Oracle EHR Real-World Data (OERWD) is a national de-identified data set useful for observational findings across 139 non-affiliated healthcare systems. Data in OERWD is associated with a de-identified provider ID categorized and documented using National Uniform Claim Committee (NUCC) taxonomy codes. There are 2,421,948 providers in OERWD associated with both their specialty and contributing organization. We profile the data for overall distribution and co-occurrence of specialties, enabling research related to treatment patterns and outcomes.

Neutrosophic Sets and Systems

Antimicrobial resistance is a global public health issue that, exacerbated by inappropriate antibiotic use, leads to reduced bactericidal effects, multidrug resistance, increased medical costs, prolonged hospital stays, and higher mortality rates. This study evaluates the antibacterial and antifungal activity of the es-sential oil of Hedyosmum cuatrecazanum Occhioni against various clinically relevant bacterial and fungal strains. The essential oil was extracted by hydrodistillation, yielding a 0.26% extract. Its components were characterized by gas chromatography coupled with mass spectrometry (GC-MS). The results demonstrate that the essential oil exhibits significant antibacterial activity, comparable to commercial antibiotics. Hedyosmum cuatrecazanum oil showed the highest antibacterial …

Foliar fungal diseases and Fusarium head blight (FHB) are among the major causes of significant yield losses in wheat production, particularly in Nebraska. Resistant cultivars and fungicide applications are common management strategies, but the overuse of fungicides has raised concerns about resistance in Fusarium graminearum populations in Nebraska. This study evaluated the effects of cultivar resistance, fungicide timing and their economic benefits. It also assessed the sensitivity of Nebraska isolates of F. graminearum and F. boothii to a triazole fungicide. Common bed bugs (Cimex lectularius) are obligatory blood-feeding pests that have adapted well to survive in challenging environmental conditions. Known for their high tolerance to dehydration, low water loss, and high critical thermal maximum, they have become difficult to manage. Environmental factors, particularly temperature and humidity, play a significant role in their survival. However, with growing resistance to insecticides, integrated pest management (IPM) strategies are now essential for effective bed bug control. This in vivo study is aimed to assess the antibiofilm and cytotoxic potential of selected microalgae isolates. Different concentration of microalgal isolates used for each of antibiofilm and cytotoxic studies. A total extract of 10 μL MTT was added to each well with LB broth, plates were read using microtiter plate spectrophotometer at 490 nm and 630 nm, respectively. The cell viability was determined in CCD-18℃ cells (3x10^3 cells/well) by MTT assay at 590 nm. IgA Vasculitis is a commonly seen presentation in the pediatric population. The constellation of symptoms typically includes purpuric rash, joint pain, abdominal pain and hematuria. This 12 year old female was seen by her pediatrician for rash on her legs, shortness of breath, cough and night sweats that all started around 2 months ago. Additionally, a 25 pound weight loss was noted over the past year. The rash appeared as nonblanching purpura on her lower extremities, lending suspicion to IgA Vasculitis. Panola Mountain Ehrlichia (PME) is an emerging zoonotic pathogen, transmitted by Amblyomma americanum ticks in the southeastern United States. It is closely related to Ehrlichia ruminantium, the causative agent of heartwater disease. Heartwater disease is an often-fatal illness of ruminant livestock present in Africa and the Caribbean. The taxonomic relationship between PME and E. ruminantium has raised concerns about the pathogenicity of PME in livestock. To determine whether cattle could be naturally infected with PME in an endemic area, we conducted a one-year longitudinal study of Angus-breed beef calves in Fayetteville, Arkansas. INTRODUCTION: Gas gangrene is a rare, often lethal infection of soft tissue that is commonly associated with the Clostridial species due to penetrating injuries. Case reports of spontaneous atraumatic gas gangrene due to the Clostridium septicum species are exceedingly rare. Atraumatic C. septicum is notably related to bowel pathologies, immunodeficiencies, and individuals with vascular insufficiency. The rapid progression of Clostridium myonecrosis contributes to its high mortality rates.

Autotomy, the voluntary loss of a limb, is a common predator evasion strategy in arthropods. Considerable research on autotomy has focused on arachnids, which have developed morphological compensations to manage the stability challenges posed by limb loss, as explained by the extra leg hypothesis. However, studies in insects are unclear about such a mechanism. In hexapods, a tripod gait means that losing any one limb disrupts one of the tripod stances. The translation of pre-clinical anti-cancer therapies to regulatory approval has been promising, but slower than hoped. The opportunistic pathogen Enterococcus faecium colonizes humans and a wide range of animals, endures numerous stresses, resists antibiotic treatment and stubbornly persists in clinical environments. The widespread application of antibiotics in hospitals and agriculture has contributed to the emergence of vancomycin-resistant E. faecium, which causes many hospital-acquired infections. In this Review, we explore recent discoveries about the evolutionary history, the environmental adaptation and the colonization and dissemination mechanisms of E. faecium and vancomycin-resistant E. faecium. Immune-related adverse events (irAEs) have become increasingly prevalent with immune checkpoint inhibitor (ICI) cancer treatment. We present a 79-year-old man with metastatic renal cell carcinoma who developed shortness of breath and hypercapnic respiratory insufficiency after his first cycle of nivolumab and ipilimumab. Laboratory data showed elevated creatinine kinase, troponins, and transaminases. Computed tomography of the chest demonstrated bilateral lower lobe atelectasis. Heart catheterization and endomyocardial biopsy were unremarkable. Electromyogram (EMG) and nerve conduction studies (NCS) of the limb muscles revealed mild diffuse myopathy, normal sensory nerve conductions, and low-amplitude motor responses. Since the onset of the COVID-19 pandemic, a variety of diagnostic approaches, including RT-qPCR, RAPID, and LFA, have been adopted, with RT-qPCR emerging as the gold standard. However, a significant challenge in COVID-19 diagnostics is the wide range of symptoms presented by patients, necessitating early and accurate diagnosis for effective management. Although RT-qPCR is a precise molecular technique, it is not immune to false-negative results. In contrast, CRISPR-based detection methods for SARS-CoV-2 offer several advantages: they are cost-effective, time-efficient, highly sensitive, and specific, and they do not require sophisticated instruments. Bourbon virus is a tickborne virus that can cause human disease. Cases have been reported in Kansas, Oklahoma, and Missouri, USA. We identified Bourbon virus-specific neutralizing antibodies in patients from North Carolina. Bourbon virus infections are likely more common than previously thought, highlighting the need for improved diagnostics and surveillance.

Broiler chicken lameness caused by bacterial chondronecrosis with osteomyelitis (BCO) is presently amongst the most important economic and animal welfare issues faced by the poultry industry, and the estimated economic loss is around USD 150 million. BCO lameness is associated with multiple opportunistic bacterial pathogens inhabiting the respiratory and gastrointestinal tracts. In cases of immune deficiency resulting from stress, injury, or inflammation of the tissue, opportunistic pathogens, mainly Staphylococcus spp., can infiltrate the respiratory or gastrointestinal mucosa and migrate through the bloodstream to eventually colonize the growth plates of long bones, causing necrosis that leads to lameness. BACKGROUND: In 2011, the Veterans Health Administration (VHA) undertook multidisciplinary efforts to improve care for patients with nonhealing foot ulcers and reduce leg amputation rates. This article examines the impact of interdisciplinary care for amputation prevention in the VHA.

METHODS: The VHA patient population was characterized using internal registries. Texas hospital admission data were used to identify sex and zip-code matched cohorts. VHA provided data on national and local frequencies of various amputations and the number of patients with diabetes.

RESULTS: Compared to matched nonveteran patients, veterans treated at VHA hospitals were older and more often self-identified as Black. They …

Immunoglobulin E (IgE)‐mediated food allergies are the most common type of food allergy, often causing rapid symptoms after exposure to allergens posing a serious health risk and a high impact on patient's and caregiver's quality of life. Omalizumab, a humanized anti‐IgE monoclonal antibody, reduces allergic reactions by binding to circulating IgE. Omalizumab has been successfully used in allergic asthma, chronic rhinosinusitis with nasal polyps, and chronic urticaria, and was recently approved for treating IgE‐mediated food allergies by the US Food and Drug Administration (FDA). Prolonged antibiotic exposure causes dangerous hematologic side effects, including neutropenia, in up to 34% of patients. Murine studies established a link between the intestinal microbiota and hematopoiesis. To identify factors that predispose to neutropenia in pediatric patients, we evaluated changes in microbiota‐derived metabolites and intestinal microbiota composition after prolonged courses of antibiotics. In this multi‐center study, patients with infections requiring anticipated antibiotic treatment of two or more weeks were enrolled. BACKGROUND: Trichuris trichiura is humans' second most prevalent soil-transmitted helminth (STH) infection after Ascaris lumbricoides, affecting approximately 460 million people worldwide. Despite its sub-optimal sensitivity, especially in low prevalence and infection intensity settings, the modified Kato-Katz (K-K) is still recommended as a diagnostic method by the World Health organization (WHO) guidelines.

METHODOLOGY/PRINCIPAL FINDINGS: Within a randomized clinical trial (RCT) comprising four treatment arms with two different anthelmintics, the present study reports an important secondary research objective to determine the diagnostic agreement between K-K and real-time PCR evaluating treatment efficacy against T. trichiura. Purpose: Although the potential transformative effect of electronic health record (EHR) data on clinical research in adult patient populations has been very extensively discussed, the effect on pediatric oncology research has been limited. Multiple factors contribute to this more limited effect, including the paucity of pediatric cancer cases in commercial EHR-derived cancer data sets and phenotypic case identification challenges in pediatric federated EHR data.
