Cytomegalovirus Associated Enterocolitis And Skin Eruption: A Pediatric Case Misdiagnosed As Acute Appendicitis, Zeynep Busra Balik, Gulsen Akoglu

Journal of Pediatric Infectious Diseases

We present a case of an immunocompetent 2-year-old boy, with cytomegalovirus-associated enterocolitis and petechial skin eruption, initially misdiagnosed as acute appendicitis. Despite abdominal pain, diarrhea and a rash for 15 days, diagnostic laparoscopy revealed no surgical pathology. Dermatological examination identified perifollicular petechiae, leading to a diagnosis of cytomegalovirus infection, which was confirmed by serology. This case emphasizes the importance of considering viral infections in pediatric patients with gastrointestinal and dermatological symptoms. It also highlights the diagnostic value of cutaneous findings in cytomegalovirus infection.

Objective: The omicron variant of coronavirus disease 2019 (COVID-19) has caused a global epidemic in 2022. Our aim was to investigate the symptom spectrum and risk factors for fever in pediatric patients with omicron infection. Methods: From April 8, 2022, to May 17, 2022, this prospective observational study included pediatric patients who attended the child-parent sections of the Fangcang shelter hospitals with laboratory-confirmed COVID-19. Patient data were collected from questionnaires. The risk factors for prolonged fever and high fever were identified by univariate and multivariate analyses. Lipid Biomarkers Predict Pneumonia Risk In Children With Influenza-Mycoplasma Co-Infections: A Retrospective Cohort Study, Kai-Jing Zhang, Xin-Feng Zhao, Xiao-Juan Lv, Xiao-Hui Huang, Jing Huang, Yan-Yan Zhu

Objective: This study evaluates the prognostic value of high-density lipoprotein (HDL-C) and low-density lipoprotein (LDL-C) levels for pneumonia risk in children co-infected with influenza and Mycoplasma pneumoniae (MP). Methods: We retrospectively analyzed 248 children with influenza-MP co-infection (January 1, 2020–October 30, 2024). Participants were grouped into pneumonia (P, n=195) and non-pneumonia (NP, n=53) based on chest imaging. Statistical comparisons of clinical parameters (WBC, lymphocytes, CRP, lipids) utilized Mann-Whitney U and chi-square tests. Variables with P<0.1 were included in logistic binary regression to identify independent risk factors. ROC analysis assessed diagnostic performance. Results: Significant differences were observed in lymphocytes (L), CRP, TG, HDL-C, and LDL-C levels between pneumonia and non-pneumonia groups (P < 0.05). CRP, HDL-C, and LDL-C were identified as independent risk factors for determining whether influenza-MP co-infections were complicated by pneumonia. CRP and LDL-C levels were risk factors associated with pneumonia risk (OR > 1), whereas HDL-C levels showed a strong protective factor association (OR < 1). The ROC curve analysis showed that the combined area under the curve (AUC) for HDL-C + LDL-C (0.878) was significantly higher than the AUCs for CRP + HDL-C (0.809), CRP + LDL-C (0.753), HDL-C (0.807), LDL-C (0.737), and CRP (0.601). The clinical DCA shows that HDL-C + LDL-C has a higher diagnostic value than CRP + HDL-C, CRP + LDL-C, HDL-C, LDL-C, or CRP alone. Conclusion: HDL-C and LDL-C levels independently predict pneumonia progression in influenza-MP co-infected children, with dual-marker assessment enhancing diagnostic accuracy.

The study aimed to assess the antibiotic susceptibility profile of P. aeruginosa isolated from burn infections in hospitals in Baquba, AL Najaf and Karbala. Goals-of-care (GOC) discussions align medical treatment with older adults’ preferences, yet are hindered by communication barriers, provider discomfort, and misinformation. Video-based decision aids improve understanding and reduce decision conflict, though data on their use in older populations remain limited. BACKGROUND: Single-pill combination (SPC) therapy improves hypertension control; however, its implementation in primary care settings remains limited. In Nigeria, there is insufficient evidence on factors influencing SPC uptake, particularly from the perspective of healthcare workers (HCWs). This study examined the implementation of SPC medications for hypertension treatment by nonphysician HCWs at primary healthcare facilities (PHCs) in Nigeria.

METHODS: An explanatory sequential mixed methods study was conducted, building on a cluster randomized controlled trial embedded within the Hypertension Treatment in Nigeria Program. Multiplex immunofluorescence and immunohistochemistry (mIF/IHC) are increasingly employed antibody-based technologies that use tissue sparingly and facilitate the detection of co-localized or neighboring biomarkers. Specifically, these platforms enable spatial analyses of the tumor microenvironment as well as extended applications, for example, describing normal tissue anatomy, autoimmunity, infectious diseases, etc. mIF/IHC has greatly enhanced biomarker discovery efforts, and a growing number of studies suggest superiority to traditional IHC. Standardization of staining approaches, reporting of image analysis strategies and resultant data is critical for facilitating cross-study comparisons, validation, deployment, and generalization of findings. Community health needs assessments (CHNAs) are vital tools for identifying population health priorities through monitoring chronic diseases and conditions; however, their effectiveness in preventing chronic diseases remains debated. This comparative study examines CHNAs from sixteen South Dakota hospitals across Avera, Sanford, Monument Health, and independent systems to evaluate their role in preventing chronic diseases. Analysis reveals significant challenges, including biased data sampling, limited community engagement, small survey samples, and inconsistent implementation strategies. BACKGROUND: Where surveillance data are limited, nationally representative electronic health records allow for geographic, temporal, and demographic characterization of the fungal diseases blastomycosis and histoplasmosis.

METHODS: We identified incident blastomycosis and histoplasmosis cases from 2013 to 2023 within Oracle EHR Real-World Data, which comprises 1.6 billion healthcare encounters nationally. To characterize spatiotemporal incidence trends, we used generalized estimating equations weighted for nonrepresentativeness of electronic health record-reporting facilities. We computed standardized incidence rate ratios (sIRRs), which relay relative differences in standardized incidence rates between regions, race/ethnicity, gender, and age subgroups and the national population.

Infectious prions readily adhere to common surfaces, retain infectivity, and are highly resistant to conventional decontamination, posing significant biosafety challenges in the medical and research environments. Recent occupational exposures underscore the urgency of improving safety measures. Here, we describe an approach combining foam-swab surface sampling and protein misfolding cyclic amplification to enhance prion surveillance. Our results demonstrate the ability to detect prions most relevant to human health directly from contaminated surfaces, even at 100 million-fold dilutions of the brain. BACKGROUND: Adults and children often respond differently to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, with adults facing a higher risk of symptomatic and severe illness. We hypothesize that children's protection from symptomatic SARS-CoV-2 may be due to more frequent respiratory viral infections, which prime their airway antiviral defenses.

METHODS: Using case-cohort and case-control analyses in the Human Epidemiology and Response to SARS-CoV-2 cohort, we evaluated whether infection with common respiratory viruses protects against SARS-CoV-2 infections and investigated airway molecular mechanisms by which this protection is achieved. However, road salts run off into local vegetation, soil systems, and (my focus) freshwater systems. Loop-mediated isothermal amplification (LAMP) is a nucleic acid amplification technique known for its specificity and efficiency in less controlled environments than laboratories. Designing novel LAMP reagents requires strict contamination control, and unlike the more commonly automated polymerase chain reaction, LAMP’s heightened sensitivity poses unique challenges to automation. To address this, we optimized automation procedures for LAMP using a robotic pipettor without altering the core assay itself. This project focuses on minimizing contamination using sealed plates, reducing operator intervention by automating pipetting, and preventing enzyme degradation using cooling plates.
