An Empty Guarantee?, Kevin Thomas Frazier Feb 2025

An Empty Guarantee?, Kevin Thomas Frazier

Ohio Northern University Law Review

Self-governance constitutes the foundation of a republican form of governance. Three pressing threats to self-governance warrant closer scrutiny of the long-forgotten and wrongly interpreted Guarantee Clause (the Clause). Election interference and other limitations on the right to vote may deny individuals the control over their officials required by republican governance. Corporate consolidation of economic, cultural, and political capital likewise threatens to disturb the ability of individuals to act as free agents—unbeholden to one or a few companies for critical information and free and able to pursue their own private goals. And, finally, intentional manipulation of citizens by state governments in …

West Virginia V. Epa: Maybe A Big Deal, But Maybe Not, Michael Lewyn Feb 2025

West Virginia V. Epa: Maybe A Big Deal, But Maybe Not, Michael Lewyn

St. John's Law Review

(Excerpt)

In West Virginia v. EPA, the Supreme Court held that the Environmental Protection Agency (“EPA”) lacked statutory authority to enact the Clean Power Plan, an EPA rule that encouraged coal-fired power plants to use non-coal sources of energy. The Court’s decision relied on the “major questions doctrine.” Under this doctrine, even if an unclear statute does not directly prohibit an administrative agency’s action, courts will reject an agency’s action when it is “asserting highly consequential power beyond what Congress could reasonably be understood to have granted.” Because the major questions doctrine is itself unclear, a variety of commentators suggested …

British Latino: Investigating Discourse-Stylistic Variables And The Construction Of Persona Within Podcasting, Jonathan A. Pye

International Journal of the Linguistic Association of the Southwest

This study expands upon previous work (McIlwaine et al., 2011; McIlwaine & Bunge, 2016) by analyzing how the Latin American community in the UK constructs identity through podcasting. The sociolinguistic construct of the persona (Campbell-Kibler, 2008; Coupland, 2007; D’Onofrio, 2020; Eckert, 2019; Podesva, 2007) is operationalized to understand how the persona of the British Latino is discursively constructed in the podcast of the British Latino Network. Using qualitative discourse analysis and values coding (Saldaña, 2013), this analysis finds that the British Latino persona is indexed in the podcast as a bricolage of four principal attributes: (1) aspirational, (2) educated, …

Abdominal/Pelvic Ct Scans Ordered Pre-Physical Exam In The Emergency Department, A Pilot Study, Ryanne M. Burke, Hannah Zazulak, Andrea M. Bodine Md

Advances in Clinical Medical Research and Healthcare Delivery

Introduction: Computerized tomography (CT) scans are effective for evaluating abdominal pain, a common emergency department (ED) complaint. In ED “physician in triage” (PIT) models, CTs can be ordered in triage. PIT has been shown to have higher clinical yield of abdominal/pelvic CTs than the traditional ED model. We explored the benefit versus radiation risk of abdominal/pelvic CTs ordered in an ED where physicians may order CTs in triage (pre-PE) to determine if results were similar to post physical exam (post PE) CTs.

Methods: In this retrospective study at Berkshire Medical Center in Pittsfield, Massachusetts, 969 patients had abdominal/pelvic CTs in …

A Compelling Case Of The Use Of Electroconvulsive Therapy In Severe Treatment Resistant Depression With Suicidality, Neha Narayanan, Ryan Tam, James Mead

Advances in Clinical Medical Research and Healthcare Delivery

Treatment-resistant depression is a condition that occurs when an individual, experiencing major depressive disorder episodes, has failed the first line of treatment with medication therapy of adequate dose and duration. This is important to be addressed as there is a high likelihood of progression to having thoughts and an intended plan for suicide. Electroconvulsive therapy is a recommended option for those who have failed medication therapy, but it has been controversial in its use due to its stigma and side effects of memory impairment. Here, we present a case of a patient who had undergone several sessions of ECT with …

Advances in Clinical Medical Research and Healthcare Delivery

Enfortumab vedotin(EV) comprises of an anti–nectin-4 monoclonal antibody bound to the chemotherapeutic agent called monomethyl auristatin E, forming an antibody-drug conjugate. It is currently approved for the treatment of locally advanced or metastatic urothelial carcinoma after disease progression on two prior lines of therapy including chemotherapy and immunotherapy. Since nectin-4 is also expressed on the surface of keratinocytes, cutaneous adverse effects are common. Our clinical image depicts a cutaneous adverse reaction to EV and its resolution after discontinuation of EV.

The Road Ahead: Emerging Therapies For Primary Iga Nephropathy, Edward J. Filippone, Rakesh Gulati, John Farber Feb 2025

The Road Ahead: Emerging Therapies For Primary Iga Nephropathy, Edward J. Filippone, Rakesh Gulati, John Farber

Division of Nephrology Faculty Papers

Primary IgA nephropathy (IgAN) is the most common form of primary glomerulopathy. A slowly progressive disease presenting in the young to middle-aged, most patients with reduced eGFR or proteinuria will progress to end-stage kidney disease (ESKD) in their lifetimes. The pathogenesis involves increased production of galactose-deficient IgA1 (Gd-IgA1) that forms immune complexes that deposit in the glomerulus, eliciting mesangial cell proliferation, inflammation, and complement activation. The backbone of therapy is supportive, including lifestyle modifications, strict blood pressure control, and renin-angiotensin system inhibition targeting proteinuria < 300 mg/day. Sodium-glucose transporter 2 inhibitors are indicated for persisting proteinuria or declining eGFR. Sparsentan is indicated for persisting proteinuria. Immunosuppression should be considered for all patients at risk for progression (persisting proteinuria and/or declining eGFR). To reduce Gd-IgA1 production, targeted-release budesonide is approved. Agents targeting B cell survival factors APRIL or BAFF/APRIL have significantly reduced Gd-IgA1 production and proteinuria in phase 2 trials but await phase 3 data for approval. To reduce inflammation, high-dose steroids are ineffective and toxic in Caucasian patients, although lower-dose regimens may be effective in Chinese patients. Complement inhibition is being actively studied. The factor B inhibitor iptacopan has conditional approval. The terminal pathway inhibitors cemdisiran and ravulizumab show promise in phase 2 studies. Our current approach for those requiring immunosuppression involves combining the reduction of Gd-IgA1 (nefecon) with suppressing the effects of inflammation (iptacopan). The optimal duration of such therapy is uncertain. Clearly, there is more to be learned with many trials underway.

The Military Order Of The Serpent And Philippine War Memory, 1904-1974, Eric Jason Kriner Feb 2025

The Military Order Of The Serpent And Philippine War Memory, 1904-1974, Eric Jason Kriner

Student Theses and Dissertations

The Military Order of the Serpent (M.O.S.) was an American anti-Filipino minstrel fraternity and veterans' organization comprised of members of the United Spanish War Veterans. To commemorate the Philippine-American War, the group dressed in Filipino blackface and paraded around the United States from 1904 to 1974. Publicly, the group’s narrative arc posited that Filipinos, by virtue of their contact with the United States Military, underwent a rapid, three-phased evolution from savage to civil. Privately, they celebrated one of the war’s atrocities, and expressed their racist hostilities and amalgamationist anxieties. During the 1920s, when Filipino emigration to the United States began …

For most researchers, academic publishing serves two goals that are often misaligned-knowledge dissemination and establishing scientific credentials. While both goals can encourage research with significant depth and scope, the latter can also pressure scholars to maximize publication metrics. Commercial publishing companies have capitalized on the centrality of publishing to the scientific enterprises of knowledge dissemination and academic recognition to extract large profits from academia by leveraging unpaid services from reviewers, creating financial barriers to research dissemination, and imposing substantial fees for open access. We present a set of perspectives exploring alternative models for communicating and disseminating scientific research. Acknowledging that …

The Association Between Online Health Information Characteristics And Appropriate Trust In Informational Quality Among Patients With High Cardiovascular Risk, Lim Woei Xian Feb 2025

The Association Between Online Health Information Characteristics And Appropriate Trust In Informational Quality Among Patients With High Cardiovascular Risk, Lim Woei Xian

Student Works (2020-2029)

More patients are turning to the Internet to understand their health and make health decisions. However, the quality of online health information (OHI) is highly variable, and it may contain misinformation. Patients who lack the necessary skills to appraise OHI may trust poor-quality OHI. Thus, this study aims to examine the association between OHI characteristics and appropriate trust in patients with high cardiovascular (CV) risk. This is a secondary analysis from a quantitative cohort study of 270 patients from a primary care clinic in Kuala Lumpur, Malaysia. Patients recorded each OHI encountered over two months in a digital information diary …

Sleep-Wake Variation In Body Temperature Regulates Tau Secretion And Correlates With Csf And Plasma Tau, Geoffrey Canet, Brendan P. Lucey, Et Al. Feb 2025

Sleep-Wake Variation In Body Temperature Regulates Tau Secretion And Correlates With Csf And Plasma Tau, Geoffrey Canet, Brendan P. Lucey, Et Al.

2020-Current year OA Pubs

Sleep disturbance is bidirectionally associated with an increased risk of Alzheimer's disease and other tauopathies. While the sleep-wake cycle regulates interstitial and cerebrospinal fluid (CSF) tau levels, the underlying mechanisms remain unknown. Understanding these mechanisms is crucial, given the evidence that tau pathology spreads through neuron-to-neuron transfer, involving the secretion and internalization of pathological tau forms. Here, we combined in vitro, in vivo, and clinical methods to reveal a pathway by which changes in body temperature (BT) over the sleep-wake cycle modulate extracellular tau levels. In mice, a higher BT during wakefulness and sleep deprivation increased CSF and plasma tau …

Faculty, Staff and Student Publications

Ion suppression is a major problem in mass spectrometry (MS)-based metabolomics; it can dramatically decrease measurement accuracy, precision, and sensitivity. Here we report a method, the IROA TruQuant Workflow, that uses a stable isotope-labeled internal standard (IROA-IS) library plus companion algorithms to: 1) measure and correct for ion suppression, and 2) perform Dual MSTUS normalization of MS metabolomic data. We evaluate the method across ion chromatography (IC), hydrophilic interaction liquid chromatography (HILIC), and reversed-phase liquid chromatography (RPLC)-MS systems in both positive and negative ionization modes, with clean and unclean ion sources, and across different biological matrices. Across the broad range …

In inflammatory diseases or following an injury, dysregulated inflammation is a common driver of pain and tissue damage. Macrophages are immune cells that contribute to the initiation, maintenance, and resolution of inflammation due to their phenotypic plasticity in response to signals from inflammatory microenvironments. Macrophages infiltrate and polarize toward a pro-inflammatory phenotype (M
