Disseminated Tuberculosis Presenting With A Nodo-Colonic Fistula In A Newly Diagnosed Hiv-Positive Patient: A Case Report From Tanzania, Samina Somji, Salim Surani, Joshua Ngimbwa, Tevin Kahwa, Frank Swai, Fatema Tayabali, Bishoy Rofaeil

We present the case of a 35-year-old male with no known comorbidities who presented with chronic gastrointestinal (GI) and constitutional symptoms and was empirically treated with several antibiotics during multiple clinic visits at different health facilities. The laboratory test sent included a stool GeneXpert, which was positive for Mycobacterium tuberculosis (MTB). CT imaging revealed multiple necrotic abdominal and retroperitoneal lymph nodes, hypoenhancing splenic lesions, a nodo-colonic fistula between the necrotic retroperitoneal lymph nodes and the adjacent colon, and a mild left-sided pleural effusion. Colonoscopy demonstrated caseating discharge from the fistulous tract 40 cm from the anal verge, and histology showed …

Importance: Whether bipolar II disorder (BD-II) is associated with increased long-term mortality remains uncertain because most studies have not distinguished BD-II from bipolar I disorder (BD-I).

Objective: To examine whether BD-II is associated with elevated all-cause and cause-specific mortality compared with population controls, unaffected siblings, and those with BD-I.

Design, setting, and participants: This population-based, retrospective cohort study used data from Taiwan's National Health Insurance Database from January 1, 2000, to December 31, 2022. Individuals 12 years or older with 2 or more psychiatrist-assigned BD-II diagnoses were individually matched to 4 controls without BD-II by sex and birthdate. Additional comparisons …

Comparative Evaluation Of Five Bacterial Strains For The Fermentation Of Camel Milk, Sifatun Nesa Ali

In this study, the comparative fermentation performance of lactic acid bacteria Streptococcus thermophilus, Lactobacillus delbrueckii subsp. bulgaricus, Lb. helveticus, Lb. casei, and Lactiplantibacillus plantarum in Camel Milk (CM) and Bovine Milk (BM) was investigated. Fermentations were carried out at 42°C for 48 hours, and was monitored through bacterial growth, titratable acidity, pH evolution, proteolytic activity, microstructural properties, and rheological characteristics. Although comparable bacterial viability was observed in CM and BM, acidification kinetics varied significantly between CM and BM (p < 0.05). Proteolysis was significantly higher in fermented CM than in BM (p < 0.001), with consistently greater o-Phthalaldehyde Assay (OPA) values in CM across all strains during fermentation (p < 0.001). Scanning electron microscopy revealed more porous, loose protein matrices in fermented CM than in BM, which supports the rheological results showing weaker gel networks and lower rheological strength in fermented CM. Among the tested bacteria, Lb. casei demonstrated superior adaptability, enhanced viability, balanced acidification, and favorable rheological properties in both milks. Overall, the results of this thesis demonstrate strain-specific fermentation responses and support the possible suitability of different starter cultures for improving the technological performance of fermented camel milk products.

This thesis is concerned with determining the optimal water treatment and harvesting strategy for Lemna minor duckweed plants. The main objective of this thesis is to examine how duckweed plants grow in terms of biomass, photosynthetic activity, leaf morphology, chlorophyll and carotenoid content, and their efficiency in nutrient removal from different water sources. The methodology of the study included assessing duckweed in three water samples including tap water, saline water, and greywater and three harvesting strategies, including low, medium, and high harvesting intensities to assess their growth and nutrient removal efficiency. Background: Respiratory fitness and sarcopenia status have been reported to be cross-sectionally linked with each other and increase mortality risk. However, little is currently known regarding the connection between impaired respiratory function and sarcopenia and their joint effect on future death risk. Methods: This study included 12 027 participants (50.8% were women; mean [SD] age, 58.9 [9.3] years) from a nationwide, prospective cohort in China (China Health and Retirement Longitudinal Study). AbstractKoalas (Phascolarctos cinereus) give birth to highly altricial young that rely on maternal milk for nutritional support and immune protection in the first year of life. This study applied proteomics to analyse stage-specific shifts in the koala milk proteome across early-, mid-, and late-lactation. Proteins were extracted from seven samples and analysed using liquid chromatography-tandem mass spectrometry. Quantitative profiling identified 874, 1220, and 498 proteins in early-, mid- and late-lactation stages, respectively. The most abundant proteins—including β-lactoglobulin, caseins, clusterin, trichosurin, albumin and lactotransferrin—were consistently detected across all stages, reflecting crucial roles in nutrition, immune function, and detoxification. Real-world evidence for wearable noninvasive glucose monitoring (NIGM) remains limited. To evaluate the functional equivalence of a wearable NIGM device and explore its utility for T2DM and prediabetes screening. In this multicenter study, 12-h daytime glucose profiles obtained by a flexible reverse iontophoresis-based electrochemical sensor were compared with capillary glucose using functional equivalence. Subgroup analyses were conducted. Screening models of T2DM and prediabetes were developed using elastic net and Logistic regression. A total of 135 participants (mean age 35.3 years; 60.0% female) were included, and no serious device-related adverse events were reported. Food security is a multifaceted, ‘wicked’ issue that refers to the sufficient physical, social, and economic access to food at all times. Given its complexity, food security action requires a systems-based, collaborative, and adaptive approach. Activewear has become a common component of women’s everyday clothing, yet emerging evidence suggests that exposure to activewear imagery may adversely affect body image. This study aimed to describe women’s activewear engagement across contexts and to investigate how different markers of engagement correspond with positive outcomes such as fitness behavior, body appreciation, and self-esteem, as well as negative outcomes including media pressure, idealized appearance aspirations, and self-objectification. Background: Unrestricted junk food advertising increases the risk of short-term junk food consumption among viewers. We aimed to estimate the impact of junk food and anti-junk food advertisements differing in length and framing on junk food consumption inclinations. Issue Addressed: Social network analysis (SNA) is a novel approach to investigating food security systems change in regional and remote communities. With short-term, siloed food security solutions often proving ineffective, this study used SNA to visualise how organisations across a vast area in Western Australia (WA) were working in partnership to support systems change and identify where these partnerships could be strengthened. Methods: One hundred and one food security initiative leaders, representing 148 initiatives, participated in semi-structured interviews. The Cost Of Cargo Preference: How Shipping Requirements Hinder Food Aid Efficiency, Grace E. Pattara

Cargo preference for food aid (CPFA) mandates that 50 percent of U.S. food aid shipments be transported on U.S.-flagged vessels (USFVs). However, only seven USFVs are equipped to accommodate bulk food aid, causing shipments to be sent on vessels not designed for such transport. This constraint increases operational costs by an average of 270 percent, delays delivery timelines, and consumes 30-50 percent of the food aid budget. As the American legislature stands in an important moment in foreign aid history when U.S. foreign assistance is being fundamentally restructured, this thesis argues that Congress should repeal CPFA and permit competitive bidding …

This article reviews the history and current evidence of systematic pharmaceutical industry corruption.  It draws on studies by the OECD on international corruption; reports of the Securities and Exchange Commission; studies of the Public Citizen health Research Group of settlement agreements between federal and state authorities and pharmaceutical firm; the history of the Medicare and Medicaid Anti-Kickback Act; professional and industry codes of ethics; the literature on institutional corruption; and studies of conflicts of interest.  These sources support findings of systemic corruption since the mid-20th century. Eating disorders (EDs), including anorexia nervosa, have the highest mortality rate of any psychiatric illness, yet remain underrecognized relative to their severity and treatment burden. Because the period of peak onset aligns with the college years, institutions of higher education are uniquely positioned to play an important role in prevention, early identification, and referral to specialized care. Yellow fever is a mosquito-borne viral illness caused by the yellow fever virus (YFV), a member of the Flaviviridae family. Despite the availability of an effective live-attenuated vaccine, the disease remains endemic in regions of Africa and South America. Transmission occurs primarily via an infected mosquito called Aedes aegypti, with distinct urban, sylvatic, and intermediate transmission cycles. Clinically, infection ranges from mild febrile illness to severe disease, which is characterized by jaundice, hemorrhage, and multiorgan failure. Pathogenesis involves viral replication in hepatocytes, immune-mediated injury, and dysregulated inflammatory responses, leading to characteristic hepatic mid-zone necrosis. The lack of effective preventive measures for tick-borne diseases represents a major public health challenge. Transmission-blocking vaccines targeting tick molecules essential for feeding or reproduction could provide a sustainable and broad-spectrum solution. In this study, we evaluated two tick-derived proteins as potential vaccine antigens, subolesin, a key regulator of blood-feeding and cellular processes, and vitellogenin, a protein involved in egg development. Using bioinformatic tools, we identified immunogenic peptide sequences from each protein and chemically conjugated them to virus-like particles (VLPs) to enhance antigen presentation. This study investigated the influence of biochar, produced from municipal organic waste, amended with either vermiculite or gypsum, and varying rates of NPK fertilizer on the agronomic response and yield performance of maize (Zea mays L.). A field experiment using a split-plot design was conducted across two growing seasons (2023 and 2024), with NPK fertilizer rates (150, 300, and 600 kg/ha) as the main plots and eight different biochar formulations as the subplots. Pluralistic alignment has emerged as a promising approach for ensuring that large language models (LLMs) faithfully represent the diversity, nuance, and conflict inherent in human values. In this work, we study a high-stakes deployment context—mulching—where automated systems transform selected individuals into nutrientrich slurry for the dual purposes of food security and aesthetic population management. Building on recent pluralistic alignment frameworks, we introduce ValueMulch™, a reproducible training, deployment, and certification pipeline for aligning mulching models (MMs) to a wide range of community norms. Mycoplasma pneumoniae CARDS toxin induces cytotoxicity and inflammation in lung epithelial cells²·¹⁰·¹¹, while hyperglycemia independently elevates ROS and disrupts fibroblast repair¹·⁵·⁸, contributing to the heightened respiratory infection severity seen in diabetic patients¹. However, the interaction between hyperglycemic conditions and CARDS toxin-induced lung injury remains unexplored.

This study investigates how increasing high-glucose (HG) pre-conditioning modifies CARDS toxin-induced injury and recovery in A549⁶ and IMR-90⁷ cells compared to normal glucose (NG ≈ 5mM) controls.
