Efficacy And Safety Of A Four-Drug, Quarter-Dose Treatment For Hypertension: The Quartet Usa Randomized Trial, Mark D Huffman, Et Al.
2020-Current year OA Pubs
New approaches are needed to lower blood pressure (BP) given persistently low control rates. QUARTET USA sought to evaluate the effect of four-drug, quarter-dose BP lowering combination in patients with hypertension. QUARTET USA was a randomized (1:1), double-blinded trial conducted in federally qualified health centers among adults with hypertension. Participants received either a quadpill of candesartan 2 mg, amlodipine 1.25 mg, indapamide 0.625 mg, and bisoprolol 2.5 mg or candesartan 8 mg for 12 weeks. If BP was >130/>80 mm Hg at 6 weeks in either arm, then participants received open label add-on amlodipine 5 mg. The primary outcome …

Low-load resistance exercise (LLRE) to failure can increase muscle mass, strength, endurance, and mitochondrial oxidative capacity (OXPHOS). However, the impact of adding blood flow restriction to low-load resistance exercise (LLBFR) when matched for volume on these outcomes is incompletely understood. This pilot study examined the impact of 6 weeks of single-legged LLBFR and volume-matched LLRE on thigh bone-free lean mass, strength, endurance, and mitochondrial OXPHOS. Twenty (12 males and 8 females) untrained young adults (mean ± SD; 21 ± 2 years, 168 ± 11 cm, 68 ± 12 kg) completed 6 weeks of either single-legged LLBFR or volume-matched LLRE. Participants …

Purpose: Central venous catheterization is a critical skill in perioperative and critical care medicine. During their postgraduate-year training (PGY), postgraduate trainees are expected to perform central venous catheter (CVC) insertion. Despite learning anatomy in medical school, the demanding nature of the hospital environment requires continuous reinforcement of their knowledge. This pilot study examines the efficacy of three-dimensional (3D) anatomy technology in teaching vascular anatomy for CVC insertion through a flipped classroom approach. Methods: An educational 3D anatomy platform "Complete Anatomy" was utilized to create central-line vascular models to teach vascular anatomy. Trainees completed an online course and a self-assessment before …

Subjective Socioeconomic Status Moderates How Resting Heart Rate Variability Predicts Pain Response, Jacinth Jia Xin Tan, Chin Hong Tan, Michael W. Kraus
Research Collection School of Social Sciences
Higher resting heart rate variability (HRV)—an index of more flexible response to environmental stressors, including noxious stimuli—has been linked to reduced perception of experimentally induced pain. However, as stress responses are adapted to one’s chronic environments, we propose that chronic exposure to threats captured by one’s subjective socioeconomic status (SSS) may shape different adaptations that produce distinct pain responses linked to higher resting HRV. Specifically, lower SSS individuals with more threat exposures may prioritize threat detection by upregulating sensitivity to stressors, such as acute pain. Therefore, higher HRV would predict greater perceived acute pain among lower SSS individuals. In contrast, …

Eosinophilic Jejunitis Presenting As Intractable Vomiting, Persistent Leukocytosis, And Ascites In A Young Adult Patient, Dawood Tahir, Shravya Ginnaram, Erkanda Ikonomi
Abington Jefferson Health Papers
Eosinophilic enteritis (EoN) poses a distinctive challenge, affecting individuals with various clinical presentations depending on the layer and extent of the bowel wall. We present a case of a 19-year-old female with abdominal pain, vomiting, and loose stools for 1 month. Labs were significant for persistent leukocytosis with peripheral eosinophilia. A computed tomography of the abdomen/pelvis demonstrated moderate abdominal ascites and moderately diffuse mucosal thickening of jejunal loops. A diagnostic paracentesis unveiled low serum ascites albumin gradient and 92% eosinophils. Push enteroscopy resulted in no significant biopsy findings, though a laparoscopic full-thickness jejunal biopsy exhibited increased eosinophils in the bowel …

Poultry litter (litter) is a nutrient dense fertilizer that increases nutritive value and yield in pastures in the mid-southern US. Nutrient losses due to runoff and nitrogen volatilization are common when broadcasting litter. As such, incorporating litter below the soil surface (subsurface) was evaluated in comparison to broadcasting in 2021 and 2022 by quantifying yield and nutritive value of annual forages. The study was a randomized complete block design with three forage treatments—sorghum-sudangrass only (Sorghum bicolor L.), cowpea only (Vigna unguiculata L.), and their mixture, and three litter application methods (broadcast, subsurface, and a no litter control). Litter …

Further reductions of crude protein in broiler diets may cause essential amino acids traditionally considered to be supplied at adequate levels to become limiting. Requirement data for histidine are currently scarce and this amino acid is uniquely able to be stored within the body. Thus, the objectives of this study were to evaluate female Cobb 500 broiler growth performance and carcass characteristic responsiveness to increasing digestible histidine to digestible lysine ratios (dHis:dLys) during a 22- to 42-day feeding period and determine if dHis:dLys in preceding feeds influence the former responses. Starter diets were formulated to contain dHis:dLys of 33% or …

The association of hydration knowledge and health habits with hydration status and fluid intake is rarely examined. We sought to determine whether knowledge or physical health behaviors predict physiological hydration status and fluid intake. Ninety-six participants (59 female; 27 ± 10 year) completed the previously validated hydration survey. Participants then recorded total fluids consumed (TFC), collected urine, and tracked void frequency for 24 h. Hydration status was assessed via 24 h urine specific gravity (USG) and osmolality (Uosm). Health behaviors included self-reported physical activity, BMI, smoking, alcoholic drinking, and sleep status. TFC was significantly correlated with 24 h …

Spirulina platensis is an emerging poultry diet source due to its essential nutrition and bioactive ingredients. Spirulina microalgae confers beneficial effects on broiler's growth performance, immune response, and antioxidant activity. We investigated the effect of Spirulina platensis on BCO lameness, one of the most driving factors for economic and animal well-being concerns in the poultry industry. Additionally, supplementation of Availa ZMC containing organic trace minerals – zinc, manganese, and copper – was shown to alleviate lameness by 20 to 25%. We hypothesized that pairing microalgae and organic trace minerals might confer a synergistic effect in reducing BCO. A 2 × …

A Typological Study Of The Provision And Use Of Communal Outdoor Space In Australian Apartment Developments, Julian Bolleter, Paula Hooper, Alex Kleeman, Nicole Edwards, Sarah Foster
Research outputs 2022 to 2026
There is a consensus that higher-density urban settings need to be accompanied by communal outdoor space (COS) to bolster the well-being of apartment residents. Nonetheless, there is a lack of studies identifying COS types in apartment buildings and systematically assessing the degree to which they provide greenery and are used by residents. In response, this study developed a COS typology for apartment buildings in Australian cities, measured the degree to which each COS type provides access to greenery, and examined which COS types received the most frequent visitation via a resident survey (n = 975). Results show that some dominant …

We aim to examine the association of sleep duration, sleep quality, late chronotype, and circadian misalignment with glycemic control and risk of complications in young adults with youth-onset type 2 diabetes followed in the Treatment Options for Type 2 Diabetes in Adolescents and Youth (TODAY) study. Self-reported sleep duration, quality, timing, and circadian misalignment were assessed via a modified Pittsburgh Sleep Quality Index (PSQI) questionnaire, and chronotype was assessed via the Morningness-Eveningness Questionnaire (MEQ). We examined diabetes complications including loss of glycemic control (defined as hemoglobin A1c ≥8%), hypertension, dyslipidemia, albuminuria, and diabetic peripheral neuropathy. Multivariable logistic regression models were …

Strengthening U.S. Jail Systems’ Response To Infectious Diseases: An Evaluation Of The Covid-19 Pandemic, Erinn Bacchus
Dissertations and Theses
Jails across the United States were struck with increased infections and deaths during the COVID-19 pandemic. Studies have shown the structural make up of jails, lack of preparedness plans, and overcrowding contributed to health risks and poor health outcomes both inside jails and local communities. Yet little research has been dedicated to strengthening jail responses to infectious disease outbreaks spanning prevention measures, data collection, and reentry planning. Gaps include information on the (1) myriad infectious disease mitigation strategies used in jails and adherence to CDC prevention guidelines, (2) development of a standardized epidemiologic surveillance system, and (3) experiences working at …

Background: Virtually the entire spectrum of liver disease is observed in association with type 2 diabetes mellitus (T2DM); indeed, T2DM is now the most common cause of liver disease in the U.S. We conducted a pilot study to investigate the relevance of increased microbial translocation and systemic inflammation in the development of liver injury in patients with T2DM. Methods: Patients with T2DM (n = 17) and non-diabetic controls (NDC; n = 11) aged 25–80 yrs. participated in this study. Serum levels of endotoxin, calprotectin, soluble CD14 and CD163, and several inflammatory cytokines were measured. In addition to standard liver injury …

Economics and Finance in Indonesia
Mental disorders can result in more significant economic losses than chronic physical illnesses. Mental disorders can be influenced by both economic and non-economic factors. This study investigates the influence of marital status and economic status interventions on depression symptoms with Propensity Score Matching (PSM). The data of this analysis was obtained from the Indonesian Family Life Survey (IFLS V). The findings indicated that individuals who were married experienced a 6% reduction in depressive symptoms, while those in the middle to high economic status category experienced a 3% decrease. Enhancing well-being and promoting effective communication play crucial roles in mitigating depressive …
