Journal of Community Hospital Internal Medicine Perspectives
We present a rare case of Bacteroides fragilis vertebral osteomyelitis, in a 20-year-old female with no underlying medical conditions. She presented with a three-week history of lower back pain, myalgias, rigors, and night sweats. The patient was febrile and tachycardic, with borderline low blood pressures. Her initial labs showed elevated inflammatory markers with no leukocytosis. MRI of the thoracic and lumbar spine demonstrated T12/L1 discitis/osteomyelitis with phlegmonous changes and concerns for epidural abscess, causing mild to moderate spinal stenosis. Bacteroides fragilis grew in an anaerobic bottle, and a CT-guided biopsy of the T12/L1 lesion grew Bacteroides fragilis. E-test sensitivities revealed …

A Unique Metastatic Spread Of Lung Adenocarcinoma To The Stomach: A Case Report And Literature Review., Aniqa Faraz, Ammad Naeem, Hafiz Muhammad Aslam
Journal of Community Hospital Internal Medicine Perspectives
Lung cancer is a leading cause of cancer-related deaths and commonly spreads to the brain, bones, liver, and adrenal glands. Gastric metastases from lung cancer are uncommon and can occur in melanoma, breast cancer, and lung cancer. Most patients show no symptoms at presentation; however, metastasis from lung cancer portends a poor prognosis. This case report highlights a rare instance of lung cancer metastasizing to the stomach in a 60-year-old female that was detected incidentally on a PET scan. The scan showed two new stomach masses with increased FDG uptake and contrast enhancement. The patient had chemoimmunotherapy after receiving palliative …

Downhill esophageal varices (EVs) were first described in 1964 by Felson and Lessure.1 They are rare with an incidence rate of 0.5% in the general population.1 They usually develop due to an increase in pressure and/or obstruction of the superior vena cava (SVC) and rarely present with hematemesis. SVC syndrome, surgical ligation of the SVC, Castleman syndrome, pulmonary hypertension, and hypercontractile esophageal motility disorders can cause this.1 We report a case of downhill EVs in an older male with alcohol use disorder with incidental finding
of SVC stenosis secondary to an old, unused chemotherapy port.

Clinicians frequently encounter patients in the hospital with mild-to-moderate COVID-19 who are at high risk for disease progression. Antiviral drugs reduce the rate of progression to severe COVID-19 when given to these patients within 5-7 days of symptom onset. To look at the utilization of antiviral medications in hospitalized patients with mild-to-moderate COVID-19, a detailed chart review was conducted of 500 randomly selected patients from the Charleston Area Medical Center COVID-19 Registry. We identified 124 patients with mild-to-moderate COVID-19 who were eligible for antiviral treatment. Only 41 of the 124 patients (33.1%) received antiviral treatment. Providers listed mild symptoms as …

The rapid increase in internet usage, digital transformation, and the rise of interconnected devices have greatly expanded the attack surface, introducing new and evolving cybersecurity challenges. Conventional security solutions frequently have difficulty adjusting to complex threats and the vast dimensionality of network traffic data, particularly in the case of imbalanced datasets. To tackle these challenges, this research introduces a Hybrid Intrusion Detection System (HyIDS-EVO) that combines the Energy Valley Optimizer (EVO) for feature selection and dimensionality reduction with machine learning classifiers, which include Support Vector Machine (SVM), Random Forest (RF), Decision Tree (DT), and K-Nearest Neighbors (KNN). The system’s effectiveness …

Playa Sediment Bioaccessibility Presents Previously Unknown Risks To Population Health In A Water-Stressed Region Undergoing Lake Desiccation: Salton Sea, Usa, Jordan Jaeger
Theses
Globally, drylands are expanding, and endorheic basins are in rapid decline. Saline lake desiccation exposes lakebed playa likely to become dust, posing potential health risks to surrounding communities. Studies investigating health risks from playa dust exposure often focus on health effects associated with the presence of airborne particulates, but fewer studies investigate health risks associated with dust source chemistry. This study examines dust source particle composition in a desiccating inland sea in a water-stressed agricultural region of California. Specifically, chemical bioaccessibility of bound trace elements uncovers risks to children, adults, and agricultural worker health. Arsenic exposure via sediment ingestion poses …

Evaluating And Supporting Transformative Agreements: Five Institutional Perspectives In Discussion, Anna R. Craft, Peggy Kain, Danica Lewis, Camille Mccutcheon, William Joseph Thomas
Libraries Professional Work
Transformative—or Read and Publish—deals are impacting the open access (OA) publishing landscape, allowing libraries to test new models for funding and incentivizing OA publishing, while also providing direct benefits for authors, including discounts and waivers on article processing charges (APCs). But how sustainable are transformative deals in light of library materials budgets and workloads for personnel who negotiate these deals, manage the implementations, and support campus authors? And how can libraries evaluate the perceived value of these deals in relation to the specific needs of their institutional users?

Beyond Words: Analyzing The Effectiveness Of Land Acknowledgements, Kendall A. Trevino
Beyond Words: Analyzing The Effectiveness Of Land Acknowledgements, Kendall A. Trevino
2025 Fall Honors Capstones Projects - Archive
Land Acknowledgements (LAs) have become common in recent years across cities, sports organizations, universities, and other institutions. However, many were created to avoid criticism rather than to pay genuine respect. Tribal members and both Indigenous and non-Indigenous scholars emphasize that LAs have specific criteria they must meet to be considered meaningful. These criteria include recognition of the past, promoting tribal sovereignty, emphasizing that Natives are still here, and a relationship is established with the tribes being acknowledged. LAs should promote tribal sovereignty and establish strong relationships with the communities they acknowledge. The research in this paper draws on academic articles, …
The Tax Rebate Trade-Off, Salvatore Ciucci
The Tax Rebate Trade-Off, Salvatore Ciucci
Fondazione Eni Enrico Mattei Working Papers
This study extends the theory of tax evasion by presenting a model of collaborative tax evasion between buyers and sellers. Buyers differ only in their level of tax morale, and tax evasion occurs when the seller fails to issue a receipt for the transaction. To counteract this, the government can disrupt the collusion between sellers and buyers by offering a tax rebate to buyers who request and retain the transaction receipt. The theoretical findings show that the tax rebate introduces a policy trade-off for the government between aggregate quantity and tax revenue. Furthermore, the marginal effect of the rebate on …
