristics in this patient population remain poorly defined.

Methods

This is a retrospective observational report from our institutional referral Redcap database reviewing ECMO characteristics for pediatric patients (age ≤ 18 years old) patients. This includes presumed TMPSMX associated severe respiratory failure, following 6 or more days of TMP-SMX exposure and lack of other diagnostic explanation.

Results

To date, at our institution we have identified 32 total adult and pediatric with clinical criteria for TMP-SMX severe …

Background: Current clinical practice guidelines support the long-term use of pharmacotherapy for obesity treatment. Historically, phentermine has been one of the most prescribed obesity medications (OMs), however, its long-term efficacy and safety have never been evaluated in a randomized trial as its market approval predates such requirements. Here we describe the design, rationale, and baseline characteristics for a 24-month, double-blind, randomized controlled trial evaluating the efficacy, cardiovascular risk, and safety of phentermine in adults with overweight or obesity.

Methods: This multicenter trial will compare outcomes among participants randomized to phentermine 24 mg daily versus placebo for 24 months. All participants …

The Importance Of International Exchange In A Time Of Worldwide Economic And Political Crisis And Change, David Anshen

This paper argues that international scholarly and cultural exchange has become especially vital in the context of intensifying global economic, political, ecological, and social crises. Drawing on Marxist theory, contemporary political realities in the United States and South Asia, and examples from world literature, the article examines how global capitalism both deepens human suffering and creates new possibilities for solidarity and resistance. Through reflections on immigration, political violence, ecological collapse, and the marginalization of the humanities, the paper highlights the role of international dialogue- facilitated by digital technologies and cross-cultural engagement- as a necessary counterforce to dehumanization and fragmentation. The world has become more complex and interconnected, with challenges that span countries and disciplines. To navigate these challenges, individuals are needed who are equipped with skills and competencies to lead in cross-cultural, cross-disciplinary environments. These leadership skills and competencies can be developed through participation in experiential learning programs. However, the impact of international research-based programs remains unexplored. This is a qualitative, phenomenological study within a bounded case that explored the lived experiences of students who participated in an international graduate student research program from a leadership development perspective. This paper rethinks Marx and Engels’ comments on imprisonment and capitalism. While there has been a tendency to suggest the authors’ comments on crime and punishment were peripheral and secondary to their major projects, such as Marx’s Capital, this paper claims, in fact, their writings on incarceration were guided by their shared theoretical project which included historicizing systems of punishment as correlating with modes of production. Also, while more has been written on their ideas of criminality, instead, this paper excavates their thoughts on prisons, which have often been neglected. How Do Social Isolation And Loneliness Vary By Age?, Jihye Baek, Cal J. Halvorsen

Social isolation and loneliness, which are experienced by many older adults, can negatively impact health and well-being. Although often used interchangeably, social isolation is defined as an objective lack of social ties (e.g., number of close family members or friends) and loneliness as an internal sense of disconnection (e.g., feeling alone even when surrounded by others). Because social relationships, health, and life circumstances change as we age, patterns of social isolation and loneliness may differ by age group. Therefore, this research brief asks, How do patterns of social isolation and loneliness differ across age groups among community-dwelling older adults?

This report details the wetland and riparian mapping methodologies implemented for the Wetland-31 Utah 2 project. The project area consists of 70 1:24,000 US Geological Survey (USGS) quads across Southeastern Utah with quads in Grand, San Juan, Emery, and Sevier counties, and Western Colorado with quads in Mesa County.

Single-family zoning has long dictated not only where people may live but also with whom. Although extensively critiqued for perpetuating racial and economic exclusion, these laws also privilege relationships defined by blood, marriage, or adoption and marginalize nontraditional families. This Article focuses on a particularly overlooked group: polyamorous families who often face legal uncertainty, social exclusion, and housing discrimination due to restrictive zoning definitions of “family.”

Energy-related conflicts are on the rise, spanning diverse issues such as the impacts of rare mineral mining on local communities, the impacts of sanctions on energy investments due to the Russia-Ukraine war, and the impacts of expanded subsidies on the electric vehicle and solar panel industries. Increasingly, companies, communities, and governments are clashing over the challenges of pursuing disparate and sometimes competing energy policies. It is critical to ensure the safe and quality supply of blood among various stakeholders; however, this remains a significant challenge in today’s world, especially in developing countries. These countries face severe problems, including blood shortages, fragmented regulatory frameworks, and inconsistent quality control measures. To tackle these challenges, countries like India are trying to implement the use of artificial intelligence (AI) and its derivative platforms to transform blood collection, testing, storage, and distribution by enabling interoperability, real-time compliance monitoring, automated donor screening, optimized inventory management, and advanced component separation. The Exactions Illusion: Sheetz’S Missing Dissent, Lee Anne Fennell, Timothy M. Mulvaney

In April 2024, the Supreme Court unanimously handed down Sheetz v. County of El Dorado, a land use exactions case billed as narrow, inevitable, constitutionally compelled, and obviously correct.  It was none of those things. Sheetz instead ushered in an open-ended shift of policy control over the built environment from state and local legislatures to courts. By its terms, the holding extended the Court’s stringent “exactions” test—which requires the government to prove nexus and proportionality between development conditions and development impacts—beyond its original context of parcel-specific administrative bargains to reach an unspecified set of legislated property conditions.
