A Case Of Disseminated Nocardia Beijingensis In An Immunocompetent Patient, Manifesting As Subcutaneous And Cns Nocardiosis., Scott Cathcart, Patricia Stogsdill, Rebecca Bell
Journal of Maine Medical Center
Introduction
Nocardiosis is an uncommon infection, typically encountered in immunocompromised patients. With only 5 cases reported thus far in the United States, we present a case of disseminated Nocardia beijingensis in an immunocompetent patient, involving the lung, subcutaneous tissue, and CNS.
Clinical Findings
A 60-year-old immunocompetent male with no significant medical history presents to our institution after a complicated medical course, beginning with vague respiratory complaints. Initial work-up revealed a large right upper lung mass of unclear etiology. A month later he had a syncopal episode followed by the development of focal neurologic deficits.

Is Rocky Mountain Spotted Fever Transmitted In Maine?, Eric J. Ray
Journal of Maine Medical Center
Introduction:
Tickborne disease occurs worldwide with recent recognition of changes in tick vector distributions. Rocky Mountain Spotted Fever (RMSF) is already widespread across the United States; however, it is being monitored for introduction to non-endemic areas.
Clinical Findings:
A 65-year-old male with a history of psoriatic arthritis on adalimumab and local tick exposures but no known recent bites presented to the emergency department with fevers, myalgias, arthralgias, and a maculopapular rash of unclear etiology. Initial testing showed transaminitis, worsened chronic thrombocytopenia, and slight neutropenia with absolute neutrophil count 2,330 (reference value: 2,400-7,600 cells per µL); however, he was stable …

Highlights Of Clinical Practice Guideline For The Management Of Community-Acquired Pneumonia: 2019 Update By The American Thoracic Society And Infectious Diseases Society Of America, Hongmei Wang, Molly Brong, Shirley Pham, Diane Dreucean
Faculty Publications
Pneumonia is the leading cause of morbidity and mortality in the United States. The American Thoracic Society and The Infectious Diseases Society of America recently published updated guidelines for the diagnosis and treatment of community-acquired pneumonia. Initial antibiotic therapy should cover for Streptococcus pneumoniae, Haemophilus influenzae, and other gram-negative bacilli. Patients who have risk factors for the development of antibiotic resistant pathogens, such as methicillin-resistant Staphylococcus aureus and Pseudomonas aeruginosa should have appropriate empiric coverage for these pathogens. The recommended duration of treatment of community-acquired pneumonia is a minimum of 5 days in conjunction with clinical improvement, which has remained …

Implementation Of A Central Line Bundle As A Quality Improvement Priority., H. Anthony Flood
Implementation Of A Central Line Bundle As A Quality Improvement Priority., H. Anthony Flood
Doctor of Nursing Practice Papers
Background: Central-line associated blood stream infections (CLABSI) are a major threat to patient morbidity and mortality. Kentucky Continue Care Hospital (KCCH), a long-term acute care hospital, experienced a significant increase in CLABSI in 2018; therefore, implementation of a central-line bundle to reduce CLABSI was identified as a quality improvement priority. Methods: This author used the Quality Improvement Model to plan, study, and evaluate the implementation of a central line bundle in KCCH following University of Louisville Institutional Review Board (ULIRB) approval, staff were educated on the new process and the audit tool using a self-study module over a 2-week period …

International Disease Epidemics And The Shadow Economy, Aziz N. Berdiev, Rajeev K. Goel, James W. Saunoris
Economics Faculty Working Papers
Adding to the emerging body of research related to the current coronavirus crisis, this paper studies the impact of disease epidemics on the worldwide prevalence of the shadow or the underground economy. The informal sector undermines compliance with government regulations and lowers tax collections. Our main hypothesis is that epidemics positively impacts the spread of the shadow economy. Using data on nearly 130 nations and nesting the empirical analysis in the broader literature on the drivers of the shadow sector, we find that both the incidence and the intensity of epidemics positively and significantly contribute to the spread of the …

Expansion Of A Food Insecurity Scale In Primary Care., Andrea Gibson
Expansion Of A Food Insecurity Scale In Primary Care., Andrea Gibson
Doctor of Nursing Practice Papers
As a household-level economic and social condition defined by limited or uncertain access to inadequate food, food insecurity serves as a health threat. Food security impacts health by increasing the risk of cardiovascular disease, type 2 diabetes, stroke, and several cancers (WHO, 2005). Expansion of the two-item food insecurity screening to a six-item short-form (USDA 6- SF) screening in primary care has the potential to identify and categorize households into three categories of food security, high/marginal, low, or very low. By differentiating food insecurity categories, clients received more appropriate referrals to services aimed at improving food security. During a recent …
Hepatitis A Educational Program In Food Service Workers., Katherine Barker
Hepatitis A Educational Program In Food Service Workers., Katherine Barker
Doctor of Nursing Practice Papers
Louisville, KY was affected by a significant outbreak of Hepatitis A in 2017-2018. Kentucky consisted of one-third of the national deaths. The outbreak influenced not only human mortality but had an economic impact on the food service industry. Many restaurants were affected, and some were forced to close due to Hepatitis A outbreaks. To address this issues, a two-step educational process was designed for management to gain understanding of hepatitis A content. The first step was to survey staff (kitchen staff, waiters/waitress, busier, etc.) to determine the compliance rate and variables that influence the compliance rate for vaccination of Hepatitis …

Attitudes, Behaviours, And The Well-Being Of Older Singaporeans In The Time Of Covid-19: Perspectives From The Singapore Life Panel, Paulin T. Straughan, William Tov, Seonghoon Kim, Terence Cheng, Stephen Hoskins, Micah Tan
ROSA Research Briefs
COVID-19 has affected all Singaporeans, regardless of age and socioeconomic status (SES). Many measures have been implemented by the government to control the spread of this disease, including restrictions on social gatherings, restrictions on overseas travel, and making it compulsory to wear a mask. Measures have also included a partial lockdown – known as the ‘circuit breaker’ – which began in April 2020. This forced Singaporeans to quickly adapt to a new normal with some doing better than others. This research brief provides an overview of how COVID-19 and its related measures have affected seniors in Singapore using data from …
Hiv/Hcv Screening Protocols In The Emergency Department., Adrianne Warren Johnson
Hiv/Hcv Screening Protocols In The Emergency Department., Adrianne Warren Johnson
Doctor of Nursing Practice Papers
Background: The United States is experiencing an epidemic of opioid drug use that has increased the number of people who inject drugs (PWID). This has resulted in an increase in the risk of transmission of blood-borne pathogens including human immunodeficiency virus (HIV) and hepatitis C virus (HCV) among this population. Although injection drug use has surpassed sexual transmission as the leading risk factor for HIV, PWID have the least access to HIV prevention, testing, treatment, and healthcare. Current guidelines recommend testing individuals from high-risk populations for HIV/HCV every six months. Offering routine, opt-out, targeted, bundled HIV/HCV testing in an emergency …

Previous studies measuring the health-related quality of life (HRQoL) of individuals with dengue focused on treatment seeking populations. However, the vast majority of global dengue cases are unlikely to be detected by health systems. Representative measurements of HRQoL should therefore include patients with disease not likely to trigger treatment-seeking behavior. This study based in Iquitos, Peru used the Quality of Wellbeing Scale-Self Administered, a survey that enquires about not only physical health, but also psychological health, self-care, mobility, and usual social activities, and rates HRQoL between 0 (death) and 1 (optimum function), to evaluate the impact of dengue on HRQoL. …

Difficulties inherent in the identification of immune correlates of protection or severe disease have challenged the development and evaluation of dengue vaccines. There persist substantial gaps in knowledge about the complex effects of age and sequential dengue virus (DENV) exposures on these correlations. To address these gaps, we were conducting a novel family-based cohort-cluster study for DENV transmission in Kamphaeng Phet, Thailand. The study began in 2015 and is funded until at least 2023. As of May 2019, 2,870 individuals in 485 families were actively enrolled. The families comprise at least 1 child born into the study as a newborn, …

The University of Louisville Journal of Respiratory Infections
Introduction: The large volume of diagnostic tests required by the response to the COVID-19 pandemic resulted in a shortage of commercial nasopharyngeal swabs. In an effort to alleviate the shortage, swabs created by 3D printing may be a solution.
Methods: We designed and produced 3D printed swabs and sought to compare their ability to detect SARS-CoV-2 in patients admitted for COVID-19 or who were suspected of having COVID-19.
Results: A total of 30 patients were swabbed with a commercial and a 3D printed swab. Results matched in 27 of 30 patients (90%). Two patients were discordant with a positive commercial …

Comparison Of Sars-Cov-2 To Sars-Cov, Mers, And Influenza A: A Systematic Review, Debbie Calabrese
Comparison Of Sars-Cov-2 To Sars-Cov, Mers, And Influenza A: A Systematic Review, Debbie Calabrese
Health Informatics Student Papers and Projects
Differences and similarities are examined among SARS-CoV-2, SARS-CoV-1, MERS and influenza A, in the context of transmissibility. Characteristics specific to the respective pathogens prove invaluable as they set the stage for intervention and management. SARS-CoV-2 bears similarities to other recently emerged infectious diseases in basic virologic ways, yet is distinctly different in terms of viral transmission dynamics. These differences arise from genomic qualities and properties specific to the virus. As ACE2 receptors are determinants for attachment and spread of the virus, and their role throughout tissues in the body is vast, an understanding of organ-level pathogenesis of SARS-CoV-2 is crucial …

How Can Econometrics Help Fight The Covid'19 Pandemic?, Kevin Alvarez, Vladik Kreinovich
How Can Econometrics Help Fight The Covid'19 Pandemic?, Kevin Alvarez, Vladik Kreinovich
Departmental Technical Reports (CS)
The current pandemic is difficult to model -- and thus, difficult to control. In contrast to the previous epidemics whose dynamics was smooth and well described by the existing models, the statistics of the current pandemic is highly oscillating. In this paper, we show that these oscillations can be explained if we take into account the disease's long incubation period -- as a result of which our control measures are determined by outdated data, showing number of infected people two weeks ago. To better control the pandemic, we propose to use the experience of economics, where also the effect of …
