Background: The trauma intensive care unit (ICU) at University of Kentucky (UK) Healthcare uses a ventilator separation protocol which provides specific guidelines on the successful weaning of patients from mechanical ventilation. However, many of the nurses are not aware of this protocol as formal education is not included in their orientation or training. This lack of knowledge can lead to risks such as increased patient morbidity, mortality, ICU length of stay, and healthcare-related costs. Research shows that using structured mechanical ventilator weaning tools in the ICU can shorten the duration of mechanical ventilation, which in turn, lowers these associated risks. …

Abstract

Background: Minimally invasive esophagectomy surgery is commonly performed by thoracic surgeons on patients with esophageal cancer. The post-operative care of these patients is meticulous and follows a protocol developed by the thoracic surgeons at the University of Kentucky. At UK Chandler Medical Center, concerns have been raised regarding nurses’ lack of education of the protocol, and therefore decreased implementation. Increasing nursing knowledge pertaining to this protocol by performing educational in-services periodically and standardizing and implementing formal education as part of nursing orientation will decrease the knowledge gap and improve patient outcomes and increase utilization of the post-operative protocol.

Background: In the United States, it is estimated that there are more than 2 million cases of Hepatitis C (HCV), with over 66,000 new cases since 2020 However, an estimated 75% of those with HCV go undiagnosed and less than 35% receive treatment within a year of diagnosis. In 2018, UK Healthcare (UKHC) implemented a universal HCV screening method using a Best Practice Advisory (BPA) for anyone 18 and older who presented to the Emergency Department and had labs ordered. Background: Adherence to diet is a challenging part of managing type 2 diabetes mellitus (T2DM). Guidelines recommend including a registered dietician and/or a diabetes educator. While this is the gold standard, there are barriers to implementation in primary care. The Starting the Conversation (STC) diet recall tool can aid primary care providers (PCPs) in providing brief nutrition counseling for patients with T2DM. Background: The benefits of breastfeeding for both mother and baby are known. It is recommended that all infants be exclusively breastfed until six months (Meek & Noble, 2022; North et al., 2021). Despite this, breastfeeding rates remain low, especially in medically fragile infants requiring care in the neonatal intensive care unit (NICU) (Rollins et al., 2016). A two-year field experiment (2022-2023) was conducted at the University of Kentucky Horticulture Research Farm in Lexington, KY to study the effects of potassium (K2O) fertilization rates on tomato yield and ripening disorders in determinate and indeterminate varieties (‘BHN-589’ and ‘Big Beef’, respectively). In the first year, K2O rates representing the soil test result-based commercial recommendation (0 lbs/acre) and sequentially greater rates (150, 300, and 450 lbs K2O/acre) were applied. The objective of the alfalfa survey was to determine the role of soil fertility in the alfalfa yield plateau. Soil samples at depths of 10, 15, and 30 cm and tissue samples were collected from 114 alfalfa stands across Kentucky and surrounding states. Results indicate that soil fertility is likely contributing but not driving the alfalfa yield plateau. The main objective of the polyhalite incubation study is to determine if POLY4 was soluble enough to support potassium (K) and sulfur (S) for plant growth. Maize stalk lodging reduces grain yields due to the permanent breakage or deformation of stalk tissue before grain harvest. To reveal new insights into the complex problem of maize stalk lodging, this dissertation will examine three primary objectives. First, the direct, diurnal influence of turgor pressure will be shown to have only marginal effects on stalk bending strength, a key trait associated with stalk lodging resistance. Previous studies of the benefits of silicon (Si) supplementation in turfgrass have yielded inconsistent results. This dissertation aimed to assess the benefits if Si supplementation in cool-season turfgrass under more well controlled conditions than many previous studies. The first study assessed the effect of Si supplementation on biomass production and Si accumulation of tall fescue (Festuca arundinacea) and bentgrass (Agrostis stolonifera) in buffered Hoagland’s media (pH 6) with four sodium silicate (Na2SiO3) treatments (0, 0.5, 1, 2, and 4 mM Si). Huanglongbing is a citrus disease devastating the worldwide citrus industry. It is caused by the pathogen Candidatus Liberibacter asiaticus (CLas), and spread through the Asian Citrus Psyllid (ACP). Pathogenic BAG5 variants recently linked to dilated cardiomyopathy (DCM) prompt further investigation into phenotypic, mutational, and pathomechanistic aspects. We explored the clinical and molecular characteristics of DCM associated with BAG5 variants, uncovering the consistently severe manifestation of the disease and its impact on the endoplasmic reticulum (ER) stress response. Analysis involved three siblings affected by DCM, along with their four unaffected siblings and consanguineous parents. Exome and Sanger sequencing identified a novel BAG5 variant, c.444_445delGA (p.Lys149AsnfsTer6), homozygous in affected siblings and heterozygous in parents and unaffected siblings. Anxiety has been improved in some, but not all, exercise or lifestyle interventions that have investigated this component of QOL. This study aimed to explore the association between body mass index (BMI) during adolescence and the risk of breast cancer in adulthood among a Chinese female population. We conducted a nested case-control study within a prospective cohort comprising 62,487 Chinese women aged 25-55 years. Adolescent BMI was retrospectively collected via interviewer-administered questionnaires and analyzed as the primary exposure variable. Breast cancer incidence served as the primary outcome. Compared to the general population, pregnant people and newborns are at a higher risk of contracting infectious diseases and experiencing worse adverse outcomes. Vaccines delivered during pregnancy, known as prenatal vaccines, represent a critical yet underused form of preventive care for these populations in the United States. Access to high-quality residential greenspace is associated with improved long-term health, particularly mental health. However, many interventions to promote “greening” or increased greenspace in urban areas have been shown through numerous qualitative studies to exacerbate environmental and health inequities, primarily through green gentrification. Our objective was to quantify the association between greenspace and mental distress in the U.S. as well as evaluate whether the association between greenspace and mental distress is modified by housing tenure. Background: The US has seen a consistent increase in obesity rates over the last sixty years across all racial and ethnic groups, with similar trends even among privileged groups. This study examines Macronutrient intake trends in US adults from 1999-2018 and their association with demographic factors: race ethnicity, age, gender, and education.

Methods: We conducted a serial cross-sectional analysis of US adults using 24-hour dietary recall data from 10 NHANES survey cycles (1999-2018). Piecewise regression models identified significant dietary pattern changes over time, by demographic groups. Statistical analysis was done in R and SAS utilizing NHANES sampling weights.

Background: The opioid crisis and climate change are two severe public health crises. There may be a synergistic relationship between the two, as extreme temperatures events (both hot and cold) may alter behavior and cause physiological stress that makes fatal opioid overdose more likely. Methods: We analyze the association between fatal opioid overdose and extreme temperature exposure using a case-crossover design and a distributed lag non-linear model, which allows us to look at the odds of death 0-6 days after temperature exposure. The United States incarcerates women at the highest rate globally (Fair & Walmsley, 2022), while simultaneously failing to provide incarcerated individuals with adequate reproductive health care. Perinatal programs, including a combination of one-on-one and group prenatal and postpartum education, labor and birth support with a doula, separation support, and lactation care have been implemented as an alternative to standard care in around 21 states (Wilson et al., 2022).
