Background: Streptoccocus Oralis is an early colonizer in dental biofilm and contributes to plaque formation and periodontal inflammation. The search for safe, natural antimicrobial alternatives has increased interest in herbal agents such as Triphala and Aloe vera. However, their combined activity against S. oralis has not been fully assessed alongside phytochemical profiling.
Aim: To evaluate the antibacterial activity of aqueous-extracted Triphala, Aloe vera, and their combination against S. oralis, including inhibition zone measurement, MIC, MBC, and HPLC characterization. Urban agriculture (UA) in the United States (U.S.) is increasingly positioned as a multi-domain strategy for addressing social determinants of health (SDOH), yet evidence is fragmented and often focused on participant outcomes rather than the organizational and environmental conditions that determine whether those benefits can be sustained, expanded, or maintained over time. This dissertation uses an exploratory sequential mixed-methods design to clarify (1) what SDOH outcomes are associated with UA in the peer-reviewed literature, (2) which internal and external factors leaders perceive as driving UA organizational success or failure, and (3) which factors predict comparative organizational functioning across a national …

Medical waste is an overlooked issue posing a significant environmental and public health threat, with 4.7 billion tons generated annually, accounting for 3% of overall waste, and wound dressing pads being a major contributor. Vitamin C, involved in all phases of wound healing, demonstrates synergy with Chitosan, a polysaccharide ideal for medical and cosmetic applications. Milkfish (Chanos chanos) skin, a dominant commercial fish in the Philippines, shows synergy with Chitosan and Vitamin C in wound healing. With rising concerns over climate change and its impacts, this study develops and evaluates a Vector Autoregressive (VAR) model to forecast climatological data in General Santos City: surface pressure, specific humidity, wind speed, air temperature, precipitation, UVA and UVB irradiance, with data from January 1, 2001 to June 30, 2024. Mean hour calculations confirmed seasonality via the QS test (p < 0.001) for all variables except precipitation; Mann-Kendall tests indicated significant trends for all tested variables, and UV irradiances were the only variables with no significant long‐term trend. The Augmented Dickey-Fuller Test (p < 0.001) established stationarity of variables, and Granger‐causal relationships were identified. Results indicated a bidirectional Granger-causal relationship between variable pairs (p < 0.001). The AIC-based model selection identified VAR(168) as the best-fitting model, effectively capturing cyclic patterns up to one week. The residuals passed the Ljung-Box tests for no residual autocorrelation (p > 0.05), and OLS-CUSUM plots indicated structural stability. Mean root-mean-square error determined out-of-sample forecasting accuracy to be good for most variables, and poor for UVA and UVB irradiance; therefore, while the VAR(168) model does generate accurate forecasts, it still presents challenges. …

A 5-month-old intact male Pomeranian puppy was presented with a 2-week history of acutely swollen face, eyelids, lips, and muzzle. A physical examination revealed focal alopecia, folliculitis, and erythema on the muzzle, periocular, and perianal areas; crusts were also found around the muzzle. The puppy received monthly oral afoxolaner and milbemycin oxime. No ectoparasite was observed based on a trichogram test. Skin cytology revealed intracellular cocci and neutrophils. A skin biopsy taken from the muzzle revealed pyogranulomatous perifollicular dermatitis with epidermal spongiosis, intraepidermal pustules, and a superficial crust. Syphilis remains a significant transfusion-transmissible infection, yet regional epidemiological data from Northern Sudan are lacking. This study aimed to establish preliminary baseline syphilis seroprevalence among asymptomatic blood donors in Merowe, Northern Sudan. A cross-sectional study was conducted in January 2022 at the Merowe blood donation center. Eighty-nine consecutive asymptomatic male blood donors were screened for anti-Treponema pallidum antibodies using a commercial Enzyme-Linked Immunosorbent Assay (ELISA) kit (Fortress Diagnostics, UK). Demographic data were collected via structured questionnaire. The overall seroprevalence was 2.25% (2/89; 95% CI: 0.6–7.9%). The donor population median age was 32 years (range: 18–64). After the genocide in Rwanda in 2014, there were as few as 100 physicians left to provide health care for the entire country. The Rwandan Ministry of Health reached out to US academic institutions for assistance in rebuilding capacity and educating Rwanda’s next generation of medical professionals. The Human Resources for Health (HRH) program in Rwanda was instituted and led by the Rwandan Ministry of Health with support from the Clinton Health Access Initiative. BACKGROUND: While cross-sectional assessments offer snapshots of care cascade progress, longitudinal analyses are essential to capture the dynamic engagement in care of people living with HIV (PLHIV). Since the outbreak of war in Sudan in April 2023 between the Sudanese Armed Forces and the Rapid Support Forces, the country’s fragile health system has deteriorated rapidly. As hospitals closed, medical supplies vanished, and millions were displaced, frontline health workers were forced to make impossible choices. This essay recounts my experience as a young physician during the collapse of Sinnar Teaching Hospital, my displacement to White Nile State, and the creation of a community-led humanitarian initiative that provided clinical care, shelter, food, and livelihood support to thousands. The Djurdjura National Park (PND, Algeria) is part of one of the world’s 36 biodiversity hot spots, recognised by the United Nations as a national biosphere reserve. It contains the Darna primary forest, a key habitat for several mesocarnivore mammals, including the golden wolf (Canis lupaster), the common genet (Genetta genetta) and the ichneumon mongoose (Herpestes ichneumon). Although their diets are primarily based on animal prey, these carnivores exhibit both qualitative and quantitative reliance on plants, a pattern that merits thorough documentation. The authors reviewed the pathophysiology, clinical genetics, phenotype, and comprehensive clinical management of Fragile X-associated Tremor/Ataxia Syndrome (FXTAS), a neurodegenerative disorder affecting FMR1 premutation carriers (55–200 CCG repeats).
Participants:
A multispecialty and multidisciplinary team of authors with backgrounds in psychiatry, pediatrics, clinical psychology, speech and language pathology, and neurosciences.
Evidence:
Review of English language sources on the clinical phenomenology, genetics, pathophysiology, and clinical management of FXTAS from 2001 (the original report) through 2025, with emphasis on the general medical, psychiatric, and neurological features of FXTAS and its multispecialty and multidisciplinary clinical management.

Introduction: Dyspepsia affects 6.9% of people globally and significantly impairs quality of life, yet it often remains undiagnosed. Undiagnosed dyspepsia (UD) affects up to one-third of patients and is prevalent among students due to stress, irregular eating, and self-medication. In Uganda, limited access to endoscopy results in many cases being undiagnosed, yet recent data among health program students remains scarce.

Anaplastic lymphoma kinase (ALK)-negative anaplastic large cell lymphoma (ALCL) is a distinct subtype of mature T-cell non-Hodgkin lymphoma characterized by large, strongly positive CD30 lymphoid cells that lack expression and rearrangement of the ALK gene. Here, we present a unique case of ALK-negative ALCL in a 34-year-old male patient who presented with acute on chronic back pain. Initial emergent MRI spine demonstrated lytic changes of L3 and L4 with probable psoas fluid collection, with high suspicion for Pott’s disease. A 67-year-old male with a history of prostate cancer, treated with prostatectomy and subsequent radiation for recurrence, presented with a complex clinical course of recurrent osteomyelitis and a persistent urosymphyseal fistula with associated phlegmon. Following stereotactic body radiation therapy (SBRT) for a T4 vertebral lesion, he developed pelvic osteomyelitis, from which Kocuria species was cultured. The progression of his disease led to a urosymphyseal fistula, necessitating a cystectomy with ileal conduit and pubic bone debridement. Despite this extensive surgical intervention, he presented one year later with recurrent ileal conduit phlegmon and osteomyelitis. This case highlights the significant diagnostic and management …

Malaria remains a significant global health threat, with increasing concern for imported and locally acquired cases in non-endemic regions. We present the case of a 35-year-old pregnant woman from Sierra Leone, living in Missouri, who despite no recent international travel, was found to have symptomatic Plasmodium falciparum malaria. Cryptococcal meningitis (CM) is a life-threatening opportunistic infection, mainly affecting immunocompromised patients, especially with advanced HIV/AIDS. Unmasking cryptococcal immune reconstitution inflammatory syndrome (C-IRIS) is a severe but relatively rare complication in the United States. Co-infection with other central nervous system (CNS) pathogens further complicates both diagnosis and management.
In this case report, we present a rare instance of CM secondary to C-IRIS alongside EBV meningitis in a postpartum, HIV-positive patient on ART. This co-infection highlights the vulnerability of immunocompromised individuals, even on ART, to multifactorial CNS infections. Pathogenic free-living amoebae such as Acanthamoeba castellanii are present in soil and water worldwide. A. castellanii causes systemic infections with very high mortality rates, yet drugs specifically targeting this pathogen are not available. Methods to reliably generate and assay cysts, which drive infection recurrence and drug resistance, are unavailable in a high-throughput format suitable for drug screening and testing. In this study, we developed a robust and reproducible protocol for encysting A. castellanii as well as a high-throughput, quantitative cyst viability assay using fluorescent live/dead staining coupled with microscopy and automated image analysis. Aortitis is a rare but serious complication of infection that results in high morbidity and mortality. The most common location is in preexisting aneurysmal disease in a patient with multiple chronic comorbidities. The pathogens often reflect gastrointestinal bacteria, the classic example being Salmonella spp., but also include other enteric flora, Staphylococcus aureus, Streptococcus spp., and more. Herein, we describe 2 cases of aortitis caused by gastrointestinal bacteria and review management of this complex syndrome.

Background: Fungal meningitis outbreaks are rare and entail high mortality rates. Beginning May 2023, we investigated fungal meningitis caused by Fusarium solani species complex occurring in US patients who received epidural anesthesia in Matamoros, Mexico.
Methods: Early epidemiological information suggested US patients with suspected fungal meningitis had undergone mostly cosmetic procedures under epidural anesthesia performed in 2 Matamoros clinics. US patients known to have received surgery at these clinics during 1 January-13 May 2023 (clinic closures date) were identified and notified by public health officials. Myeloid malignancies carrying somatic DNMT3A mutations (DNMT3Amut) are refractory to standard therapy. DNMT3Amut leukemia cells accumulate toxic DNA double-strand breaks (DSBs) and stalled replication forks, rendering them dependent on DNA damage response (DDR). We report here that DNA polymerase theta (Polθ), a key element in DSB repair by end-joining (Polθ-mediated end-joining [TMEJ]) and in fork restarting, promotes survival and proliferation of DNMT3Amut leukemia cells. Polθ is overexpressed in DNMT3Amut leukemia cells due to abrogation of PARP1 PARylation-dependent UBE2O E3 ligase-mediated ubiquitination and proteasomal degradation of Polθ. This paper is part of a larger clinical practice guideline on the diagnosis and management of parapneumonic effusion and empyema in children, developed by the Infectious Diseases Society of America. In this paper, the panel provides recommendations on the appropriate size of thoracostomy tube for drainage. The panel's recommendations are based upon evidence derived from systematic literature reviews and adhere to a standardized methodology for rating the certainty of evidence and strength of recommendation according to the GRADE (Grading of Recommendations Assessment, Development, and Evaluation) approach.

Background: Filifactor alocis is a Gram-positive, obligate anaerobe that has emerged as a diagnostic marker of periodontitis. Neutrophils and macrophages are key players in oral immune homeostasis. Neutrophils are short-lived cells and constitutively undergo apoptosis. This apoptotic program can be accelerated or delayed by bacterial interactions. Macrophages clear apoptotic neutrophils via “efferocytosis,” a crucial mechanism for resolving inflammation and preventing tissue damage. The efferocytic process downregulates the expression and release of proinflammatory cytokines such as interleukin (IL)-6 and IL-23 while promoting anti-inflammatory responses.
