Doctor of Nursing Practice Final Project Abstract

PURPOSE

This policy-focused Doctor of Nursing Practice (DNP) quality improvement (QI) project aims to secure grant funding through the Gus Schumacher Nutrition Incentive Program (GusNIP) to implement a produce prescription program at a community clinic in Texas. The program targets low-income adults (18-65 years) with type II diabetes who experience food insecurity and are affected by Medicaid non-expansion.

BACKGROUND

Texas’ decision not to expand Medicaid has exacerbated food insecurity among low-income residents, particularly those managing type II diabetes. This decision creates a healthcare policy gap that this project aims to address with a food-as-medicine program.

Mysid shrimp (Neomysis americana) play a pivotal role in estuarine food webs by linking primary producers to higher trophic levels and serving as an important food source for commercially valuable fish. Despite their ecological significance, mysids are often under sampled, leading to gaps in our understanding of their population dynamics and life history traits. My study investigated N. americana over a full year (June 2023 to May 2024) in the temperate Damariscotta River Estuary in Maine. Work related musculoskeletal injuries are a significant concern in physically demanding environments such as large retail stores, often leading to high workers’ compensation costs and decreased productivity. This evidence to practice paper explores the potential benefits of integrating industrial athletic trainers (ATs) into standard workplace safety programs within retail settings. Drawing from existing literature, the paper highlights ATs’ expertise in injury prevention, ergonomic assessment, rehabilitation, and health education. Studies indicate that incorporating ATs results in fewer workplace injuries, faster return to work times, and substantial cost savings. The study investigated the impact of crime on interstate road transport operations in north-central Nigeria. The study relies on descriptive statistics to describe the features of the populations. 200 questionnaires were purposefully administered to the four major interstate motor parks in the capital of some north-central states (i.e., Minna, Lokoja, Benue, and Lafia) using random sampling techniques. The gathered data was analysed using percentages and charts, while the T-test statistics were used to test the study hypothesis. The rates of obesity and suicidal behavior have continued to increase over the years among young adults in the United States. Previous research has indicated a relationship between obesity and suicidal behavior, primarily among women, yet minimal research has been done to examine factors that explain this relationship (i.e., mediators). This study investigated the role of depression, body dissatisfaction, body shame and guilt, insomnia, physical health, and social anxiety on obesity and suicidal ideation among 209 college-aged Latina women. For years, minor league baseball players received salaries far below the federal minimum wage, despite working sixty hours per week. It was not uncommon for these professional athletes to share cramped hotel rooms, sleep on floors, or live out of their cars. Most had to secure loans and pursue odd jobs to make ends meet during the offseason. Such sacrifices were necessary for minor leaguers to achieve their dreams of reaching the major league level, particularly as those abysmal working and living conditions became entrenched by adverse legislation and judicial rulings.

Objective: The World Health Organization has identified intrinsic capacity and functional ability as key constructs in enabling healthy ageing. However, functional ability is distinct from actual levels of social and physical participation, which research has shown to be associated with health and wellbeing. Following recipes while cooking is an important but difficult task for visually impaired individuals. We developed OSCAR (Object Status Context Awareness for Recipes), a novel approach that provides recipe progress tracking and context-aware feedback on the completion of cooking tasks through tracking object statuses. OSCAR leverages both Large-Language Models (LLMs) and Vision-Language Models (VLMs) to manipulate recipe steps, extract object status information, align visual frames with object status, and provide cooking progress tracking log. Food selectivity involves avoiding certain foods based on type, texture, brand, smell, temperature, or presentation, and it affects 80% of children with intellectual and developmental disabilities. Although several antecedent-based and consequence-based behavioral interventions exist to increase acceptance and consumption of food in this population, one antecedent-based intervention that has shown promise both within and outside of a feeding context is the high-probability instructional sequence, which consists of the delivery of three high-probability instructions before delivery of a low-probability instruction. The typical modern diet is brimming with food additives used in food processing. Additives such as antibiotic residues, GMOs, pesticide residues, anti-caking agents, emulsifiers, artificial sweeteners, colorants, flavor enhancers, and preservatives. Most of these additives seem to shift the gut microbiome towards more pro-inflammatory signals. When all these additives are considered as an amalgam, such as with the modern processed diet, the ramifications on the gut microbiome are concerning. Obesity, which is characterized by low-level chronic inflammation, is likely exacerbated by these food additives. Consequently, most of these additives should be avoided or banned for optimal health.

Economic research has documented significant short, medium, and long-term labor market consequences following recessions (Goodman & Mance, 2011; Yagan, 2017; Farber, 2011). The Covid-19 recession triggered an unprecedented economic downturn, disproportionately impacting employment in industries with high concentrations of low-income and female workers. In this paper, I leverage the substantial spatial variation in employment shocks to examine their effects on earnings and employment outcomes in both the short and medium terms. I find remarkably stable trends in earnings and employment when comparing counties experiencing high employment shocks with those that did not. Background

Prediabetes affects one in three US adults. Over 80% of individuals with prediabetes are unaware of having the condition. Evidence-based clinical practice guidelines (CPG) recommend screening adults 35 to 70 years old with a BMI of 25 or higher. Prediabetes treatment consists of lifestyle modification, referrals to nutrition counseling, and metformin for individuals who cannot make lifestyle changes. Diagnosis and treatment of prediabetes reduces the risks or delays the progression to type 2 diabetes.

Introduction: Rare solid tumors account for one-quarter of cancers among adults in the United States, but few resources have been devoted to their treatment. We evaluated the efficacy of pembrolizumab, a programmed cell death-1 inhibitor, in patients with rare solid tumors.

Methods: We conducted a phase 2 basket trial that included patients with rare, advanced tumors. Patients were enrolled in the study in nine tumor-specific and a 10th cohort of miscellaneous rare histologies. Patients received pembrolizumab 200 mg intravenously every 21 days. Climate change-induced drought stress is a significant constraint on global rice (Oryza sativa L.) production, threatening food security. This study evaluated the drought resilience of 15 diverse rice genotypes from the USDA mini-core collection under field, greenhouse, and osmotic stress conditions. Field trials assessed reproductive-stage drought tolerance based on panicle length (PL), number of spikelets per panicle (NSP), and spikelet sterility (SS). Greenhouse experiments examined moisture retention at the vegetative stage. Purpose: The Pregnancies' Care Integrating translational Science, Everywhere Network was established to investigate specific placental disorders (pregnancy hypertension, preterm birth, fetal growth restriction and stillbirth) in sub-Saharan Africa. We created a repository of clinical and social data with associated biological samples from pregnant and non-pregnant women. Alongside this, local infrastructure and expertise in the field of maternal and child health research were enhanced.

Schizophrenia is a psychiatric disorder impacting behavior, emotions, and communication. Atypical antipsychotics are increasingly prescribed due to their significant reduction of both positive and negative symptoms and low risk of extrapyramidal side effects (EPS) in schizophrenia patients. This thesis explores the influence of brand identity and media on shaping evolving perceptions of health across generations. The three generations considered are Generation X, Millennials, and Generation Z. The initial hypothesis predicted the popular media and brands that each generation interacts with are most likely to influence their perception. Additionally, the health brands most popular with each generation would influence their overall health perception, based on the identity and values of the brand. This qualitative study examines the perception of Artificial Intelligence (AI) and how this technology can advance sustainable development in small businesses in Central Appalachia. Participants described how they leveraged AI-enabled technologies to address economic challenges as well as how they overcame the technological barriers often associated with emerging technologies. Findings reveal that AI can significantly improve efficiency, streamline workflows, and enhance visibility for Appalachian entrepreneurs. However, the research also highlights challenges, including trust issues, limited resources, and resistance to change, which hinder adoption. Delayed cerebral ischemia (DCI) is a significant complication of aneurysmal subarachnoid hemorrhage (aSAH). This study profiled immune responses after aSAH and evaluated their association with DCI onset. Twelve aSAH patients were enrolled. Leukocyte populations and cytokine levels were analyzed in cerebrospinal fluid (CSF) and peripheral blood (PB) on days 3, 5, 7, 10, and 14 post-aSAH. PB mononuclear cells (PBMCs) were collected, and their cytokine production quantified following stimulation. Mixed-effects models reveal distinct immune cell dynamics in CSF compared with blood. Aims/hypothesis: Growing evidence suggests that timing may be an important aspect of physical activity that influences cardiometabolic health. However, the current literature is inconclusive regarding the time of day that physical activity offers the greatest metabolic advantages. We investigated associations between hourly physical activity levels and diabetes mellitus and glycaemic biomarkers in a cross-sectional and nationally representative sample of US adults.

Methods: We studied 7074 adults (mean age 48 years; 52% women) from the National Health and Nutrition Examination Survey (2011-2014). Physical activity was measured by actigraphy. A monitor-independent movement summary (MIMS) unit was used to derive the total activity …

Beyond Opioids: The Role Of Polysubstance Exposure In Neonatal Abstinence Syndrome Treatment And Hospitalization, Caelyn Harrison

This thesis explores the impact of prenatal polysubstance exposure, specifically focusing on the need for pharmacological treatment for Neonatal Abstinence Syndrome (NAS) and the duration of their hospital stay. A retrospective analysis of 462 infants, including 121 exposed to polysubstances during pregnancy, showed significant associations between polysubstance exposure and adverse neonatal outcomes. Infants with prenatal polysubstance exposure were more likely to require pharmacological treatment for NAS (52.8%) compared to non-exposed infants (38.7%), as determined by chi-square analysis (χ²(1, N = 462) = 7.36, p = .007). Introduction: TLR9 agonists are immunomodulators that have been of interest for combined use with cancer immunotherapy. TLR9 agonists, such as lefitolimod (MGN1703), significantly increased Th1 response in preclinical models and have demonstrated efficacy in early clinical trials. This trial assessed the safety and preliminary efficacy of the combination of lefitolimod and ipilimumab in patients with advanced solid tumors.

Methods: This was a single-center, open-label, investigator-initiated phase I trial conducted at The University of Texas MD Anderson Cancer Center.
