This qualitative dominant, exploratory sequential case study aimed to identify stakeholder perceptions of technology within a one-to-one technology initiative and outline any possible barriers. The study examined the perceptions of teachers, parents, and administrators within a rural K-12 public school district in Pennsylvania. The study provided school district stakeholders with data they can use to improve existing and future initiatives by early intervention within areas of concern identified by the study. Background: Guidelines recommend ≥2 weeks of antifungal therapy after candidemia clearance and for invasive candidiasis (IC). This post-hoc analysis evaluates Day 7 pooled data from the phase 2 STRIVE and phase 3 ReSTORE trials to explore early antifungal activity.

Methods: Adults with candidemia and/or IC received weekly rezafungin 400/200 mg or daily caspofungin 70/50 mg for ≤4 weeks. Efficacy was evaluated in the modified intent-to-treat population via all-cause mortality (ACM; primary endpoint; 20% noninferiority margin), mycological eradication, and time to negative blood culture (TTNBC) at days 7, 14, and 30 (TTNBC assessed only in patients with candidemia). With over 5 million attributed deaths per year, physical inactivity is a major global public health issue. Although the importance of physical activity is well recognized within the scope of obesity and cardiometabolic disease prevention and control, its broader benefits for the health of individuals and societies are yet to be fully harnessed. About half the global population is now at risk of contracting dengue, a mosquito-borne viral infection that can cause severe morbidity and fatalities. Effective dengue control depends on controlling the mosquito vector, but finding the right mix of vector control policies has proved challenging. Using a content analysis of 208 Hansard records from parliamentary proceedings in Singapore, where dengue outbreaks have significantly increased in both frequency and magnitude since the early 1990s, we trace the processual evolution of Singapore’s anti-dengue policy mix from 1960 to 2023 and conclude that the evolution of dengue control policies is consistent with a customized …

Background: A perceived shortage of dental hygienists in the United States has been linked to delays in preventive care and concerns about declining oral health outcomes. In response, some states have proposed alternative pathways permitting internationally trained dentists, dental students, and oral preventive assistants (OPAs) with limited or inconsistent education requirements to provide dental hygiene services. Limited evidence exists regarding whether these strategies would effectively address access to care and workforce challenges. Nonprofit organizations play a fundamental role in meeting community needs when government resources are limited or nonexistent. For decades, scholars and practitioners have recognized the importance of nonprofit-government partnerships for effective public service delivery. This study examines faith-based organizations as a specific type of hybrid nonprofit organization. As social, economic, and environmental challenges intensify, the need for community-based programs led by faith-based organizations also increases. Listeria monocytogenes is a facultative intracellular foodborne pathogen capable of crossing the placental barrier, leading to miscarriage, stillbirth, and neonatal sepsis, and has a fetal/neonatal fatality rate of ~20–30%. Although the importance of canonical virulence factors such as Internalin A (InlA) and Internalin B (InlB) in L. monocytogenes crossing of the placental barrier has been demonstrated, clinical strains lacking functional canonical internalins have been reported to cause severe neonatal disease, suggesting additional mechanisms of placental translocation.

United States citizens rely on state government employees to provide essential public services. During the COVID-19 pandemic, these employees responded by administering both routine and novel procedures to protect public health, often placing them on the frontlines and at increased risk of contagion. Concurrently, the pandemic-driven economic downturn created budget shortfalls that threatened the job and financial security of the state workforce. Together, these conditions substantially increased job demands for state government employees.

Introduction

Opioid misuse and abuse during pregnancy can adversely affect maternal and infant outcomes. Factors that drive the burden of opioid misuse and abuse are multifaceted and understudied. This study sought to explore predictors of opioid misuse and abuse among pregnant women in the United States.

Methods

Secondary data analysis of the National Survey on Drug Use and Health (NSDUH) data from 2015-2019 was performed. The primary outcomes were past-year opioid misuse, past-month misuse, and past-year opioid abuse. Data analysis included weight-adjusted univariate, bivariate, and multivariable logistic regression.

Results

Background. Mathematical models guide tuberculosis (TB) target-setting, yet most assume homogeneous “all-to-all” mixing. We compared projected intervention impacts between an all-to-all compartmental model and a Barabási–Albert (BA) scale‑free social network model under otherwise identical disease assumptions.

Methods. We calibrated transmission parameters so both models produced similar baseline trends, then introduced vaccination (coverage 30–70%; efficacy 80–95%) and treatment (20–50% increases in recovery) after a 400‑day burn‑in. Outcomes were assessed 300 days post‑intervention.

Results. Under 60% coverage, increasing vaccine efficacy from 80% to 95% yielded smaller projected reductions in active TB with the network model than with all‑to‑all mixing. Treatment improvements showed …

The escalation of ecological degradation, marked by climatic instability, precipitous biodiversity loss, and systemic chemical infiltration, necessitates a shift toward integrated biotechnological intervention. This analysis considers how the trajectory of diverse advanced methodologies, including environmental DNA (eDNA) surveillance, CRISPR-based genetic restitution, and synthetic biology-driven bioremediation, might be synthesized into a unified Planetary Health Framework. Habitat loss and fragmentation are major drivers of biodiversity decline and can alter the physiological condition of wildlife through changes in habitat quality, resource availability, and community composition. The Atlantic Forest of South America is one of the world’s most threatened biodiversity hotspots. This study examined relationships among biodiversity, landscape structure, and chronic stress in nonvolant small mammals within the Reserva Natural Tapytá in eastern Paraguay using hair glucocorticoid concentrations as indicators of long-term physiological stress. Importance: Current pretransplant clinical scoring systems fail to accurately estimate post-liver transplant (LT) survival, making recipient selection challenging. Persistent immune dysfunction contributes to early LT recipient mortality but is not captured by existing models.

Objective: To identify plasma biomarkers of pretransplant immune dysfunction and to develop a biomarker-based pre-LT risk stratification tool to predict post-LT mortality.

The lymphatic system is a highly branched endothelial tubular network that facilitates the migration of immune cells from the peripheral tissues to lymph nodes (LNs) and other lymphoid organs. Complement factors are essential for innate and adaptive immune functions. Complement anaphylatoxin C5a is crucial in vascular endothelial cell activation and lymphocyte polarization. Understanding the impact of C5a and its cognate receptor C5ar1 signaling on lymphatic function could provide new insights into the mechanisms of immune dysregulation observed in chronic inflammatory diseases. We demonstrate that acute C5a challenge in wildtype C57B6/J mice significantly reduced lymph propulsion compared to their C5ar1-deficient counterparts. …

Background: Subarachnoid neurocysticercosis (SANCC) is an uncommon but severe form of Taenia solium infection. There is limited evidence to guide clinical management. We aimed to describe the clinical and laboratory features of a cohort of patients with SANCC, and assess the impact of different management strategies on their clinical course.

Methods: We performed a multicentre retrospective cohort study with patients from 15 medical centres across nine states in the USA. Problem: Reproductive- aged women are one of the populations most at risk for iron deficiency and depression in the United States. While many studies have looked at the relationship between these two illnesses, none have explored it from a biocultural framework, examining how race/ethnicity affects this relationship. Using data from NHANES, this thesis examined the relationship between iron status (total body iron) and depressive symptoms (PHQ-9) among reproductive-aged women in the U.S. and how race/ethnicity affects it. BACKGROUND: Pediatric oncology and hematopoietic stem cell transplant (HSCT) patients have elevated risk for

METHODS: We retrospectively studied 276 stools longitudinally collected over 34 months from 32 HSCT and 12 oncology patients treated at a single tertiary center. Specimens were cultured for

RESULTS: Baseline cultures were positive in 16 (50%) HSCT patients and 2 (12%) oncology. On subsequent samples, 64% of patients who were initially negative acquired colonization: 8 of 15 (53%) HSCT and 8 of 10 (80%) oncology. IMPORTANCE: Despite rising hepatitis C virus (HCV) incidence in pregnant people, studies demonstrate that fewer than 50% of perinatally exposed infants receive HCV testing. Few studies report treatment of infected infants or factors affecting testing.

OBJECTIVE: To identify birthing parent and child factors associated with appropriate HCV testing for perinatally exposed infants and characterize HCV care for children with HCV.

Human astroviruses (HAstVs) are a global cause of pediatric gastroenteritis and can cause disseminated infection in immunocompromised hosts. Murine astrovirus (muAstV) causes acute asymptomatic infections in immunocompetent mice and chronic infection in immunodeficient models and has provided important insights into AstV pathogenesis in vivo. MuAstV can protect immunodeficient mice from other enteric viruses via robust induction of antiviral cytokine interferon-lambda (IFN-λ), with recent findings implicating goblet cells and enterocytes as both cell types infected by muAstV as well as potential sources of IFN-λ in vivo. BACKGROUND: Antimicrobial resistance among gram-negative pathogens is a global threat, with children particularly vulnerable due to limited treatment options and age-specific clinical outcomes data. Ceftazidime/avibactam (CZA) is effective against resistant gram-negative bacteria and demonstrates high success rates in adults; however, pediatric data remain limited. This study examined CZA use in children across United States medical centers.

METHODS: This multicenter, retrospective, observational cohort study included pediatric patients (< 18 years) who received ≥48 hours of CZA for suspected or confirmed gram-negative infections between February 2015 and October 2023. The primary outcome was clinical success, defined as occurrence of all 3 components: (1) absence of all-cause mortality within 30 days of CZA initiation, (2) absence of microbiologic and clinical recurrence within 30 days of CZA discontinuation, and (3) resolution or improvement of infection-related signs and symptoms during CZA therapy without modification of therapy due to clinical failure. Secondary outcomes included adverse events attributable to CZA and resistance development to CZA.

RESULTS: One-hundred patients were included, with most receiving CZA for targeted therapy (67%). The median (IQR) age and weight were 9.8 (1.9-15.9) years and 24.2 (12.2-49.9) kg, respectively. Nearly all patients (88%) had …

This review critically examines the advancements in biosynthesizing ZnFe₂O₄ NPs using various plant extracts, including Aloe vera, Hibiscus rosa-sinensis, Aegle marmelos, Eucalyptus leaves, Tragacanth gum, Piper nigrum, and Moringa oleifera, among others. It emphasizes their dual role as reducing, capping, chelating, and stabilizing agents. Despite of advances in cardiac and surgical care, some children with complex congenital heart disease remain cyanotic in adulthood. The grown-up cyanotic congenital heart diseases are heterogeneous due to their underlying cardiac anatomy and physiology. Other than cardiovascular problems, they patients are also faced with multi-system complications in the long run. Therefore, these patients need nuanced and comprehensive assessment, regular follow-up monitoring, and multi-facet management. Mathematical Modelling and Numerical Simulation with Applications

Various infectious diseases, such as Tuberculosis and Hepatitis B virus (HBV), caused by Mycobacterium bacteria and hepatitis B virus, respectively, pose serious threats worldwide. This paper examines the spread of these diseases using a mathematical model that incorporates control measures, including vaccination and awareness programs. We use a system of differential equations with control variables and apply Pontryagin's principle to determine optimal control strategies. The stability of the Disease Free Equilibrium (DFE) has been analysed and demonstrated to be both locally and globally asymptotically stable when the basic reproduction number remains below unity, thereby ensuring disease elimination. Leprosy remains a public health problem in Indonesia. In 2023, Indonesia ranked third globally in the number of leprosy cases, with 14,376 reported cases, after India (107,851 cases) and Brazil (22,773 cases). One of the key strategies for leprosy control in Indonesia is the implementation of preventive measures, including home visits and the administration of chemoprophylaxis using single-dose rifampicin (SDR).
