Gut microbiota have been implicated in severe malaria in murine models, but their contribution to the pathogenesis of severe malaria in children is unknown. Here we show through analysis of gut bacteria in stool samples from two separate African studies enrolling children with severe malaria, and children from local communities, that children with severe malaria have gut bacteria dysbiosis. Among children with severe malaria, there is increased abundance of Enterobacteriaceae that associates with multiple clinical complications of severe malaria. Moreover, increased abundance of Escherichia coli was a predictor of post-discharge mortality. Metagenome analysis identify elevated metabolic pathways and genes supporting …

The Institute for Health Policy and Practice (IHPP) at the University System of New Hampshire commissioned Scientific Health to conduct an evidence-based review of the relationship between criminal justice system involvement and health outcomes. This research synthesized findings from recently published, peer-reviewed academic literature and relevant reports to examine how correctional system involvement affects physical health, mental health, and overall well-being among incarcerated individuals, their families, and communities. The review identified common themes related to behavioral health, chronic conditions, and systemic impacts, providing insight into the broader health implications of mass incarceration and deinstitutionalization. BACKGROUND: Of patients who receive standard concomitant chemoradiation (CRT; intensity-modulated radiation therapy [IMRT] plus cisplatin) for locally advanced head and neck cancer (HNC), approximately two-thirds will develop severe oral mucositis (SOM), limiting their ability to eat solids (WHO grade 3) or drink liquids (WHO grade 4). In a randomized, double-blind phase 2b trial, avasopasem manganese substantially reduced duration and incidence of SOM versus placebo. This phase 3 trial further assessed avasopasem's reduction of SOM due to CRT.

In public health research, diverse perspectives are vital to identify biases that homogenous teams might miss. Since publication metrics influence career progression, we investigated publication rate disparities within a School of Public Health. We analysed 18 322 peer-reviewed publications by 513 affiliated researchers between 2014 and 2023 using multivariable regression models and network analysis to assess the impact of gender, ethnicity, job level and centrality in the School’s research network on publication rates. The Children’s Hospitals in Africa Mapping Project (CHAMP) survey was developed and implemented to assess the capabilities of some of the best resourced sub-Saharan African hospitals serving children. The aim was to evaluate hospital facilities, infrastructure, equipment, supplies, services, staffing, and readiness to care for children amid public health emergencies. This report analysed a subset of survey questions that characterised the hospitals and assessed facilities, equipment, supplies, infrastructure and capacity to respond to emergencies and outbreaks. Twenty-four sites were recruited. Twenty hospitals from 15 countries completed the survey from 2018 to 2019. This portion of the CHAMP study identified issues …

Efficient bioreactor clarification for harvesting virus particles is often challenging. Tangential flow filtration is attractive as it can be easily adapted for batch and perfusion operations. Here the feasibility of using reverse asymmetric hollow fiber membranes, where the more open support structure faces the feed stream, has been investigated for harvesting adeno associated virus serotype 2. The open support structure of these membranes stabilizes a secondary membrane consisting of rejected particulate matter. Economic sanctions engender the disturbance of financial transactions between the sanctioner and the targeted nations. The Russia-Ukraine war also restricted Russia's entry into financial and commodity markets, with reverberating effects on the global market. Hence, the study aims to elucidate the relationship between armed conflicts and economic sanctions enforced by the G7 nations, the crude oil market, and its corresponding volatility index, OVX. It seems that heightened levels of ambiguity, conflict, nervousness, and hostility have contributed to an increase in the fluctuation of the energy market, leading to an extreme response to economic sanctions. Despite national media attention and significant policy investment, substance use epidemics continue to impact Central Appalachia. Inequities affecting rural communities and persistent stigma create significant barriers to well-being and recovery. Recovery, however, is more than a healthcare issue; it is a relational and community-rooted process that must involve education, policy, and care. In this article, we reframe deficit-oriented conceptions of recovery to highlight the vital role of relational care and community collaboration in promoting wellness in rural Appalachia. We offer the Community Resiliency Model (CRM) as a practical, evidence-informed strategy to foster emotional regulation, coregulation, and community resilience. We call …

Objectives: To describe the utilization of the My Visit application for video calls between patients admitted to intensive care units (ICUs) and their families during the .COVID-19 pandemic as an alternative to hospital visits. Methods: The My Visit (Zeyareti) application was launched in 2 National Guard Health Affairs hospitals in Riyadh, Saudi Arabia, in response to the COVID-19 pandemic in March 2020 and extended to all ICUs in August 2020. Objectives: To examine the relationships between the neutrophil-to-lymphocyte ratio (NLR), the platelet-to-lymphocyte ratio (PLR), and the systemic immune-inflammation index (SII) in myringosclerosis after ventilation tube insertion (VTI). Methods: We conducted this descriptive case-control study between March 2022 and September 2024. A total of 75 children aged 6 to 12 years with a history of VTI were enrolled. Myringosclerosis+ (n=43) included children with myringosclerosis, while Myringosclerosis- (n=32) included children without myringosclerosis. Preoperative values for NLR, PLR, and SII were collected by hematological testing and calculations. Background: Patients with Human Immunodeficiency Virus (HIV)have an atypical imaging pattern of lymphoma. There is paucity of literature on differences in tumor volume or burden of disease amongst HIV positive patients compared with HIV negative patients and how this correlates with clinicopathological parameters of aggressiveness and prognosis.
Methods: This was a retrospective cross-sectional study of patients with non-Hodgkin lymphoma which were categorized into HIV positive and HIV negative. The tumor burden, disease sites, international prognostic score and Ki-67 index were recorded. Background: Early diagnosis of TB with drug susceptibility testing (DST) is critical to achieve successful treatment outcomes. We aimed to develop and test a novel colorimetric, 12-well, thin-layer agar-based test to assess its accuracy for TB diagnosis and DST in a clinical setting in Southern Mozambique.
Methods: Development of the first prototype of the second generation (2G) test in the laboratory setting followed by a cross-sectional diagnostic accuracy study with consecutive recruitment of subjects with microbiologically confirmed TB using GeneXpert MTB/RIF Ultra.

Spectral graph theory is a subfield of algebraic graph theory that studies the matrices associated with graphs. It lives at the nexus of Linear Algebra and Combinatorics. Many intriguing results in the domains of Matrix Theory and Combinatorics have come from studying the eigenvalues of graph matrices; in fact, several open problems in both areas have been resolved. Beyond its theoretical appeal, spectral graph theory has found meaningful applications in theoretical chemistry, particularly in the mathematical classification of chemical graphs. Background
COVID-19 vaccination campaigns have reduced disease severity and fatalities around the globe. Global data shows that 67.7% of the general population are vaccinated, and Tanzania has reported appealing findings of over 70% coverage among individuals over 18 years of age. However, a group-specific assessment of vaccination coverage is needed in sub-Saharan Africa. This study aimed at assessing the COVID-19 vaccination uptake and associated factors among cancer patients owing to their vulnerability.
Methods

This qualitative phenomenological study explored how elementary principals led the implementation of a newly adopted core literacy curriculum and how they perceived their emotional competence in this role. Grounded in implementation science (see D. L. Fixsen et al., 2010), the transtheoretical model (see Prochaska & DiClemente, 1982), and emotional intelligence (see Goleman et al., 2001), the study examined both the leadership approaches and emotional competencies principals relied on when leading curriculum implementation in their schools. Evaluation of the durable efficacy and safety of long-acting injectable therapy for HIV is needed in African populations. In a multicenter, open-label phase 3b trial, 512 African adults with HIV-1, stable on first-line oral therapy, with screening plasma viral load (VL) < 50 copies ml−1 and without past virologic failure were randomized (1:1) to continue oral therapy or switch to cabotegravir (600 mg) and rilpivirine (900 mg) intramuscular injections every 8 weeks (optional 4-week oral lead-in). VL was monitored every 24 weeks. Here the primary outcome for our analysis up to 96 weeks was VL < 50 copies ml−1, using the Food and Drug Administration snapshot algorithm (noninferiority margin 10%) in the intention-to-treat exposed population. At 96 weeks, 247/255 (97%) in the long-acting group and 250/257 (97%) in the oral therapy group had VL < 50 copies ml−1 (difference −0.4%; 95% confidence interval −3.1% to 2.0%), demonstrating noninferiority. Adverse events of severity grade ≥3 occurred in 41/255 (16%) in the long-acting group and in 22/257 (9%) in the oral therapy group, mostly considered unrelated to the study drug; only one treatment-related adverse event in the long-acting group led to a decision to discontinue treatment (injection-site abscess). Cabotegravir and rilpivirine long-acting therapy produced durable virologic suppression, met the prespecified noninferiority endpoint compared with oral therapy and demonstrated an acceptable safety and tolerability profile. Long-acting therapy may be considered for use in African treatment programs. PACTR registration: 202104874490818.

Multilevel selection (MLS) is a foundational framework that expands the concept of natural selection beyond the traditional focus on individuals or genes to larger units, like groups. Despite strong empirical support and applications in a broad range of biological fields, MLS, often referred to as group selection, has a controversial history, which has impeded its wide use and led to incorrect inferences on the sources and strength of selection.
