Objective: This study aimed to retrospectively analyze clinical data from pediatric patients with Mycoplasma pneumoniae pneumonia (MPP), investigate the expression and clinical significance of serum IL-36γ, and to develop and validate an early identification model for severe MPP (SMPP). Methods: Clinical data were collected from 345 children diagnosed with MPP in our department between July 1, 2023, and December 31, 2024. Based on diagnostic criteria, enrolled patients were categorized into an SMPP group (n=145) and a non-severe MPP (NSMPP) group (n=200). Serum IL-36γ levels at admission were measured using ELISA. Objective: This study examines whether the oral mycobiome of pediatric patients with newly diagnosed hematologic or oncologic malignancies differs from that of healthy children before treatment begins. Methods: The study included 117 children under 18 years of age: 67 patients and 50 healthy controls. Oral swab/rinse samples were cultured on various agar media, and fungal species were identified using conventional methods, API ID 32C, and MALDI-TOF MS. Antifungal susceptibility testing of non-albicans Candida (NAC) isolates was performed using VITEK2. Results: Yeast growth was observed in 58.2% of patients and 68% of healthy children. Objective: Crimean–Congo hemorrhagic fever (CCHF) and brucellosis are two zoonoses that co-circulate in overlapping endemic areas and share similar clinical features. They may occur concurrently in the same patient. Methods: We aimed to investigate the occurrence of concurrent CCHF and brucellosis in pediatric patients from our endemic region. Results: We reviewed 31 patients younger than 18 years who, between January 2018 and December 2024, underwent concurrent testing with CCHF PCR and the Brucella serum agglutination test (SAT), along with their presenting complaints. Objective: Meteorin-like protein (METRNL), soluble CD14 subtype (presepsin), and bone morphogenetic protein 9 (BMP9) show promising for sepsis diagnosis and prognosis assessment in children. Their diagnostic and prognostic utility is comprehensively assessed in this study. Methods: A total of 360 children, aged 3-14 years, with sepsis, infection, and healthy groups were retrospectively analyzed. Receiver Operating Characteristic curve evaluated the diagnostic performance of METRNL, presepsin, and BMP9 for sepsis. The 28-day prognostic information was collected for the sepsis group. Bronchiectasis, although less common in children than in adults, is an important long-standing respiratory disease marked by permanent enlargement of the bronchi and impaired airway function. The main symptom is a chronic wet cough. This condition presents a considerable health challenge in children, especially in developing countries where access to healthcare may be limited. Although cystic fibrosis (CF)-related bronchiectasis is relatively common, non-CF bronchiectasis, often triggered by post-infectious causes, predominates in pediatric populations. The diagnosis of bronchiectasis is confirmed by demonstrating a bronchoarterial ratio > 0.8 on high-resolution computed tomography. This doctoral dissertation comprehensively investigates the underexplored phenomenon of noise-induced extreme events (EE). The word “extreme” is accompanied by an occurring “event” when the deviation is extreme or higher than that of regular occurrences. These extreme occurrences are rare, abrupt, sudden, and irregular, often causing a profound impact on the system and its surroundings. Tsunami, earthquakes, solar flares, and tornadoes are such events that do not occur often but still significantly cause damage to mankind. Human sex ratios at birth (SRBs) shape population composition and are closely linked to maternal health and gender discrimination. In the context of environmental change, SRBs may theoretically be skewed by physiological or behavioral responses to exposure to extreme heat. However, evidence for this is limited. In this study, we estimate the effect of prenatal exposure to temperature on birth sex by linking survey data on 5 million live births in 33 sub-Saharan African countries and India with high-resolution temperature data. To distinguish between spontaneous and induced abortions, we exploit sociodemographic differentials, exposure timing, and regional differences in son preference. …

The Disproportionate Impact Of Hiv On Young Women In Durban: Exploring Gendered Vulnerabilities And Socioeconomic Barriers, Adrian A. Nolasco

South Africa has a high rate of HIV, with young women in Durban especially affected due to social, economic, and cultural challenges. This study looks at the experiences of young women living with HIV and the views of healthcare professionals caring for them in KwaZulu-Natal. Using a qualitative, phenomenological approach, semi-structured interviews were conducted with five women and three medical professionals to explore how HIV affects their lives, the challenges they face, and how they cope.

The findings show that both structural and personal barriers make it hard for young women to stick to treatment. Long waits at clinics, limited …

In light of the COVID-19 pandemic, the Centers for Disease Control and Prevention (CDC) provided funding to states as part of the CDC Disparities Grant initiative. The aim of the funding was to reduce chronic disease and COVID-19–related health disparities among vulnerable communities through four unique strategies. Funding was awarded to the Indiana Department of Health (IDOH) to begin an Indiana chapter of this initiative, called Indiana Healthy Opportunities for People Everywhere (I-HOPE). Introduction: In 2024, 48.4million Americans experienced a substance use disorder (SUD),with opioid use disorder representing a significant concern. In 2024, an estimated 8 million individuals misused opioids, contributing to 54,743 fatal overdoses. College students are particularly vulnerable, with 20% reporting illicit substance use within the past month, highlighting the urgent need for focused interventions in this population.

Social work professionals often experience demanding and intense situations while facing organizational restructuring during crisis situations. The purpose of this generic qualitative study was to explore the viewpoints of frontline social workers regarding their ethical decision-making processes in response to the mandatory modifications of the service delivery protocol during the COVID-19 pandemic. Enck’s biomedical and ethical decision-making framework grounded the study. Data were collected from semi-structured interviews with 10 frontline social workers in Connecticut who had been employed with a social service agency for at least 3 years, but no more than ten. More than 38 million Americans have diabetes, a chronic illness requiring daily self-management. This challenge is further complicated when patients lack health literacy skills, yet 36% of U.S. adults function at or below basic health literacy. Low health literacy and diabetes contribute to poor health outcomes, disproportionately affecting vulnerable populations. This quasi-experimental, descriptive, correlational study examined the impact of health literacy on A1C awareness, glycemic control, and diabetes knowledge in adults with type 2 diabetes, and evaluated a teaching intervention that used visual aids teach-back. Access to effective behavioral treatment is critical for the imprisoned population and newly released individuals to reduce reentry into the penitentiary system. Social workers within correctional facilities can determine the need for treatment, how often treatment is required, what specific treatment is necessary, and what resources will be needed to provide those treatments. A qualitative exploratory design was used in this study. I explored the quality of behavioral health treatment provided to the incarcerated population. Introduction: HPV vaccination rates for adolescents in the United States are below recommended levels. The Adolescent Vaccination Program (AVP) guides pediatric clinics on how to implement evidence-based strategies to increase HPV vaccination rates. These strategies comprise the adoption of (1) immunization champions, (2) provider assessment and feedback, (3) continuing education, and (4) prompts, (5) parent reminders, and (6) parent education. Objective: This study aimed to assess the relationship between nonthyroidal illness syndrome (NTIS) and extubation outcomes in pediatric patients requiring mechanical ventilation. Methods: This prospective cohort study included 59 patients who underwent mechanical ventilation in the pediatric intensive care unit of a tertiary care university hospital from July 2020 to June 2021. Each patient underwent laboratory testing including serum levels of TSH, free T3, free T4, and reverse T3 on the first and third days of mechanical ventilation. Results: On the first day of mechanical ventilation, 62.7% of patients had low free T3, which significantly increased to 78% of patients …

Background

Protein malnutrition remains a major global health problem and frequently co-occurs with alcohol use, opioid exposure, and infectious disease, particularly in low- and middle-income settings. The liver is central to both metabolism and inflammatory regulation, yet the effects of a malnutrition-like nutrient environment on hepatic responses to combined metabolic and inflammatory insults are not well defined. Cytochrome P450 3A4 (CYP3A4), a key enzyme involved in opioid metabolism, is highly sensitive to inflammatory signaling and may be especially vulnerable under conditions of nutritional deficiency.
